血管内光学相干成像技术检测易损斑块研究进展

摘要:动脉粥样硬化斑块破裂引发血栓形成是临床急性冠脉综合征的根本原因。易损斑块的早期识别与及时处理是避免临床心血管事件的关键。目前已经有很多影像学方法应用于冠脉内易损斑块的检测,但易损斑块的形态学及其破裂导致血栓形成的机制仍是临床医生关注的重点。血管内光学相干成像技术(OCT)是一种基于心脏导管的检查方法,它利用近红外光,可获取高达10 μm分辨率的影像,从而在细胞水平实现体内血管壁微结构成像。随着新一代频域OCT(FD-OCT)的发展,这一技术在易损斑块研究中发挥积极作用。现将OCT在易损斑块相关研究中的进展作一综述。

Intravascular optical coherence tomography in detecting vulnerable coronary plaque:an update

Abstract: Rupture of a vulnerable plaque and the subsequent thrombus formation are the main cause of acute coronary syndrome (ACS). The key to avoid clinical cardiology event is the early detection and prompt management of vulnerable plaques. Many imaging modalities have been introduced to identify thin-capped fibroatheroma (TFCA), but the morphology and the mechanism of vulnerable plaque rupture are still great concern of clinicians. Intravascular optical coherence tomography (OCT) is an examination based on cardiac catheter. It uses near-infrared light to obtain up to 10 μm high-resolution images, to display the in vivo micro-structure of blood vessel wall. With the development of a new generation of frequency-domain OCT (FD-OCT), OCT will play an active role in TCFA. We reviewed here the studies of TCFA involving OCT.

研究已经明确临床心脏事件主要原因在于不稳定斑块破裂,并继发血栓形成[1]。Muller等[2]最早提出了“易损斑块(vulnerable plaque)”的概念。随后,Libby[3]将这一概念进一步明确,提出了炎症浸润的纤维帽等组织学概念。1997年,Burke等[4]对心源性死亡患者的病理学研究证实:导致猝死事件的根本原因是斑块破裂并继发血栓形成,并明确将“易损斑块”定义为“薄纤维帽的动脉斑块”(thin-capped fibroatheroma,TCFA),特指具有巨噬细胞浸润的,厚度<65 μm的纤维帽覆盖的坏死中心这一特殊类型的动脉粥样硬化斑块。2003年Naghavi等[5]结合大量回顾性病理解剖研究结果,发表了第1份对易损斑块的国际共识,指出易损斑块不仅容易破裂,形成血栓,而且可快速进展为临床事件。同时提出了易损斑块的组织病理学标准,即主要标准:(1)活动性炎症(单核细胞/巨噬细胞/T淋巴细胞浸润);(2)大的脂质核、纤维帽薄;(3)内皮剥脱,表面血小板聚集;(4)斑块裂隙;(5)狭窄程度>90%。次要标准:(1)表浅钙化小结节;(2)斑块呈浅黄色;(3)斑块内出血;(4)内皮功能障碍;(5)血管正性重塑。正是基于这样的病理生理研究基础,越来越多的研究集中于早期发现稳定性患者存在的易损斑块。

近20年,随着血管内超声技术(intravascular ultrasound,IVUS)的广泛应用,对不稳定斑块的临床研究也逐步深入。Rodriguez-Granillo等[6]利用较先进的组织虚拟影像IVUS(VH-IVUS),对IVUS检测下TCFA做了进一步说明,要求必须具备以下3项标准中的两项,即:薄纤维帽的斑块;管腔面积≤4.0 mm2;斑块负荷≥70%。2011年PROSPECT研究中利用高频血管内超声(RF-IVUS)对697例急性冠脉综合征(acute coronary syndrome,ACS)患者进行3年随访研究,结果发现,非罪犯病变造成的主要心血管事件与原有治疗的ACS靶病变相似,其相关危险因素为:斑块负荷≥70%;检测出TCFA;最小管腔面积≤4.0 mm2[7]。尽管IVUS能分辨较深的血管组织,但其分辨率为100~150 μm,对于动脉粥样斑块的细微结构缺乏清晰的认识,很难达到病理解剖中关于不稳定性斑块薄纤维帽的标准(<65 μm)。因此临床迫切需要更加精确的检测方法以区分易损斑块,减少临床事件的发生。光学相干成像技术(optical coherence tomography,OCT)的空间分辨率达到了10~15 μm,穿透距离为1~3 mm,不仅能区分内外弹力膜,而且可以清晰地分清血管壁内膜、中膜及外膜,获得类似血管壁组织水平的图像,对于斑块的识别具有特殊优势[8, 9, 10]1 OCT技术的发展

OCT是使用低相干宽带光源和干涉原理对物体进行透射成像的技术。其原理类似于超声成像,区别只是用波长为1 250~1 350 nm的远红外光代替了声波。2001年,Lightlab Imaging Inc( Wesrford,MA,USA)首次报道了OCT技术在冠状动脉内获得的高清晰图像,此后OCT技术在冠心病介入领域中的应用逐渐增多。虽然美国心脏协会(AHA)/美国心血管造影和介入学会(SCAI) 2011年发布的经皮冠状动脉介入治疗(PCI)指南对于OCT技术的建议等级没有明确定论[11],但随着该项技术的发展以及临床的广泛应用,使得它在冠心病介入治疗中发挥越来越明显的作用,尤其是在评价血管内不稳定斑块方面,显示出更加安全、可靠的优势。

2010年开始在临床广泛应用的第2代C7-XR OCT系统(St Jude Medical,St Paul,MN),采用Dragonfly成像导管,成像速度达到100帧/s。其操作简单,经常规6F的指引导管,沿着普通0.014英寸(1英寸=2.54 cm)的导丝送至病变远端。该仪器不需阻断血流,只要通过导管注射生理盐水或造影剂(15~20 mL/序列),后撤速度达到20 mm/s,扫描长度可达50 mm,相对第1代具有操作简单、无需阻断血流、临床并发症减少等优点[12]。最近,更新的一代 IlumienTM系统(St. Jude Medical,St. Paul,MN,USA) 已经在一部分国家及地区应用,该系统回撤速度可快至20~36 mm/s,并可获得更长的检测范围(50~75 mm)。同时该系统结合了无线血流储备分数(FFR)导丝系统,可同时完成管腔内斑块的形态学及功能学检查。而Ilumien OptisTM系统,更增加了3D成像功能,并可实现同步冠脉造影定位,以指导PCI过程。Terumo公司生产的"LUNAWAVETM OFDI系统也在回撤长度、冠状动脉造影影像同步性等方面有新的改进。

2 OCT检测下不稳定斑块的形态学研究

早期组织学研究已证实,OCT检测成像与组织学检查具有良好的相关性,对于纤维斑块的敏感性为71%~79%,特异性为97%~98%;而纤维钙化斑块敏感性及特异性分别为95%~96%和97%;对于IVUS较难分辨的脂质斑块,也分别高达90%~94%和90%~92%[13]。OCT极高的分辨率,可以准确评判接近管腔的细微结构,尤其是诊断斑块不稳定成分,如纤维帽的厚度、 巨噬细胞浸润、脂质斑块大小、钙化结节、斑块破裂、浸润以及血栓等[11, 12, 13, 14, 15, 16]。目前,绝大多数研究仍延续了早期Yabushita等根据尸体解剖研究的结果,即结合血管解剖研究结果对血管内斑块的分类方法,将OCT影像下血管动脉粥样硬化斑块分为纤维斑块、钙化斑块及脂质斑块[14]。2012年,血管内光学相干成像国际工作组撰写委员会对IVOCT领域内关键问题的相关标准和推荐进行了整理,基于国际性IVUS的共识标准[17],结合现有的临床研究,制定了针对OCT的共识,对各类血管内斑块的定义、特征及测量制定了相应的标准[18]

2.1 OCT对纤维帽厚度的研究

纤维帽的厚度是斑块易损性最重要的决定因素[19]。由于纤维帽主要由平滑肌细胞及胶原纤维组成,在OCT上表现为高密度、低反射的影像,因此OCT可以清楚分辨带有薄的纤维帽的易损斑块。临床上,被大家广泛接受的具有易损倾向的纤维帽厚度为65 μm,这一数值源自重要的病理学研究[4],该研究针对导致猝死的41个破裂斑块的病理组织学检查,发现破裂斑块纤维帽为(23±15) μm,其第95百分位值为65 μm,并将具有这一特征的粥样斑块定义为TCFA。Jang等[19]首次将具有薄纤维帽(最薄处<65 μm)的富含脂质的斑块(在一个斑块内脂质成分占据2个象限,即180°)定义为OCT意义上的TCFA。目前为止,OCT仍然是人体内测量纤维帽厚度最为精确的检测手段,但是由于血管本身存在10%~20%的组织收缩特性,65 μm的截点并不是唯一。Kubo等[10]进一步证实,ACS患者中,TCFA多达83%,而急性心肌梗死患者的纤维帽厚度仅为(49±21) μm。在最近一项研究中,发现破裂斑块纤维帽厚度中位数是116 μm,而未破裂的为182 μm,斑块破裂的纤维帽厚度为<80 μm[20]

相对ST段抬高心肌梗死(STEMI)患者斑块纤维帽的平均47 μm的厚度,非ST段抬高心肌梗死(NSTEMI)患者为53.8 μm,稳定性患者为102.6 μm[19]。不同临床状态下,斑块纤维帽的厚度也存在一定差异[21, 22]。上述结果在最近的一项针对猝死患者的病理组织学研究中得到进一步验证。该研究发现,破裂斑块纤维帽<54 μm,且管腔狭窄程度多<74%;TCFA纤维帽厚度范围为54~84 μm。因此有理由认为,纤维帽厚度<85 μm或管腔狭窄程度>75%的病变应引起重视[23]。不仅如此,早期Tanaka等[24]研究发现,虽然TCFA是导致ACS的主要因素,一些附带较厚纤维帽的稳定性斑块(>65 μm)在运动时也会诱发破裂,导致急性心脏事件。自发性破裂斑块的纤维帽厚度明显薄于运动后破裂的斑块(50 μm vs 90 μm),而后者也多发生于斑块的肩部,这可能与炎症反应及斑块肩部的高剪切力有关。这一现象近期也得到来自国内研究的进一步证实[25]。正是由于既往关于斑块纤维帽厚度的绝大多数数据来自尸体解剖以及已经破裂的斑块,因此,关于纤维帽破裂的界限的研究仍是临床科研关注的重点。

2.2 OCT对坏死中心面积的研究

纤维帽覆盖下的坏死中心是不稳定斑块的重要特征。脂质坏死中心在OCT上表现为边界不清晰的高密度区域。既往组织病理学及IVUS研究均提示斑块纵向所占象限或弧角与易损性相关,但目前仍没有明确定论[18]。而且,由于OCT的穿透性不强,对于血管壁深层结构分辨不如IVUS,因此在测量脂质斑块及坏死中心面积上存在缺陷。近期,随着3D技术的进展,也有一些针对斑块容积的探索性研究[26]

2.3 OCT对不稳定斑块其他特征的研究

炎症反应是另一构成斑块不稳定性的重要原因[27]。OCT极高的分辨率可以准确评判接近管腔的细微结构,尤其是诊断斑块不稳定成分,如巨噬细胞浸润、新生微血管、钙化结节、斑块破裂、侵蚀以及血栓等。

2.3.1 巨噬细胞浸润

纤维帽内炎症细胞浸润提示高度破损倾向。由于斑块内巨噬细胞体积可达30~50 μm,使得OCT很容易分辨。Tearney等[28]证实OCT测量的巨噬细胞密度与组织细胞学测量结果相关度极高。而对于不同状态下冠心病患者的血管病变检测显示,不稳定斑块患者的斑块内巨噬细胞增多。同样,“罪犯斑块”内巨噬细胞密度多于“非罪犯病变”,而破裂斑块内最多[27]。巨噬细胞的聚集以及密度的增加,提示斑块不稳定,成为OCT检测斑块不稳定的标志[29]。但由于检测者个人水平不同,很难在OCT下对巨噬细胞进行定量研究,且目前尚不清楚它在斑块破裂中的作用机制。

2.3.2 新生微血管

新生内膜滋养血管反映了斑块的不稳定性,OCT能够区分直径50~100 μm的微血管[30]。而新生血管多与薄的纤维帽共存,对他汀类药物治疗反应差,较难出现斑块回缩[26, 29]。这与既往的病理学研究结果吻合[31]

2.3.3 血管正向重塑

既往的病理学研究已明确说明,血管管腔的正向重塑同样也是斑块不稳定的一种表现[32]。这一点在Raffel等[33]的研究中得到充分的证实。由于OCT的组织穿透力不如IVUS,因此在正向重塑方面研究不多。但Kume等[10]曾对比了组织学及OCT影像,发现正向重塑的斑块含有较多的脂质以及更大的斑块负荷,而这也正符合不稳定斑块的形态学特点。现有OCT研究也显示血管壁的正向重塑与斑块的不稳定性一致[34, 35]

2.3.4 斑块破裂及侵蚀

尽管普遍认为斑块的破裂是造成ACS的主要原因,以往研究也发现其他因素同时参与了这一过程,如斑块侵蚀、钙化结节等。根据2012年OCT国际工作组的专家共识,分别对破裂的斑块(ruptured plaque)及OCT概念中侵蚀(erosion)做了明确说明。破裂的斑块经常出现在OCT检测的TCFA中,表现为内膜撕裂、破坏或斑块纤维帽夹层。当注射显影剂时,内膜没有显影或显影不充分,甚至形成空洞。OCT检测下侵蚀是指血管内膜表面不正常或不连续并附有微小血栓,但没有纤维帽破损及空洞形成[18]。最近的一项研究发现,相对稳定性心绞痛患者,不稳定心绞痛患者出现斑块破裂及侵蚀的情况较多[36]。而在STEMI 患者中,破裂的斑块明显多于NSTEMI[21, 37]。Jia等[38]将造成ACS的斑块分为破损斑块、钙化结节、明确的侵蚀斑块、可能的侵蚀斑块和其他共5类,据此分类,发现126个ACS靶病变中,44%为破损斑块、23%为经OCT确认的侵蚀性病变、另外8%为钙化结节;结合临床特征进一步分析发现,出现斑块侵蚀的患者更年轻,且非ST段抬高ACS发生率更高。

不同类型冠心病患者的冠脉血管内均可见到脂质斑块,但其中斑块破裂、溃疡形成、纤维帽厚度、最小管腔面积及血栓发生情况均有明显不同[36]。管腔狭窄、斑块表面形成溃疡并少量血栓形成,可能是BraunwaldⅠ级心绞痛患者发生ACS的机制;而Braunwald Ⅲ级的不稳定性心绞痛患者中即使在管腔足够大的情况下,TCFA破裂发生血栓的概率明显多于其他类型。同样,在STEMI患者中,斑块破裂明显多于NSTEMI患者(70% vs 47%),而破裂的纤维帽可能是前向血流减慢的影响因素[21]

与稳定性患者相比,ACS患者的非罪犯血管存在较多不稳定特性,包括较多脂质斑块、更薄的纤维帽、更多的靠近管腔的新生血管。ACS患者中,65%可检测到TCFA,而平均每例患者存在(1.9±1.4)个富含脂质的斑块。这也提示ACS是系统性、广泛的血管疾病,而不是某个血管的局部问题[39]。这些研究对于深入探讨ACS的机制、明确治疗策略具有深远的意义。

2.3.5 血栓

OCT不仅可以检测出血栓,而且可区分红色及白色血栓,并证实血栓的存在可能是斑块不稳定的标识[16, 40],这一点是IVUS不能实现的。及时识别血栓,明确血栓性质,对于减少手术并发症,改善预后都有积极的意义。

3 应用前景及不足

越来越多的OCT指导下PCI治疗中发现,富含脂质的斑块在治疗过程中,容易出现相关并发症。因此,术前明确管腔内斑块性质的识别对于介入手术术中策略选择有着突出的意义[41, 42, 43, 44]。此外,作为血管内斑块形态学检测标准,OCT指导下易损斑块的发现及诊断已经成为临床检测其他生化指标及无创检测技术的临床应用及发展的检测手段[45, 46]。OCT对于易损斑块纤维帽的评判具有明显优势,已有研究利用这一检测手段来评价临床使用他汀类降脂药物的效果[47]

OCT在易损斑块诊断中存在以下问题:(1)组织穿透力较弱,仅为2~3 mm,因此对于血管壁深层结构,尤其是斑块面积及血管重塑方面存在不足;对于内径粗大的血管以及左主干开口部位,成像不理想。(2)无论是纤维帽测量、巨噬细胞的定量检测,目前都缺乏统一的标准,需要更多的实体数据。(3)虽然目前OCT检查已无需阻断血流,但为了获得良好的图像,依然需要快速冠脉内注射造影剂,增加了造影剂的使用。(4)由于OCT成像速度快,相对IVUS其斑块定位性差,往往需要钙化斑块及分支血管协助定位。但随着新一代冠脉影像同步技术的应用,这一缺陷已得到明显改进。

综上所述,易损斑块是造成心脏事件的重要启动因素,因此,早期、准确发现这些破裂倾向的斑块具有重要的临床意义。同其他检查方法相比,OCT具有高度的分辨率,对于血管内膜研究具有绝对的优势。现有研究结果证实,OCT对于易损斑块的检测与组织学具有极高的相关度,是研究易损斑块的可行性方法,有望在易损斑块的形态学研究及斑块破裂导致ACS的机制研究方面发挥积极作用。
