子宫内膜癌(endometrial cancer, EC)为女性生殖道常见的恶性肿瘤之一,占女性恶性肿瘤第6位,分别占发达国家及发展中国家女性恶性肿瘤第4位和第7位[1]。随着手术病理分期的实施及临床研究的进展,EC的诊治更加规范化,但在微创手术、根治性子宫切除术(radical hysterectomy, RH)及前哨淋巴结(sentinel lymph node, SLN)切除方面仍存在诸多争议。本文旨在探讨EC手术治疗的选择与其争议,为临床决策提供参考。
1 子宫内膜癌的微创手术治疗EC的手术发展非常迅速,从开腹到腹腔镜手术再到达芬奇机器人,由多孔到单孔腹腔镜,微创手术已经成为EC的主要术式。2018年美国德州大学安德森癌症中心在新英格兰医学杂志发表的LACC研究结果显示,早期子宫颈癌患者腹腔镜/机器人手术无瘤生存率(disease-free survival, DFS)和总生存率(overall survival, OS)低于开腹手术,使广大临床医师重新思考EC微创手术的安全性[2]。
1.1 腹腔镜手术GOG 2222-LAP2是一项前瞻性随机对照研究(randomized controlled trial, RCT),2 626例Ⅰ期/ⅡA期(FIGO 1988)EC患者来自美国和英国的6家医疗中心(1996年~2005年),随机行腹腔镜或开腹手术[3]。研究结果显示,两组间的术中并发症比例相似; 腹腔镜组的术后并发症(肠梗阻、感染和心律失常等)低于开腹组(P < 0.01);腹腔镜组与开腹组的3年累计复发率(11.4% vs 10.2%,P > 0.05)、5年复发率(13.7% vs 11.6%,P > 0.05)及5年OS(84.8% vs 84.8%,P > 0.05)差异均无统计学意义。
LACE试验也是一项多中心RCT,760例Ⅰ期EC患者来自澳大利亚、新西兰和中国香港地区的20个妇科肿瘤中心(2005年~2010年)。结果显示,腹腔镜组与开腹组的DFS分别为81.6%和81.3%,差异为0.3%(95% CI:5.5%~6.1%),符合等效性标准; 两组间的复发率(8.1% vs 7.9%,P=0.93)及OS(92.6% vs 93.2%,P=0.76)差异也无统计学意义[4]。
Galaal等[5]应用Cochrane系统Meta分析9项研究,结果与上述一致,腹腔镜手术与开腹手术的OS(HR=1.04,95% CI:0.86~1.25)及DFS(HR=1.14,95% CI:0.9~1.43)差异均无统计学意义(均P > 0.05);与开腹组比较,腹腔镜组住院时间更短,术中出血量更少; 另外,两组间的脏器损伤率(膀胱、输尿管、肠管和大血管损伤)、输血率、围术期死亡率及严重的术后并发症发生率相当。
从目前的研究来说,早期EC患者腹腔镜手术具有较高的可行性与安全性。由于上述研究的病例多为FIGOⅠ期患者,尚未见Ⅱ期患者大样本的前瞻性报道。需要关注Ⅱ期EC患者微创手术复发的可能相关因素如举宫器和二氧化碳气腹等外,还需考虑到Ⅱ期患者与子宫颈癌类似的子宫颈累犯,其行微创手术可能有更大的肿瘤播散风险。
1.2 达芬奇机器人手术Gaia等[6]纳入2005年~2010年的8项EC研究行Meta分析,包括机器人手术589例、开腹手术606例和腹腔镜手术396例,结果表明与开腹及腹腔镜组比较,机器人组腹主动脉旁及盆腔淋巴结切除数目差异均无统计学意义(均P > 0.05),但术中出血量减少; 机器人及腹腔镜组中转开腹率比较差异无统计学意义(P=0.06),两者的手术及住院时间少于开腹组; 三组之间的血管/肠管/泌尿道损伤、血栓栓塞发生率和输血率比较,差异均无统计学意义(均P > 0.05)。Gala等[7]回顾性分析Medline和Cochrane试验注册中心2012年前发表的关于机器人和非机器人手术在治疗妇科良恶性疾病方面比较的44项研究,发现在治疗EC方面,机器人较开腹手术有更大优势,但与腹腔镜手术比较,尚有争议。目前机器人手术应用于EC的安全性已经得到多方验证,和腹腔镜比较各有优点,选择何种术式,取决于患者知情选择、医师手术能力及设备可获得性等。
2 根治性子宫切除术与Ⅱ期子宫内膜癌2018年及以后的NCCN指南推荐Ⅱ期EC可采用单纯子宫切除术(simple hysterectomy, SH)或RH[8],目前RH和SH对Ⅱ期EC患者的生存获益尚存在争议。Cohn等[9]和Orezzoli等[10]分别回顾性分析162例和81例Ⅱ期EC患者,均发现RH组的5年OS高于SH组(均P < 0.05)。而Jiang等[11]和Phelippeau等[12]的研究显示,RH组与SH组之间的OS及DFS差异均无统计学意义(均P > 0.05),权衡术后辅助放疗的影响后,两组间比较,差异仍无统计学意义(均P > 0.05);但接受RH者围术期及远期并发症发生率更高[12]。一项meta分析(10项研究共2 886例)[13]及目前样本量最大的一项回顾性研究(7 552例)[14]得出的结论也均与Phelippeau等[12]一致。
为了进一步分析上述结果产生差异的原因,本文检索1993年至2020年有关Ⅱ期EC患者SH/RH治疗的文献,结合Jiang等[11]和Liu等[13]的报告,进行汇总(表 1),其中3篇源于北美肿瘤数据库——监测、流行病学和结果数据库(Surveillance,Epidemiology and End Results,SEER),1篇来源于美国国家癌症数据库(National Cancer Data Base,NCDB)[11-29]。结果显示,以FIGO 1988年分期系统为标准的研究多认为RH优于SH,而以FIGO 2009年分期系统为标准的研究均认为两者的OS无差异,RH甚至增加死亡风险(HR=1.24,P=0.245)[13];美国及意大利的研究多提示RH优于SH,而芬兰、日本、韩国、土耳其、墨西哥及中国的研究认为两者之间的OS比较,差异均无统计学意义(均P > 0.05),结果偏倚的原因考虑可能与地域和手术时期有关; 另外回顾性研究混杂因素众多,如样本量、术者的手术环境及技术等。

目前有关EC子宫颈累及对宫旁侵犯预测价值的研究报道极少。Watanabe等[30]报道334例行RH的EC患者,术后病理检查示28例累及宫旁,其中仅10例累及子宫颈,但均合并淋巴脉管间隙浸润。结果提示,EC子宫颈累及对宫旁侵犯的预测价值较低。Lee等[23]对71例Ⅱ~Ⅳ期EC患者行RH,术后病理示所有宫旁侵犯者均为Ⅲ期及以上患者,肌层浸润深度及淋巴结转移与宫旁侵犯相关。鉴于RH不能改善Ⅱ期EC患者的生存期,且增加围术期并发症,因此Ⅱ期EC患者选择RH需谨慎。
3 前哨淋巴结显影在子宫内膜癌的应用目前早期EC患者是否需行系统性淋巴结切除(systematic lymphadenectomy, SLAD)及其切除范围尚无定论。SLAD能提高EC分期的准确性,有助于判断预后及指导术后辅助治疗。但欧洲2项RCT均表明盆腔淋巴结切除术(pelvic lymphadenectomy, PLND)对早期EC患者无生存获益,反而增加下肢淋巴水肿、淋巴囊肿以及神经血管和输尿管损伤等并发症[31-32]。由于这2项研究未行系统的腹主动脉旁淋巴结切除(para-aortic lymphadenectomy, PALND),且未考虑术后辅助治疗的影响,其结果存在一定的争议。Todo等[33]对691例EC患者行PLND或PLND+PALND,发现后者OS高于前者(HR=0.53, P=0.0005);进一步分析显示低危患者两组之间的OS比较差异无统计学意义(P > 0.05),中高危患者两组之间的OS比较,差异具有统计学意义(P=0.000 9)。
3.1 SLN显影的示踪剂选择及注射方式FILM RCT试验发现吲哚菁绿(indocyanine green, ICG)在检测SLN方面较放射性胶体(99mTc)和蓝染料具有更高的敏感度和总检出率,所有转移的SLN均在ICG显影下被检出[34]。Kang等[35] meta分析表明,EC子宫颈注射的SLN检出率高于其他注射方法,能准确预测盆腔淋巴结转移情况,但腹主动脉旁淋巴结检出率相对较低; Kim等[36]研究表明,相较于ICG单纯子宫颈注射,子宫颈注射联合宫角注射对转移淋巴结及腹主动脉旁SLN的检出率更高(P < 0.01)。
3.2 SLN病理超分期检查的临床意义SLN需行病理超分期检查[37],即连续薄层切片行HE及免疫组织化学染色,鉴别微转移(直径0.2~2.0 mm)和孤立细胞团转移(isolated tumor cells, ITC; 直径 < 0.2 mm),但其临床意义尚未明确。Plante等[38]研究519例EC患者发现,SLN阳性率16.4%,其中50.6%为大转移灶,12.9%为微转移灶,36.5%为ITC; 多数ITC者未接受术后辅助治疗,3年PFS分别为58.6%、85.5%和95.5%,提示淋巴结微转移及ITC者预后较好。因此不能将淋巴结微转移等同于淋巴结阳性,否则会导致低危患者不必要的术后辅助治疗,但也不能完全忽略其对预后可能存在的影响。
3.3 SLN显影精确性及安全性的评价最大规模的前瞻性研究FIRES试验纳入385例Ⅰ期EC患者,结果显示SLN总检出率86.1%,41例淋巴结阳性者,36例行SLN活检,其中35例SLN阳性,SLN活检检出ⅢC期的敏感度为97.2%,阴性预测值为99.6%,假阴性率为2.7%[39]。Bogani等[40] meta分析发现,SLN活检技术较SLAD对阳性淋巴结的检出率更高(14.7% vs 9.9%, P=0.002),但两者的腹主动脉旁阳性淋巴结的检出率(OR=0.93, P=0.86)、术后总复发率(4.3% vs 7.3%, P=0.63)及淋巴结复发率(1.2% vs 1.7%, P=0.29)比较,差异均无统计学意义,提示SLN活检技术可作为评价EC淋巴结是否转移的一种可行办法。
但SLN显影在高危EC患者中的应用仍有争议。Ye等[41]前瞻性研究发现,低危患者SLN检测转移淋巴结的敏感度是100%,阴性预测值为100%,假阴性率为0%,而高危患者检测的敏感度、阴性预测值及假阴性率分别为20.0%、83.3%和80.0%,提示高危患者SLN显影的准确性可能较差。但Papadia等[42]研究显示,高危EC患者SLN单侧及双侧检出率为100%和90.5%,检出转移淋巴结的假阴性率、敏感度及阴性预测值分别为10.0%、90.0%和97.1%。SHREC试验发现,高危EC患者双侧SLN显影率为81.7%(二次注射后为94.9%),检测阳性淋巴结的敏感度为98.1%,阴性预测值为99.5%[43]。因此SLN显影并不降低高危EC患者转移淋巴结的检出率。
关于生存获益,Eriksson等[44]对低危患者的研究及Schlappe等[45]对高危患者的研究结果一致,EC患者SLN切除组和SLAD组的OS及DFS比较,差异均无统计学意义(均P > 0.05)。Casarin等[46]研究表明,SLN切除组较SLAD组手术时间更短,术中出血量更少,淋巴水肿的发生率更低,严重围术期并发症发生率、再入院率及二次手术率比较,差异均无统计学意义(均P > 0.05)。以上研究均证实SLN显影在EC应用中的可行性与安全性。
4 结语综上所述,EC微创手术的肿瘤学结局与开腹手术比较无明显差异,且围术期并发症更少,易被广大医患所接受。RH与SH对Ⅱ期EC患者预后的影响存在争议,且RH围术期并发症多,Ⅱ期EC患者术式选择RH需谨慎。与传统淋巴结切除术比较,EC患者SLN显影及活检的转移淋巴结检出率更高,损伤小,且对生存率无明显影响,适用于早期低危EC患者,对中高危患者是否有利尚需更多研究探索。
