LVIS支架辅助栓塞颅内破裂动脉瘤

摘要目的 探讨LVIS支架辅助栓塞颅内破裂动脉瘤的安全性和有效性。方法 回顾性分析2016年1月—2019年6月中山市人民医院脑血管介入科使用LVIS支架辅助栓塞颅内破裂动脉瘤118例,分析临床资料、栓塞率及并发症等情况。结果 118例颅内破裂动脉瘤均成功栓塞。术后即刻栓塞结果:RaymondⅠ级栓塞82例(69.5%),Ⅱ级栓塞29例(24.6%),Ⅲ级栓塞7例(5.9%)。术中LVIS支架完全打开116例(98.3%),未完全打开2例(1.7%)。术中形成脑血栓6例(5.1%),其中3例及时给予替罗非班治疗后消失,3例用药后仍有脑血栓及脑梗死症状。术后发生血栓事件2例(1.7%),经过积极治疗后1例遗留有神经功能缺损症状。术后随访6~12个月,动脉瘤复发6例(5.1%),支架内狭窄5例(4.2%),未发生再出血,疗效良好(mRS评分0~2分)102例(86.4%)、疗效不良16例(13.6%)。结论 LVIS支架辅助栓塞颅内破裂动脉瘤安全、可行。

LVIS stent-assisted embolization of ruptured intracranial aneurysm

Abstract: Objective To investigate the safety and efficacy of LVIS stent-assisted embolization in treating ruptured intracranial aneurysms. Methods A retrospective analysis was performed on the clinical data, embolization rate, and complications of 118 cases of ruptured intracranial aneurysm who were admitted to the Department of Cerebrovascular Intervention of Zhongshan People's Hospital from January 2016 to June 2019 and treated with LVIS stent-assisted embolization. Results All 118 ruptured intracranial aneurysms were successfully embolized. The embolization results immediately after operation were as follows:Raymond grade I embolization was achieved in 82 cases (69.5%), grade II embolization in 29 cases (24.6%), and grade III embolization in 7 cases (5.9%). The LVIS stent was fully open in 116 cases (98.3%) during the operation, and not fully open in 2 cases (1.7%). Six patients (5.1%) had cerebral thrombosis during the operation, of which 3 were timely treated with tirofiban and then the thrombosis disappeared, while the other 3 still had symptoms of cerebral thrombosis and cerebral infarction after medication. Postoperative thrombotic events occurred in 2 cases (1.7%), of which 1 case still had symptoms of neurological defects after active treatment. During the postoperative follow-up period of 6-12 months, aneurysm recurred in 6 patients (5.1%), in-stent stenosis occurred in 5 patients (4.2%), and no rebleeding occurred in any patient; 102 patients (86.4%) had good outcome (modified Rankin Scale:0-2) and 16 (13.6%) had poor outcome. Conclusions LVIS stent-assisted embolization is safe and feasible in treating ruptured intracranial aneurysms.

颅内动脉瘤破裂出血在出血性脑血管病中发病率较高,首次破裂出血的致残率和病死率在15%~25%[1],单独栓塞复发率较高[2]。支架辅助下栓塞颅内动脉瘤可明显减少复发率,LVIS支架其金属覆盖率为22%,支架网眼约1 mm,可起到更好的血流导向作用,目前广泛用于临床辅助颅内动脉瘤栓塞[3]。本文通过回顾性分析我院118例LVIS支架辅助栓塞颅内破裂动脉瘤的临床资料,分析其疗效及并发症发生情况。

1 资料和方法 1.1 临床资料

2016年1月—2019年6月中山市人民医院脑血管介入科随访到的118例LVIS辅助颅内破裂动脉瘤的临床资料。其中,男性56例,女性62例;年龄26~78岁,平均(56±21)岁;Hunt-Hess分级Ⅰ级38例,Ⅱ级56例,Ⅲ级18例,Ⅳ级6例;临床表现主要有头痛、呕吐,神经功能障碍,意识障碍等。所有患者发病后均行颅脑CT证实有蛛网膜下腔出血,86例患者术前行脑血管CTA证实有颅内动脉瘤,118例患者均通过全脑数字减影血管造影(DSA)检查确诊有颅内动脉瘤。根据动脉瘤发生位置:前交通动脉瘤18例,后交通动脉瘤31例,颈内动脉瘤26例,大脑中动脉瘤15例,大脑前动脉瘤6例,脉络膜前动脉瘤2例,基底动脉瘤8例,大脑后动脉瘤2例,椎动脉瘤6例,小脑后下动脉瘤3例,小脑上动脉瘤1例。根据动脉瘤形状:囊状动脉瘤104例,血泡样动脉瘤6例,夹层动脉瘤7例,假性动脉瘤1例。

1.2 方法

所有患者均在出血后72 h内在全麻下行LVIS支架辅助下动脉瘤栓塞术,术中常规肝素化,常规股动脉穿刺,置入6F导引导管(Cordis,美国),根据DSA情况了解动脉瘤大小、位置、形状及其与载瘤动脉的关系,选择合适工作位,选用支架指征[4]:①绝对宽颈:瘤颈>瘤体;②相对宽颈:0.5≤瘤颈/瘤体或瘤颈≥4 mm;③夹层动脉瘤。应用Traccess 14微导丝(MicroVention,美国),Headway 21微导管(MicroVention,美国)及Echelon 10微导管(Medtronic,美国)。微导管塑形后将Headway 21微导管送入载瘤动脉远端,Echelon 10微导管送入动脉瘤腔内。术中根据情况选择半释放技术(先部分填塞动脉瘤,再半释放Lvis支架覆盖动脉瘤颈,随后栓塞动脉瘤,最后完全释放支架)或者后释放技术(先完全栓塞动脉瘤,后释放Lvis支架)。术中释放支架时,先静脉推注负荷量替罗非班0.8 μg/kg,推注时间5~10 min,再持续静脉泵入替罗非班0.1 μg/(kg·min),一般用12~24 h[5]。术后长期口服阿司匹林100 mg/d,口服氯吡咯雷75 mg/d 6周后停药,术后双联抗血小板药物重叠6小时后停用替罗非班。

2 结果 2.1 即刻栓塞情况

根据Raymond分级评价动脉瘤即刻栓塞程度,Ⅰ级为完全栓塞,Ⅱ级为次全栓塞,Ⅲ级部分栓塞,RaymondⅠ级栓塞82例(69.5%),Ⅱ级栓塞29例(24.6%),Ⅲ级栓塞7例(5.9%),栓塞有效率94.1%。

2.2 手术并发症

术中LVIS支架良好116例(98.3%),打开不良时,经导丝、微导管通过支架,必要时球囊扩张后大部分可以打开(图 1)。通过上述办法仍未完全打开支架2例(1.7%),其中1例位于颈内动脉虹吸弯,1例位于大脑中M1分叉处(图 2)。支架内血栓事件8例(6.8%),术中形成6例(5.1%),其中3例在导指引管内推注替罗非班8~10 ml后消失,3例遗留神经功能缺损症状,2例因支架未完全打开导致的(图 2)。术后发生血栓事件2例(1.7%),术后复查CT未见脑出血后再次造影证实支架内血栓,并给予替罗非班,其中1例遗留神经功能缺失症状。

2.3 随访结果

术后随访6~12个月,临床疗效评价按照改良Rankin量表评分(mRS)评估,mRS评分0~2分为疗效良好。动脉瘤复发定义为以下任意一种情况[6]:①责任动脉瘤再出血;②责任动脉瘤再治疗;③患者不明原因死亡。动脉瘤复发6例(5.1%),3例为颈内动脉血泡样动脉瘤,1例为椎动脉夹层动脉瘤(图 3),2例为后交通动脉瘤。支架内狭窄5例(4.2%),均无明显神经功能缺失症状,暂时无进一步处理,口服抗血小板及他汀类药物,2例为大脑中动脉瘤(图 4),1例颈内动脉瘤,1例大脑前动脉瘤,1例基底动脉瘤。术后未发生再出血,疗效良好102例(86.4%)、疗效不良16例(13.6%)。

3 讨论

颅内动脉瘤破裂出血急性期行支架辅助下动脉瘤栓塞术是动脉瘤栓塞中常见的治疗策略。LVIS支架具有良好的顺应性,贴壁性,金属覆盖率高,血流导向性好[7, 8]。急性期使用LVIS支架辅助栓塞动脉瘤同时也可能出现严重并发症。

首先术中可能出现支架打开不良,LVIS支架在虹吸段及大脑中M1分叉,前交通分叉等部位可能出现打开不好,有文献报告支架展开成功率为93.5%[9]。本组支架完全展开116例(98.3%),有2例支架打开不良(1.7%),打开不良时,通过摆动支架导丝,微导丝成襻配合微导管反复按摩支架,必要时球囊扩张等,大多情况可以打开。本组有2例通过上述办法仍打开不良并出现并发症,其中1例位于颈内动脉虹吸弯,在虹吸段有s型弯,1例位于大脑中动脉分叉部位。

支架内血栓事件报道很多,支架内血栓可能和支架打开不良,支架扭曲,贴壁不良,局部金属覆盖率过高有关[10],支架内血栓发生率大约0%-5%[11]。本文中支架内血栓有8例(6.8%),术中脑血栓形成6例,及时给予替罗非班等处理后3例好转,3例仍遗留神经功能缺失症状,2例术中因支架打开不良形成的。术后脑血栓事件2例,造影发现后及时用替罗非班或者阿替普酶后1例好转,1例仍遗留有神经功能缺失症状。动脉瘤出血急性期行LVIS支架辅助下栓塞动脉瘤,目前支架的微导管到位后我们就开始外周静脉泵入替罗非班0.1 μg/(kg·min),术中发现血流减慢时及时在导指引管内推注6~8 ml替罗非班。

术后支架内狭窄发生率约0%~86.7%[12]。支架内狭窄主要和血管内皮增生,动脉硬化,抗血小板药物使用、支架打开不全等有关[13]。无症状的指甲内狭窄建议保守药物治疗,密切随访[14]。本组术后随访出现支架内狭窄5例,本组资料5例病例主要位于血管转折处或者血管较细的远端血管,这可能也和支架贴壁欠佳,血管内皮增生等有关,如果发现有血管狭窄,建议双联抗血小板药物使用时间为6~12个月,必要时加用他汀类药物,本组病例术后发现有狭窄病例无神经功能症状,未进一步处理,密切观察随访。

术后动脉瘤复发率及再出血率较低,有文献报道大约7.4%[15]。本文术后复发6例,其中3例为血泡样动脉瘤,术后再出血率较低,本组术后无再出血,一般术后半年复查DSA,对于颈内血泡样动脉瘤,复发率极高,一般术后2周,3个月,6个月复查DSA,早期发现复发,早期处理。

总之,LVIS支架辅助栓塞颅内破裂动脉瘤安全有效,但长期疗效仍需严密、长期的随访及随机试验。
