2010年与2014年中国学生校内体育锻炼状况比较及影响因素分析

摘要: 目的 比较中国9~22岁学生2010年与2014年校内体育锻炼1 h的概率及影响因素。方法 利用2010年与2014年全国学生体质与健康调研数据,选取年龄为9~22岁的汉族学生和9~18岁的藏族学生(西藏自治区)为研究对象,将研究对象分为小学生(9~12岁)、初中生(13~15岁)、高中生(16~18岁)和大学生(19~22岁)。采用χ2检验比较不同年度校内体育锻炼1 h的概率之间的差异,采用log-binomial回归计算不同年龄组间校内体育锻炼1 h的概率的RR值。结果 2010年中国9~22岁学生校内体育锻炼1 h的概率为20.5%,2014年为23.8%。相较于2010年,2014年中国9~22岁学生校内体育锻炼1 h的概率整体有所上升(P < 0.001),且上升幅度东部>西部>中部地区(P < 0.05)。各年龄组校内体育锻炼1 h的概率均有所上升(P < 0.05),且小学生校内体育锻炼1 h的概率上升幅度高于初、高中生和大学生(P < 0.05)。除中部地区的初、高中生校内体育锻炼1 h的概率不升反降外,其余各年龄组东中西部地区学生校内体育锻炼1 h的概率均上升;东部地区的中小学生校内体育锻炼1 h的概率上升幅度最大,但大学生校内体育锻炼1 h的概率的上升幅度在3个地区中最小。各省学生校内体育锻炼1 h的概率的变化情况差异较大,小学生表现尤为明显。仅有3个省份4个年龄组校内体育锻炼1 h的概率均显著上升。以9~12岁年龄组学生的体育锻炼1 h的概率为参照,13~15岁的RR值从2010年到2014年几乎无变化。16~18岁的RR值从2010年的0.34(95%CI:0.33~0.35)增加到2014年的0.36(95%CI:0.36~0.37,P < 0.001),19~22岁的RR值从2010年的0.33(95%CI:0.32~0.34)增加到2014年的0.43(95%CI:0.42~0.44,P < 0.001)。结论 相较于2010年,2014年各年龄组学生校内体育锻炼1 h的概率均上升,但省间差异较大,这说明国家政策虽然起到了一定的作用,但各省应针对各自的情况采取有针对性的措施来提高学生体育锻炼1 h的概率。此外,高年龄组学生的体育锻炼也应当给予更多的重视。

Comparison of status of physical activity time at school and influencing factors in students in China, 2010 and 2014

Abstract: Objective To compare the probability of physical activity (PA) time ≥ 1 hour at school and influencing factors in students in China between 2010 and 2014. Methods We used the data of 2010 and 2014 Chinese National Survey on Students' Constitution and Health (CNSSCH). The surveys covered the Han students aged 9-22 years and Tibetan students aged 9-18 years (Tibet). The participants were primary school students (9-12 years old), junior high school students (13-15 years old), senior high school students (16-18 years old) and college students (19-22 years old). The probability of PA time ≥ 1 hour at school was compared using χ2 tests between 2010 and 2014 survey years. Log-binomial Regression was used to estimate the relative risk (RR) for the probability of PA time ≥ 1 hour at school in different age groups. Results The overall probability of PA time ≥ 1 hour at school in students aged 9-22 years was 20.5% in 2010, and 23.8% in 2014. The difference between 2010 and 2014 was significant (P < 0.001). On the whole, the probability of PA time ≥ 1 hour at school increased in the eastern, central and western areas in 2014 compared with 2010 (P < 0.05), and the increase range was highest in eastern area, followed by western area and central area (P < 0.05). The probability of PA time of 1 hour at school in all age groups increased in 2014 compared with 2010 (P < 0.05). The increase in the probability of PA time ≥ 1 hour at school was larger in primary school students than that in high school students and college students (P < 0.05). The probability of PA time ≥ 1 hour at school increased in all age groups in three areas, except in junior and senior high school students in the central area where the probability of PA time ≥ 1 hour at school decreased. The probability of PA time ≥ 1 hour at school in primary and junior/senior high school students in the eastern area had the greatest increase, but in college students it had the smallest increase in the three areas. From 2010 to 2014, the changes in the probability of PA time ≥ 1 hour at school in students in different provinces were quite different, especially in primary school students. The probability of PA time ≥ 1 hour at school in four age groups increased in only three provinces. With the probability of PA time ≥ 1 hour at school in primary school students as the reference, the RR in junior high students had no significant change between 2010 and 2014. The RR in senior high school increased from 0.34 (0.33-0.35) in 2010 to 0.36 (0.36-0.37) in 2014, and the RR in college students increased from 0.33 (0.32-0.34) in 2010 to 0.43 (0.42-0.44) in 2014. Conclusions The probability of PA time ≥ 1 hour at school in all age groups increased in 2014 compared with 2010, but there were great differences among provinces. The findings of our study suggests that although the national policy played a certain role, the provinces should take specific measures to improve the probability of PA time ≥ 1 hour at school in students according to their own conditions. In addition, more attention should be paid to PA of older students.

体力活动对促进儿童青少年健康成长和发展具有积极的作用[1-5]。WHO建议5~17岁的儿童青少年每天应进行至少1 h中等到高强度的体力活动[6],我国于2007年发布《中共中央国务院关于加强青少年体育增强青少年体质的意见》(7号文件),也提出“确保学生每天锻炼1 h”[7]。然而,全球约有80%的青少年(13~15岁)未达到WHO的推荐标准[8]。在大多数国家,缺乏体力活动的趋势正在迅速增加,这一现象不仅出现在高收入国家,而且越来越多地出现在低收入和中等收入国家[8-9]。1项对2005-2018年关于中国中小学生体育锻炼1 h的概率的研究的荟萃分析发现[10],中国中小学生体育锻炼1 h的概率仅为31.1%。虽然有研究表明[11-16],中国学生体育锻炼1 h的概率有随年龄逐步下降的趋势,然而,较少有研究分析和比较地区水平上各年龄组学生体育锻炼1 h的概率差异的变化。本研究利用全国学生体质与健康调研数据,比较9~22岁学生2010年与2014年校内体育锻炼1 h的概率的地理分布以及年龄差异。

资料与方法

1.资料来源:2010与2014年全国学生体质与健康调研,涉及中国31个省(直辖市、自治区),选取年龄9~22岁的汉族学生和9~18岁的藏族学生(西藏自治区)为研究对象。将研究对象分为小学生(9~12岁)、初中生(13~15岁)、高中生(16~18岁)和大学生(19~22岁)。

2.抽样方法:2010年、2014年两次调研采用同样的调查方法。对于9~18岁的汉族学生,在调查的30个省份(除西藏)内根据社会经济状况选择“好”“中”“差”3片地区[在各省份抽样时综合考虑地市的5个指标(区域生产总值、人均年总收入、人均食物消费量、人口自然增长率和区域社会福利指数),在每个省内将各市的社会经济发展水平从好到差排序并分为3段,每段内随机抽取市,分别定义为“好”“中”“差”片区]。片区内分城乡采用分层随机整群抽样的调查方法,首先确定调研点校(基本沿用1985年以来的点校),再以年级分层,以教学班为单位随机整群抽样构成调研样本。19~22岁年龄组除不分片区外,其余抽样过程同9~18岁汉族学生。9~22岁汉族学生按城、乡、男、女分为4类,每岁一组。9~18岁汉族学生每片每类每个年龄组样本含量为50人;19~22岁汉族学生每类每个年龄组样本含量为100人。9~18岁的藏族学生在西藏拉萨地区进行抽样,按男、女性分为两类,每岁一组,每类每个年龄组样本含量为100人[17]。最终收集到的样本共456 934人,其中2010年样本人数为229 158人,2014年为227 776人;汉族学生452 716人,藏族学生4 218人。

3.分析指标:①体育锻炼时间:由调研统一发放的自填式问卷收集学生体育锻炼时间信息,体育锻炼时间是指学生在校期间参加所有体育活动(包括课间操、体育课和课外体育活动)的时间总和,按照7号文件中“确保学生每天锻炼1 h”的建议,分为体育锻炼时间<1 h组和体育锻炼时间≥1 h组。②居住地区:东部地区包括北京、天津、河北、辽宁、上海、江苏、浙江、福建、山东、广东、海南11个省份,中部地区包括黑龙江、吉林、山西、安徽、江西、河南、湖北、湖南8个省,西部地区包括内蒙古、广西、重庆、四川、贵州、云南、西藏、陕西、甘肃、青海、宁夏、新疆12个省份。

4.统计学分析:利用SPSS 20.0软件进行数据整理及分析。剔除每日锻炼时间缺失的样本19 495人(原样本量的4.3%),最终纳入分析的样本量为437 439人。采用χ2检验分析率之间的差异。结合文献及预分析,考虑到指标率>10%,采用log-binomial回归模型,以9~12岁学生体育锻炼1 h为参照,对于19~22岁学生,控制省份、城乡、性别变量,对于13~15岁及16~18岁学生,控制省份、城乡、性别、省内经济水平变量,计算年龄组间校内体育锻炼1 h的概率的RR值,检验水准α=0.05。

结果

1.基本特征:研究对象共有437 439人。其中男生218 325人,女生219 114人;按地区划分,东部地区158 353人,中部地区116 997人,西部地区162 089人;按年龄组分,9~12岁131 098人,13~15岁106 920人,16~18岁105 136人,19~22岁94 285人。

2.全国校内体育锻炼1 h的概率及变化情况:2010年全国9~22岁学生校内体育锻炼1 h的概率为20.5%,2014年为23.8%,两年的差异有统计学意义(P < 0.001)。2010年学生校内体育锻炼1 h的概率前三的省份分别是吉林(37.0%)、江苏(31.4%)和北京(31.3%),校内体育锻炼1 h的概率最低3个省份是安徽(12.1%)、河南(12.4%)和山西(12.6%);2014年学生校内体育锻炼1 h的概率前三的省份分别是北京(38.4%)、河北(38.0%)和江苏(36.4%),最低的3个省份是安徽(12.9%)、河南(14.7%)和广西(15.7%)。相较于2010年,2014年大部分省份的校内体育锻炼1 h的概率有所增高,其中以河北(18.2个百分点)、甘肃(10.8个百分点)和西藏(9.7个百分点)增长最多;有8个省份校内体育锻炼1 h的概率反而下降,其中以吉林(-10.5个百分点)、云南(-5.5个百分点)、湖北(-4.3个百分点)3省下降最多。见表 1

3.各年龄组全国校内体育锻炼1 h的概率及变化情况:无论男生、女生,校内体育锻炼1 h的概率都随着年龄增长而下降,但高中生和大学生差异无统计学意义(P>0.05);相较于2010年,2014年各年龄组校内体育锻炼1 h的概率都有所上升(P < 0.05);从上升幅度看,小学生校内体育锻炼1 h的概率上升幅度高于初、高中生和大学生(P < 0.05)。见图 1

4.各年龄组东中西地区校内体育锻炼1 h的概率及变化情况:整体上,相较于2010年,2014年东中西部地区校内体育锻炼1 h的概率均上升(P < 0.05),增长幅度东部>西部>中部地区(P < 0.05)。相较于2010年,2014年中部地区的初、高中生校内体育锻炼1 h的概率不升反降,初中生下降1.4个百分点,高中生下降0.8个百分点。东部地区的中小学生校内体育锻炼1 h的概率上升幅度最大,但东部大学生校内体育锻炼1 h的概率上升幅度在3个地区中最小。见表 2

5.各省不同年龄组学生各省校内体育锻炼1 h的概率变化情况:相较于2010年,2014年各省学生校内体育锻炼1 h的概率的变化情况差异较大,小学生表现尤为明显。相比于其他年龄组,小学生校内体育锻炼1 h的概率上升幅度超过10个百分点的省份最多,达到5个,但同时校内体育锻炼1 h的概率显著下降的省份数也最多,且下降集中在西部地区省份(4个下降的省份/12个西部省份)。初中生校内体育锻炼1 h的概率显著下降的省份数有4个。高中生和大学生校内体育锻炼1 h的概率变化情况以小幅度(5个百分点以内)上升居多。4个年龄组校内体育锻炼1 h的概率均显著上升的省份仅有3个,分别是新疆、辽宁和福建。见表 3

6. 2010、2014年校内体育锻炼1 h的概率年龄组间差异的比较:以9~12岁年龄组学生校内体育锻炼1 h的概率为参照,13~15岁与其的RR值在2010年与2014年均为0.55(95%CI:0.54~0.56)。16~18岁的RR值从2010年的0.34(95%CI:0.33~0.35)增加到2014年的0.36(95%CI:0.36~0.37,P < 0.001),19~22岁的RR值从2010年的0.33(95%CI:0.32~0.34)增加到2014年的0.43(95%CI:0.42~0.44,P < 0.001),表明高中生及大学生与小学生的校内体育锻炼1 h的概率的差距在缩小。见表 4

讨论

本研究发现,相较2010年,2014年中国9~22岁学生的校内体育锻炼1 h的概率均有所升高。既往研究显示[18],2004-2011年中国中小学生的体育锻炼时间略有下降。自2007年7号文件发布后,“阳光体育运动”在全国范围内广泛开展[7],这在很大程度上促进了中小学生加强体育锻炼。根据历次全国学生体质与健康调研结果,1985-2005年中小学生身体素质呈现持续下降趋势,2010年下降趋势基本得到遏制,到2014年时呈现向好的趋势[19]。身体素质与体育锻炼时间密切相关[20],学生身体素质的加强与本研究得到的2010-2014年校内体育锻炼1 h的概率的趋势基本一致。此外,小学生校内体育锻炼1 h的概率的上升幅度高于中学生和大学生,这可能是由于高年级学生花更多的时间在学业上[21],这种情况下同样的政策可能对小学生的效果更为明显。

尽管从全国范围看2014年各年龄组校内体育锻炼1 h的概率相较于2010年均上升,但省份间差异较大。造成这种地区差异的原因很多,从政策角度来看,政策的执行程度可能是重要原因之一。比如,在2010年,河北省的学生校内体育锻炼1 h的概率在全国居于中游,但2014年已跃居全国第二。河北省积极响应7号文件精神,持续多年举办了阳光体育冬季长跑活动,并进行了监督和激励[22]。2011年,河北省发布《河北省切实保证学生每天一小时校园体育活动实施细则》,“细则”中规定了多项促进学生体育锻炼的具体措施[23]。这提示我们,提高全国学生体育锻炼1 h的概率不能光靠“一刀切”,要因地制宜,根据不同的地区情况采取相应的促进措施。

值得注意的是,在小学生校内体育锻炼1 h的概率上升幅度高于高中生和大学生的情况下,高中生和大学生校内体育锻炼1 h的概率的上升比例却高于小学生,这更加表明了高中生与大学生校内体育锻炼1 h的概率的基础水平较低。研究表明,大学生在大学期间养成的体育锻炼习惯对成年期的体育锻炼行为有长期的影响,从而对健康状况产生长远的影响[24-25],这提醒我们要加强对大学生体育锻炼行为的重视。改变大学环境,将体育锻炼融入日常活动中,成为生活方式的一部分是提高大学生体育锻炼1 h的概率的有效策略方向[26]

本研究的局限性在于只调查了学生在校体育锻炼时间,未包括课外体育锻炼时间。然而,有研究显示,中国学生在上学日的平均体育锻炼时间长于休息日[27]。了解学生在校体育锻炼时间情况从而采取针对学校的干预措施和政策是提高学生体育锻炼时间最有效的方法之一。此外,体育锻炼时间的获得是通过学生自填式问卷,可能存在报告偏倚,但是考虑到是全国性的大样本,客观测量不具有可行性。除锻炼时间外,锻炼强度也是影响学生身体素质的重要因素,限于条件,本研究未收集到学生体育锻炼强度的信息。今后的研究中应纳入体育锻炼强度维度收集信息及进行分析。
