肺炎球菌结合疫苗与其他儿童常规免疫疫苗同时接种对免疫原性和安全性影响

摘要:婴幼儿期是免疫规划的关键时期,需要接种的疫苗较多,为了使免疫程序更为便捷,可在不干扰疫苗免疫原性和安全性的情况下,将多种疫苗同时接种。肺炎球菌结合疫苗(PCVs)是预防儿童肺炎球菌性疾病的重要手段,在全球儿童中广泛应用。本文对PCVs与其他儿童常规免疫疫苗同时接种的免疫原性及安全性进行了系统综述,发现PCVs与用于预防百日咳、白喉、破伤风、麻疹、风疹、腮腺炎、水痘、脊髓灰质炎、甲肝、乙肝、C群脑膜炎球菌、ACWY群脑膜炎球菌、b型流感嗜血杆菌等疾病的疫苗同时接种对PCVs本身及其他疫苗的免疫原性均不产生明显影响,且具有良好的安全性。在儿童免疫规划过程中,可将PCVs与其他常规免疫疫苗同时接种,从而在不降低疫苗免疫原性和安全性的情况下,减少接种次数,提高免疫效率。

Impacts of simultaneous vaccination of pneumococcal conjugate vaccine and other routine vaccines on immunogenicity and safety of the vaccines

Abstract: Infancy is an important period for the implementation of planned immunization and various vaccines should be administered during this period. In order to make the immune process more convenient, several vaccines may be administered concomitantly under the condition of no interference on immunogenicity and safety. Pneumococcal conjugate vaccines (PCVs), which is effective for the prevention of Pneumococcus-related diseases, has been globally applied among children. The study systematically reviewed published literatures on immunogenicity and safety when PCVs administered concomitantly with other routine pediatric vaccinations, and analyzed their interactions of co-administration. Based on the analyses, few impacts on the immunogenicity of both PCVs and other routine pediatric vaccines were observed when PCVs were administered concomitantly with vaccines against pertussis, tetanus, diphtheria, measles, mumps, rubella, varicella, polio, hepatitis A, hepatitis B, Meningococcus serogroup C, meningococcal ACWY, Haemophilus influenzae type b, and expressed acceptable safety profiles. The results of the study suggest that PCVs could be administered concomitantly with other routine pediatric vaccines so as to reduce the frequency of vaccinations and improve the efficiency of planned immunization under acceptable immunogenicity and safety profiles.

肺炎球菌性疾病是严重的公共卫生问题之一,也是 < 5岁儿童疫苗可预防的首要死因 [1],给全球儿童带来了严重的健康威胁和沉重的经济负担。肺炎链球菌也是引起中国婴幼儿和老年人发病和死亡的重要病因[25],全球 < 5岁儿童肺炎球菌性疾病病例数最高的10个国家中,中国位居第2位,占全球总病例数的12 % [6]。然而,婴幼儿免疫系统发育不完全,常规多糖疫苗难以诱导产生有效的保护性抗体,而肺炎球菌结合疫苗(pneumococcal conjugate vaccines,PCVs)是T细胞依赖性抗原,在婴幼儿体内具有良好的免疫原性,可用于婴幼儿预防肺炎球菌性疾病。自2000年首个肺炎球菌结合疫苗PCV7在美国上市以来,血清型覆盖范围更广的PCV10和PCV13相继获批上市,在全球儿童中应用十分广泛。婴幼儿期是免疫规划的重要时期,需要接种的疫苗种类较多。一般情况下,为了使儿童常规免疫更为便捷,可在确保不发生免疫干扰且安全性良好的情况下,将多种疫苗同时接种,从而减少接种次数,方便儿童的免疫流程。2012年,WHO立场文件报道,肺炎球菌结合疫苗与用于预防百日咳、白喉、破伤风(无细胞和全细胞疫苗)、乙型肝炎、脊髓灰质炎(灭活疫苗和口服减毒活疫苗)、b型流感嗜血杆菌、麻疹、腮腺炎、风疹、水痘、C群脑膜炎球菌(结合疫苗)、轮状病毒等疾病的单价疫苗或联合疫苗同时接种,其免疫原性和反应原性并不会发生明显变化,且不会降低疫苗在儿童中的安全性和耐受性[6]。肺炎球菌结合疫苗在全球儿童中应用非常广泛,随着PCV7和PCV13在中国大陆地区的相继上市,其在中国儿童中的应用也越来越普遍,因此,疫苗联合接种的问题也越来越受到医务工作者和儿童家长的重视。目前已有多项临床研究报道了PCV7或PCV13与其他儿童常规免疫疫苗同时接种的免疫原性及安全性,为探讨肺炎球菌结合疫苗与其他儿童常规免疫疫苗同时接种对免疫原性及安全性的影响,从而用于进一步指导其在儿童中的应用,本文对此类研究进行了如下综述。

1 同时接种对疫苗免疫原性的影响 1.1 PCV7疫苗

PCV7是全球首个获批上市的肺炎球菌结合疫苗,早期的研究发现,PCV7与儿童常规免疫疫苗同时接种对疫苗的免疫原性无明显影响。美国一项多中心临床试验对比研究了PCV7在婴幼儿中与白喉、破伤风、无细胞百日咳三联疫苗(diphtheria,tetanus,acellular pertussis vaccine,DTaP)、乙肝疫苗(hepatitis B virus,HBV)、脊髓灰质炎灭活疫苗(inactivated polio virus,IPV)、b型流感嗜血杆菌疫苗(haemophilus influenza type b,Hib)等同时接种和间隔接种的免疫原性,研究共包括3个试验组:第1组在2、4、6月龄分别同时接种DTaP-HBV-IPV(三者联合接种)、Hib和PCV7等3种疫苗;第2组在相同时间分别同时接种DTaP、HBV、IPV(三者独立接种)、Hib和PCV7等5种疫苗;第3组在2、4、6月龄时分别同时接种DTaP-HBV-IPV和Hib疫苗,并在每次接种结束后2周各接种1剂PCV7,然后分别测定所有婴幼儿7月龄时血清中各类抗体水平,研究结果显示,第1组婴幼儿抗白喉、破伤风、脊髓灰质炎及百日咳等抗体水平均非劣于第2组和第3组,3组婴幼儿抗HBV和抗Hib抗体水平相当,PCV7所有血清型在各组中的血清阳性率均很高,且各组间无明显差异,说明PCV7与DTaP-HBV-IPV及Hib疫苗同时接种具有良好的免疫原性,各疫苗的免疫反应均未受到明显干扰[7]。此外,还有多项试验研究了PCV7在儿童中与DTaP-HBV-IPV联合疫苗、Hib疫苗、甲肝疫苗(hepatitis A virus,HAV)、C群脑膜炎球菌结合疫苗和麻疹、腮腺炎、风疹、水痘联合疫苗(measles,mumps,rubella,varicella vaccine,MMRV)等儿童常规免疫疫苗同时接种的免疫原性,研究结果均显示PCV7与这些疫苗同时接种对PCV7本身及其他疫苗的免疫原性均无明显影响[814]

1.2 PCV13疫苗

PCV13与其他儿童常规免疫同时接种时,对这些疫苗的免疫原性不会产生明显干扰。2006 — 2008年德国和西班牙的2项多中心随机双盲Ⅲ期临床试验对比研究了PCV13及PCV7分别与儿童常规免疫疫苗同时接种的免疫原性,德国的研究分别在婴幼儿2、3、4月龄时各接种1剂PCV13或PCV7,且同时各接种1剂DTaP-HBV-IPV/Hib疫苗,并在11~12月龄时所有疫苗均加强1剂;西班牙的研究分别在婴幼儿2、4、6月龄时各接种1剂PCV13或PCV7,且同时各接种1剂DTaP-IPV/Hib疫苗,并在15月龄时所有疫苗均加强1剂;然后2项研究均分别测定基础免疫及加强免疫1个月后各类抗体的几何平均浓度或几何平均滴度,研究结果显示,婴幼儿在三剂基础免疫后对各类抗原均能产生良好的免疫应答,而加强免疫后各类抗体水平进一步大幅提高,且PCV13接种组与PCV7接种组婴幼儿的各类抗体水平相当,说明PCV13与PCV7一样,与用于预防百日咳、白喉、破伤风、乙肝、脊髓灰质炎、b型流感嗜血杆菌等疫苗同时接种时对这些疫苗的免疫原性无明显影响[15]。西班牙另一项随机、双盲、平行对照试验研究了PCV13及PCV7分别与C群脑膜炎球菌 – 破伤风类毒素结合疫苗(meningococcal serogroup C-tetanus toxoid conjugate vaccine,MnCC-TT)、DTaP-HBV-IPV/Hib疫苗及DTaP-IPV+Hib疫苗同时接种的免疫原性,研究过程中选择性测定了抗C群脑膜炎球菌的功能抗体滴度以及抗白喉和抗破伤风的IgG抗体浓度,结果发现接种了PCV13或PCV7的婴幼儿基础免疫后抗C群脑膜炎球菌、抗白喉及抗破伤风的抗体浓度达到相应合格水平的婴幼儿百分数均 ≥ 96.6 %,加强免疫后均 ≥ 99.4 %,且2组婴幼儿体内抗体水平相当,说明PCV13非劣于PCV7,两者均对MnCC-TT疫苗、DTaP-HBV-IPV/Hib及DTaP-IPV+Hib疫苗的免疫原性无明显干扰 [16]。除此之外,意大利一项多中心、随机、双盲、平行对照Ⅲ期临床试验也研究了PCV13及PCV7分别与DTaP-HBV-IPV/Hib在婴幼儿3、5、11月龄时同时接种的免疫原性,研究结果同样表明DTaP-HBV-IPV/Hib各抗原在PCV13组的免疫原性均非劣于其在PCV7组的免疫原性[17]。巴西地区针对PCV7及PCV13与儿童常规免疫同时接种的临床试验也表明,PCV7及PCV13与白喉疫苗、破伤风疫苗、全细胞百日咳疫苗、Hib、口服脊髓灰质炎疫苗(OPV)、轮状病毒疫苗等同时接种不会造成免疫原性的相互干扰[18]。一项全球多中心Ⅲ期临床试验在婴幼儿2、4、6月龄时均同时接种DTaP-IPV/Hib、HBV、PCV7或PCV13和脑膜炎球菌A、C、W-135、Y群四价结合疫苗[quadrivalent (A,C,W-135,and Y群) meningococcal vaccine conjugated to CRM197,MenACWY-CRM],并在12月龄时同时接种MenACWY-CRM、PCV7或PCV13和麻疹、腮腺炎、风疹联合疫苗(measles,mumps,and rubella vaccine,MMR),然后分别测定儿童体内抗A、C、W135、Y群脑膜炎球菌抗体水平,并同时测定其他疫苗的免疫应答阳性率,结果表明婴幼儿体内抗血清型A、C、W135、Y群人补体血清杀菌力试验滴度 ≥ 8的受试者比例分别为89 %、95 %、97 %和96 %,且其他疫苗的免疫应答阳性率也很高,说明PCV7或PCV13与MenACWY-CRM等疫苗同时接种对这些疫苗的免疫原性均不产生明显影响[19]。此外,美国还开展了2项随机、双盲、对照试验,均对比研究了PCV13和PCV7与儿童常规疫苗同时接种时这些疫苗的免疫原性,研究中婴幼儿分别在2、4、6和12~15月龄各接种1剂PCV13或PCV7,同时在2、4、6月龄时各接种1剂用于预防白喉、破伤风、无细胞百日咳、灭活脊髓灰质炎、乙肝和b型流感嗜血杆菌的疫苗,而在12~15月龄均同时接种1剂麻疹、腮腺炎、风疹、水痘和甲肝疫苗,然后分别测定基础免疫和加强免疫1个月后各类抗体的几何平均浓度/滴度,结果发现PCV13组婴幼儿体内各抗体水平均非劣于PCV7组,基础免疫后PCV13组与PCV7组婴幼儿各类抗体几何平均浓度/滴度的比值为0.91~1.33,加强免疫后则为0.83~1.03,均符合预期的非劣性标准[20]。以上研究通过PCV13与PCV7的非劣性试验说明了PCV13与儿童常规免疫疫苗同时接种对这些疫苗的免疫原性不会产生明显影响。

PCV13与其他儿童常规免疫疫苗同时接种时,PCV13本身的免疫原性也不会受到明显干扰。意大利一项Ⅲ期临床试验研究了PCV13及PCV7与DTaP-HBV-IPV/Hib疫苗在婴幼儿3、5、11月龄时同时接种的免疫原性,结果发现接种PCV13的婴幼儿在第2剂基础免疫后肺炎球菌各血清型IgG抗体浓度 ≥ 0.35 μg/mL的婴幼儿比例为86.5 %~98.5 %(血清型6B和23F略低,分别为58.4 %和68.6 %),IgG抗体几何平均浓度为1.15~3.42 μg/mL(血清型6B和23F分别为0.41和0.61 μg/mL),调理吞噬试验抗体滴度 ≥ 1 : 8的婴幼儿比例为94.8 %~100.0 %;而11月龄加强免疫后各疫苗血清型IgG抗体浓度 ≥ 0.35 μg/mL的婴幼儿比例增加至93.9 %~100.0 %(包括血清型6B和23F),IgG抗体几何平均浓度为3.02~10.30 μg/mL(血清型3为1.22 μg/mL),调理吞噬试验抗体滴度 ≥ 1 : 8的婴幼儿比例为97.9 %~100.0 %,且发现接种PCV13的儿童各疫苗血清型抗体水平均不低于接种PCV7的儿童,说明PCV13与DTaP-HBV-IPV/Hib疫苗同时接种对PCV13本身的免疫原性无明显影响[17]。西班牙的一项临床试验研究了PCV13在婴幼儿中与MnCC-TT、DTaP-HBV-IPV/Hib及DTaP-IPV+Hib疫苗同时接种的免疫原性,分别测定了第2、3剂和加强剂PCV13接种后1个月各疫苗血清型的抗体浓度,结果发现基础免疫后各疫苗血清型IgG抗体浓度 ≥ 0.35 μg/mL的婴幼儿比例 ≥ 93.0 %(血清型3为86.2 %),而加强免疫后则增加至98.1 %~100.0 %(血清型3为93.6 %),且各血清型抗体几何平均浓度均较高,说明PCV13与儿童常规疫苗同时接种时同样具有很强的免疫原性[16]。此外,将MenACWY-CRM、DTaP-IPV/Hib、HBV、MMR、PCV7或PCV13疫苗在婴幼儿中同时接种的全球多中心Ⅲ期临床试验发现,所有儿童对PCVs的免疫应答均较高,各疫苗血清型抗体浓度 ≥ 0.35 μg/mL的儿童比例均 > 90 %,说明PCV7或PCV13疫苗与MenACWY-CRM、MMR等疫苗同时接种并不会影响PCVs本身的免疫原性 [19]。除上述各项研究外,还有多项研究表明PCV13疫苗与儿童常规免疫疫苗同时接种对PCV13疫苗本身及其他疫苗的免疫原性均不会产生明显影响[2125]

2 同时接种对疫苗安全性的影响

2012年WHO立场文件援引欧洲的研究报道,4 429名健康婴儿在接种PCV13疫苗的同时接种其他常规儿童期疫苗,结果约10 %的受种者出现了注射部位不良反应、发热、激惹、食欲减退和睡眠增加和/或减少,注射部位不良反应在 > 12月龄儿童中较在婴儿中更为多见,发热 > 39℃的发生率为1/100至 < 1/10,呕吐和腹泻的发生率为1/1 000至 < 1/100,超敏反应(包括皮疹、面部水肿、呼吸困难)和中枢神经系统失调(包括抽搐和低渗低反应症状)的发生率为1/10 000至 < 1/1 000 [6]。此外,西班牙针对PCV13和PCV7疫苗在健康婴幼儿中的研究显示,当PCV13或PCV7疫苗与其他儿童常规免疫疫苗同时接种时,多数局部不良反应均为轻度,接种部位出现明显压痛的婴幼儿比例 ≤ 4 %,基础免疫和加强免疫后均未发现局部肿胀病例,PCV13接种组与PCV7接种组在局部不良反应及大多数全身反应方面发生率均相当,均未出现死亡病例,且无婴幼儿因不良反应而退出研究[16]。意大利在健康婴幼儿中的临床研究表明,PCV13或PCV7疫苗与DTaP-HBV-IPV/Hib疫苗同时接种时,PCV13组与PCV7组婴幼儿不良反应发生率相近,2组局部反应程度均为轻度,一般持续1~2 d后消失,基础免疫和加强免疫后出现明显压痛的婴幼儿百分数分别 ≤ 5.0 %和 ≤ 8.5 %;大部分出现发热的婴幼儿均为轻度(38.0~39.0℃),基础免疫和加强免疫后出现中度发热(39.0~40.0℃)的婴幼儿比例分别 ≤ 7.0 %和 ≤ 12.5 %,全身性不良反应的持续时间均 ≤ 4 d;整个研究过程中无死亡病例,PCV13组未出现与疫苗接种相关的严重不良事件;以上均说明PCV13与DTaP-HBV-IPV/Hib疫苗在婴幼儿中同时接种具有良好的安全性[17]。全球多中心临床试验研究也表明PCV7或PCV13疫苗在婴幼儿中与MenACWY-CRM、DTaP-IPV/Hib、MMR、HBV等疫苗同时接种具有良好的耐受性[19]。此外,美国、德国、巴西等地区的研究均表明PCV13联合其他疫苗同时接种的安全性证据均非劣于PCV7与相同疫苗同时接种的安全性,说明PCV13疫苗与多种儿童常规免疫疫苗同时接种具有良好的安全性[18, 21, 23, 26]

3 小 结

综上所述,肺炎球菌结合疫苗与儿童常规免疫疫苗同时接种时,各类疫苗均能诱导机体产生良好的免疫反应,同时接种对肺炎球菌结合疫苗本身及其他儿童常规免疫疫苗的免疫原性均无明显影响,且具有良好的安全性。婴幼儿期是免疫规划的关键时期,需要接种多种疫苗,为了使免疫程序更为便捷,可以将PCV13疫苗与儿童常规免疫疫苗同时接种,从而在确保良好免疫原性和安全性的情况下减少接种次数,提高免疫效率。
