MSM是指与男性发生性行为的男性,主要包括四类人群:男性同性恋者、男性双性恋者、男性变性欲者和男性异性恋者。MSM以同性恋者居多,其次为双性恋者。处于性活跃期的MSM占性活跃期男性的2%~4%,2007年中国疾病预防控制中心公布的数字估计我国MSM超过1000万人。从全球来看,近年来 MSM的HIV感染率快速上升,在拉丁美洲MSM已经成为艾滋病流行主要人群,2012年多米尼加HIV新发感染者中MSM占33%,秘鲁占56%[1];在美国及东南亚、非洲等地MSM的HIV感染率为14%~18%[2]。在我国,MSM是艾滋病流行的主要人群之一,2007、2009和2011年中国艾滋病疫情报告中估计当年HIV新发感染者中,MSM感染者构成比为12.2%、32.5%和29.4%。截止2011年底,全国估计HIV感染者人数为78万人中,经同性传播占17.4%[3];2013年1-9月新发现的7万例HIV/AIDS中同性传播比例为20.8%[4]。研究报告显示,2003-2011年我国MSM的HIV感染率从0.9%快速上升至6.3%[5],在重庆等地MSM的HIV感染率已达16.5%[6]。因此,MSM成为HIV感染率上升最快同时也是当前艾滋病防治的重点人群。与此同时,随着艾滋病患者的增多和治疗标准的调整,越来越多的感染者得到了免费的抗病毒治疗(ART)。截止2013年,全球估计存活3 530万例HIV感染者,其中1 060万人已经参加了ART,治疗人数较2012年增幅超过20%[1]。截止2014年1月31日,我国共报告现存活约44.4万例HIV/AIDS中已有22.7万例正在接受ART[7]。
MSM普遍存在多性伴、无保护性肛交、群交、商业性性行为等高危性行为,我国MSM既往HIV检测比例仅为32.7%[8],很多感染者未及时发现,以及较低的安全套使用率等无保护的高危行为持续存在,导致了HIV在MSM中的快速传播和蔓延。同时,还有相当一部分MSM迫于社会及传统道德压力,大部分会选择婚姻或与异性交往,在无保护情况下与女性发生性行为,使得MSM成为HIV由高危人群向一般人群传播的桥梁人群[9]。作为传染源的MSM中的HIV感染者在预防和控制HIV传播方面起着重要作用。MSM中的HIV感染者如果和普通MSM发生无保护性行为,则导致MSM中HIV继续传播,因此,亟需对MSM中的HIV感染者高危行为采取针对性干预。
1.未接受ART的MSM中HIV感染者存在着高危性行为:国外一些关于MSM中的HIV感染者的研究显示,该人群无保护性肛交发生率为34%~57%[10, 11, 12],高危性行为在该人群中并未减少。一项包括30个研究的Meta分析显示,美国18 121名MSM中的HIV感染者中,43%的研究对象发生过无保护性行为[13];在台湾,一个横断面研究显示,56%的MSM中的HIV感染者发生过无保护性行为[14]。一项Meta分析显示,我国MSM近6个月肛交发生率为81.7%、多性伴发生率为59.7%,而安全套坚持使用率仅为37.9%[15],高危性行为普遍存在,导致艾滋病流行传播的因素广泛存在,亟需对该人群开展针对性的高危行为干预。因此,MSM中的HIV感染者减少高危性行为在控制HIV在MSM中传播起着重要作用。
2. 接受ART的MSM高危行为:随着HIV感染者CD4+T淋巴细胞计数不断降低,很多MSM接受免费ART。MSM在接受ART后身体状况明显好转,减少了机会性感染和艾滋病相关死亡,ART良好的治疗效果可能使无保护性行为增加。美国HPTN052研究表明,早期ART可以使异性传播途径中的HIV新发感染率降低96%[16],很多MSM中的HIV感染者认为早期ART同样可以在同性传播途径中起到作用,在接受ART后体内病毒得到完全抑制,从而降低了传播给他人的风险,安全套使用率降低,导致持续的无保护性行为增加。国外多项研究表明,接受ART的MSM中的HIV感染者发生肛交性行为时安全套使用率降低[17];多项研究显示,在接受ART后,MSM无保护性行为增加[17, 18, 19]。美国最新的一项研究显示,接受ART的感染者因为体内病毒载量降低,认为传播给他人HIV的风险降低,从而导致发生性行为时安全套使用率降低[20]。在美国一项136名MSM在接受ART>3个月的研究显示,约10%的研究对象持续存在着高危性行为并且体内存在着较高的病毒载量(>1 000 copy/ml)[21]。同时,一些研究显示,接受ART与高危性行为改变无关[22]。此外,在埃塞俄比亚的横断面研究显示,接受ART的感染者其高危性行为少于未接受者[23],可能原因是接受ART的感染者接受治疗时身体状况已经很差,其身体状况明显低于未接受者,性活跃度降低。虽然关于ART与高危性行为的改变有争议,但大量的研究证据显示,在MSM的ART覆盖率大幅提高的同时,MSM中的HIV感染者的性传播疾病(STD)患病率升高、高危性行为发生变化如安全套使用率降低[22, 24, 25]。多项研究显示,由于持续的高危性行为、较高STD感染率和生殖系统炎症,MSM中的HIV感染者在接受ART后,其精液中依旧可以检测到HIV[13, 26, 27]。最新研究显示,83名MSM接受ART后,其血液中检测不到HIV,但在25%研究对象的精液里检测到了HIV[28]。接受ART的 MSM中的HIV感染者中,发生无保护性行为的研究对象,其精液中更容易发现HIV(发生无保护性行为的研究对象精液中存在HIV的概率是未发生无保护性行为的7倍),有STD/尿道炎的研究对象精液中发现HIV的概率是无STD/尿道炎的27倍[28]。已接受ART的MSM中,由于持续的高危性行为和较高的STD患病率,精液中还是存在HIV[27]。因此,MSM在接受ART的同时可能存在着持续的高危性行为,导致HIV继续传播,对接受ART的MSM开展相关高危性行为监测已刻不容缓。
WHO 2013年6月30日发布《治疗和预防HIV感染的新指南》推荐CD4+T淋巴细胞计数由原来的低于350 cell/mm3提高到低于500 cell/mm3开始治疗[29]。而我国最新的治疗标准也是提高到低于500 cell/mm3开始治疗[30]。随着感染人数增多和治疗标准的调整,将会有越来越多的MSM中的HIV感染者开始接受早期ART。大部分感染者接受治疗时处于HIV期,其身体状况较好,性行为活跃,如果发生性行为时不采取保护措施,很容易导致HIV在MSM中传播。研究显示,ART依从性差的人,其ART效果也差,容易治疗失败从而产生耐药,导致耐药病毒株的传播[14, 28]。在美国开展的接受ART的MSM研究中显示,HIV耐药病毒传播率为14.2%[31];而国内在19个省(市)开展的研究显示,MSM中耐药病毒比例为4.9%[32],远高于其他高危人群。因此,接受ART的MSM可能导致HIV耐药毒株的传播流行,一旦HIV耐药毒株流行起来,将会对艾滋病预防与控制带来巨大挑战。
3. 接受ART的MSM梅毒感染率情况:HIV和梅毒的传播途径一致,美国的一项研究显示,MSM感染梅毒后感染HIV的概率是未感染梅毒的2~3倍[33],因此在监测艾滋病疫情时常用梅毒作为监测指标,由于调查中获得MSM高危性行为的结果,多以自我报告(self-report)的方式得到,不能客观反映真实情况,因此也常用梅毒感染率作为评价HIV高危性行为的客观指标[34]。在国内开展的一项Meta分析中,从2001-2008年中国MSM的HIV合并梅毒的感染率为9.1%[35]。国内的一项包含2005-2011年82篇文章的Meta分析中显示,MSM合并HIV感染率为4.4%,合并梅毒感染率为11.1%[15]。最新研究显示,MSM中的HIV感染者的梅毒感染率和接受ART有关,国内进行的横断面研究中显示,MSM中的HIV阳性人群中,接受ART是感染梅毒的危险因素(OR=3.848)[36];以色列的研究中显示,MSM在接受ART后更容易感染梅毒[37]。在荷兰的研究中显示,MSM中的HIV感染者在接受ART后,梅毒的感染率从0.5%上升至0.8%[38]。这些研究提示,MSM中的HIV感染者接受ART后梅毒感染率增加,在一定程度上提示接受ART后其高危性行为增加。
结论及建议:目前国内尚未报道关于接受ART的MSM中的HIV感染者高危性行为变化的研究,对MSM接受ART后其高危性行为是否发生改变,怎样改变,目前尚不清楚。但国外已经有相关研究中提示MSM接受ART后,无保护高危性行为增加或继续存在着传播HIV的风险。对于接受ART的MSM中的HIV感染者,其性伴HIV若是阳性,则可能导致不同类型亚型HIV感染和耐药性病毒传播;但性伴如果是阴性,则需要提供策略,防止HIV阳性传向HIV阴性人群。目前早期ART在异性传播上可以预防96%新发感染[16],MSM通过早期治疗后在一定程度上可以降低HIV感染率[39],但对于接受治疗的MSM其存在的无保护性行为、精液中存在着HIV、梅毒感染率上升、HIV耐药毒株等危险因素存在导致HIV的继续传播流行,因此对于接受治疗后的MSM高危性行为仍不能掉以轻心。
目前常规艾滋病防治工作中对于接受ART的MSM并未开展系统性的研究,其高危性行为如何变化尚不清楚。因此有必要对MSM接受ART后对性伴类型、性伴HIV感染状态、同性性行为、异性性行为及同性商业性性行为、安全套使用情况、酒精/新型毒品滥用等开展调查。
