阿尔茨海默病(Alzheimer’s disease，AD)，也称老年痴呆，是一种与年龄高度相关的神经退行性疾病，临床主要表现为进行性认知功能障碍、记忆力下降、语言和视觉空间技能障碍、行为和人格改变，最终可能导致死亡[1]。其主要病理特征包括细胞外β淀粉样蛋白(β-amyloid protein，Aβ)聚集形成的老年斑(senile plaque，SP)、细胞内以过度磷酸化的tau蛋白为主形成的神经纤维缠结(neurofibrillary tangles，NFT)和神经元丢失等[2]。
近年来，研究者发现沉默信息调节因子2同源蛋白1(silent mating-type information regulation 2 homolog 1，SIRT1)的激活与AD的保护作用密切相关[3]。临床观察，健康老人的血清SIRT1水平比青年人明显降低，AD患者和轻度认知功能损伤患者的SIRT1水平又较健康老人明显降低，提示血清SIRT1水平可能是早期诊断AD的潜在生物标志物。AD模型动物(如APP/PS1和Tg19959小鼠等)也显示β淀粉样斑块沉积与SIRT1水平呈反比[4]。
SIRT1激活剂，如白藜芦醇、20(S)人参皂苷Rg3、人参皂苷Rb3、人参皂苷F1、人参皂苷F2、三七皂苷Ft1、重楼皂苷Ⅰ和五味子乙素等，具有抗氧化应激、抗炎、能量稳态调节、神经保护等活性，在AD的治疗过程中显示重要价值[5]。本文就SIRT1与AD研究进展进行综述。
1 SIRT1抑制β淀粉样蛋白生成和沉积沉默信息调节因子2(silent information regulator 2，Sir 2)蛋白家族是普遍存在于人类和其他哺乳动物体内的烟酰胺腺嘌呤二核苷酸(nicotinamide adenine dinucleotide，NAD+)依赖的组蛋白去乙酰化酶类。哺乳动物中发现有7个Sir 2样蛋白(sirtuins)，即SIRT1~SIRT7，在调控细胞应激、代谢、生长、衰老和凋亡等方面发挥着重要作用[6]。SIRT1是哺乳动物中与Sir 2最为相似的同系物，其激活与多种神经退行性疾病，如AD、帕金森病(Parkinson’s disease，PD)、亨廷顿病(Huntington’s disease，HD)、神经运动障碍和多发性硬化等改善有关[7]。
β淀粉样前体蛋白(β-amyloid precursor protein，APP)经过β-分泌酶(β-site APP cleaving enzyme 1，BACE1)酶切形成膜结合片段C99，APP C99通过膜整合蛋白酶复合物γ-分泌酶(γ-secretase)连续分裂后，可形成一系列β-淀粉样肽(Aβ)，如Aβ42和Aβ40，其中Aβ42易在细胞外聚集产生老年斑，在胞内也可形成聚集，且所形成的低聚体或寡聚体在AD形成过程中亦有重要作用。γ-分泌酶由4个膜整合蛋白组成，包含19次跨膜螺旋的复合体，包括早老素Presenilin(PS1)、Aph-1、Pen-2和Nicastrin 4个亚基，其中PS1是执行酶活功能的膜整合蛋白酶(intramembrane protease)活性亚基[8]。
21世纪初，研究者发现多酚化合物白藜芦醇是SIRT1激动剂，具有保护AD的作用[9]。在研究SIRT1激动与减少β淀粉样斑块形成机制时发现，AD模型动物(如APP/PS1小鼠、Tg19959小鼠)脑内β淀粉样斑块的沉积与SIRT1水平呈反比，动物给予白藜芦醇后可明显减轻大脑斑块沉积。SIRT1通过活化具有α分泌酶家族成员的ADAM10转录，增加APP的α-分泌酶切比例，进而减少AD小鼠脑组织中Aβ含量[10]。SIRT1还可通过降低BACE1和Rho相关蛋白激酶1(Rho-associated，coiled-coil-containing protein kinase，ROCK1)的活性，减少Aβ的含量[11]。有趣的是，短期的热量限制也可增加SIRT1表达，并显示对脑部Aβ的清除作用[12]。
2 SIRT1可抑制tau蛋白过度磷酸化细胞骨架由胞质内蛋白质丝组成的纤维状网架系统，可维持细胞基本形态，并参与细胞分裂、运动和物质运输等基本生命活动，在神经元变性过程中起重要作用。微管系统是蛋白质丝主要成分，由α、β微管蛋白和微管相关蛋白组成。tau蛋白是一种微管相关蛋白(microtubule-associated proteins，MAP)，可与微管相结合并保持微管的稳定性。微管蛋白与tau蛋白结合是早期微管组成的核心，促进其他微管蛋白在此核心上延伸聚集形成微管。tau蛋白通过异构和磷酸化控制微管的稳定性。AD患者脑组织神经纤维缠结的主要成分是异常修饰(如磷酸化)的tau蛋白所形成的双螺旋纤丝，通过神经元变性、诱导线粒体和p53等凋亡途径，导致神经元轴突转运障碍、乙酰胆碱释放减少、蛋白酶体活性抑制等，促进神经退行性病变[13-14]。
临床研究显示，AD患者、高龄人群大脑皮层中SIRT1水平明显降低，AD患者大脑皮层中SIRT1的基因和蛋白水平与认知损伤、tau蛋白磷酸化累积程度呈负相关[15]。整体动物研究，敲除tau过表达转基因小鼠的SIRT1基因，动物脑组织中tau的乙酰化和磷酸化水平升高，认知功能障碍加剧，动物死亡率增加[16]。SIRT1激动剂白藜芦醇可通过抑制过氧化物酶增殖体激活受体γ辅助活化因子1α(peroxisome proliferator-activated receptor gamma coactivator-1-alpha，PGC-1α)和p53的乙酰化过程，减轻tau蛋白高度磷酸化小鼠的神经元损伤，并改善其学习记忆能力。体外培养神经元，SIRT1可直接在多个位点使tau蛋白去乙酰化[17]，在一种高表达tau的神经细胞中，tau蛋白乙酰转移酶p300表达水平升高，tau的乙酰化和磷酸化增加，SIRT1水平明显降低，并伴随着Aβ积累[15]。
3 SIRT1可调节神经元突触可塑性正常成年人脑中，突触可塑性是学习记忆形成的基础，长时程增强(long-term potentiation，LTP)是神经元突触可塑性的类型之一，是一种长时间的突触前和突触后神经元同时激动出现的突触传递的增强，在学习记忆形成过程中起到重要作用。AD发病过程中，海马和皮层的突触可塑性发生紊乱，突触功能丧失、坍塌，学习记忆能力下降，LTP的突触后效应出现较多异常[18]。
整体动物研究，特异性敲除小鼠大脑SIRT1基因，与野生型小鼠比较，脑部形态、基础神经递质合成未见明显差异，但是动物出现明显的短期、长期记忆能力缺陷以及空间学习记忆能力的降低，神经突触可塑性明显损伤，突触前膜和CA1区神经元的突触标记蛋白降低，突触素水平明显降低[18]。脑源性神经生长因子(brain-derived neurotrophic factor，BDNF)表达与神经可塑性密切相关，SIRT1可增加脑中BDNF水平，从而增加树突棘数目、神经连接性以及记忆功能，SIRT1增加BDNF水平与甲基-CpG结合蛋白2(MeCP2)的乙酰化有关[19]。cAMP反应元件结合蛋白(cAMP response element binding protein，CREB)在哺乳动物学习记忆调节过程起重要作用，SIRT1可通过miR-134途径增加CREB表达，从而提高CREB依赖性神经元代谢、存活以及可塑性相关基因的表达[20]。
4 SIRT1可抑制神经过度免疫炎症反应神经过度免疫炎性反应可促进AD的发生和发展，AD患者脑部的Aβ是导致炎症反应的重要介质。AD患者大脑中存在明显的非特异性免疫炎性反应，且在斑块形成早期即可起作用。Aβ诱导炎症机制主要包括：① 小胶质细胞的激活：小胶质细胞过度激活是AD情况下神经免疫炎症产生的主要诱因，在AD早期，小胶质细胞激活可通过吞噬和降解Aβ、清除损伤细胞、调节神经免疫炎症反应、清除谷氨酸和分泌神经营养因子等发挥神经保护作用。随着疾病发展，神经炎性斑周围可出现大量激活的小胶质细胞和大量神经免疫炎性因子，如肿瘤坏死因子-α(tumor necrosis factor-α，TNF-α)、γ-干扰素、前列腺素E2(prostaglandin E2，PGE2)、自由基等，通过神经免疫炎症级联反应、Aβ清除障碍、兴奋性毒性物质产生以及氧化应激损伤等途径导致胆碱能神经元变性、死亡，也可进一步促进APP的表达、刺激β-分泌酶，导致Aβ沉积以及细胞凋亡;② 血管内皮β细胞通过多种途径参与Aβ介导的炎症反应，如血管内皮细胞上的晚期糖基化终产物受体(receptor for advanced glycation end products，RAGE)和低密度脂蛋白受体相关蛋白-1(low-density lipoprotein receptor-1，LRP-1)参与脑内外Aβ转运，使Aβ穿过血脑屏障在脑中大量积聚，进而激活小胶质细胞、激活炎症信号通路。
中枢神经系统中，SIRT1激动剂可以抑制Aβ引起的小胶质细胞激活，通过降低Aβ诱导的核因子-κB(nuclear factor-κB,NF-κB)信号通路，减轻炎症损伤[21]。此外，SIRT1还可以抑制小胶质细胞活化后促炎因子白细胞介素-6(IL-6)、TNF-α的生成[22]，并可直接抑制小胶质细胞分化，促进其凋亡[23]。白藜芦醇也可抑制Aβ1-42诱导的星形胶质细胞活化以及促炎因子的生成[24]。SIRT1抑制炎症的机制可能是SIRT1使p65去乙酰化，同时阻断NF-κB依赖性基因表达的反式激活。聚腺苷酸二磷酸核糖转移酶-1[poly(ADP-ribose)polymerase-1，PARP-1]是NF-κB依赖性转录共活化剂，其激活可大量消耗NAD+，导致SIRT1抑制以及NF-κB信号通路的活化，SIRT1可通过去乙酰化以抑制PARP-1表达。在表观遗传学水平，SIRT1通过H4K14去乙酰化，并募集更多阻抑复合物，进而抑制NF-κB依赖性的炎症基因表达。SIRT1去乙酰化，并激活组蛋白甲基SUV39H1，从而抑制诱生型炎症基因的表达[25]。
5 SIRT1可通过Akt通路介导AD的保护作用Akt是一种丝氨酸/苏氨酸激酶，分子质量约为60 ku，因其与已经发现的PKA和PKC高度同源，故又被称为蛋白激酶B(PKB)。Akt可激活一系列底物磷酸化级联反应，激活或抑制下游靶分子介导蛋白合成、细胞生长、糖原代谢等生物活动，促进细胞存活。磷脂酰肌醇3-激酶/蛋白激酶B(phosphatidylinositol 3-kinase/protein kinase B，PI3K/Akt)信号通路独立于丝裂原活化蛋白激酶(mitogen-activated protein kinase，MAPK)、PKC信号通路促进细胞存活，对抗损伤因子发挥神经元的保护作用，激活Akt可保护细胞膜的完整性，防止凋亡因子诱导的神经元细胞膜上的磷脂酰丝氨酸外翻，阻断细胞凋亡，参与学习、记忆的调节。AD发生时，PI3K/Akt信号通路抑制，可促进tau蛋白磷酸化[26]。
糖原合成酶激酶-3β(glycogen synthase kinase-3β，GSK-3β)、哺乳动物雷帕霉素靶蛋白(mammalian target of rapamycin，mTOR)、叉头蛋白转录因子O(forkhead transcription factors of the o class，FoxO)3a等作为PI3K/Akt的下游蛋白，可参与到Akt对AD的保护过程。Akt可通过促进GSK-3β的磷酸化，降低tau蛋白磷酸化，减轻细胞死亡；PI3K/Akt/mTOR信号通路活化可控制神经元自噬，对抗AD进程；Akt活化，可促进FoxO3a磷酸化，抑制神经元凋亡[27]。
Akt可逆性的乙酰化和去乙酰化过程调节具有生物活性。在Akt的PH区域，氨基酸的乙酰化位点是在K14和K20。一般认为，SIRT1可使这些乙酰化的赖氨酸位点去乙酰化，该过程可促使Akt和PIP3结合，继而促进Akt膜移位，激活Akt活性[28]。
SIRT1和Akt可以共同调控细胞凋亡。SIRT1能够使具有促凋亡活性的蛋白如Bax和p53等去乙酰化，发挥抗凋亡作用；SIRT1还可以使Ku70去乙酰化，减少嵌插在线粒体膜上的Bax水平，抑制凋亡过程[29]。在SIRT1抑制凋亡的这个过程中，Akt主要是通过抑制Hdm2介导的Ku70降解，来维持Ku70的水平，从而辅助SIRT1使Ku70去乙酰化，持续发挥抗凋亡作用[30]。
6 小结自AD发现一百多年来，发病率伴随老龄化社会的日益加剧，关于其致病机制、药物治疗关注度越来越高。目前的研究重点还主要集中于Aβ沉积和tau蛋白过度磷酸化。而近来越来越多证据证实SIRT1激动剂可抑制β淀粉样蛋白沉积、tau蛋白磷酸化、炎症反应以及调节神经突触可塑性，在改善AD认知障碍方面具有确切作用，这些发现为研究AD以及治疗AD的药物开发提供了新的思路。
