晚期非小细胞肺癌免疫治疗的研究进展

摘要: 近年来,免疫治疗为肺癌领域带来革命性突破,改变了肺癌的治疗格局,让更多的晚期肺癌患者得到了长期生存的机会,已经成为晚期NSCLC患者不可或缺的治疗手段。目前,免疫治疗仍是肺癌领域炙手可热的研究热点,免疫治疗策略百花齐放,新型免疫药物层出不穷,研究进展日新月异。因此,为了更好地了解晚期NSCLC免疫治疗的发展变化,本文就免疫治疗的临床研究进展进行系统、全面的综述。

Research Progress of Immunotherapy for Advanced Non-small Cell Lung Cancer

Abstract: In recent years, immunotherapy has made revolutionary breakthroughs in the field of lung cancer and has become an indispensable treatment for patients with advanced NSCLC. More patients treated with immunotherapy have achieved long-term survival. At present, immunotherapy is still a hot research hotspot. In order to better understand the development and changes of immunotherapy for advanced NSCLC, this article systematically and comprehensively reviews the clinical research progress of immunotherapy.

0 引言

在我国,肺癌的发病率和死亡率均位居所有恶性肿瘤首位,其中非小细胞肺癌(non-small cell lung cancer, NSCLC)占所有肺癌的85%左右,且大部分患者就诊时就已是晚期,通常预后较差[1]。为了改善晚期NSCLC患者的生存状态,多年来一直在不断地探索有效治疗手段,分子靶向治疗在晚期NSCLC首先取得进展,显著提高了驱动基因阳性人群的生存状态,但是对于无驱动基因突变的NSCLC患者仍然缺乏有效的治疗手段。近年来,以程序性死亡受体1(programmed cell death-1, PD-1)或程序性死亡受体-配体1(programmed cell death-ligand 1, PD-L1)抑制剂为主的免疫治疗取得历史性突破,改变了无驱动基因突变人群的治疗格局,更多的晚期患者获得了长期生存。目前,免疫治疗在晚期NSCLC领域研究进展迅速,本文就免疫治疗的最新临床研究进展进行综述。

1 晚期NSCLC一线免疫治疗进展 1.1 PD-L1高表达人群一线免疫治疗

NSCLC一线免疫治疗首先在PD-L1高表达人群获得了突破,KEYNOTE 024研究证实在PD-L1高表达人群中(PD-L1≥50%),与标准化疗相比,帕博利珠单抗可以改善患者的总生存时间(overall survival, OS),两组的OS分别为26.3月和13.4月(HR=0.62, 95%CI: 0.48~0.81),5年OS率分别为31.9%和16.3%,提示PD-L1高表达人群免疫单药治疗可以获得长期生存[2-3]。随后KEYNOTE 042研究将帕博利珠单抗的适应证扩大到PD-L1≥1%的人群,但是亚组分析提示生存获益仍主要来源于PD-L1高表达的人群[4]。IMpower110研究评估了阿替利珠单抗对比标准化疗在PD-L1高表达人群(TC3/IC3)的疗效,结果显示免疫治疗和化疗的中位OS分别为20.0月和13.1月(HR=0.59, P=0.01)[5];EMPOWER-Lung1研究评估了PD-L1≥50%人群Cemiplimab对比化疗的疗效,结果显示Cemiplimab和化疗两组的OS分别为未达到(not reached, NR)和14.2月(HR=0.57, P=0.0002)[6],这两项研究再次验证了免疫单药治疗在PD-L1高表达人群中的疗效。目前,除了帕博利珠单抗以外,美国食品和药品监督管理局(Food and Drug Administration, FDA)也批准了阿替利珠单抗和Cemiplimab用于PD-L1高表达人群的一线治疗。

为了进一步提高PD-L1高表达人群免疫治疗的疗效,多项研究探索了免疫联合治疗的策略,WJOG10718L是一项单臂、Ⅱ期研究,评估了阿替利珠单抗联合贝伐珠单抗一线治疗PD-L1≥50%人群的疗效,结果显示,客观缓解率(objective response rate, ORR)为64.1%,无进展生存时间(progression-free survival, PFS)为15.9月[7],与免疫单药治疗的历史数据对比,免疫联合抗血管生成的治疗策略提高了ORR和PFS,需要Ⅲ期的临床研究进一步验证。KEYNOTE 598研究是一项随机对照的Ⅲ期研究,评估帕博利珠单抗联合伊匹木单抗对比帕博利珠单抗联合安慰剂治疗PD-L1高表达人群的疗效,结果显示,帕博利珠单抗联合伊匹木单抗组的中位PFS为8.2月,帕博利珠单抗联合安慰剂组的中位PFS为8.4月(HR=1.06, P=0.72),两组的OS分别为21.4月和21.9月(HR=1.08, P=0.74), PD-1抑制剂和CTLA-4抑制剂的联合治疗并没有提高PD-L1高表达人群的疗效,反而增加了不良反应,该研究因此也终止了入组[8],对于PD-L1高表达人群,双免疫联合治疗可能并不是一种合适的治疗策略。

1.2 所有人群中的免疫一线治疗

为了扩大免疫治疗的获益人群,在无论PD-L1表达状态的所有人群中,多种免疫联合治疗策略进行了探索。化疗是驱动基因阴性患者的治疗基石,临床前研究表明,化疗可以通过释放肿瘤细胞抗原、调节T细胞功能、重构免疫微环境和减少免疫抑制细胞等机制激活免疫反应,与免疫治疗联合使用具有协同作用[9-10]。在非鳞状NSCLC领域,KEYNOTE 021G这项Ⅱ期研究首先证实了无论患者PD-L1的表达状态如何,帕博利珠单抗联合标准化疗可以改善患者的ORR[11],随后KEYNOTE 189这项Ⅲ期研究再次证实了免疫联合化疗可以提高PFS和OS,更新的数据显示帕博利珠单抗联合化疗与单独化疗两组3年的OS率分别为31.3%和17.4%[12-13]。在鳞状NSCLC领域,KEYNOTE 407研究同样证实了帕博利珠单抗联合化疗在全部人群中改善了患者的PFS和OS[14],两组3年的OS率分别为29.7%和18.2%[15]。KEYNOTE 407研究中国扩展队列也首次报道了中国人群免疫联合化疗的数据,结果提示中国人群数据与全球数据保持一致[16]。近两年国产的PD-1/PD-L1抑制剂也有多项研究报道了结果,在非鳞状NSCLC领域,CAMEL研究证实了卡瑞利珠单抗联合化疗可以改善PFS和OS,随后ORIENT 11研究、RATIONAL 304研究证实信迪利单抗、替雷利珠单抗联合化疗也改善了患者的PFS,OS数据仍不成熟[17-19]。在鳞状NSCLC领域,ORIENT 12研究、RATIONAL 307研究、CAMELsq研究也均获得了成功[20-22]。目前,免疫联合化疗已经成为晚期NSCLC一线标准的治疗策略,并且这种联合策略在所有人群中均初步看到了长期生存的曙光。

抗血管生成药物贝伐珠单抗联合化疗是免疫药物问世以前标准的一线治疗方案,在抗血管联合化疗的基础上加上免疫治疗是否会取得更好的疗效?基础研究显示,抗血管生成药物作用于肿瘤微环境,可以降低骨髓来源的抑制细胞(myeloid-derived suppressor cells, MDSCs)和调节性T细胞(T-regulatory cell, Tregs)的活性,同时使肿瘤血管结构正常化,促进T细胞浸润肿瘤,抗血管药物与免疫治疗联合应用可以提高抗肿瘤活性[23]。IMpower150研究是首个免疫联合抗血管及化疗的Ⅲ期研究,结果显示,与贝伐珠单抗联合化疗相比,阿替利珠单抗联合贝伐珠单抗及化疗可以改善患者的PFS和OS[24],因此,目前这种联合治疗也被FDA批准作为一线可选择的治疗方案。但是与免疫联合化疗的数据对比,目前这种四药联合策略尚未显示出足够的OS获益优势,考虑到四药联合治疗可能带来更高的不良反应,因此探索这种联合治疗的适合人群尤为重要。IMpower150研究亚组分析结果显示,对于肝转移以及肿瘤负荷较大的人群,更能从四药联合治疗中获益[25-26];生物标志物的探索性分析结果显示,在KRAS突变的患者中,应用这种联合治疗获益更为显著[27]。另一项应用纳武利尤单抗联合贝伐珠单抗及化疗对比贝伐珠单抗联合化疗的Ⅲ期研究也达到了主要终点PFS(12.1月vs. 8.1月, HR=0.56, P < 0.0001),再次验证了这种四药联合用药模式的疗效[28]。除了免疫联合大分子抗血管药物外,免疫联合小分子多靶点抗血管药物的研究也正在开展,如帕博利珠单抗联合仑伐替尼及培美曲塞/铂类治疗转移性非鳞状NSCLC患者的Ⅲ期研究正在进行中[29]。此外,一项ⅠB期的研究评估了安罗替尼联合信迪利单抗一线治疗NSCLC的疗效和安全性,结果提示ORR达到72.7%,中位PFS为15月,显示了颇具前景的初步疗效[30],未来免疫联合抗血管治疗可能为一线去化疗提供一种全新的治疗策略。

PD-1/PD-L1和细胞毒性T淋巴细胞相关蛋白4(cytotoxic T-lymphocyte-associated protein 4, CTLA-4)是目前最重要的两个免疫靶点,CTLA-4抑制剂可在免疫效应启动阶段促进T细胞增殖,阻断T细胞抑制信号,基础研究显示联合阻断PD-1/PD-L1和CTLA4通路存在协同抗肿瘤作用[31],并最早在黑色素瘤患者中证实了这种双免疫联合策略的抗肿瘤活性。CheckMate227是一项随机、对照Ⅲ期研究,主要评估了纳武利尤单抗联合伊匹木单抗对比含铂双药化疗一线治疗驱动基因阴性晚期NSCLC患者的疗效,Part 1最终分析结果显示,PD-L1≥1%患者接受纳武利尤单抗联合伊匹木单抗和标准化疗两组的OS为17.1月和14.9月,PD-L1 < 1%患者两组OS分别为17.2月和12.2月,提示无论PD-L1是否表达都可以从双免疫联合治疗中获益[32]。CheckMate227研究是晚期NSCLC中双免疫治疗首次获得阳性结果的Ⅲ期研究,基于此项研究结果,FDA也批准了纳武利尤单抗联合伊匹木单抗一线治疗PD-L1≥1%且基因阴性NSCLC患者的适应证,为一线免疫“去化疗”方案提供一种新的选择。虽然Checkmate227研究获得了成功,但并非所有的双免疫联合治疗均获得了一致的结果,MYSTIC和NEPTUNE这两项双免疫联合治疗的Ⅲ期研究没有达到主要终点,考虑原因可能与研究设计时选择的终点指标不同所致[33]

CheckMate227研究证实纳武利尤单抗联合伊匹木单抗改善了晚期NSCLC患者的OS,但是双免疫联合组和化疗组的两条生存曲线存在交叉现象,提示双免疫联合治疗存在着早期进展和死亡的患者。在双免疫联合治疗的基础上加上短疗程的化疗,能否达到快速的控制症状、减轻肿瘤负荷,让患者长期生存?CheckMate9LA这项Ⅲ期研究评估了纳武利尤单抗联合伊匹木单抗及2周期的含铂双药化疗对比标准化疗方案的疗效,结果显示,纳武利尤单抗联合伊匹木单抗及化疗的中位OS是15.6月,明显高于化疗组的10.9月(HR=0.66, 95%CI: 0.55~0.80)[34],FDA也批准了纳武利尤单抗联合伊匹木单抗及短疗程化疗一线治疗无驱动基因突变晚期NSCLC的适应证。

近年来,新型免疫靶点药物层出不穷,除了PD-1/PD-L1和CTLA4抑制剂以外,不断在探索新的肿瘤免疫靶点,包括针对共抑制性受体、协同刺激分子、肿瘤微环境中的免疫细胞等新型免疫药物的研究都在如火如荼进行中。TIGIT为免疫抑制性检查点受体,可以抑制T细胞和NK细胞介导的肿瘤杀伤功能,同时抑制TIGIT及PD-1受体具有协同杀伤肿瘤作用,增强抗肿瘤免疫应答。一项Ⅱ期研究评估TIGIT抗体Tiragolumab联合阿替利珠单抗对比安慰剂联合阿替利珠单抗一线治疗NSCLC的疗效,结果显示Tiragolumab联合治疗组的ORR为31.3%,安慰剂治疗组的ORR为16.2%,两组PFS分别为5.4月和3.6月[35]。该研究提示TIGIT抑制剂Tiragolumab联合阿替利珠单抗能够提高ORR和PFS,未来需要Ⅲ期研究进一步验证。

目前,多种一线免疫治疗方案获批于临床使用,包括免疫联合化疗、抗血管生成治疗、双免疫联合治疗、双免疫联合短程化疗等,在临床实践中如何选择合适的治疗策略是需要思考的问题,在进行治疗决策时,要充分考虑联合治疗的疗效、安全性、患者的生活质量等问题;此外,治疗方案对后续治疗的影响及经济效价比也是选择治疗策略的重要参考因素。

2 晚期NSCLC二线/后线免疫治疗进展 2.1 单药免疫治疗

免疫单药治疗最早在二线治疗实现了突破,KEYNOTE 010、CheckMate 017/057、OAK等几项Ⅲ期研究奠定了免疫单药在晚期NSCLC二线治疗中不可撼动的地位[36-39]。随着随访时间延长,免疫单药二线治疗部分人群显示了长期生存获益,CA209-003研究纳武利尤单抗治疗5年的OS率为16%[40],KEYNOTE 001研究帕博利珠单抗在经治人群中5年OS率为15.5%[41],CheckMate 017/057研究中纳武利尤单抗组的5年OS率为13.4%[42]。CheckMate 078研究是首个报告中国肺癌人群免疫二线治疗的Ⅲ期研究,结果显示,纳武利尤单抗和多西他赛组的OS分别为11.9月和9.5月(HR=0.75, 95%CI: 0.61~0.93),更新的3年OS率分别为19%和12%[43]。ORIENT 03研究和RATIONAL 303研究结果也显示信迪利单抗或替雷利珠单抗对比多西他赛二线治疗NSCLC,显著延长了OS [44],目前,免疫单药治疗已经成为二线治疗的标准治疗方案,且部分人群能够得到长期生存。

2.2 免疫联合治疗

尽管免疫单药二线治疗改善了生存状态,但其客观缓解率并不令人满意,为了进一步提高免疫治疗的疗效,多项研究探索了免疫联合治疗的策略。一项卡瑞利珠单抗联合阿帕替尼在二线及后线治疗晚期非鳞NSCLC的Ⅰ/Ⅱ期临床研究数据显示,免疫联合抗血管治疗的ORR为30.8%,PFS为5.9月[45],该结果要优于免疫单药治疗的历史数据。另一项Ⅱ期PROLUNG研究,评估帕博利珠单抗联合多西他赛对比单药多西他赛二线治疗NSCLC的疗效,结果显示免疫联合化疗的ORR为42.5%,明显优于单药多西他赛组的15.8%,联合治疗组和单药多西他赛组的PFS分别为9.5月和3.9月[46],免疫联合抗血管治疗或联合化疗是潜在可选择的治疗策略。

有多项研究对免疫治疗与新型靶点药物的联合进行了探索,KN046是一种PD-L1和CTLA-4的双克隆抗体,在一项Ⅱ期、多中心的KN046-201临床研究中,评估KN046在晚期NSCLC患者中的有效性和安全性,结果显示,KN046二线治疗NSCLC的PFS为3.68月(95%CI: 3.35~7.29),鳞癌和非鳞癌患者中PFS分别为7.29月和3.58月,与PD-1/PD-L1单药治疗的历史数据相比,体现出获益优势[47]。M7824是TGF-β和PD-L1双克隆抗体,其二线治疗NSCLC的ORR为23.8%,中位DOR为18月,2年的OS率为34.6%[48],双克隆抗体在二线治疗显示了具有前景的疗效。

癌症疫苗是近年来一种新兴的肿瘤免疫疗法,通过识别特定癌细胞上存在的蛋白质,激活机体自身抗肿瘤免疫,进而杀伤和清除肿瘤细胞。在肺癌领域,癌症疫苗显示了初步的疗效,新型肿瘤疫苗OSE-2101在NSCLC的Ⅲ期临床试验中取得成功,既往接受过PD-1/PD-L1免疫治疗失败的患者,接受肿瘤疫苗OSE-2101治疗后,与化疗相比,12月生存率提高10%,1年生存率达到46%,超过了预设的25%指标[49]。目前多种癌症疫苗在研发过程中,未来可能为NSCLC免疫治疗增添更多的治疗选择。通过上述研究结果,我们初步看到了免疫联合策略在二线及后线治疗中的前景,未来还需要前瞻性的Ⅲ期研究进一步证实。

3 EGFR突变人群免疫治疗进展

表皮生长因子受体(epidermal growth factor receptor, EGFR)和间变性淋巴瘤激酶(anaplastic lymphoma kinase, ALK)等驱动基因阳性人群能否使用免疫治疗仍存在争议。多项研究显示,与EGFR野生型患者相比,EGFR突变型患者PD-L1的表达水平更低,肿瘤突变负荷的数值更低,且突变型患者肿瘤浸润性淋巴细胞的水平较低,这种独特的肿瘤微环境决定了EGFR突变人群免疫治疗效果欠佳[50-52]。一项荟萃分析比较了在EGFR突变人群中二线应用多西他赛和PD-1/PD-L1抑制剂的疗效,结果显示免疫治疗的疗效并未优于多西他赛化疗[53]。ATLANTIC研究首次前瞻性评价了EGFR/ALK突变阳性患者免疫治疗的疗效,结果显示,度伐利尤单抗在三线及以上治疗基因突变患者的ORR在PD-L1≥25%人群中为12.2%,在PD-L1 < 25%人群中仅为3%,两组的OS分别为13.3月和9.9月,提示PD-L1表达阳性的人群免疫治疗可能效果更优[54]

为了提高EGFR突变人群免疫治疗的疗效,有研究探索了应用特瑞普利单抗联合培美曲塞及铂类治疗EGFR敏感突变且靶向治疗耐药后的人群,结果显示,ORR为50.0%,中位PFS为7.0月[55],免疫联合化疗成为一种颇具前景的治疗策略,目前免疫联合化疗的几项Ⅲ期研究也正在进行中,包括KEYNOTE789、CheckMate722和TREASURE等。免疫联合抗血管生成治疗及化疗是另一种具有前景的治疗策略,IMpower150研究EGFR突变亚组人群结果显示,贝伐珠单抗联合阿替利珠单抗及紫杉醇/卡铂方案与贝伐珠单抗联合紫杉醇/卡铂方案比较,两组的PFS分别为9.7月和6.1月,OS分别为29.4月和18.1月[25],结果提示免疫联合抗血管生成治疗及化疗对于EGFR突变人群是一种有效的治疗策略,目前也有ORIENT 31这项Ⅲ期临床研究正在进行中。未来,对于EGFR突变且TKI耐药的人群,采用免疫联合治疗可能是一种具有前景的治疗策略。

4 总结与展望

免疫治疗使NSCLC进入了更好的时代,免疫单药治疗成为PD-L1高表达人群一线治疗的标准方案,免疫联合治疗策略进一步实现了全人群的覆盖,免疫联合化疗、免疫联合抗血管生成治疗及化疗、双免疫联合治疗以及双免疫联合短程化疗均已成为驱动基因阴性晚期NSCLC一线治疗可选的治疗策略;免疫单药治疗是NSCLC二线治疗的基石,免疫联合化疗、抗血管治疗可能带来更好的疗效,需要在Ⅲ期研究中进一步验证;在EGFR突变阳性患者中免疫联合策略也显示了极具前景的疗效,未来值得期待;此外,新的靶点药物、双克隆抗体以及癌症疫苗等新型免疫药物的研发将进一步助力免疫治疗在NSCLC领域的发展,未来免疫治疗将造福更多的肺癌患者。
