摘要: 目的 分析中国成年人不同饮酒行为与肥胖的关系。方法 本研究利用中国慢性病前瞻性研究基线调查数据,根据BMI和腰围(WC)分别划分一般性肥胖和中心性肥胖,采用logistic回归分析饮酒行为与一般性肥胖和中心性肥胖间的关系。结果 本研究纳入249 873名调查对象,男性饮酒量与BMI和WC间大体呈“J”形的曲线关系。以不饮酒者作为对照,少量饮酒组患一般性肥胖和中心性肥胖的比例较低,男性的OR值分别为0.65(0.59~0.71)和0.93(0.88~0.98),女性的OR值分别为0.77(0.65~0.91)和0.89(0.80~0.99);男性大量饮酒组患一般性肥胖(OR=1.21,95% CI:1.12~1.32)和中心性肥胖(OR=1.33,95% CI:1.27~1.40)的比例最高。饮酒频率为3~5 d/周组的BMI和WC值较高,患中心性肥胖的比例也最高(男性:OR=1.23,95% CI:1.16~1.31;女性:OR=1.13,95% CI:0.99~1.28)。20岁前开始每周饮酒的男性患中心性肥胖的比例是不饮酒者的1.24倍(95% CI:1.16~1.33)。饮啤酒者患一般性肥胖的比例较低(男性:OR=0.74,95% CI:0.67~0.82;女性:OR=0.54,95% CI:0.43~0.68)。结论 少量饮酒患肥胖的比例较低,大量饮酒者患肥胖的比例较高,开始饮酒的年龄越早肥胖的比例越高。

Abstract: Objective To evaluate the correlation between alcohol consumption and obesity in adults in China. Methods The information about alcohol consumption were collected at the baseline survey of the China Kadoorie Biobank.The general obesity and central obesity were defined by BMI and waist circumference (WC) respectively.Logistic regression model was employed to examine the relationship of drinking behavior with general obesity and central obesity. Results A total of 249 873 adults were included.A J-shaped relationship was observed between alcohol consumption and obesity measurement index (BMI and WC) in men.Compared with non-drinkers, the proportion of general obesity and central obesity were lower in light drinkers (men:OR=0.65, 95%CI:0.59-0.71 and OR=0.93, 95%CI:0.88-0.98; women:OR=0.77, 95%CI:0.65-0.91 and OR=0.89, 95%CI:0.80-0.99).In men, the proportion of general obesity and central obesity was highest in heavy drinkers (OR=1.21, 95%CI:1.12-1.32; OR=1.33, 95%CI:1.27-1.40).BMI and WC were higher in those with a drinking frequency of 3-5 d/week, with largest of proportion of central obesity (men:OR=1.23, 95%CI:1.16-1.31; women:OR=1.13, 95%CI:0.99-1.28).The risk for central obesity in men who began drinking every week before 20 years old was 1.24 times higher than non-drinkers (95%CI:1.16-1.33).Those who drank beer had lower proportion of general obesity (men:OR=0.74, 95%CI:0.67-0.82; women:OR=0.54, 95%CI:0.43-0.68). Conclusion The proportion of obesity was lower in light drinkers but higher in heavy drinkers; and the earlier drinking started, the higher the risk for obesity was.

饮酒是2016年全球全死因和伤残调整寿命年的第七位危险因素,导致15~49岁人群中,3.8%的女性和12.2%的男性死亡[1]。有研究认为饮酒是肥胖的危险因素[2],但也有研究提出不同的观点[3],在10万名英国人和苏格兰人的横断面研究中发现饮酒频率与肥胖间存在“钟”形的关系曲线,经常饮酒的人与不饮酒的人间不存在差异[4]。目前,饮酒与肥胖间关系还没有形成一致的结论,且国内鲜有详细的研究报道。本研究采用中国慢性病前瞻性研究(CKB)基线调查数据进行研究分析,以探讨中国成年人饮酒行为与肥胖间的关系,为指导人们控制体重提供科学依据。

对象与方法

1.研究对象:CKB项目于2004-2008年在全国包括5个城市地区(黑龙江省哈尔滨市南岗区、山东省青岛市李沧区、江苏省苏州市吴中区、广西壮族自治区柳州市、海南省海口市美兰区)和5个农村地区(河南省辉县市、甘肃省天水市麦积区、四川省彭州市、浙江省桐乡市、湖南省浏阳市)招募年龄在30~79岁的调查对象,并完成基线调查512 715人[5-7]。本研究纳入基线调查中从不饮酒者和每周饮酒者311 245人,并依次剔除自报患有糖尿病、癌症、中风/小卒中、消化道溃疡、肾脏疾病、肺结核、慢性肝炎/肝硬化等疾病37 189人,BMI值异常(BMI<15 kg/m2和BMI>50 kg/m2)251人,近几年内饮酒量明显变化者23 932人,最终纳入249 873名调查对象进行数据分析。

2.研究内容:包括一般人口学信息(地区、年龄、性别、文化程度、家庭经济收入、婚姻状况)、吸烟状况、体力活动水平及饮食状况;饮酒状况由调查员面对面问卷调查获得,本研究中从不饮酒者是指过去一年从不饮酒,饮酒者是指每周饮酒≥1次者。对饮酒者进一步收集以下内容:

(1)饮酒品种:通常情况下包括:啤酒、黄酒/米酒、葡萄酒/果酒、高度白酒(≥40°)、低度白酒(<40°)。

(2) 饮酒频率:1~、3~、6~7 d/周。

(3) 饮酒量:根据自报的5类饮酒品种和每天饮酒量估计个体每天摄入的纯酒精量(g)[8]。根据《中国居民膳食指南(2016)》将饮酒量分为:①少量饮酒组:男性<25 g/d,女性<15 g/d;②中等量饮酒组:男性25~50 g/d,女性15~30 g/d;③大量饮酒组:男性≥50 g/d,女性≥30 g/d。

(4) 开始饮酒的年龄:<20、20~、30~、≥40岁。

肥胖测量指标:由经过统一培训的调查员使用身高仪测量身高,TANITA TBF-300GS体质构成分析仪测量体重,软皮尺测量腰围(WC)。测量WC时,皮尺置于髂前上棘和第12肋下缘连线的中点水平环绕腹部。上述指标测量读数分别以cm和kg为单位,具体数值精确到0.1。BMI由体重(kg)除以身高(m)的平方获得。肥胖的判定标准参考2006年《中国成人超重和肥胖症预防控制指南》[9]:根据BMI(kg/m2)分为4组:低体重(<18.5)、正常体重(18.5~)、超重(24.0~)、肥胖(≥28.0);根据WC(cm)分类中心性肥胖:男性WC≥85.0 cm、女性WC≥80.0 cm。

3.统计学分析:计量资料采用x±s表示,组间差异使用方差分析;计数资料使用频数(%)表示,组间差异使用χ2检验。本研究采用协方差分析,描述饮酒量、饮酒频率、饮酒种类和开始每周饮酒的年龄与肥胖测量指标(BMI和WC)间的关系;分别采用单因素和多因素logistic回归,分析饮酒行为与中心性肥胖和一般性肥胖间的关系。模型均调整地区、年龄、文化程度、家庭年收入、婚姻状况、吸烟状况、体力活动和摄入水果、肉类、蔬菜的频率。线性趋势检验时,将饮酒量、饮酒频率与开始每周饮酒的年龄按连续性变量纳入模型进行检验。性别间交互采用似然比检验,比较有饮酒行为与性别交互项模型和无交互项模型的差异。数据分析使用Stata 14.0软件,双侧检验,以P<0.05为差异有统计学意义。

结果

1.人口学特征:本研究共纳入249 873名调查对象,平均年龄52.2岁,其中男性74 655人,占29.9%。每周饮酒者占总研究人群的17.5%,其中,大量饮酒者占总人群的6.8%。少量饮酒组的城市所占比例、大学及以上文化程度、已婚、每天摄入水果的比例都最高。通常情况下,饮酒频率为6~7 d/周者的比例最高,饮酒种类主要以高度白酒、啤酒和低度白酒为主,开始每周饮酒的年龄在20~岁组的比例最高。见表 1

2.每周饮酒行为与BMI和WC间的关系:男性中少量饮酒组的BMI和WC最低,饮酒量与BMI和WC间大体呈“J”形关系,见图 1;女性中从不饮酒者的BMI值最高(23.65 kg/m2),大量饮酒组的WC值最高(79.12 cm)。与其他饮酒频率组比较,男性饮酒频率为6~7 d/周者的BMI和WC值较低;女性饮酒频率为1~2 d/周者的BMI和WC值最低。BMI值在啤酒组最低,男性和女性分别为22.94 kg/m2和22.84 kg/m2;男性BMI值在葡萄酒/果酒组最高,为23.31 kg/m2,WC值在高度白酒组最高,为81.99 cm,女性BMI值在黄酒/米酒组最高,为23.70 kg/m2,WC值在低度白酒组最高,为79.22 cm。男性开始每周饮酒的年龄与BMI和WC均值呈负相关,<20岁时最高,分别为23.35 kg/m2和82.17 cm。见表 2

3.饮酒行为与一般性肥胖间的关系:正常体重组作为对照组(133 165人),一般性肥胖作为病例组(24 893人),调整混杂因素后,少量饮酒与中等量饮酒者患一般性肥胖比例较低,男性大量饮酒时患一般性肥胖的比例较高(OR=1.21,95%CI:1.12~1.32);饮酒频率为6~7 d/周者患一般性肥胖的比例较低(男性OR=0.79,95%CI:0.73~0.85;女性OR=0.72,95%CI:0.61~0.85)。饮啤酒者患一般性肥胖的比例较低(男性OR=0.74,95%CI:0.67~0.82;女性OR=0.54,95%CI:0.43~0.68)。男性开始每周饮酒的年龄≥30岁时患一般性肥胖的比例较低,女性≥20岁时患一般性肥胖的比例较低。见表 3

4.饮酒行为与中心性肥胖间的关系:根据WC值将调查对象分为中心性肥胖组(101 690人)和非中心性肥胖组(148 183人),调整混杂因素后,少量饮酒组患中心性肥胖的比例较低;男性中等量饮酒(OR=1.12,95%CI:1.06~1.17)和大量饮酒(OR=1.33,95%CI:1.27~1.40)患中心性肥胖的比例较高。男性饮酒频率为6~7 d/周者患中心性肥胖的比例最低(OR=1.08,95%CI:1.04~1.13)。男性中,饮葡萄酒/果酒、高度和低度白酒者患中心性肥胖的比例均较高,女性饮啤酒和葡萄酒/果酒者患中心性肥胖的比例较低。男性开始每周饮酒的年龄<20岁时患中心性肥胖的比例最高(OR=1.24,95%CI:1.16~1.33)。

讨论

本研究发现少量饮酒组患一般性肥胖和中心性肥胖的比例较低,男性大量饮酒组肥胖的比例最高。任何频率的饮酒患中心性肥胖的比例均较高,饮酒频率为6~7 d/周者患一般性肥胖的比例较低。男性饮葡萄酒/果酒和白酒者患中心性肥胖的比例较高。男性中开始每周饮酒的年龄越早肥胖的比例越高;女性20岁后开始饮酒者患一般性肥胖的比例较低。

目前,饮酒量与肥胖间关系的结论不一致。澳大利亚的一项前瞻性队列研究在对1 621名研究对象随访15年后认为饮酒与肥胖间不相关[10],但本研究发现男性饮酒量与BMI和WC间呈“J”形关系,且无论是男性还是女性,少量饮酒组患一般性肥胖和中心性肥胖的比例低,与日本成年人和部分女性人群的研究结果一致[3, 11-13]。对于大量饮酒者,男性患一般性肥胖和中心性肥胖的比例均较高,与过往研究结果一致[14-18],可能因为酒精会增加体内脂肪的积聚[19]、改变肝脏类固醇代谢,从而导致中心性肥胖的发生。之前有研究认为女性大量饮酒同样会增加肥胖的风险[14],但本研究并没有发现女性大量饮酒与肥胖间存在相关。

男性饮酒频率与肥胖的测量指标间大体呈“钟”形的关系;每天饮酒者患一般性肥胖和中心性肥胖的比例较低,与之前英国进行的研究一致[4]。这可能是由于酒精主要通过乙醇脱氢酶通路、微粒体乙醇氧化系统和过氧化氢酶氧化3种途径进行代谢,高频率的饮酒可能激发微粒体乙醇氧化系统增加能量的消耗[20],从而可能出现高频率饮酒肥胖风险较低的结果。不同于之前英国报道的女性从不饮酒者肥胖的风险高于5~7 d/周者[4],本研究认为女性1~2 d/周和6~7 d/周患一般性肥胖的比例较低,未发现饮酒频率与中心性肥胖间存在相关。

饮啤酒者患一般性肥胖的比例较低,可能是由于饮啤酒者来源于食物的能量摄入量较低[21]。男性饮葡萄酒/果酒和高度白酒者患中心性肥胖的比例较高,而女性中并没有发现饮酒种类与中心性肥胖间存在相关。男性开始每周饮酒的年龄越早,出现肥胖的比例越高,提示饮酒可能是青少年体重增加和转向超重/肥胖的重要潜在风险因素,与过往研究结果一致[22-25]

目前国内研究饮酒与肥胖的相关报道非常有限,本研究较以往的研究样本量大,体格测量指标均由调查员亲自测量获得,避免自报偏倚。但本研究为横断面研究,不能验证饮酒与肥胖间的因果关联,还需要做进一步的前瞻性研究加以论证。通过分析饮酒行为与肥胖的关系,对今后指导人们健康合理饮酒,制定科学合理的饮酒规则,预防肥胖发生,以及减少饮酒所造成的健康损害具有重要的公共卫生学意义。
