参与式工效学在预防工作相关肌肉骨骼疾患中的应用

摘要: 工作相关肌肉骨骼疾患是一种慢性非致死性疾病,是职业人群常见的健康问题,尤其在劳动密集型行业。在国外,参与式工效学已广泛应用于工作场所中肌肉骨骼疾患的预防。本文依据参与式工效学的组织、实施及其影响因素和特点,介绍参与式工效学在预防肌肉骨骼疾患中的应用。参与式工效学方式可帮助工人识别作业过程中的危险因素并制定相应的干预方案。企业可通过开展参与式工效学项目改善工作条件、减少肌肉骨骼疾患相关危险因素的暴露及预防相关疾患的发生,以促进职工健康。

Functions of participatory ergonomics programs in reducing work-related musculoskeletal disorders

Abstract: Work-related musculoskeletal disorders (MSDs) are most commonly seen in all the occupational non-fatal injuries and illnesses for workers, especially those who are involved in labor-intensive industries. Participatory ergonomics is frequently used to prevent musculoskeletal disorders. This paper gives an overview of a historical perspective on the use of participatory ergonomics approach in reducing the health effects of labor-intensive industries. Progress, barriers and facilitators on the organization, implementation and evaluation of participatory ergonomics programs are studied. Participatory ergonomics seems a successful method to develop, prioritize measures to prevent MSDs. Participatory ergonomics can help industries reduce musculoskeletal injuries and disorders, improve workplace condition and promote health conditions of the workers.

工作相关肌肉骨骼疾患(work-related musculoskeletal disorders,WMSD)是指从事具有重复性动作、较重的静态/动态负荷或强制性体位等特征的职业活动所引起的一系列肌肉骨骼疾患,如腰背痛、肌腱综合征、手臂振动病和腕管综合征等,其特征是疼痛和活动受限[1-2]。WMSD在发达国家已成为职业人群最主要的健康问题之一,并列入职业病范畴。目前WMSD相关工效学干预主要为工程干预、管理干预和个体行为干预三类[3],其中工效学干预措施有助于改善工作条件、减少WMSD的发生,但工作场所中发生的WMSD其工效学干预存在一些问题,如工人较难接受新的工作方式等[4]。参与式工效学(participatory ergonomics,PE)是近年发展起来的工作场所工效学干预模式,但在我国尚无应用,为此笔者复习PE的概念、组织与实施及其影响因素和特点。

1. PE概况: PE对预防WMSD的发生具有很好效果。可通过招募多学科成员以及其他利益相关者(如工人、管理人员及工效学专家等),培训工效学知识,识别并分析工作场所和作业过程中出现的不良结局及其危险因素,提出解决方案及其实施过程[5]。PE可以让工人最大程度地参与识别危险因素、提出改进措施、改善工作条件,以更好控制WMSD的发生,所以PE通常被用于工作场所中WMSD的干预[6]。帮助工人识别工作场所或劳动中WMSD的危险因素,比如不良姿势、重复性劳作、外部负荷等,并采取措施减少其发生[7]。大量研究表明PE可减少肌肉骨骼系统疼痛及伤害、减少因病误工、降低因WMSD产生的医疗费用、提高生活质量及生产效率等[8-10]。目前北欧[11]、北美[12]及日本[13]、澳大利亚[14]等国家和地区已应用PE对WMSD进行干预,并广泛应用于纺织业[15]、木工业、零部件制造业[16-17]、建筑业[18]、印刷业、肉制品加工[19]及医护人员[9, 11]、环卫工人、农民[20]、军人、门卫、图书管理员[8]、出租车司机[21]等职业人群的WMSD预防。

PE概念是一个涵盖多种想法和实践的术语[22]。Wilson和Haines把PE定义为职工参与计划并控制大量的职业活动,利用足够的知识和工作经验影响劳动过程及健康结局,并达到预期目标的一个过程。Kuorinka[23]认为由关键人员参与的PE干预是实用的解决问题的方式之一,并由劳动者、管理人员、人机工效专家、健康与安全管理人员以及研究人员组成的工作小组实施,通过在作业场所中建立人机工效学方法,对作业场所进行改进[24],小组成员齐心协力改善工作条件,最终对企业职工的健康产生积极的影响[13, 25]。美国和欧洲国家对PE的应用不同,在美国[26],PE较倾于微观水平的工效学干预,如防护工具的开发及实施;而在欧洲[27],PE多用于宏观水平的综合干预,包括所有利益相关者的参与。

2. PE的组织与实施:

(1)构建团队:通常由人机工效学专家、工人及企业管理者参与。工效学专家提供技术和专业指导;工人具有丰富的工作经验,不仅在识别危险因素的过程中发挥着重要作用,也可提出相应的解决方案;企业管理者可以提供政策及管理上的支持,并对干预的实施具有积极推动作用。

(2) 实施流程:包括5个步骤(表 1)。

3. PE的方法:

(1)识别危险因素阶段:此阶段主要采用问卷调查、现场观察和直接测量的方法进行流行病学调查,收集基本信息以及识别作业场所中可改善肌肉骨骼疾患问题及分析其危险因素。

问卷调查法主要有面对面问卷调查、自评的问卷调查、电话调查问卷和网络调查问卷。调查问卷法主要用于调查PE干预前后WMSD的发现及其危险因素以及对PE过程的评价。目前有北欧国家肌肉骨骼系统疾患标准调查表、荷兰肌肉骨骼疾患调查问卷、美国职业安全卫生研究所(NIOSH)症状调查问卷、OSHA200日志和OSHA300日志等,其中前两者问卷的信度和效度已得到验证,并由杨磊等译为中文版,已在我国使用[32]

现场观察法是通过PE团队对劳动现场的观察,收集劳动者工效学危险因素的信息。目前的方法有工效学快速接触检查表(QEC)、OWAS、Checklist、RULA、REBA等,其中由美国Surrey大学工效学研究中心开发的工效学快速接触检查表评价身体背部、肩部/手臂、手腕和颈部在姿势负荷、力量负荷、移动频率、持续时间、振动等方面的暴露状况,同时还综合考虑了工人对工作负荷的反应及工人心理等因素。该方法已被多个国家研究人员使用[33-34]

直接测量法主要通过测量生物力学或者肌电图的方式,测量工作状态下工人的肌肉负荷及肌肉疲劳情况。可在直接测量肌电信号的同时进行计算机处理,并通过信号频谱特征变化评价局部肌肉的疲劳[35];另一种方式是劳动者佩带测量仪,应用电磁光谱或者电子扫描技术分析其作业姿势和角度[36]

(2)制定方案及实施计划阶段:此时PE主要采用专题小组讨论的方式,又称焦点团体访谈(focus group interview)。即通过召集一组同类研究对象组成专题讨论小组,由主持人引导小组成员对研究问题广泛、深入、自由地交换意见和观点,最后得出讨论结果,为一种定性研究方法。工作小组在对作业场所进行现场观察、问卷调查以及直接测量的基础上,对可能引起WMSD的危险因素进行讨论分析,提出可行的工效学干预措施,最终制定出实施方案和计划。专题小组讨论法贯穿PE研究的始终。

(3)过程评价:常用于评价PE的实施过程,并帮助解释研究结果,也可揭示干预措施是否按照研究意图实施(如研究对象的参与度、依从性、满意度及获得的经验等),并评价干预项目实施是否成功,最终通过过程评价收集信息指导干预的实施[37]

目前主要通过定性和定量方法评价PE的实施过程[38]。如Gupta等[16]采用IGLO(individual,group,leader,organization)四水平定性-定量过程评价框架,分别从个人水平、PE小组水平、领导层以及组织水平评价PE过程;Visser等[39]通过对研究对象的可及性、信息传递量、信息接收量、精确度、完成度、满意度及劳动者行为的改变评价其实施过程;Dale等[40]通过研究者与劳动者一对一交流,从交流的频率、劳动者态度以及改变行为的意愿评价实施干预的过程;Jaegers等[41]利用逻辑模型评价PE项目实施过程中志愿者的依存性、参与度、实施频率、持续时间等;Driessen等[42]通过Linnan-Stecler框架[43]对参与度、可及性、依从性、满意度、信息传递量及信息接收量等评价PE的实施过程;Guimar?es等[44]利用宏观工效分析法(macroscopic analysis of ergonomics)验证工人对参与式工效学的接受程度,还通过宏观工效分析和设计(macro ergonomics analysis and design)方法把工人的需求同专家的观察相结合;此外还有研究者通过研究对象的参与度评价PE实施过程[45];Matthews等[46]通过PE项目职工态度调查问卷(Employee Perceptions of Participatory Ergonomics Questionnaire,EPPEQ)评价员工对项目实施有效性的看法。

4. PE实施的影响因素:①管理人员、监察人员及工人对PE的支持;②人机工效学知识、技能以及培训;③包括时间、物质及人力成本等可利用的资源;④PE团队,包括专家委员会和行为干预促进团队成立情况;⑤项目实施过程中沟通情况(非工作中的人际关系);⑥团队成员的组织管理能力;⑦制定计划及完成的能力;⑧PE团队成员,包括工效学专家、管理者、推动者等;⑨合作能力;⑩企业对生产效率的要求可能影响PE实施的过程;人员调整、对PE了解程度、参与者的工作性质等。其中最主要的影响因素是对PE的支持、人机工效学的知识及可利用的资源[47-48]

5. PE的特点:劳动者对自身行业的劳动条件、方式、特点的认识强于其他行业的人员,因此在实际工作中劳动者的工作经验可有助于提出综合解决问题的方式,由他们设计的WMSD防护/干预方法更易被其他人员接受[49],故相比被动的干预,由劳动者直接参与的干预方式更容易达到理想的效果。

PE主要特点:①把劳动者、管理者、工效学专家和研究者组织成具有综合能力和经验的集体,对WMSD危险因素进行有效估计及干预[15];②能帮助劳动者建立干预方式,并适应不同国家、地区及不同行业劳动者价值观和文化的需要[19];③可缩小研究者与劳动者的干预需求之间的鸿沟,以提高干预措施的接受度和实行率[50];④可更好地适应作业场所、任务、条件及环境的需要[47];⑤可激励劳动者自发参与到识别WMSD危险因素、提出解决方法并评估干预措施的过程中,最终解决实际问题[51];⑥PE的研究人员与参与者之间的关系为平等互助,并有助于产生新的想法、改变个体行为习惯以及接受长时间的干预[52];⑦PE的最终目标是把干预过程的主导权及控制权传递给劳动者,提高干预的效果[53];⑧PE项目的进行需要足够的时间和管理支持,以及良好的沟通和资源[7],否则会影响干预的效果。

6. 实例分析:在荷兰,最主要的职业性肌肉骨骼疾患是下背痛(LBP)和颈部疼痛(NP),对个人、企业以及社会造成了很大的影响。对此,荷兰学者开展PE干预研究以评价其预防效果[37]

该研究为随机对照试验,研究对象来自4个企业36个部门共5 759名工人,采用分层抽样方法在各部门抽取150人,并将36个部门分层随机分配到干预组和对照组。两组工人首先观看肌肉骨骼疾患干预的教学视频,了解肌肉骨骼疾患的基本知识及其干预措施,该视频已有研究证明其对预防LBP和NP并无效果,仅作为一种假干预(sham intervention)[54]。分别测量基线及随访6个月后的心理和工效学危险因素,心理因素通过工作满意问卷(Job Content Questionnaire)测量,工效学危险因素通过荷兰肌肉骨骼调查问卷(Dutch Musculoskeletal Questionnaire)测量。

首先构建PE团队,由10人组成,包括8名工人和管理者及职业安全与健康协调员各1人;干预组的18个部门,建立了16个工作小组。

第一步收集工作场所信息(拍摄工组场所中可能引起LBP和NP因素的照片或录像,填写调查问卷收集LBP和NP危险因素信息);第二步通过焦点小组讨论分析LBP和NP危险因素;第三步采用头脑风暴法针对LBP和NP危险因素提出可采用的工效学干预措施;第四步根据成本、相对优势、兼容性、关注度、可行性制定干预措施及实施计划;第五步按照实施工效学干预措施及时对其评价和修改;第六步通过4种方式进行过程评价:①对工作小组成员进行访谈,询问对实施工效干预的期望及LBP和NP的效果;②对干预组和对照组进行访谈,询问是否知道第五步中的工效学干预措施以及是否实施工效学干预措施,如果实施了干预措施,询问其预防LBP和NP的有效性如何;③分别调查工作小组成员和工人的满意度(1~10 分量表)和工效学干预措施实施情况(问卷调查)。

研究发现PE干预能提高工人心理因素中的决定自主权和决策权,但可能由于随访时间较短,在减少工效学危险因素方面效果较差。

7. 结论:WMSD与作业过程中的诸多因素有关,需要通过多学科参与改善劳动者的作业条件,而PE干预方式具有多学科知识综合的性质,不仅能降低WMSD的发生,还可以提高生产效率,降低生产成本[15]。与非PE相比,由企业管理人员及人体工效学专家共同发现解决问题,实施参与式工效干预,是一种人性化、低成本、高效的干预方式[30],并可有效地降低劳动者工效学危险因素的暴露[55],而且也为企业工作场所健康促进提供策略[56],改善工效系统,增进员工参与意识,加强安全管理,使企业既符合职业健康安全法规有关要求又提高生产效率[30]
