哮喘是一类影响全球三亿多人的慢性气道炎症性疾病, 主要症状包括气道高反应性和可逆性气流受限, 并引起反复发作的喘息、气急、胸闷或咳嗽等[1]。哮喘是由多种炎症细胞, 如嗜酸性粒细胞、T细胞、肥大细胞、中性粒细胞和树突状细胞及多种细胞组分参与的慢性气道炎症性疾病[2]。根据诱导痰细胞的比例, 哮喘可以分为4个亚型:嗜酸性粒细胞性哮喘、中性粒细胞性哮喘、混合(嗜酸性粒细胞与中性粒细胞)性哮喘以及寡细胞性哮喘[3]。痰液中嗜酸性粒细胞至少占3%为嗜酸性粒细胞性哮喘[4]。关于中性粒细胞性哮喘, 目前尚没有确切的标准。Nair等[5]认为痰液中性粒细胞数占65%以上并且嗜酸性粒细胞占3%以下即为中性粒细胞性哮喘。痰细胞中嗜酸性粒细胞和中性粒细胞的数量均高为混合性哮喘, 而两者处于正常值范围的哮喘则为寡细胞性哮喘[6]。中性粒细胞性哮喘患者的主要特点如下:相对糖皮质激素抵抗、较长的吸烟史、易感染病毒、痰液中性粒细胞数目增多以及IL-8分泌过多等。不同于其他类型哮喘, 中性粒细胞性哮喘无基质膜变厚现象[7]。另外, 中性粒细胞也参与其他各类哮喘的发展过程[8]。
中性粒细胞在非特异性细胞免疫系统中起着十分重要的作用, 它处于机体抵御微生物病原体, 特别是化脓性细菌入侵的第一线。当炎症发生时, 它们被趋化性物质吸引到炎症部位发挥作用[9]。活化的中性粒细胞可以释放很多物质, 如氧自由基、多种蛋白酶、颗粒以及一些炎症因子等, 这些物质可能影响气道结构和功能。IL-8等中性粒细胞的趋化剂可以持续招募中性粒细胞到肺泡腔, 使得肺实质细胞损伤, 肺间质结构破坏以及气道损伤[10]。本文对中性粒细胞在哮喘中的作用加以阐述。
1 哮喘患者气道中性粒细胞炎症产生的原因气道炎症是哮喘的重要特征之一, 并与哮喘的发病密切相关。以嗜酸性粒细胞为主的气道炎症已有广泛研究, 然而对于气道积累过量的中性粒细胞在哮喘发病中的作用研究较少。有研究发现, 在重症哮喘与急性加重期哮喘患者中, 中性粒细胞数量增多的现象更为明显。在重症哮喘患者中, 有80%的患者其痰液中性粒细胞增多; Wenzel等[11]发现, 相对于健康者, 重症哮喘患者的肺泡灌洗液中中性粒细胞数量较多; 突发性死亡的哮喘患者中, 其气道黏膜下层中性粒细胞数目远大于嗜酸性粒细胞[5]。一些重症哮喘患者也会表现混合中性粒细胞和嗜酸性粒细胞的炎症[12]。
哮喘患者中性粒细胞数量增多的原因主要包括以下几个方面:
1.1 中性粒细胞相关趋化因子表达增加在某些哮喘患者肺泡灌洗液和血浆中检测到中性粒细胞相关的趋化因子表达显著上调[13]。哮喘患者血液中循环的中性粒细胞需要在炎症因子的刺激下才可以趋化到炎症部位。促使中性粒细胞迁移到肺部的相关趋化剂主要有白三烯B4(LTB4)、IL-8、TNF-α等, 其中大部分由支气管上皮细胞所分泌。哮喘患者气道中性粒细胞数量增多主要是因为IL-8和TNF-α的表达升高[14]。
1.2 中性粒细胞呈抗凋亡性哮喘患者气道中性粒细胞呈现抗凋亡的特性, 中性粒细胞寿命的延长, 导致其在气道中的数量增多。Nair等[5]通过控制哮喘患者激素摄入量发现, 摄入高剂量激素类药物的哮喘患者血液和痰液中性粒细胞数量增多, 但其活性与轻度哮喘患者相比并无太大差异。激素类药物通过抑制中性粒细胞的凋亡延长寿命, 因而使得哮喘患者体内中性粒细胞数量增多。此外, 机体发生炎症时, ATP可以通过自分泌或旁分泌的形式作用于中性粒细胞提高其生存活性[15]。哮喘患者痰液中抗凋亡的中性粒细胞可能是由死亡的气道细胞释放的ATP所致。另有研究证实, 中性粒细胞自身也可以在低氧条件下释放ATP[13]。
1.3 中性粒细胞清除受到限制巨噬细胞能够通过吞噬来清除入侵的微生物、自身变异的细胞和死亡细胞的残骸。衰老的中性粒细胞会被巨噬细胞吞噬而清除[16]。哮喘患者巨噬细胞的吞噬能力下降, 导致中性粒细胞的清除受限, 因而中性粒细胞数量增多[17]。
此外, 气道中性粒细胞的数量也会受一些其他因素的影响, 如年龄、饮食、吸烟、大气污染物、气道菌群等[18]。
2 中性粒细胞的功能与哮喘的发生、发展 2.1 哮喘患者中性粒细胞的功能改变研究表明, 中性粒细胞在哮喘患者中的功能受到了一定的影响, 中性粒细胞功能的改变对于调控哮喘的发展过程具有重要的意义[14]。当组织发生炎症时, 循环系统中的中性粒细胞可以被招募到炎症部位发挥作用。中性粒细胞的极化、黏附、趋化、吞噬、活性氧的释放以及脱颗粒等都在这一过程中发挥重要作用[19]。临床研究显示, 哮喘患者的中性粒细胞的趋化活性与健康者相比明显增强, 这可能是由于中性粒细胞对于刺激剂IL-8的敏感性提高[20]。此外, 在微生物的刺激下, 中性粒细胞可以产生活性氧, 丰富的氧自由基可以起到杀菌的作用[21]。哮喘患者中性粒细胞活性氧的释放受到一定的抑制, 这可能导致哮喘患者易被病原体感染。然而轻度哮喘患者中性粒细胞活性氧的产生相比健康者更为强烈, 过多活性氧的产生可能导致哮喘加重和气道结构重构[22]。
2.2 中性粒细胞作为调节细胞影响哮喘的发展过程中性粒细胞作为哮喘的调节细胞, 可以侵润到气道中, 释放一系列的细胞因子和炎症介质, 调节其他细胞的功能, 从而在哮喘中发挥作用[10]。活化的中性粒细胞可以分泌IL-6、IL-8、IL-13、干扰素γ、粒细胞巨噬细胞集落刺激因子(GM-CSF)、TGF-β等[23]。TGF-β可以促使气道成纤维细胞的增殖[24]。由中性粒细胞产生的制瘤素M (oncostatin M, OSM)能够促进气道平滑肌细胞和成纤维细胞的增殖, 同时, 也可以刺激气道细胞产生血管内皮生长因子(vascular endothelial growth factor, VEGF)。OSM与VEGF协同作用能够促进气道重构[25]。此外, 参与气道重构的其他细胞因子如LTB4和血小板活化因子(PAF)也可由中性粒细胞产生[26]。中性粒细胞释放的基质金属蛋白酶9(MMP-9)、弹性蛋白酶、髓过氧化物酶等也参与哮喘气道的重构[27]。在哮喘患者的肺泡灌洗液、痰液以及血清中检测到大量的MMP-9可以诱导嗜酸性粒细胞迁移到气道从而参与哮喘的发展。有研究发现, IL-8刺激的中性粒细胞可以导致嗜酸性粒细胞在气道积累, 这一现象可能通过MMP-9、LTB4、PAF等调控完成[12]。
3 辅助性T (Th)17/Th1细胞调控中性粒细胞炎症大部分哮喘气道炎症是由Th2型细胞因子调控的气道嗜酸粒细胞所介导的, 但近期研究表明, 气道中性粒细胞炎症主要由Th17和Th1细胞调控[28]。当气道受到过敏原刺激时, T细胞被激活, 进而Th17和Th1细胞被激活, Th17细胞分泌Th17A和Th17B导致中性粒细胞数量增多和活化, 活化的Th1细胞产生的干扰素γ和TNF-α可能也参与其中[29]。
在重症哮喘患者中, 特别是中性粒细胞性气道炎症患者中IL-17A和IL-17E分泌增多。IL-17可以刺激气道上皮细胞和成纤维细胞产生大量的中性粒细胞趋化相关因子, 如IL-8、GM-CSF等[30]。这些炎症反应可能是由包含IL-17受体A和受体C的受体复合体所介导的NF-κB信号通路引起[31]。有文献报道, Th17在中性粒细胞性哮喘中的作用可能与NLRP3(NACHT, LRR and PYD domains-containing protein 3)炎性小体相关, NLRP3炎性小体由NOD样受体蛋白3(NLRP3)、接头分子凋亡相关斑点样蛋白9(apoptosis-associated speck-like protein containing a CARD, ASC9)及效应分子酶原1(procaspase-1)组成[32], 在中性粒细胞性哮喘患者中, NLRP3炎性小体与其活化剂血清淀粉样蛋白A (SAA)表达上调。哮喘患者易受到细菌或真菌感染, 这些细胞或真菌成分可以作为病原体相关分子模式(PAMP)被Toll样受体所识别, 进而激活NF-κB转录因子产生前IL-1β和前IL-18。NLRP3炎性小体可以通过活化caspase-1剪切前IL-1β和前IL-18, 使其转变为成熟形式发挥作用[33-34]。大量IL-1β的产生可以促进Th17细胞的活化, 进而分泌更多的Th17A和Th17E[35]。
除Th17细胞外, Th1细胞在中性粒细胞性哮喘中也发挥着一定的作用。研究发现, 重症哮喘患者中Th1细胞来源的细胞因子干扰素γ和TNF-α表达量较高[35]。TNF-α能够使内皮细胞细胞间黏附分子-1(ICAM-1)的表达增加, 使得中性粒细胞与内皮细胞的黏附能力增强, 最终导致更多的中性粒细胞迁移到气道中[36]。干扰素γ可以抑制中性粒细胞凋亡, 延长其寿命[13]。
4 中性粒细胞性哮喘的治疗对于中性粒细胞性哮喘患者, 吸入性激素类药物治疗的疗效较差, 而新型小分子药物由于具有更强的特异性, 成为治疗中性粒细胞性哮喘的热点, 如特异性的拮抗中性粒细胞趋化相关因子与受体结合的CXCR2拮抗剂、阻碍其合成的5-脂氧合酶活化蛋白(FLAP)抑制剂等。生物制剂在疾病缓解方面具有起效快、耐受性好的优势, 因此也备受青睐, 如IL-17阻断剂和TNF-α阻断剂能够抑制Th17/Th1所介导的中性粒细胞炎症, 从而缓解中性粒细胞性哮喘。
4.1 新型小分子药物 4.1.1 CXCR2拮抗剂阻碍中性粒细胞趋化因子与CXCR2的结合CXCR2受体表达于中性粒细胞表面, 可以通过与配体CXCL3、CXCL5、CXCL6(IL-6)、CXCL-7及CXCL-8(IL-8)结合来调控中性粒细胞的趋化[37]。CXCR2拮抗剂AZD8309可以缓解脂多糖诱导的气道中性粒细胞性炎症[38]。有研究发现, CXCR2拮抗剂SCH527123能够减少哮喘患者痰液中的中性粒细胞数目和提高其哮喘控制问卷(ACQ)分数[39]。另外一种CXCR2抑制剂AZD5069与激素药物或是LABA (β受体激动剂)混用, 虽然可以减少持续性哮喘患者痰液中性粒细胞数目, 但并不能缓解其病情[40]。关于CXCR2受体拮抗剂在哮喘方面的作用还需要进一步评估。
4.1.2 FLAP抑制剂阻碍中性粒细胞趋化剂LTB4的合成中性粒细胞趋化剂LTB4的合成需要FLAP[41]。实验证明, FLAP拮抗剂GSK2190915可以抑制体内LTB4的形成, 但其并不能减少哮喘患者痰液的中性粒细胞数量及缓解哮喘病情[42]。
4.2 生物制剂 4.2.1 IL-17阻断剂抑制IL-17与IL17RA的结合关于IL-17的单克隆抗体已经进入临床研究。如布罗达单克隆抗体(brodalumab)可以与IL-17RA受体结合以此阻断IL-17A、IL-17F、IL-17A/IL-17F异二聚体以及IL-25的活性, 缓解了部分哮喘患者的病情, 但并未达到治疗哮喘药物的指标[43]。有研究发现哮喘患者服用激素类药物会导致大量Th17细胞和中性粒细胞寿命变长, 提示依赖于IL-17的中性粒细胞哮喘可能是由于摄入了高剂量激素类药物所致[44]。其他因素如大气污染或肥胖也与IL-17高表达的中性粒细胞性哮喘相关[18]。这些复杂因素导致探究IL-17在中性粒细胞性哮喘中的作用变得十分困难。
4.2.2 TNF-α阻断剂抑制TNF-α与其受体的结合使用TNF-α阻断剂能够阻碍TNF-α与其受体间的相互作用, 进而抑制下游的信号通路。TNF-α的抗体干预可以缓解哮喘患者的中性粒细胞炎症, 有效减少气道中性粒细胞数目和适度缓解气道高反应, 但因其不良反应较大不适合作为临床药物[45]。
5 结语哮喘是一种具有异质性的慢性炎症性疾病。较为严重的哮喘亚型患者气道更易发生中性粒细胞大量累积, 表明中性粒细胞在哮喘的发展中有着不可忽视的作用。目前对于中性粒细胞在哮喘中扮演的角色的认识具有一定的局限性。参与哮喘发生发展的因素较为复杂, 这导致研究中性粒细胞在哮喘中的作用有一定的困难。近年来有研究发现, 患有气道中性粒细胞炎症的哮喘患者显示系统性炎症, 血浆中IL-6、C反应蛋白有较高表达水平。而对于嗜酸性粒细胞炎症或寡细胞炎症的哮喘患者, 血浆中的IL-6或C反应蛋白的含量与健康者相比并无太大差异。该结果证实了气道中性粒细胞炎症会导致系统性炎症, 但系统性炎症对于中性粒细胞炎症是否具有一定的作用尚不清楚[46]。目前认为, 与依赖于Th2细胞的嗜酸性粒细胞炎症不同, 中性粒细胞炎症主要由Th17和Th1细胞所调控, 但具体作用机制还需进一步研究。对于中性粒细胞炎症的治疗, 吸入性的激素药物不敏感。更好地了解中性粒细胞在哮喘中的作用机制, 可以为治愈中性粒性细胞炎症哮喘提供有效的依据。
