山东省2016-2017年人群烟草使用及其影响因素调查

摘要: 目的 了解山东省人群烟草使用现状以及主要影响因素。方法 运用多阶段分层整群抽样方法,在山东省36个区(县)216个村(居委会)抽取调查对象,使用非条件logistic回归模型进行多因素分析。结果 有效调查6 271人,经过复杂加权计算后,≥ 15岁人群现在吸烟率为23.93%,其中男性为45.58%,女性为1.18%;不同年龄组人群现在吸烟率差别较大,45~64岁年龄组现在吸烟率最高(25.07%);不同文化程度人群吸烟率不同,初中文化程度者现在吸烟率最高(28.94%);农村现在吸烟率(24.98%)高于城市(23.08%);现在每日吸烟率为20.23%,吸烟者开始吸烟平均年龄为21.21岁,日平均吸机制卷烟量为16.31支;在调查所有曾经和现在吸烟者中,戒烟率为20.79%。多因素logistic回归分析结果显示,性别、年龄、职业、地区、健康知识得分与吸烟行为存在关联。男性现在吸烟率高于女性(OR=49.625,95%CI:37.832~65.093),45~64岁组现在吸烟率高于15~24岁组(OR=1.830,95%CI:1.048~3.194),农林牧渔从业者现在吸烟率分别高于医务人员(OR=0.403,95%CI:0.187~0.866)和离退休人员(OR=0.648,95%CI:0.481~0.873),中部地区居民现在吸烟率低于东部(OR=0.724,95%CI:0.606~0.865),吸烟危害健康知识得分1~3分组现在吸烟率高于4~6分组(OR=1.432,95%CI:1.240~1.654)。结论 山东省人群吸烟率处于较高水平,应对吸烟者开展控烟健康教育等综合性干预措施,以降低人群吸烟率。

Survey on tobacco use and associated factors in population in Shandong province, 2016-2017

Abstract: Objective To understand the status of tobacco use and related influencing factors in population in Shandong province. Methods Stratified multi-stage cluster sampling was used to select respondents from 216 villages (communities) of 36 districts (counties) in Shandong province. Influencing factors on smoking were analyzed by logistic regression model. Results The adequate sample size was 6 271 participants. After complex weighted, the current smoking prevalence was 23.93%, 45.58% in males and 1.18% in females. The Public's smoking rates varied widely among different groups in the population. 45-64 age group had the highest smoking rate (25.07%). Smoking rates were substantially different in education levels, with the highest in junior high school (28.94%). The rate was higher in the rural area (24.98%) than that in the urban areas (23.08%). The average daily smoking rate was 20.23%. The average age of initiating smoking was 21.21 years. The average daily cigarette intake was 16.31 cigarettes. Among all the former and current smokers, the quitting rate was 20.79%. Multiple logistic regression model analysis showed that gender, age, occupation, region, and health knowledge score were correlated with smoking behavior. The current smoking rate of men was much higher than that of women (OR=49.625, 95%CI: 37.832-65.093). The current smoking rate in 45-64 age group was higher than that in the 15-24 age group (OR=1.830, 95%CI: 1.048-3.194). The current smoking rate of medical (OR=0.403, 95%CI: 0.187-0.866) and retired personnel (OR=0.648, 95%CI: 0.481-0.873) were lower than those engaging in agriculture, forestry, animal husbandry and the fishery, respectively. The prevalence of residents living in the central part showed lower rate on current smoking than that in the eastern region (OR=0.724, 95%CI: 0.606-0.865). The current smoking prevalence of smoke hazard in 1-3 score group was higher than that in the group with 4-6 score (OR=1.432, 95%CI: 1.240-1.654). Conclusions Smoking rate in adults in Shandong remained stable and at a high level. Comprehensive intervention measures such as tobacco control and health education should be carried out to reduce the smoking rate.

烟草流行是全球面临的最大公共卫生威胁之一,每年使世界各地800多万人失去生命,其中有700多万人死于直接使用烟草[1]。全球28个国家人群烟草流行调查显示,现在吸烟率为22.5%[2]。2018年中国≥15岁人群吸烟率为26.6%[3]。与既往调查结果相比,吸烟率呈现下降趋势。为了解山东省居民吸烟行为及其影响因素,对吸烟行为进行科学干预,本研究对2016-2017年山东省人群吸烟情况和相关危险因素进行调查。

对象与方法

1. 研究对象:调查样本来自中国人群烟草流行监测,2016、2017年山东省分别有16个国家监测点,2017年增加了4个省级监测点,共调查36个区(县)、108个街道(乡镇)、216个村(居委会),实现山东省16市全覆盖,确保抽取样本具有全省代表性。调查对象为≥15岁非集体居住的居民。

2. 调查方法:采用多阶段分层整群抽样方法,第一阶段使用容量比例概率抽样法(PPS)抽取区(县),第二阶段采用PPS法在每个监测点抽取3个街道(乡镇),第三阶段使用PPS法在每个街道(乡镇)抽取2个居委会(村),第四阶段每个居委会(村)随机抽取40个家庭户,第五阶段利用电子设备自动抽取1名家庭成员作为调查对象。

3. 调查内容:调查对象的基本情况、烟草使用、戒烟、以及人们对烟草使用的知识和态度等信息。问卷由中国CDC控烟办设计,调查员经过统一培训,入户进行调查。现场质控员对问卷进行核查,省级督导员抽取5%的调查问卷进行复核。

4. 分析指标:现在吸烟者为调查时存在吸烟行为的人;现在每日吸烟者为调查时每天都吸烟的人;戒烟者为过去曾吸过烟,但调查时已不再吸烟的人。吸烟危害健康知识6题:吸烟会不会造成严重疾病、中风、心脏病、肺癌、阴茎勃起障碍与低焦油低危害。

5. 统计学分析:使用EpiData 3.0软件建立数据库,使用SAS 9.3软件进行数据清理和复杂调查数据分析,计算各指标的率(构成比)及95%CI。吸烟行为的影响因素分析使用logistic回归模型,不同率(构成比)的差异比较采用χ2检验。

结果

1. 一般情况:2016-2017年连续2年应调查6 401人,实际完成6 271人,应答率为97.97%。加权后,男性占51.25%,女性占48.75%;城乡居民比例分别为55.19%和44.81%;年龄组以45~64岁和25~44岁为主,占总人数的74.38%;文化程度主要为小学和初中,占69.80%(表 1)。

2. 吸烟情况:

(1)现在吸烟率:调查人群经过复杂加权计算后,≥15岁人群现在吸烟率为23.93%,其中男性为45.58%,女性为1.18%。不同年龄组现在吸烟率差别较大,45~岁组吸烟率最高(25.07%);15~岁组吸烟率最低(16.30%)。不同文化程度人群吸烟率不同,初中文化程度者现在吸烟率最高(28.94%);小学及以下文化程度者为19.53%。农村现在吸烟率高于城市,分别为24.98%、23.08%。现在每日吸烟率为20.23%,男性为38.59%,女性为0.94%;不同年龄组中,现在每日吸烟率45~岁组最高(21.53%);不同文化程度中,初中文化程度者每日吸烟率最高(24.50%)。见表 2

(2)开始吸烟年龄:每日吸烟者开始吸烟年龄为21.21(95%CI:20.72~21.71)岁,最小年龄为16岁,最大年龄为57岁。男性开始每日吸烟年龄为21.14(95%CI:20.63~21.64)岁,女性开始每日吸烟年龄为24.38(95%CI:21.53~27.32)岁。

(3)吸烟量:每日吸烟者日均吸机制卷烟量为16.31(95%CI:15.12~17.49)支,最少平均每天吸1支,最多为60支,其中以11~20支和 > 20支为主,分别占28.78%和47.72%。男性吸烟以11~20支为主,女性以1~5支为主,男女吸烟量差异有统计学意义(χ2=23.346,P < 0.001);45~64岁组每日吸烟量 > 20支明显高于其他年龄组,不同文化程度吸烟量差异无统计学意义(χ2=15.069,P=0.089)。

3. 戒烟情况:在调查所有曾经和现在吸烟者中,戒烟率为20.79%。现在吸烟者准备在1个月内戒烟意愿比例为6.82%,考虑在12个月内戒烟比例为13.45%,会戒烟但不会在12个月内为21.57%,不打算戒烟者为48.32%。

4. 对烟草危害的认知:公众对吸烟导致肺癌的认知度较高,为85.80%;对吸烟会导致其他3种疾病(中风、心肌梗死和勃起障碍)的认知度较低,分别只有47.72%、52.04%和27.76%。知晓吸烟能导致以上4种疾病的比例仅为24.08%,其中小学及以下文化程度者知晓率为16.47%,大专及以上文化程度者知晓率为39.98%;医务人员知晓率为72.69%,明显高于其他职业人群。

5. 吸烟行为的多因素分析:调整混杂因素之后,以是否现在吸烟为因变量,以性别、城乡居住、年龄、职业、文化程度、地区及吸烟危害健康得分作为自变量,运用非条件logistic回归模型进行分析。结果显示,男性现在吸烟率高于女性(OR=49.625,95%CI:37.832~65.093),45~64岁组现在吸烟率高于15~24岁组(OR=1.830,95%CI:1.048~3.194),农林牧渔从业者现在吸烟率高于医务人员(OR=0.403,95%CI:0.187~0.866)和离退休人员(OR=0.648,95%CI:0.481~0.873),中部地区居民现在吸烟率低于东部(OR=0.724,95%CI:0.606~0.865),吸烟危害健康知识得分1~3分人群现在吸烟率高于4~6分(OR=1.432,95%CI:1.240~1.654)。见表 3

讨论

山东省居民吸烟率为23.93%,低于四川省、陕西省[4-5],但高于江苏省[6]。每日吸烟者开始吸烟年龄为21.21岁,高于北京市调查的19.2岁[7];戒烟率为20.79%,高于2018年全国调查结果[3]。吸烟开始年龄的推迟,吸烟年数相应的减少,吸烟成瘾程度相对减低,有助于吸烟量的降低,提高戒烟的成功率。

男性吸烟率高于女性,与国内外调查相似[3, 8]。男性吸烟与食物、酒精消费以及社会环境的关系较为密切,当男性经常和其他人一起吸烟时,他们的吸烟率就明显上升;女性吸烟行为主要是受到环境因素和情绪影响[9]。目前,各地控烟干预措施是不分性别的,如果考虑到男女性吸烟行为的差异,那么这些措施将更为有效。

吸烟与经济水平不是简单的正相关或负相关的关系。一般来说,低收入者吸烟率高。有研究表明,高收入国家吸烟与收入的独立效应不明显或不存在,但确实发生在低收入国家和一些中等收入国家[10-11]。也有研究显示,收入与吸烟行为的关系很小,甚至不显著[12]。本研究经济水平较高的居民吸烟率高,可能烟草消费占个人可支配收入的比例较低。经济水平较低地区居民吸烟率高,原因是低收入者往往有较差的居住环境、较大的生活压力和经济困难,这些因素不仅助长了吸烟,而且也一直阻碍了戒烟[13]

随着年龄的增长,吸烟率呈现先升后降趋势。> 45岁人群吸烟率高,可能与该人群社会生活压力大有关。这个结果很重要,意味着吸烟者到中年,烟草对健康的危害开始出现。65岁以后吸烟率开始下降,可能是老年人有更多的时间接受健康信息,重视烟草对疾病的影响;部分老年人患有慢性病,能够听从医生的建议,主动降低吸烟量或戒烟有关。

体力劳动者吸烟率高于其他职业,与相关研究一致[14]。农林牧渔从业者劳动强度高,收入低,工作环境艰苦,容易产生压力[15]。该人群文化程度不高,居民健康素养水平较低,获取科学健康信息的能力较差,对吸烟危害健康知识了解有限,导致吸烟率的升高。

我们发现,健康认知是吸烟的保护因素,与Yang等[16]调查结果相同。健康教育可以促进健康行为的形成,因此,应充分运用被证实有效的健康教育策略,警告吸烟有害健康和倡导戒烟的好处,增加吸烟者对健康知识的了解,减少烟草使用,更有可能增强戒烟的意愿,增加戒烟次数和戒烟成功率。

本研究未发现文化程度与吸烟行为存在关联。一般文化程度低者吸烟率高[17],或者是文化程度越高,吸烟率越低[13]。但少数研究显示,文化程度高者吸烟率高[18]。高文化程度代表掌握更多的健康知识,更容易获得参与健康促进带来的好处。文化程度越低,吸烟者使用戒烟工具的可能性越小,更容易与其他吸烟者一起交往聚集。

总之,我们认为减少烟草的流行,应采取一些新的监管措施,如制定无烟环境的法律法规,提高烟草销售价格,健康警示图片上烟包,提升居民健康素养水平,充分发挥综合性控烟政策在降低烟草使用中的作用。
