哺乳动物包括人类的骨骼发育过程主要是通过维持破骨细胞骨吸收和成骨细胞骨形成的活性平衡。成骨细胞和破骨细胞是骨骼形成发育的两大关键性细胞。成骨细胞是来自多能的间充质干细胞(mesenchymal stem cells,MSCs),位于骨表面,占整体骨细胞的4%~6%,主要涉及骨骼形成过程[1]。成骨细胞分化是骨生成的前提,成骨细胞分化是一个复杂的过程,主要分为这三个阶段:骨祖细胞的增殖,细胞外基质的产生,基质的矿化[2]。这些阶段会表达相应成骨细胞的标志基因,包括Runt相关转录因子(Runt-related transcription factor 2,Runx2),Osterix(Osx),碱性磷酸酶(alkaline phosphatase,ALP),Ⅰ型胶原蛋白(collagen type Ⅰ,Colla Ⅰ),骨桥蛋白(osteopontin,OPN),骨钙蛋白(osteocalcin,OCN)等[3]。这些基因受局部和全身的信号因子行为的调控,如转化生长因子-β(transforming growth factor-β,TGF-β),骨形态蛋白(bone morphogenetic protein,BMPs),胰岛素样生长因子(insulin-like growth factor,IGF),血小板源性生长因子(platelet-derived growth factor,PDGF),β-连环蛋白(β-catenin),成纤维细胞生长因子(fibroblast growth factor,FGF)等。其中FGF是多效性的生长因子,在许多器官中它主要是通过与其受体(fibroblast growth factor receptors,FGFR)结合发挥着调控各种细胞的增殖,迁移,分化和凋亡的作用[4]。不管是在胚胎骨骼形成还是出生后骨骼发育过程中,FGF/FGFR信号都发挥着重要的作用。FGF/FGFR信号主要是通过调节涉及骨祖细胞复制,成骨细胞分化和凋亡的各种基因的表达来调控骨形成[5-6]。在此对FGF及FGFR家族,成骨细胞分化过程中标志性基因相应的标志物,FGF/FGFR信号调控成骨细胞分化作用等方面进行综述。
1 FGF及FGFR家族成纤维细胞生长因子(Fibroblast growth factor,FGF)是属于蛋白类家族,该家族成员共有23个,FGF1-23包括人和鼠的FGFs,其中鼠的FGF15与人的FGF19直接同源。FGFs大约由150~300个氨基酸组成,其中心区域是由120个氨基酸组成的保守核心区域,该核心区域具有高度同源性,30%~60%的氨基酸序列是相同的。迄今为止,FGFs由七个亚家族组成:FGF1/2、FGF3/7/10/22、FGF4/5/6、FGF8/17/18、FGF9/16/20、FGF11/12/13/14、FGF15/19/21/23。根据FGFs不同的作用机制可将其分为三类:内分泌(FGF15/19/21/23)、旁分泌(FGF1-10、FGF16-18、FGF20、FGF22) 和胞分泌FGF11/12/13/14 [7]。旁分泌FGFs在氨基和羧基末端分别具有分泌信号序列和硫酸乙酰肝素(heparan sulfate,HS)相关的结合位点。旁分泌FGFs是作为局部分泌信号通过扩散方式作用于附近的靶细胞。旁分泌FGFs调节生物活性的过程是利用HS作为辅助因子,特异性结合细胞表面的FGF受体(Fibroblast Growth Factor Recepter,FGFRs)。FGFRs主要包括4种:FGFR1、FGFR2、FGFR3、FGFR4。HS是参与FGF与FGFR相互结合不可缺少的重要组成部分,它有利于FGF与FGFR稳定结合,增加FGF-FGFR二聚体的亲和力从而提高受体活性[9-10]。不同于旁分泌FGFs,内分泌FGFs与HS结合能力较弱,导致该FGFs没有局部信号功能[7-8]。内分泌FGFs在氨基和羧基末端分别是具有分泌信号序列和Klotho结合位点,需要膜蛋白αKlotho/βKlotho作为辅助因子与FGFR相结合而发挥作用,而不是通过HS直接结合FGFR来发挥作用[9-10]。但是最终旁分泌和内分泌FGFs都是通过激活四种不同的FGFRs来展现其生物活性。许多研究已经发现FGFRs细胞内区域的酪氨酸磷酸化可激活细胞内关键的信号通路,包括促分裂素原活化蛋白激酶(mitogen-activated protein kinase,MAPK),磷脂酰肌醇3-激酶-Akt[Phosphoinositide 3-kinase (PI3K)-AKT,PI3K-Akt]和磷脂酶Cγ/蛋白激酶Cα(Phospholipase Cγ/Protein kinase Cα,PLCγ/PKCα)信号通路,其中MAPK信号通路中主要的三种信号分别是细胞外调节蛋白激酶1/2(extracellular regulated protein kinases1/2,ERK1/2),c-Jun氨基末端激酶(c-Jun N-terminal kinase,JNK)和p38丝裂原活化蛋白激酶(p38MAPK)[11]。大多数FGFs(除了FGF11-14) 都能与酪氨酸激酶受体发生特异性结合,从而激活FGFR信号,并启动该信号转导的各种途径,最终发挥着相关生物学功能。FGFs在骨骼发育中发挥着重要的作用,大多数研究已经表明旁分泌类的FGFs,比如FGF2在骨中很活跃。内分泌类FGFs中的FGF23对骨组织也起重要作用。FGF2和FGF23在成骨细胞系中表达,并与其受体FGFR结合转导细胞内的信号,对成骨细胞的增殖、迁移、分化和凋亡发挥重要作用[12]。胞分泌类的FGFs,FGF11-FGF14主要在神经内起作用,在骨中不活跃。
2 成骨细胞分化的相关基因及标志物成骨细胞分化的过程,主要是骨基质蛋白的不断合成和沉淀。这个过程受各种不同的形态发生素,激素,生长因子,细胞因子,细胞外基质蛋白等调控。这些外部信号主要是通过启动几种级联和转录因子来调节成骨细胞的分化作用。已知这几种转录因子可控制骨发育和成骨细胞分化,转录因子包含有Runx2,Osx,Dlx5,Msx2,fos和Twist,其中Runx2和Osx是成骨细胞分化中两个至关重要的转录因子[13]。Runx2是首个描述成骨细胞特异性的转录因子,并且是成骨细胞分化的关键因子。Komori等[14]和Otto等[15]研究发现Runx2敲除的小鼠显示成骨细胞分化停滞,并且导致成骨细胞和骨发育的缺乏,但是软骨细胞没有变化。Mundlos等[16]发现在人类骨骼发育过程中若Runx2杂合突变会引起颅骨锁骨发育不全(cleidocranial dysplasia,CCD),表现特征是锁骨发育不全,身材矮小,牙齿赘齿以及其他骨骼模式和生长的变化。同时杂合Runx2基因敲除的小鼠显示类似人类的CCD疾病。另一种转录因子Osx是一种含锌指的转录因子,其在成骨细胞不断分化过程中起主导作用。Osx属于转录因子Kruppel样家族的亚群(Subgroup,Sp),也称为Sp7,其特征是一个三锌指DNA连接结构域位于靠近蛋白质的羧基末端的转录因子[17]。研究报道敲除Runx2基因的小鼠,Osx没有表达,然而敲除Osx基因的小鼠,Runx2有表达,由此说明Runx2在Osx的上游起作用,Runx2表达先于Osx的表达,Runx2是激活Osx转录所必需的。转录因子相互协作可调控成骨细胞分化和功能特征性的几种相关基因表达。在成骨细胞分化过程中的陆续表达基因,包括ALP,Colla Ⅰ,OPN,OCN等[18]。ALP是同源二聚体糖蛋白,在成骨细胞分化中ALP的作用是产生磷酸,与钙生成磷酸钙沉积于骨中,有利于成骨细胞的成熟,钙化,是成骨细胞分化早期的标志物之一。另一个早期分化标志物是Colla Ⅰ,它可促使细胞外基质不断地成熟,进一步形成矿化结节,并且胶原不断地合成、成熟,可加快成骨细胞表型分化。OPN是成骨细胞分化中期的标志物,它存在于骨基质中,是属于所占比重最大的非胶原蛋白。OPN具有一种特别的细胞粘附序列——精氨酸-甘氨酸-天冬氨酸(RGD)序列,该序列能与细胞或羟基磷灰石表面相互结合,有利于羟基磷灰石晶体的形成,促进钙化物沉积,加快骨形成。OCN又称为骨钙素(Bone gla protein,BGP),两者可结合写成OCN/BGP,是成骨细胞分化过程中合成和分泌的另一类非胶原蛋白,可以特异性地与Ca、Pi相结合,产生羟基磷灰石结晶,进入矿化期,是成骨细胞成熟的标志物,也是成骨细胞分化晚期的标志物。
3 FGF/FGFR信号调控成骨细胞分化的作用骨形成是一个复杂的过程,主要是涉及间充质细胞分化过程,首先是间充质细胞逐步分化为前体成骨细胞,随后形成成熟骨形成细胞,这个过程受转录因子,系统激素,局部生长因子以及细胞-细胞和细胞-基质之间的相互作用的控制[19]。研究表明FGF/FGFR信号在成骨细胞中起重要作用,其在成骨细胞中的作用是复杂的,因为它们取决于FGF和FGFR表达的类型、细胞成熟阶段、各种微环境(蛋白聚糖和蛋白相互作用),这些因素可能增强或减弱FGF/FGFR在成骨细胞中的作用。如FGF2信号促进前体成骨细胞增殖,从而扩大了该细胞群,进而使得成骨细胞在骨环境中所占比重增大[20]。据Jeon等[21]报道外源FGF7通过激活ERK-Runx2信号通路刺激胚胎干细胞(Embryonic stem cells,ESCs)中的成骨细胞分化,但是对成骨细胞的增殖不起作用。Lin等[22]发现FGF8可刺激连接蛋白43(Connexin 43,Cx43) 表达,该蛋白主要存在于成骨细胞中,在一些情况下调节成骨细胞的增殖和分化,因此FGF8可在成骨细胞分化中通过调节Cx43发挥作用。近年数据显示,FGF18也是一个成骨细胞分化过程的必不可少的自分泌阳性调节因子,介导ERK1/2和PI3K信号激活FGFR1和FGFR2 [23-24]。除了体外研究,还有一些体内的相关研究报道,在胚胎发育和出生阶段,小鼠的遗传研究揭示了FGF信号转导在成骨细胞谱系细胞中的多种作用。Montero等[25]研究发现缺乏FGF2小鼠在成年阶段,显示总骨量减少,这是由于前成骨细胞增殖减少和成骨细胞分化功能下降。之后Ohbayashi等[26]报道在缺乏FGF18小鼠胚胎中显示成骨间充质细胞增殖下降,cells不能形成成熟成骨细胞。
FGFR表达和功能可调控成骨细胞谱系分化的各个阶段。在间充质细胞和骨祖细胞中,FGFR1和FGFR2在维持间充质细胞的多能性方面起重要作用,主要是通过抑制细胞衰老[27]。体内研究证实FGFR1控制成骨细胞的不同成熟阶段。在骨祖细胞中FGFR1失活会导致细胞增殖增加,但会延迟分化和基质矿化,而在正在分化的成骨细胞中FGFR1缺乏会增强矿化和增加FGFR3的表达[28]。FGFR2也在成骨细胞分化中发挥着重要作用。Yu等[29]研究发现若是小鼠出现有条件缺乏FGFR2或FGFR2c、具有突变或间充质FGFR2的剪接变体这几种情况的话都会表现出Runx2表达减少,使得迟发性骨化。之后Miraoui等[30]在间充质细胞中研究发现组成型FGFR2激活ERK1/2和PKCα信号进而增加成骨细胞分化和减少脂肪细胞分化,促进Runx2表达。这说明FGFR2是成骨细胞成熟的正向调控因子。尽管FGFR3主要在胚胎发育软骨细胞中表达,但也有相关报道称缺乏FGFR3的小鼠会导致成骨细胞成熟和小梁骨的矿化减少,进而出现骨质减少的症状[31],这说明FGFR3在出生后骨代谢的成骨细胞发育中也起到重要作用。来源于转录活化蛋白1缺失(signal transducers and activators of transcription,Stat1-/-)小鼠的成骨细胞通过FGFR1或FGFR2起作用,表现为FGFR3表达减少和FGF18的表达增加,说明改变FGFR表达的组成部分会影响反馈回路,它可控制FGF依赖性成骨细胞生成[32]。FGF结合细胞表面硫酸类肝素蛋白多糖(heparan sulfate proteoglycan,HSPGs),它是FGF/FGFR的共同受体,可增强FGF/FGFR结合。早期研究表明,一些HSPG如多配体聚糖(syndecans)在骨生成期间可在骨环境中表达,影响成骨细胞对FGF2的反应。体外数据显示细胞表面分泌的HSPGs可以进一步扩增FGF2信号传导和成骨细胞分化[33-34]。因此,通过FGF/FGFR信号调控成骨细胞分化不仅依赖于FGF和FGFR的所有组成成分表达,也依赖于调节细胞信号传导和FGF/FGFR生物活性的辅助细胞外因子的表达。
在成骨细胞中,FGF/FGFR信号可调控与成骨细胞相关信号通路。其作用机制是FGF与FGFR的结合会引起FGFR二聚化,使得内源性酪氨酸残基磷酸化,进一步促成几种信号传导通路的活化[10]。FGFR酪氨酸激酶结构域激活促使成纤维生长因子受体底物2(fibroblast growth factor receptor substrate2,FRS2α)直接磷酸化和其他衔接蛋白结合,包括PLCγ,信号转导物,激活物的转录1(Stat)。FRS2α的磷酸化招募衔接蛋白生长因子受体结合蛋白2(growth factor receptor-bound protein 2,GRB2),该蛋白可通过接头蛋白Gab1(GRB2-associated-binding protein 1,GAB1) 作用于SOS蛋白(SOS蛋白是编码鸟苷释放蛋白的基因sos的产物,son of sevenless)和PI3K/Akt通路,进而激活MAKP通路[35-36]。SOS和PI3K,FGF的下游可以激活几种不同的MAPK途径,包括ERK1/2,JNK和p38 MAPK[11]。在成骨细胞谱系中,FGFR-介导激活ERK1/2,PLCγ/PKCα和PI3K/Akt信号调控成骨细胞不同分化阶段[11]。在早期前体成骨细胞中,FGFR信号主要激活ERK1/2增加细胞增殖[37]。在更成熟的细胞中,FGF2激活ERK1/2调控Runx2的表达,促进成骨细胞分化[38]。在人类成骨细胞中,FGF6一方面可通过激活ERK信号作用于细胞增殖,另一方面可激活Akt信号影响细胞分化[39]。在胚胎干细胞中,FGF7通过激活ERK/Runx2信号促进成骨分化。在前成骨细胞(MC3T3-E1) 中,FGF8激活ERK1/2-MAPK信号从而刺激细胞分化。在小鼠前体成骨细胞中,FGF18-介导FGFR1/FGFFR2的激活可诱导ERK1/2信号和PI3K信号参与Runx2表达和成骨细胞分化[23]。
4 其他信号通路调控成骨细胞作用 4.1 BMP信号除了FGF/FGFR信号直接作用于成骨细胞外,FGF也可与其他信号相互作用来调控骨生成,比如骨形成蛋白(bone morphogenetic protein,BMP)信号[40]。Naganawa等[41]报道缺乏FGF2小鼠,其骨组织表达BMP减少。Jiang等[42]研究颅骨骨化期间的成骨细胞发现FGF2治疗可刺激BMP2基因表达。在人和小鼠骨细胞中,BMP2与FGF2相互作用可刺激成骨细胞功能。在成骨细胞中,FGF2是通过促进Runx2的核定位和Smad1/5/8蛋白(Drosophila mothers against decapentaplegic protein1/5/8,Smad1/5/8) 磷酸化来增强BMP2信号传导[43]。在体外和体内成骨细胞研究,发现FGF2、FGF9和FGF18都会抑制BMP拮抗剂(Noggin)的表达,从而影响细胞功能作用[44]。因此,具有多种机制调节FGF和BMP信号相互作用,进一步调节成骨细胞分化。
4.2 Wnt/β-catenin信号遗传和功能研究表明FGF/FGFR与Wnt蛋白/β-连环蛋白(Wnt/β-catenin)信号功能性地相互作用可以调控成骨细胞的分化。Felber等[45]报道FGF/FGFR和Wnt/β-catenin信号相互作用作用于前成骨细胞促进骨形成,共同刺激Osx基因表达。Maruyama等[46]发现在颅缝发育期间Wnt/β-catenin信号通过平衡FGF/FGFR和BMP信号通路的活性来控制干细胞更新,增殖和分化。在骨祖细胞中,发现FGF1拮抗Wnt/β-catenin信号传导,导致成骨细胞的分化被阻断[47]。同时在体内研究发现来自FGF2-/-的小鼠,在其成骨细胞中Wnt基因表达显著地减少,显示成骨细胞分化减少。Fei等[48]进一步研究发现,外源性FGF2可促进β-catenin的核积累,使得FGF2-/-的小鼠成骨细胞可分化,表明在成骨细胞中内源性FGF2是通过激活Wnt/β-catenin信号,从而促进细胞分化。
4.3 PTH信号FGF/FGFR信号也可与甲状旁腺素(parathyroid hormone,PTH)信号相互协调作用于成骨细胞。Hurley等[49]在前成骨细胞MC3T3-E1和新生小鼠颅骨中研究发现PTH可调控FGF2和FGFR表达,进而调控成骨细胞。Sabbieti等[50]研究证实内源性FGF2有利于PTH作用于成骨细胞增殖、分化和凋亡,FGF2和PTH共同作用可以增加Runx2蛋白表达。这些研究表明FGF/FGFR信号自身以及与其他主要信号通路相互作用对调控成骨细胞分化发挥关键作用。
5 FGF与颅缝早闭功能获得性FGFR突变会诱导颅骨缝合的过早骨化(颅缝早闭)出现Apert,Crouzon和其他综合征[51-52]。分子机制显示FGFR突变是复杂的,其包括组成型或配体依赖性FGFR激活功能丧失和细胞受体的运输改变[53]。遗传和功能研究表明,FGFR突变可引起信号通路和下游靶基因的激活进而诱导过早缝合融合。研究者们[52, 54-55]通过几种老鼠模型的不同成骨细胞Apert FGFR2(S252W)突变研究,发现它们可激活ERK1/2,p38 MAPK,AKT,β-catenin或PLCγ的表达,进而促成过早颅骨骨化。在小鼠中,颅缝过早融合会增加Runx2表达。在人颅缝早闭中,Apert FGFR2(S252W)突变促进成骨细胞基因表达和颅骨成骨,主要是与激活PLCγ和PKCα信号有关。此外,在人类成骨细胞中Apert FGFR2(S252W)突变c-CBL(Casitas B-Linesge lymphoma gene CBL gene)基因募集引起Src基因(sarcoma gene)家族成员LYN和FYN,还有α5β1整合素的泛素化和下调,促使成骨细胞分化和凋亡增加[56]。在Apert成骨细胞中,增加c-CBL的募集也会引起PI3K泛素化和降解,导致PI3K信号的衰减进而降低成骨细胞活性[57]。因此,FGFR-介导相关信号通路的活化和调节相关基因的下调会诱导颅缝早闭。
6 结语在骨骼发育过程中,成骨细胞分化起着重要作用,它是骨骼形成的重要前提阶段。如果成骨细胞分化出现异常,会使得骨代谢异常,从而导致各种骨骼疾病如颅缝早闭,骨质疏松和异位骨化等。在成骨细胞分化过程中,FGF/FGFR信号具有正负两方面的调控作用。除了自身发挥作用外,FGF信号也可与BMP,Wnt和PTH等信号相互作用,它们主要是作用于Runx2和Osx两个关键的转录因子,组成复杂的调控关系,一起参与调节成骨细胞分化,影响成骨细胞分化不同阶段的各个基因的表达。尽管目前FGF/FGFR信号对成骨细胞影响研究取得了很大进展,但是对于它在调控成骨细胞分化方面的具体机理研究还有待提高。今后通过研究人员不断的努力研究,会使得这方面研究机理更加清楚明了。从而为临床治疗和预防由成骨细胞分化异常引起的相关骨骼疾病(颅缝早闭,骨质疏松和异位骨化等)提供理论依据。
