老年HIV/AIDS衰弱综合征及其相关研究进展

摘要: 衰弱综合征是由机体退行性改变和多种慢性疾病引起的机体易损性增加的综合征,与跌倒、残疾和死亡等不良结局密切相关。随着抗病毒治疗的开展,艾滋病患者的平均生存期限逐渐延长,老年HIV/AIDS人数增加,衰弱综合征在该人群中的患病率增加。老年衰弱综合征的发生与HIV感染及抗病毒治疗药物的不良反应有关。早期识别与干预老年HIV/AIDS的衰弱综合征可以减少不良临床事件,提高其生存质量。

HIV/AIDS related frailty syndrome in the elderly and related research progress

Abstract: Frailty syndrome, caused by degenerative changes in the body and the body vulnerability due to a variety of chronic diseases, is associated with adverse outcomes, such as fall, disability and mortality. With the development of antiretroviral therapy, the average life span of HIV/AIDS patients is extended, the number of elderly living with HIV/AIDS has increased, resulting the increase of the incidence of frailty syndrome in this population. The incidence of frailty syndrome in the elderly is associated with HIV infection and adverse reaction of antiretroviral therapy. Early assessment and intervention of frailty syndrome in elderly HIV/AIDS patients can reduce adverse clinical events and improve the quality of life.

随着抗病毒治疗(ART)的广泛开展,HIV/AIDS生存时间逐渐延长,≥50岁HIV/AIDS人数逐年增多[1]。预计到2030年,全球约有73%的HIV/AIDS年龄将>50岁[2-3]。根据美国CDC定义,老年HIV/AIDS的年龄≥50岁[4]。研究表明,HIV/AIDS的老年综合征(geriatric syndrome,GS)发生率较非HIV感染者人群更高[5]。GS是老年人由于多种疾病或多种原因造成的同一种临床表现或问题的症候综合征[6]。衰弱综合征(衰弱)作为重要的老年综合征之一,其在青壮年HIV/AIDS中的患病率约为8%[7-8],而在老年HIV/AIDS的患病率可达15.4%[9]。有研究显示,HIV感染者比非HIV感染者平均提前约15年发生衰弱[10-11],这可能与HIV感染及ART药物的毒副作用有关[12-13]。衰弱与老年HIV/AIDS主要的不良临床结果密切相关,包括可使跌倒发生率和日常生活能力受损风险增加、住院率和死亡率升高等[14-15],显著影响HIV/AIDS的健康相关结局。因此,了解衰弱的发病机制,及早识别和提供相关干预措施对避免或减少老年HIV/AIDS不良临床结局的发生及提高其生存质量具有重要意义。

1.研究现状:目前对衰弱暂无统一公认的定义,但普遍认为其主要是由于机体退行性改变和多种慢性疾病引起的机体易损性增加的一种重要的老年综合征[16-17]。其核心是老年人生理储备下降或多系统异常,对健康人不足以致病的刺激亦可导致不良临床事件的发生[18]。各系统的功能失调是衰弱发生的主要途径,主要表现为免疫失调与炎症反应、神经内分泌改变、激素失调等[19]。相比于青壮年HIV/AIDS,老年HIV/AIDS的衰弱发生率更高[9]。目前对老年HIV/AIDS衰弱的研究主要集中在衰弱的发生机制及相关不良临床结局等方面。

(1)发生机制:在HIV/AIDS人群和非HIV感染者人群中,随着年龄的增长,机体出现全身免疫激活和慢性炎症,这个过程被称为“炎症老化”,该过程与衰弱综合征的发生有关[20]。然而,在老年HIV/AIDS中,除了年龄因素外,还受HIV感染与ART药物的影响。一方面,HIV可造成全身系统性的疾病,其中免疫系统为主要受影响的系统之一,免疫系统中与衰老相关的免疫标志物被提前激活[21-22]。免疫系统受长期激活作用和持续慢性炎症反应可导致免疫老化,加速机体衰老,使HIV/AIDS更早出现与年龄相关的衰弱[23-24]。Piggott等[25]研究发现,相比于其他疾病患者,HIV/AIDS体内的炎症生物标志物如正核因子kB(NFkB)、人可溶性TNF受体(sTNFRI)和白细胞介素(IL-6)与衰弱呈显著相关。在该研究的纵向分析中还发现,炎症减轻与延缓衰弱进程和衰弱程度恢复显著相关[26]。此外,ART药物可使细胞线粒体受损,机体发生炎症反应,进一步加速衰弱的发展[27]。相比之下,Collerton等[28]则未发现青壮年HIV/AIDS的衰弱与炎症反应之间存在关联。其他研究还发现衰弱与延迟开始治疗HIV、艾滋病相关神经认知功能障碍、合并感染、身体功能损害、吸烟、生活方式等因素有关[19, 29]

(2)相关临床结局:Erlandson等[14]在一项横断面研究中证明,衰弱指数作为一种评估衰弱程度的工具,每增加一个百分点,老年HIV/AIDS跌倒的风险就增加3.1%。Önen等[15]研究还发现衰弱与老年HIV/AIDS住院风险增加有关。再者,Gustafson等[30]研究显示在HIV/AIDS人群中,许多与衰弱相关的共病发生率更高,这些共病包括代谢性疾病、肝肾疾病和精神疾病。相比于青壮年HIV/AIDS,衰弱可进一步加重老年HIV/AIDS的疾病负担和医疗负担。因此,需要对衰弱的发生机制及影响因素进行更深入地研究。

2.评估工具:有效的衰弱评估是干预的前提。国际目前尚无针对老年HIV/AIDS衰弱风险的评估工具。在HIV/AIDS中,使用最多的测量工具是衰弱表型,其次是衰弱指数。近年来,也有学者开始尝试研究针对AIDS衰弱的评估工具,如衰弱相关表型(frailty-related phenotype,FRP)[31]和退伍军人老龄化队列研究风险(the Veterans Aging Cohort Study,VACS)指数[32]等。

(1)FRP:Fried等[16]在1998年提出的FRP包括不明原因体重减轻、自身疲乏、体力活动减少、行走速度下降、握力低下5个临床指标,符合1项衰弱指标则计1分,满分为5分,分值越高表明衰弱程度越重。该评估方法已被应用于多种流行病学研究,可预测死亡等不良临床结果[33-34]。在一项多中心艾滋病队列研究中[31],研究人员定义了FRP,指满足包括计划外的体重减轻、疲劳、低体力活动水平和缓慢的步行速度中的任意3项特征。研究结果表明,该表型类似于衰弱表型;与非HIV人群相比,HIV/AIDS出现衰弱症状的风险可能更高,FRP可能更早形成[31]

(2)衰弱指数:是2002年Mitnitski等[35]在“老年人累积健康缺陷”的概念上开发的衰弱测量工具,可以对衰弱度进行逐级描述。该指数以健康缺陷为衰弱变量,共有70种健康缺陷项目,包括心理、生理、既往史等多个维度,衰弱指数范围为0~1,衰弱界限为0.25,超过0.25则被认为是衰弱[36]。在此基础上,Guaraldi等[37]在意大利北部从2004-2014年构建了一个基于大量HIV临床队列研究的健康缺陷累积的衰弱指数。该指数包含了37个与HIV和行为风险因素无关的变量,结果显示该指数能有效预测死亡率和发病率。但未来还需要更多研究来验证该衰弱指数在量化老年HIV/AIDS衰弱方面的可靠性。

(3)VACS指数:在HIV/AIDS中,除了FRP和衰弱指数外,VACS指数也被提出作为评估生理衰弱程度和器官系统的累积损伤的工具。VACS指数首先是在一个退伍军人队列中开发和验证的,包括HIV特异性和一般器官损伤的评估,是预测HIV/AIDS全因死亡率的一个强有力的预测工具[38]。它包括年龄和常规收集的临床生物标志物[包括CD4+T淋巴细胞(CD4)计数、病毒载量、血红蛋白、肾小球滤过率、肝纤维化指数和丙型肝炎感染];在非HIV个体中,CD4计数被界定为>500个/μl,HIV-1 RNA被假定为无法检测[32, 39]。有研究显示,初入重症监护病房时的VACS指数评分可以预测HIV/AIDS和其他疾病患者30 d的死亡率[32]。VACS指数被证明还可以预测HIV/AIDS和其他疾病患者1年和5年的死亡率[39]。有研究显示,在男性HIV/AIDS中,VACS指数与肌肉质量、身体功能以及神经认知功能有关[40-42]。故可以考虑将其作为老年HIV/AIDS衰弱的一个评估工具进行验证。

以上3种评估工具均为预测型工具,可预测HIV/AIDS的不良临床事件的发生率和死亡率。未来研究可考虑纳入更广泛的领域,如认知领域和特定的HIV领域,HIV感染时间、抗病毒治疗药物的种类、服药时间等,以更准确评估老年HIV/AIDS的衰弱。

3.干预措施:衰弱的病因较复杂,往往需要多学科的干预与治疗。目前针对老年人衰弱的干预研究主要包括运动干预、营养干预和多模式干预[43]。由于老年HIV/AIDS群体的特殊性,有效的衰弱干预措施更加迫切与关键,但目前针对老年HIV/AIDS衰弱的干预尚处于初级阶段。

(1)运动干预:运动干预是衰弱最主要的干预措施[44-45],相关研究显示运动干预可有效改善衰弱老年人的步态能力和平衡能力,并降低跌倒风险[43]。Veeravelli等[46]对年龄(57.2±9.2)岁的小样本衰弱HIV/AIDS进行为期6周的运动干预,研究结果显示,运动干预可改善衰弱老年HIV/AIDS身体的平衡性和灵活性,对改善与HIV感染有关的衰弱可能有一定的作用。

(2)营养干预:研究显示,在青壮年HIV/AIDS中,热量和蛋白质摄入不足与衰弱和肌肉减少症有关[47]。而有效的营养干预可改善衰弱患者的营养状况,提高机体的免疫能力[48]。有研究报道HIV/AIDS的营养状况与衰弱存在相关性。Smit等[49]的研究发现衰弱的≥45岁HIV/AIDS人群有较低的蛋白质摄入趋势。说明增加蛋白质的摄入量可能与改善老年HIV/AIDS的衰弱有关。

(3)多模式干预:有研究显示,多模式干预在衰弱中能发挥更大的作用。Ng等[50]将246人分为营养干预组、认知干预组、运动干预组、联合干预组、对照组,结果显示,营养(OR=2.98)、认知(OR=2.89)、运动(OR=4.05)和联合(OR=5.00)干预组的衰弱评分和状态在12个月内均显著高于对照组,联合干预组的效果最佳。由于HIV对认知功能和衰弱有直接损害作用[51],故多因素联合干预可能对老年HIV/AIDS衰弱的预防和管理更有效,目前尚缺乏相关研究。

(4)个性化老年护理模式:除了上述干预措施外,根据“全面老年医学评估”(Comprehensive geriatric evaluation,CGA)确定的损害程度来进行个性化定制的干预措施也越来越受到关注[52]。CGA被定义为关注衰弱老年人的医疗、心理和功能状况,以此来制定综合的治疗计划并进行长期多方面、跨学科的评估[53]。Ekdahl等[54]比较了为期3年的以CGA为基础的护理和常规护理对老年人死亡率、住院天数、医疗总费用的影响。结果显示,和接受常规护理的老年人相比,受到CGA管理模式的老年人死亡率更低,住院天数更少,且CGA管理模式并没有产生较多的医疗费用。大部分CGA研究主要在普通老年人群中进行,未来老年HIV/AIDS衰弱的管理与干预可尝试借鉴CGA模式。

4.总结与展望:衰弱在老年HIV/AIDS中发生率更高,更易发生骨折、跌倒、残疾和死亡。目前针对老年HIV/AIDS衰弱的研究主要集中在衰弱的发生机制、评估及相关临床结局等方面。对如何有效改善老年HIV/AIDS的衰弱还有待进一步深入研究。早期识别与干预老年HIV/AIDS的衰弱对提高临床治疗/护理效果、提高其生活质量及节约医疗资源具有重要意义。未来研究可通过编制评估老年HIV/AIDS衰弱状况的临床评估工具,进而设计更具针对性的干预措施,以期能维持或改善其身体功能和生存质量,降低衰弱在老年HIV/AIDS中的发生率。
