摘要: 目的 分析遗传和环境因素分别对成年男性吸烟、戒烟和开始吸烟年龄<20岁3个吸烟行为相关维度的影响。方法 利用电子问卷面对面调查法获得中国双生子登记系统中9省/市的6458对25岁及以上成年男性双生子的吸烟行为相关信息,通过拟合结构方程模型,计算遗传和环境对吸烟相关行为的影响。结果 成年男性双生子吸烟、戒烟以及开始吸烟年龄<20岁3个维度的最佳模型均为ACE模型,其遗传度分别为0.26(0.19~0.34)、0.27(0.19~0.37)和0.05(0.00~0.14);调整地区和年龄因素后,3个维度的遗传度分别为0.26(0.19~0.34)、0.31(0.00~0.74)和0.05(0.00~0.14)。结论 吸烟相关行为的3个维度均受到遗传因素的影响,但环境因素对这类行为的影响更大,其中戒烟行为的遗传度较高,受到环境影响较小;开始吸烟年龄则受环境影响最大。

Abstract: Objective To analyze the influences of genetic and environmental factors on smoking behavior, smoking cessation and onset age of smoking less than 20 years in male twin adults. Methods A face-to-face questionnaire was conducted to collect data from 6 458 pair male twins aged ≥25 years registered in 9 provinces (municipality) in China. The heritability of three smoking related behaviors were calculated by using structural equation models. Results The ACE models were the best models of the three dimensions of smoking, i.e. smoking behavior, smoking cessation and onset age of smoking less than 20 years for male twins, and the corresponding heritability of these behaviors were 0.26 (0.19-0.34), 0.27 (0.19-0.37) and 0.05 (0.00-0.14), respectively. When adjusted for area and age, the heritability of these three behaviors were 0.26 (0.19-0.34), 0.31 (0.00-0.74) and 0.05 (0.00-0.14), respectively. Conclusions All the three smoking related behaviors were affected by genetic factors, but environment factors had more effect on them. For smoking cessation, the heritability was highest, but the influence of environmental factors was lowest. Meanwhile, for onset age of smoking, the influence of environmental factors was highest.

吸烟是慢性非传染性疾病的危险因素之一。据WHO调查结果显示,2008-2010年全球吸烟率为30%。2010年中国男性现在吸烟率为53%,远高于世界水平[1]。已有研究表明,吸烟行为同时受到遗传和环境因素的作用,吸烟行为的遗传度在11%~78%之间[2, 3, 4, 5]。国内外研究发现多巴胺奖赏系统(rewarding system)、血清素通路及代谢酶类等的相关基因能影响吸烟行为[6];而居住的地理位置、社会经济地位等环境因素也能对吸烟相关行为产生一定影响[7, 8, 9, 10]。吸烟行为按是否吸烟、是否戒烟和开始吸烟年龄3个维度描述。

中国双生子登记系统(Chinese National Twin Registry,CNTR)自2001年起在我国的南方和北方、农村和城市募集双生子,是中国最早的以人群为基础的双生子登记系统[11],2011-2013年CNTR在北京市、天津市、上海市、四川省、浙江省、江苏省、山东省、黑龙江省及青海省9个项目点募集了3万多对双生子,对其进行基本登记、随访登记和深入登记调查[12]。本研究利用CNTR提供的信息,采用双生子研究设计计算遗传度,定量估计吸烟行为3个维度受遗传和环境因素的影响。

对象与方法

1. 研究对象:CNTR中2011-2013年深入登记的≥25岁男男双生子对作为研究对象,共6 555对,剔除卵型信息和吸烟情况缺失的双生子97对,最终共纳入6 458对。

2. 研究方法:

(1)指标测定:CNTR问卷中针对吸烟情况询问双生子“您是否吸烟?”。选项包括否、是和已戒烟。对于后两者则进一步询问其开始吸烟年龄。CNTR中的吸烟定义:平均每日吸纸烟≥1支(或采用其他吸烟方式消耗相当于1支纸烟的烟草量),并且已经连续吸≥1年;戒烟定义:停止吸烟半年及以上。本研究在CNTR问卷调查获得信息的基础上从3个维度对成年男性双生子的吸烟行为进行描述:吸烟:在CNTR调查中回答“是”和“已戒烟”者为吸烟者(ever smoker),回答“否”者为不吸烟者(never smoker);戒烟:在CNTR调查中回答“已戒烟”者为戒烟者(smoking cessation),回答“是”者为非戒烟者(non-smoking cessation);开始吸烟年龄:在吸烟者中,参照我国人群开始吸烟年龄的现况[13, 14],将吸烟者分为开始吸烟年龄<20岁和≥20岁两组。

(2)卵型判别:CNTR通过问卷结合建模的形式对双生子的卵型进行判断。性别不同的双生子为异卵双生子(dizygotic twins,DZ)。对于性别相同的双生子,则根据年龄、性别以及问题“你们这对双生子长得像不像”建模。利用建模的方法对CNTR收集对象中的191对性别相同的双生子进行了卵型鉴别,通过与基因检测金标准相比,一致率为87.0%[12]

CNTR中采用的问题:你们这对双生子长的像不像(请回答目前或近期的状况)? ①我们两个长的非常像,很容易被外人混淆;②我们两个长得与普通的兄弟姐妹差不多,外人通常可以区分;③不好说。

3. 统计学方法:首先对研究对象的吸烟相关情况、年龄、卵型、地区分布进行描述。对于分类变量采用频数和构成比等统计量进行描述,对于连续变量采用均值±标准差和M等统计量进行描述。并根据 χ2检验和均值t检验确定组间差异是否具有统计学意义。采用阈值模型(liability model)的结构方程模型(structural equation model,SEM)分别计算吸烟行为、戒烟行为及开始吸烟年龄<20岁的遗传度。该模型假设一个二分结局变量的现有分布是源于一个潜在的存在阈值的正态分布。因此,将该二分变量转换成一个均值及标准差均和原来相等且符合正态分布的连续变量,并在横坐标上找到一个阈值使其右侧的曲线下面积等于原二分变量的阳性概率,再按照连续变量的方法进行SEM拟合[15, 16]。SEM将双生子的表型总变异分解为加性遗传效应(A)、非加性遗传效应(D)、共同环境效应(C)和特殊境效应(E)。同卵双生子(monozygotic,MZ)两成员之间A、D的相关系数(r)均为1,DZ两成员A、D的r分别为1/2、1/4。MZ与DZ两成员Cr均为1,Er均为0。本研究未区分共同抚养和分开抚养的双生子,无法区分D和C,故MZ之间表型相关系数rMZrDZ的两倍时,拟合ADE模型;反之,则拟合ACE模型。

考虑吸烟行为存在地区和年龄差异,因此调整这两者进行ACE及其嵌套模型的拟合。在模型中将年龄作为连续型变量调整,项目地区作为二分类变量调整。依据秦岭-淮河一线将项目地区划分成南、北方两组,其中南方组包括上海市、四川省、浙江省、江苏省,北方组包括北京市、天津市、山东省、黑龙江省及青海省。

采用最大似然估计法,根据-2倍对数似然函数值的(-2 log likelihood,-2LL) χ2检验(α=0.05)确定参数对模型拟合是否有贡献,但因E包含残差,所以不能从模型中剔除。以AIC(Akaike’s information criterion)评价模型的拟合优劣程度,选择AIC值最小的模型为最优模型。遗传度(heritability)是指A占总表型方差的比例,用于表示群体中表型差异归因于遗传因素作用的大小[17]。一般认为,遗传度>70%为高度遗传,遗传度<30%为低度遗传。

采用SPSS 19.0软件进行统计学描述和检验,检验水准α=0.05,采用R3.1.0和OpenMx软件包1.32进行结构方程的拟合及遗传度的估计[18, 19]

结 果

1. 一般情况:见表 1。共纳入男性双生子6 458对,平均年龄(41.2±11.5)岁,其中MZ 4 333对,占67.1%;北方3 699对,占57.3%。共6 291名(48.7%)调查对象报告吸烟。吸烟者中有313人(5.0%)戒烟;1 816人(30.8%)报告在20岁以前开始吸烟。不同卵型双生子的年龄、地区及吸烟情况的分布差异有统计学意义(均P<0.05),戒烟及开始吸烟年龄<20岁在不同卵型中的差异无统计学意义,P值分别为0.134和0.906(表 2)。

为使各亚组样本量均衡,以研究对象的中位年龄40岁为界分组。≥40岁组的吸烟率和戒烟率均高于<40岁组,前者开始吸烟年龄<20岁的比例低于后者。从地区分组来看,南方男性双生子的戒烟率高于北方,但北方开始吸烟年龄<20岁的比例低于南方。上述各分组之间比较差异均有统计学意义(P<0.05)。但南北方项目点的吸烟率差异无统计学意义(表 3)。

2. 结构方程模型的拟合及遗传度估计:研究对象吸烟、戒烟和开始吸烟年龄<20岁3个表型在MZ中的r值分别为0.71(0.70~0.73)和0.38(0.34~0.42)和0.87(0.86~0.88),在DZ中分别为0.56(0.53~0.59)、0.28(0.21~0.34)和0.73(0.70~0.76),均rDZrMZ的一半,故拟合结构方程模型时选择ACE模型。

各表型调整后的模型均优于调整前。戒烟行为调整后的CE和AE模型可接受,开始吸烟年龄<20岁的CE模型可接受。成年男性双生子吸烟行为相关3个维度的最佳模型均为调整后的ACE模型。成年男性双生子吸烟、戒烟和开始吸烟年龄<20岁的遗传度分别为0.26(0.19~0.34)、0.31(0.00~0.74)和0.05(0.00~0.14)。其中遗传度最大的是戒烟行为,受到环境因素影响最大的是开始吸烟年龄<20岁,其中C的影响为0.89(0.81~0.94),E的影响为0.06(0.04~0.08)。各表型调整和未调整年龄和地区模型的拟合结果分别见表 46

讨 论

本研究利用CNTR分析了成年男性吸烟、戒烟和开始吸烟年龄<20岁3个吸烟相关行为受到遗传和环境影响的相对大小,拟合模型得到双生子吸烟行为、戒烟行为和开始吸烟年龄<20岁的遗传度分别为0.26、0.31和0.05。可见受到遗传影响最大的是戒烟行为,受到环境影响最大的是开始吸烟年龄<20岁。

有研究显示,遗传因素对是否吸烟、是否戒烟、何时开始吸烟的影响在人群生长发育的各个阶段程度不同[20],而不同的地区环境对人群影响各异[21, 22, 23, 24]。本研究在分析遗传和环境的影响时控制了年龄和地区的影响。

对于吸烟行为,国内外现有研究结论不尽相同。Li等[25]开展的一项Meta分析综合了国外6项研究,包含17个队列共计28 871对双生子,得到吸烟行为的遗传度在0.30左右。我国青岛地区一项双生子研究得到吸烟行为遗传度为28%[26],与本研究结果基本一致。而Maes等[27]的一项双生子研究发现的吸烟遗传度为75%;国内其他研究得到的遗传度较本研究略高(0.69[3]和0.67[28]),可能是由于研究开展时间、涉及人群和调查范围及统计学方法不同造成的。另外,上述研究采用基因检测或问卷结合基因检测的方法鉴定卵型,准确率高于本研究。本研究卵型鉴定的准确率为87.0%,有研究者认为可能由于高估了E而造成对遗传效应的低估,但这一结论还有待进一步研究[29]。与上述研究认为吸烟行为受遗传影响不同,也有研究认为是否吸烟仅受共同环境的影响[30]

对于戒烟行为,本研究得到的遗传度为0.31(95%CI:0.00~0.74),可能是由本次研究对象的戒烟率较低导致。但本研究结果的点估计值与巴西的一项家系研究根据研究对象是否戒烟分组发现的持续吸烟调整年龄协变量后的遗传度0.32[31]相近。与澳大利亚(0.48)、瑞典(0.61)、芬兰(0.50)[32]、越南(0.70)[33]等双生子研究发现戒烟行为属于中度遗传行为的结论相符。但在土耳其开展的一项研究发现,尼古丁依赖行为的遗传度为0.80,属于高度遗传[34]。而有研究表明,不能成功戒烟的个体很大程度上归结于尼古丁依赖相关基因突变[35, 36]。由此可见,在尚未形成尼古丁依赖的吸烟者中进行干预,可能取得较好的戒烟效果[37]

国内外针对开始吸烟年龄的研究发现其遗传度从0到0.62不等[3, 31, 38],但未见针对开始吸烟年龄早晚的研究报道。本研究发现开始吸烟年龄<20岁的遗传度较低,仅为0.06,受到环境因素的影响高达0.94,其中共同环境影响为0.87。这与我国一项利用同病率研究开始吸烟年龄遗传度的结论一致,该研究发现MZ双生子的开始吸烟年龄相关系数低于DZ,即遗传对开始吸烟早晚的影响很小[3]。此外,有研究发现开始吸烟年龄低和个体进展为尼古丁依赖的关系密切[38]。因此,对环境因素(尤其是共同环境)的干预在吸烟行为开始早晚中有关键作用,并有可能进一步预防尼古丁依赖。

本研究使用的数据来自CNTR,数据质量高,规模大,涉及省份众多。在卵型判断部分,采用问卷和建模结合的方法对双生子的卵型进行判断,准确度较高[39, 40],能够满足大型流行病学调查的需要[41]。但相对于运用基因法和问卷法鉴定准确率达到90%以上的其他研究[42, 43],本研究卵型鉴定的准确度略低,对双生子的卵型存在一定无差异错分。这会对结果的稳定性造成影响,有研究认为这可能造成对遗传效应的低估,但还需进一步研究[44]。另外本研究属于横断面调查,调查对象信息的获取采用自报形式,开始吸烟年龄、是否吸烟及是否戒烟等信息可能出现回忆偏倚和报告偏倚。CNTR在调查中针对吸烟、戒烟等制定了严格的信息标准;在调查中是否吸烟、是否戒烟和开始吸烟年龄、年龄、地区均采用客观指标收集信息;并统一严格培训调查员,运用计算机辅助的电子问卷对研究对象进行面对面访谈,尽可能地减少信息偏倚。

目前我国利用双生子进行吸烟行为遗传度和环境因素影响的研究还不多,本研究具有一定现实意义。本研究利用高质量样本,以经典双生子法同时研究吸烟相关行为的遗传度,比较环境对这3种行为的影响为环境干预提供了合适的切入点。本研究验证了吸烟相关的3个维度均受到遗传因素的影响,但环境影响对吸烟相关的影响更大[22, 45]。现有前瞻性研究显示实施禁烟政策能够取得良好效果[22],进行环境干预能有效控制吸烟。通过严格制定禁烟相关的校纪校规、增加宣教力度、号召家庭配合等手段能够降低低龄吸烟的发生率[46],避免青少年过早暴露于烟草的危害。
