母乳喂养是为婴儿健康生长和发育提供理想食品和营养的最好方式,同时也是繁育过程的基本组成部分和母亲健康的重要指标[1]。世界卫生组织(World Health Organization, WHO)和联合国儿童基金会(United Nations International Children's Emergency Fund, UICEF)[2]建议生命最初6个月应进行纯母乳喂养;在婴儿6个月龄时增加有足够营养和安全的补充(固体)食品,同时持续进行母乳喂养至两岁或两岁以上。通过母乳喂养改善儿童发育并降低卫生成本,可为个人家庭以及国家层面带来经济收益[3]。6个月的纯母乳喂养率提高到至少50 %是2025年全球营养目标的6个目标之一[4]。然而在全球范围内,母乳喂养水平并不乐观,离所期望的目标还有较大差距。多项研究表明,重返工作是母亲停止母乳喂养的一个重要因素[5 – 8],目前国内对这方面的研究较少。本文就国内外工作场所母乳喂养支持现状、工作场所母乳喂养支持的理论模型、评价工具、干预措施进行综述,为中国工作场所母乳喂养支持的相关研究提供参考与借鉴。
1 母乳喂养概述柳叶刀母乳喂养系列研究显示,母乳喂养对婴儿和母亲的短期和长期健康都有好处[9]。母乳喂养的持续对增进个人体质、促进人口健康、提高人口素质具有非常重要的作用与意义[10]。保障女性母乳喂养权益是构建生育友好型社会环境的重要内容[11]。在全球不同背景下,影响母乳喂养的关键因素可能是社会文化,与社会和组织群体的规范有关。低收入国家母乳喂养的障碍和决定因素包括文化信仰、教育、配方奶的营销以及卫生保健等[12]。高收入国家的障碍包括肥胖、重返工作岗位、缺少家庭的支持、公共场所哺乳的尴尬和教育水平[12]。
2 工作场所母乳喂养概述当今时代,女性地位和教育水平快速提高,女性以广泛就业的方式全面参与社会发展已成为世界潮流。但是面对激烈的竞争,工作压力及妻子、母亲和员工等多重身份的责任,职业女性在产后返回工作岗位,往往面临着继续母乳喂养的多重困难。即便我国制定了相应的政策规定,但具体的实施及效果还尚不清楚。许多母亲在返岗工作后继续母乳喂养中遇到了障碍,这使得母乳喂养的中断比建议的或是预期的要短[13]。由于工作或家庭等因素所造成的产妇负担过重,在母乳喂养失败的比例中有所上升[14]。为了使妇女成功地协调母乳喂养和工作,在产后前6个月,要充分了解可以促进母乳喂养尤其是纯母乳喂养的确切要求和支持系统[15]。
3 工作场所母乳喂养研究现状 3.1 工作场所母乳喂养的影响因素许多女性在生育后正常上班,此时她们或许能够继续哺乳,但由于缺乏相应的支持,她们可能决定停止哺乳或提前结束哺乳[16]。多项研究显示,目前我国职业女性返岗后继续母乳喂养的情况不容乐观,母乳喂养持续时间和纯母乳喂养率较低[17 – 19]。并且在产妇的主观意识影响中,部分产妇由于工作原因放弃哺乳[14]。国外的多项研究表明,影响职业女性母乳喂养的因素主要分为母亲层面、家庭层面、工作层面等几个方面。(1)母亲层面:主要包括返岗工作后母亲对母乳喂养的信心、态度[20 – 21],母乳喂养、婴儿护理及自身营养有关的知识和技巧[22]。职业女性对母乳喂养政策和法律的意识和应用也是影响返岗后母乳喂养的重要因素[23]。(2)家庭层面:家人的支持对协调母乳喂养与工作,在工作后继续进行母乳喂养非常重要[22, 24]。从国际上来说,有证据表明,男性的带薪产假,在提高母乳喂养率方面是有效的[25]。(3)工作层面:促进和阻碍工作场所母乳喂养的因素[26 – 27]包括工作环境(如工作场所的社会支持和来自工作场所主管的支持[28])、工作单位母亲友好政策、工作时间安排(如产假[29]和灵活的工作安排[30])、工作场所的工具设施、领导和同事的看法与支持[31]等。在所有的研究中,通过提供私人空间、保证休息以及雇主和同事支持来支持哺乳的工作政策是一致的主题[32 – 34]。
3.2 工作场所母乳喂养支持现状在全球范围内,作为职业母亲持续哺乳的决定因素,产假长度和工作场所政策缺乏一致性,为实现婴儿纯母乳喂养的目标,需要采取一致的方法[32]。尽管立法支持工作场所母乳喂养,但在工作场所的行为受文化影响很大[35 – 36]。虽然越来越多的大公司已经接受了建立适合母乳喂养的政策,在工作场所建立泌乳哺育休息间,但职业女性继续母乳喂养的有效性是不确定的[37]。因此,需要有工作场所的哺乳政策和干预措施,以帮助雇主克服哺乳支持障碍。缺乏休息时间、吸奶和储存乳汁的设施不足、缺乏促进母乳喂养的资源、缺乏雇主和同事的支持,这些都是那些希望能够在工作场所泌乳从而继续母乳喂养的职业母亲所面临的挑战[37 – 38]。可以促进或抑制母乳喂养的工作场所政策包括需要额外的休息,泌乳的私人场所,以及雇主的全面支持[39]。
4 工作场所母乳喂养支持的评价工具目前国外学者已经研制出评价工作场所母乳喂养支持情况的工具。主要包括两部分,一部分是用来研究雇主对母乳喂养态度和支持情况,公开发表的共4篇,所发展量表为母乳喂养态度调查表(Attitude Toward Breastfeeding Questionnaire, ATBQ)[40]、雇主母乳喂养态度量表(Employer Attitude Towards Breastfeeding, EATB)[41]、工作场所婴儿喂养调查表(the Worksite Infant Feeding Survey, WIFS)[42]和雇主对产妇母乳喂养支持状况调查表(Employer Support for Breastfeeding Questionnaire, ESBQ)[43]。二是从雇员的角度出发,评价工作场所母乳喂养支持情况的自我报告工具,主要包括员工母乳喂养支持感知问卷(Employees′ Perceptions of Breastfeeding Support Questionnaire, EPBS-Q; Greene SW, Olson BH,2008)[44 – 45];重返工作母乳喂养支持评估工具(Return to Work Breastfeeding Assessment Tool, Bar-Yam, 1998)[46 – 47];工作场所母乳喂养支持量表(Workplace Breastfeeding Support Scale、WBSS(Bai, 2008))[48];感知母乳喂养支持评估工具(Perceived Breastfeeding Support Assessment Tool, PBSAT; Hirani, 2013)[49]。目前我国尚缺少工作场所母乳喂养的评价工具。
5 工作场所母乳喂养干预研究 5.1 工作场所母乳喂养支持的理论基础社会生态系统理论和计划行为理论较多地被应用评价和预测工作场所母乳喂养情况。社会生态模型认为,多重因素影响人的健康行为,例如母乳喂养这一健康行为[50]。它是用以考察人类行为与社会环境交互关系的理论[51],多位学者基于该理论研制评价工作场所母乳喂养支持的工具[44, 49]进行相关研究。计划行为理论被广泛应用在母乳喂养方面,该理论可以帮助预测母亲的母乳喂养行为并帮助实施有效的干预[52]。Rojjanasrirat等[43]基于计划行为理论研制评价雇主对母乳喂养支持的问卷。
5.2 工作场所母乳喂养干预模型世界母乳喂养行动联盟(The World Alliance for Breast-Feeding Action,WABA)发起的对母亲友善的职场倡议(Mother-Friendly Workplace Initiative, MFWI)[53]得到了多个国家的支持和参与,并且建议相应的策略应聚焦于时间、空间或距离和支持3个基本概念上。Yimyam等[53]基于WABA建议的3个基本概念和泰国政府提倡的母乳喂养角活动构建了工作场所母乳喂养支持模式,并将该模式应用工作场所中,结果有效提高了6个月的母乳喂养率。
5.3 工作场所母乳喂养干预措施在国外已有关于工作场所母乳喂养支持性干预的相关报道和项目。例如,2015年由W Kellogg基金会(The WK Kellogg Foundation)赞助开发的母乳喂养雇主支持时间(Breastfeeding Employer Supported Time, BEST)项目,旨在工作场所建立哺乳支持[54]。Lennon等[54]建立了一种基于导师学员的工作场所母乳喂养雇主支持时间项目,Magner等[36]在工作场所成立母乳咨询委员会(Workplace Expression of Breast Milk Advisory Committee, WEBMAC),运用员工健康计划在工作场所倡导一种母乳喂养的文化,促进母乳喂养这一健康行为。Hirani等[55]的研究显示,促进工作场所母乳喂养的措施主要包括教育和指导职业母亲母乳喂养、提高雇主对母乳喂养的认识、在工作场所设置相应的设施、灵活的工作环境、母亲和婴儿友好政策。Spatz等[56]的研究结果显示,基于综合医院的员工哺乳支持项目可能会提高母乳喂养始动、纯母乳喂养和持续时间。Dinour等[13]的研究显示工作时保持母乳喂养不仅是可能的,尤其是在雇主提供支持时,尽管有些雇主有更广泛的母乳喂养支持政策和做法,但是所有雇主都可以实施符合公司预算和资源的母乳喂养支持计划。在我国,除了相关政策法规支持外,具体到相应工作场所母乳喂养的干预研究尚未见报道,目前缺乏具体的理论依据和干预措施。
6 小 结我国对工作场所母乳喂养的研究较少,且缺少相对完善的评价工具。近年来,母乳喂养成为全球关注的焦点,尤其是公共及工作场所的母乳喂养支持情况。国家卫生健康委员会新出台的多个政策方针和指导意见,都提到要在工作单位和用人单位配置标准化的母婴设施,支持女性生育后重返工作岗位,鼓励用人单位制定有利于职工平衡工作与家庭关系的措施。保障女性母乳喂养权益是构建生育友好型社会环境的重要内容。目前,我国的生育保障制度主要为面向国家机关、企事业单位及其他各类经济社会组织工薪劳动者提供生产阶段的经济保障,对母乳喂养权益的经济、物质、制度保障都比较匮乏。虽然有相应政策的支持,但具体到用人单位,其具体的实施情况,其中存在的问题则缺少数据支持,且目前我国缺乏工作场所母乳喂养支持状况的评估工具和相对应的干预措施。未来应借鉴国外的经验引进或研制出适合我国的工作场所母乳喂养评价工具,形成相应的干预框架和具体的干预措施,从而改善职业女性母乳喂养水平,创造母乳喂养友好型的工作环境和生育友好型的社会环境,进而提高母乳喂养水平,改善妇女及婴幼儿健康。
