近年来,艾滋病在我国MSM中快速蔓延[1]。HIV暴露后预防(PEP)指当HIV阴性个体在有可能经由血液、精液、直肠或阴道液体或者乳汁暴露于HIV之后(不超过72 h),及早采用为期28 d的抗病毒治疗,使得尚未感染的人群,在暴露于高感染风险后,能有效阻断HIV感染风险[2]。
WHO已将PEP推荐用于MSM、吸毒等重点人群的非职业性PEP[3],以阻断因无保护性行为、不安全注射和被性侵等暴露者感染HIV。欧洲、澳大利亚、美国等对PEP的应用进行了标准化[4-6]。美国还在2019年将暴露前预防(PrEP)、PEP和发现即治疗一起作为终结艾滋病流行的生物学干预手段[7-8]。本研究分析MSM对PEP服务的使用意愿及其相关因素,为制定MSM科学防控策略提供参考依据。
对象与方法1. 研究对象:纳入标准:①年龄≥18岁的男性;②最近6个月发生同性肛交或口交;③知情同意。
2. 研究方法:采用横断面调查设计,于2019年9-11月,通过“i卫士”微信公众号在全国范围进行调查对象的网络招募,并使用问卷星平台生成电子问卷开展调查。
3. 样本量计算:采用PASS 11.0软件计算,其中p为PEP服务的使用意愿率,根据研究设置为80%,设置α=0.05,u=1.96(双侧检验),容许误差设为2.8%,根据查阅文献,设定PEP服务的使用意愿率为4%,考虑无效问卷和应答率,实际样本量扩大30%,估算样本量为1 338例。样本量估算公式:
4. 统计学分析:使用SPSS 23.0软件进行统计学分析。对于多选题变量采用多重响应进行频率和交叉表分析。采用χ2检验对不同特征研究对象PEP服务的使用意愿进行单因素分析,结合专业判断和筛选自变量,以PEP服务的使用意愿为因变量,采用多因素logistic回归模型进行影响因素分析,计算0R值(95%CI)。双侧检验,检验水准α=0.05。
结果1. 基本情况:招募MSM共1 782人,有效应答率为85.1%(1 517/1 782)。研究对象年龄M(P25,P75)为25(20,32)岁,听说过PEP服务的研究对象占72.5%(1 100/1 517);PEP服务的使用意愿为87.9%(1 333/1 517);寻求PEP服务的研究对象中,能够完成28 d药物服用的为64.1%(234/365)。
2. PEP服务的使用意愿:单因素分析结果显示,研究对象PEP服务的使用意愿在居住地、年龄、本地居住时间、职业、每月可支配收入、最近3个月内是否有同性肛交、肛交方式、群交行为、性伴HIV感染状况、新型毒品和/或助兴剂使用、PEP服务知识知晓、HIV检测史等方面的差异均有统计学意义。见表 1。
3. PEP服务的使用意愿分析:校正居住地、年龄、本地居住时间、职业、每月可支配收入等因素后,多因素logistic回归分析结果显示,更愿意使用PEP服务的因素包括年龄 > 25岁(OR=1.807,95%CI:1.090~2.995)、曾经做过HIV检测(OR=1.953,95%CI:1.171~3.256)、至少了解1条PEP知识(OR=2.163,95%CI:1.468~3.186);不了解性伴HIV感染状况者(OR=0.602,95%CI:0.407~0.890)的PEP服务使用意愿较低。见表 1。
4. 听说过PEP服务的使用意愿分析:校正居住地、年龄、本地居住时间、职业、每月可支配收入等因素后,听说过PEP服务的研究对象中,多因素logistic回归分析结果显示,更愿意使用PEP服务的因素包括居住地为浙江省(OR=1.942,95%CI:1.097~3.438)、年龄 > 25岁(OR=2.431,95%CI:1.331~4.439)、至少了解1条PEP知识(OR=3.714,95%CI:1.532~9.007)、通过MSM社区组织/志愿者/卫生专业机构了解PEP服务(OR=1.902,95%CI:1.096~3.301)。见表 2。
讨论国内近年来开始探索PrEP和PEP对MSM的HIV感染的影响,但主要以PrEP为主[9-10],主要认为PrEP的使用意愿与行为存在较大差距,而PEP可见效果较明显[11]。听说过PEP服务的研究对象占72.5%,PEP服务的使用意愿为87.9%,与国外相关文献比较,高于美国纽约(59.2%)[12],与南非接近(71.1%)[13],但低于伦敦(88.3%)[14]。一项全球系统综述和Meta分析发现,MSM听说过PEP的占51.6%(95%CI:40.6%~62.5%)[15]。本研究的MSM对PEP知晓和使用意愿高于国外一些起步较早的国家。这可能与本研究通过“i卫士”微信公众号平台开展的艾滋病相关宣传活动有关。
本研究发现,影响MSM对PEP服务的使用意愿中,更愿意使用PEP服务的因素包括年龄 > 25岁、曾经做过HIV检测、至少了解1条PEP知识;不了解性伴HIV感染状况者的PEP服务的使用意愿较低,即缺乏PEP知识、低估的HIV暴露风险、PEP服务的可及性低、无HIV检测史者、不了解性伴HIV感染状况以及社会歧视,都会影响和阻碍其对PEP服务的使用意愿[12, 15]。了解较多PEP服务的途径,其卫生专业机构、社会公益小组/志愿者可能因为专业性和身份认同等原因,也提高了PEP服务的使用意愿[13]。其他因素如年龄 < 25岁者,多以经济尚未独立的学生为主,机关事业单位工作人员则顾虑身份保密问题,影响其对PEP服务的使用意愿。
国际上MSM对于PEP服务的知晓和利用依然存在不足,阻碍因素主要为PEP服务的知晓程度不高、药物副作用引起的依从性低、寻医用药过程复杂、不了解其性伴HIV感染状况等,导致错过PEP服务的最佳时机[16],其中,坚持28 d的PEP比例仅为67.3%的主要障碍因素是药物副作用[17]。本研究认为,未寻求或接受PEP的最重要原因是对PEP知晓程度不够、PEP费用太高(约3 000元/月)。
PEP服务虽然能够高效减少MSM的HIV感染风险,但是,对于增加MSM高危性行为仍然存在争议[18],美国有研究发现,曾经接受过PEP服务者并未增加高危性行为,但是,正在接受或未曾接受,想法相反[19]。
建议充分运用社交媒体加强PEP服务的宣传教育,降低社会歧视,提高目标人群对PEP知晓率,营造积极的社会影响氛围[13, 20-21];二是要充分发挥MSM社会组织或公益小组/志愿者积极性和卫生专业机构的PEP服务的专业培训;三是探索将PEP纳入医保,降低服务对象的经济负担[12]。
本研究存在不足。一是通过“i卫士”微信公众号开展线上招募和调查存在样本偏倚;二是横断面调查无法探索因果关联,三是HIV感染状况存在缺失数据,影响其对PEP使用意愿的数据分析。
综上所述,MSM对PEP服务的使用意愿较高,年龄、PEP知识、是否了解性伴HIV感染状况是其相关因素。MSM社区组织、志愿者及卫生专业机构是开展PEP服务宣传和推广的重要途径。
