全球不同控烟措施应用场景下潜在的短期控烟效果的Meta分析

摘要: 目的 在梳理全球控烟措施效果的模拟预测研究现状、理清不同控烟措施的实施场景的基础上,系统分析提高烟草税、全面无烟环境立法、烟草包装健康警示、防止向未成年人销售烟草、禁止烟草营销、提供戒烟帮助、开展控烟宣传活动(七大类控烟措施)不同应用场景下潜在的短期控烟效果。方法 系统检索PubMed、Embase、EconLit、PsychINFO和CINAHL数据库截至2022年4月有关控烟措施效果模拟预测的研究。根据纳入和排除标准收集资料,采用R软件对七大类控烟措施不同应用场景下潜在的短期控烟效果进行Meta分析。结果 共纳入22篇文献,研究地点涉及16个国家,其中美国、墨西哥和意大利分别进行5、3和2次研究。纳入研究均选取了提高烟草税、全面无烟环境立法和开展控烟宣传活动措施,部分研究选取了防止向未成年人销售烟草措施(21项)、禁止烟草营销措施(20项)和提供戒烟帮助及烟草包装健康警示措施(19项)。Meta分析结果显示,提高烟草税对不同年龄组的价格弹性影响不同,15~17岁组的价格弹性最大,为0.044(95%CI:0.038~0.051);工作场所全面无烟环境立法的潜在短期控烟效果高于对餐厅和室内其他公共场所;防止向未成年人销售烟草措施对$ < $16岁组的影响大于16~17岁组;其他控烟措施的实施力度越强,潜在的短期控烟效果影响越大。七大类控烟措施间比较发现,提供戒烟帮助的戒烟率增加幅度最大,为0.404(95%CI:0.357~0.456);防止向未成年人销售烟草表现为优时,对$ < $16岁组影响的吸烟率下降幅度、开始吸烟率下降幅度最大,分别为0.292(95%CI:0.269~0.315)和0.292(95%CI:0.270~0.316)。结论 通过Meta分析获得七大类控烟措施不同应用场景下潜在的短期控烟效果。从短期效果看,提供戒烟帮助将大幅度提高戒烟率,完全落实防止向未成年人销售烟草措施时将大幅度降低 < 16岁组的吸烟率和开始吸烟率,为我国和其他国家控烟措施模拟研究提供重要的参考依据。

Potential short-term effects of tobacco control scheme under various application scenarios of tobacco control measures across the globe: a Meta-analysis

Abstract: Objective based on summarizing the simulation and prediction of tobacco control measures across the globe and sorting out the various scenarios of tobacco control measures, the potential short-term effects of seven tobacco control measures in different scenarios were systematically analyzed. Methods Until April 2022, PubMed, Embase, EconLit, PsychINFO, and CINAHL databases were used to retrieve literature about tobacco control measures simulation and prediction models across the globe. Inclusion and exclusion criteria were strictly followed. Meta-analysis for the potential short-term effects of seven tobacco control measures in different scenarios was performed using the R software. Results A total of 22 papers covering 16 countries were selected. Five studies were conducted in the United States, three in Mexico, and two in Italy. There were all papers with the measures to tax increases, smoke-free air laws, and mass media campaigns, 21 papers with youth access restrictions, 20 with marketing restrictions, and 19 with cessation treatment programs and health warnings. The tax increases had diverse influences on the price elasticity of different age groups. The price elasticity in the age group 15-17 years was the highest, which was 0.044 (95%CI: 0.038-0.051). The potential short-term effects of smoke-free air laws in workplaces were higher than in restaurants and other indoor public places. The effects of youth access restrictions were greater in the age group < 16 years than in the age group 16-17. The stronger the implementation of other measures, the greater the potential short-term effects. A comparison of seven tobacco control measures showed that the cessation treatment programs increase in cessation rate was the highest, 0.404 (95%CI: 0.357-0.456). The reduction in smoking rate and reduction in initiation rate of youth access restrictions strongly enforced and publicized was the highest in the age group < 16 years, 0.292 (95%CI: 0.269-0.315), and 0.292 (95%CI: 0.270-0.316). Conclusions The potential short-term effects of seven tobacco control measures in different scenarios were evaluated more accurately and objectively through Meta-analysis. In the short term, cessation treatment programs will substantially increase smoking cessation rates, and strong youth access enforcement will sharply reduce smoking and initiation rates among adolescents under 16. These results also offer strong data-related support for the simulation and prediction of tobacco control measures in China and other countries.

烟草危害是当今世界严重的公共卫生问题,2021年《全球烟草流行报告》显示,烟草使用每年造成全球超过800万人死亡,其中700余万人死于直接使用烟草,约120万人(6.5万名儿童)死于二手烟暴露,每年造成1.4万亿美元的经济损失,相当于全球年度国内生产总值的1.8%,而在发展中国家这一损失比例高达40%[1-3]。中国是主要的烟草生产国和消费国,烟草消费占世界的40%以上,我国约有3.16亿烟民,占全球吸烟者的1/4[4],约有7.4亿非吸烟者暴露于二手烟[5]。我国每年因烟草使用死亡的人数超过200万[6],若不采取有效控烟措施,预计2050年将增加至300万[7]

为应对全球烟草流行对健康的严重威胁,WHO从减少烟草需求的角度提出MPOWER(M:监测烟草使用与预防措施;P:保护人们免受烟草烟雾危害;O:提供戒烟帮助;W:警示烟草危害;E:禁止烟草广告、促销和赞助;R:提高烟草税)措施以指导各国有效控烟[8-10]。2021年《全球烟草流行报告》指出,巴西和土耳其已全部落实MPOWER措施,并取得显著的控烟效果[1]。在MPOWER措施的指导下,我国控烟工作取得一定进展[5, 11],但与MPOWER措施要求仍存在很大差距,包括尚没有公共场所禁止吸烟的全国性立法;烟草包装华丽,未有图形警示;烟草制品税率和价格均偏低,卷烟零售税率远低于WHO所建议的75%等[10]

SimSmoke模型通常用于预测开始实施某一项MPOWER措施或同时实施多种MPOWER措施对吸烟率的影响,已在巴西、美国等20多个国家使用[12],例如,巴西若2010年开始提高烟草税率至零售价的75%,利用SimSmoke模型估计至2050年吸烟率将下降14%,避免47万人过早死亡。而SimSmoke模型中潜在的短期控烟效果这一关键参数通常由政策制定者、烟草控制专家等商讨确定[13],而专家小组对MPOWER措施、本国控烟现状和这一参数的了解程度等因素会对其确定产生一定影响。另外,由于各国控烟措施实施现状、经济发展水平和居民对吸烟危害的认识不同,不能直接照搬其他国家的潜在短期控烟效果值进行模拟预测研究[14]。因此,亟需对其统一标准,供各国借鉴与参考,使各国SimSmoke模型预测结果更为准确与科学。

本研究汇总全球SimSmoke模型研究,主要考虑提高烟草税、全面无烟环境立法、烟草包装健康警示、防止向未成年人销售烟草、禁止烟草营销、提供戒烟帮助和开展控烟宣传活动(七大类控烟措施)及其不同的应用场景,了解其短期控烟效果的指标和大小的可获得性,并通过Meta分析获得不同控烟措施按WHO推荐要求实施的潜在的短期控烟效果大小,为我国和其他国家进行长期控烟效果的模拟预测提供重要的参考依据。

资料与方法

1. 文献检索:系统检索PubMed、Embase、EconLit、PsychINFO和CINAHL数据库中关于控烟措施模拟预测模型的相关文献资料。截止日期为2022年4月,以“SimSmoke”“MPOWER”及同义词进行检索,并辅以文献追溯法收集更多相关文献。

2. 文献筛选与资料提取:由2位研究者根据纳入、排除标准独立筛选文献、提取资料及交叉核对,如遇分歧,交由第3位研究者裁决。初步通过文献标题和摘要排除明显不符合要求的文献,对可能符合纳入标准的文献进一步阅读全文进行复筛,以确定其是否纳入本研究。

3. 纳入与排除标准:纳入标准:①控烟措施模拟预测相关研究;②明确报告不同控烟措施的潜在短期控烟效果影响指标及大小;③预测指标至少有一个为吸烟率;④针对整个国家。排除标准:①评估已实施的控烟措施效果;②未明确报告不同控烟措施的潜在短期控烟效果的指标及大小;③综述、通信、评论、报告等非原始数据研究;④研究仅针对特殊人群:儿童、孕妇、男性等;⑤针对某一州或省。

4. 数据处理与统计学分析:首先,收集2019年全球疾病负担研究(GBD2019)各国对应年份≥15岁吸烟者数和分年龄组人口数、全球成人烟草调查(GATS)和全球青少年烟草调查(GYTS)(https://www.cdc.gov/tobacco/data_statistics/surveys/index.htm)分年龄组吸烟率,并计算各国相对应非吸烟者数(人口数与吸烟者数之差);再根据摘录数据中不同控烟措施的潜在短期控烟效果值,即吸烟率下降幅度(RSR)、戒烟率增加幅度(ICR)和开始吸烟率下降幅度(RIR),计算各国对应年份吸烟者减少数(吸烟者数×RSR或吸烟者数×ICR)和开始吸烟者减少数(非吸烟者数×RIR)。最后,将RSR、ICR转化为总吸烟者减少数与总吸烟者数的比值,RIR转化为总开始吸烟者减少数与总非吸烟者数的比值。

采用R软件中的Meta程序包对统计量进行合并,计算七大类控烟措施不同应用场景下总RSR、ICR和RIR及其95%CI,并绘制森林图和漏斗图。采用I2Q检验评估纳入研究间的异质性,若I2 < 50%,P > 0.05表明研究间的异质性可以接受,采用固定效应模型;否则采用随机效应模型,说明各独立研究结果间不同,不仅是由于抽样误差造成的。采用漏斗图和Egger检验以确定纳入文献是否存在发表偏倚,当Egger检验P < 0.05时,则认为不存在发表偏倚。

结果

1. 文献筛选过程:5个数据库共检索到相关文献725篇,剔除重复文献276篇。通过阅读题目和摘要,获得101篇,进一步阅读全文后,最终纳入分析22篇[13-34]。文献筛选流程见图 1

2. 纳入研究基本特征:研究时间为2005-2021年,以2010年以后为主。纳入的22篇文献共涉及16个国家,其中美国、墨西哥和意大利分别进行5、3和2次研究。22篇文献均为SimSmoke模型,均选取了提高烟草税、全面无烟环境立法和开展控烟宣传活动措施。其中共17篇文献选取了七大类控烟措施,而选取防止向未成年人销售烟草措施、禁止烟草营销及提供戒烟帮助和烟草包装上印制健康警示措施分别有19、20和21篇文献。

3. Meta分析结果:

(1)提高烟草税:15~、18~、25~及≥35岁组的价格弹性依次降低,分别为0.044(95%CI:0.038~0.051)、0.031(95%CI:0.024~0.039)、0.019(95%CI:0.015~0.024)及0.010(95%CI:0.009~0.011)。见表 1Q检验结果提示,18~、25~及≥35岁组的价格弹性均不存在异质性(P > 0.05,I2=0),而15~17岁组的价格弹性存在异质性(P < 0.05,I2≥50%)。

(2)全面无烟环境立法:对工作场所全面无烟环境立法的RSR、RIR及ICR均最高,RSR和ICR为0.067(95%CI:0.060~0.076),RIR为0.066(95%CI:0.059~0.074);餐厅次之,RSR和ICR为0.021(95%CI:0.017~0.024),RIR为0.021(95%CI:0.018~0.025);其他室内公共场所最低,RSR和ICR为0.010(95%CI:0.009~0.011),RIR为0.010(95%CI:0.009~0.010)。见表 1Q检验结果显示,对工作场所和餐厅全面无烟环境立法的RSR、RIR及ICR均存在异质性(P < 0.05,I2≥50%),其他室内公共场所的RSR、RIR及ICR均不存在异质性(P > 0.05,I2=0)。

(3)烟草包装健康警示:在烟盒包装上印制强、中等及弱健康警示的RSR、RIR及ICR均依次降低,且ICR明显高于RSR和RIR。RSR分别为0.025(95%CI:0.019~0.032)、0.014(95%CI:0.010~0.019)及0.009(95%CI:0.007~0.011);ICR分别为0.059(95%CI:0.046~0.075)、0.030(95%CI:0.021~0.041)及0.016(95%CI:0.013~0.020);RIR分别为0.023(95%CI:0.016~0.031)、0.009(95%CI:0.004~0.014)及0.009(95%CI:0.008~0.010)。见表 1Q检验结果显示,在烟盒包装上印制强、中等及弱健康警示的RSR、RIR及ICR均存在异质性(P < 0.05,I2≥50%)。

(4)防止向未成年人销售烟草:防止向未成年人销售烟草方面表现优、良的RSR和RIR明显高于表现差时,且对 < 16岁青少年的影响明显大于16~17岁。< 16岁组,防止向未成年人销售烟草方面表现优、良和差的RSR分别为0.292(95%CI:0.269~0.315)、0.153(95%CI:0.128~0.179)和0.031(95%CI:0.028~0.033);RIR分别为0.292(95%CI:0.270~0.316)、0.154(95%CI:0.128~0.180)和0.031(95%CI:0.029~0.034);16~17岁组RSR分别为0.162(95%CI:0.146~0.178)、0.091(95%CI:0.076~0.106)及0.021(95%CI:0.019~0.022);RIR分别为0.189(95%CI:0.178~0.200)、0.101(95%CI:0.085~0.119)及0.020(95%CI:0.019~0.022)。见表 1Q检验结果显示,防止向未成年人销售烟草方面表现为优和良时16~17岁组的RSR和表现为差时 < 16岁组的RSR和16~17岁组的RIR不存在异质性(P > 0.05,I2=0),其余存在异质性(P < 0.05,I2≥50%)。

(5)禁止烟草营销:全面、适度及低限度禁止烟草营销的RSR、RIR及ICR均依次降低,RSR分别为0.064(95%CI:0.055~0.074)、0.036(95%CI:0.028~0.045)及0.014(95%CI:0.011~0.017),RIR分别为0.079(95%CI:0.068~0.090)、0.045(95%CI:0.035~0.058)及0.014(95%CI:0.011~0.017),全面、适度禁止烟草营销的ICR分别为0.041(95%CI:0.036~0.047)、0.026(95%CI:0.021~0.032),低限度禁止烟草营销不会增加戒烟率。见表 1Q检验结果显示,全面、适度及低限度禁止烟草营销的RSR、RIR及ICR均存在异质性(P < 0.05,I2≥50%)。

(6)提供戒烟帮助:提供戒烟药物、开展戒烟热线及100%简短戒烟干预和随访措施的RSR为0.050(95%CI:0.042~0.059),ICR为0.404(95%CI:0.357~0.456),ICR明显高于RSR。见表 1Q检验结果显示,提供戒烟帮助的RSR和ICR均存在异质性(P < 0.05,I2≥50%)。

(7)开展控烟宣传活动:开展大型、中型及小型控烟宣传活动的RSR、RIR及ICR均依次降低,RSR及ICR分别为0.057(95%CI:0.049~0.065)、0.031(95%CI:0.027~0.036)及0.010(95%CI:0.008~0.012),RIR分别为0.055(95%CI:0.047~0.063)、0.032(95%CI:0.027~0.037)及0.010(95%CI:0.008~0.013)。见表 1Q检验结果显示,开展大型、中型及小型控烟宣传活动的RSR、RIR及ICR均存在异质性(P$ < $0.05,I2$ \ge $50%)。

(8)不同控烟措施间RSR、ICR和RIR进行比较:防止向未成年人销售烟草表现优时,对 < 16岁组青少年影响的RSR、RIR最大,分别为0.292(95%CI:0.269~0.315)和0.292(95%CI:0.270~0.316);提供戒烟帮助的ICR最大,为0.404(95%CI:0.357~0.456)。见表 1

4. 发表偏倚分析:Egger检验结果提示,提高烟草税对18~24和≥35岁组的价格弹性、工作场所全面无烟立法的RSR、RIR和ICR等可能存在发表偏倚。而选取中等健康警示和防止向未成年人销售烟草表现良好应用场景的文献 < 10篇,尚无法进行Egger检验,通过漏斗图提示,中等健康警示的RIR和防止向未成年人销售烟草表现良好时 < 16岁组的RSR可能存在发表偏倚。使用剪补法后,结果无明显变化,显示结果比较稳健。

讨论

纳入文献中共有16个国家进行了SimSmoke模型研究,并报道了控烟措施不同应用场景的潜在短期控烟效果指标及大小,模拟预测结果与实际吸烟率变化吻合度均较高,其中美国、墨西哥和意大利已进行多次模拟研究[15, 23, 31]。本研究基于全球控烟措施效果的模拟预测研究,通过Meta分析评估七大类控烟措施不同应用场景下潜在短期控烟效果指标和大小,研究结果相对于直接照搬其他国家或仅基于本国专家商讨结果更为客观、准确,可以促使已进行SimSmoke模型研究的国家更新预测结果,又可以为从未进行SimSmoke模型研究或未选满七大类控烟措施的国家提供更加准确、可靠的参考。

不同控烟措施间比较显示,提供戒烟帮助的ICR最大,由于大多数吸烟者会产生尼古丁依赖,每日吸烟者在无帮助的情况下尝试戒烟的复吸率高达90%~95%,而提供有效的戒烟帮助能够帮助吸烟者应对生理、心理成瘾,并帮助其建立支持戒烟的社会环境,从而大大提高戒烟的成功率[35]。防止向未成年人销售烟草表现为优时,对 < 16岁组影响的RSR、RIR最大,青少年吸烟不仅影响身心健康,还可能导致成年后对烟草制品产生依赖,因此防止向未成年人销售烟草不仅促进未成年人的身心健康发展,而且从根源上减少烟草使用人口[36]。虽然与其他控烟措施不同场景下短期控烟效果相比,提高烟草税措施的价格弹性非最大值,但多项研究显示提高烟草税是减少吸烟最有效的手段[1, 22, 37]。基于本Meta分析结果进一步开展具有我国代表性的长期控烟效果模拟预测研究,可以为我国完善和实施MPOWER系列措施提供重要的科学依据和指导,早日实现“健康中国2030”≥15岁人群吸烟率降至20%的控烟目标[38-39]

从全球SimSmoke模型模拟研究中摘录控烟措施不同应用场景下潜在短期控烟效果时,仅有RSR、ICR和RIR单一值。本研究在进行Meta分析时,考虑到各国人口数、吸烟率并不相等,利用GBD2019、GATS和GYTS数据将其转化为各国吸烟数或开始吸烟数变化情况,进一步结合各国总吸烟者数和总非吸烟者数计算合并RSR、ICR和RIR,总RSR、ICR和RIR值的范围得到收敛,结论更全面、可靠[40],更适合于各国开展控烟措施模拟预测研究。

本研究存在局限性。不同控烟措施场景下多个潜在短期控烟效果纳入研究间异质性较大,可能与研究国家间人口数有别、吸烟率数据不充分等有关。潜在短期控烟效果参数汇总多基于国外的控烟措施模拟研究结果,可能与我国控烟现状与国情存在差异,尚不能直接用于我国控烟措施模拟预测研究。另外,纳入的研究选取在烟盒包装上印制中等健康警示等控烟措施的国家相对较少,尚无法进行Egger检验发表偏倚,只能通过漏斗图进行主观判断。
