能量饮料是指包含了咖啡因以及其他成分(糖、氨基酸、牛磺酸、中草药提取物及B类维生素等)的饮料,自从1987年第一次面市,在全世界迅速流行[1-2]。咖啡因是能量饮料最主要的成分,它能刺激中枢神经系统,让大脑误认为身体并不疲倦,从而使人感到清醒、充满能量[3-4]。由于市场上销售的能量饮料并没有标注其所含咖啡因成分、份量及副作用,因此会误导消费者大量饮用,增加咖啡因中毒的危险[5-6]。目前,能量饮料消费是学校心理健康领域研究的一个新问题[1],研究者们围绕能量饮料对青少年身心健康的危害开展了大量研究,本文对已有研究进行综述,希望能够为能量饮料管理、青少年健康维护、相关研究开展提供参考。
1 能量饮料在青少年中流行现状研究者们大多采用问卷(自陈量表)调查测量青少年能量饮料的消费,让青少年对在1年、1个月或1周内能量饮料的饮用情况进行自我评估[2, 7-8], 并对其进行人口学变量分析。结果表明,能量饮料的消费在青少年中非常普遍,他们第一次饮用能量饮料的年龄大约为16岁[9-11]。年龄、性别与能量饮料的消费有关,随着年龄的增长,能量饮料的消费量在减少[12-13];由于能量饮料在广告中宣称能够增强男性体能,因而男性比女性更多地饮用能量饮料[14-16]。
2 能量饮料对青少年的生理危害能量饮料对青少年健康的许多负面影响都已被证实,如:头痛、紧张、焦虑、失眠、躁狂、精神失常、脱水、胃肠不适、面色潮红、多尿、抽搐、中风、戒断症状、龋齿、心律失常、心跳加速,甚至死亡[17-18]。下面主要从体重、头痛、心脏、神经系统和死亡五个方面详细介绍能量饮料对生理的负面影响。
2.1 对体重影响能量饮料主要从2个方面影响青少年的体重。一方面,能量饮料饮用会影响青少年的饮食习惯,使其养成不吃早饭、摄入过多含糖饮料等对身体有害的饮食行为。Blum等[19]调查了164名儿童超过2年的能量饮料消费和体质指数之间的关系,发现随着青少年能量饮料消费增加,牛奶饮用量显著减少。Larson等[20]发现大学生能量饮料的使用和不健康的饮食习惯有关,如更多食用苏打汽水及冷冻餐食、更少食用水果及蔬菜、不吃早餐、不健康的减肥行为等[2]。另一方面,能量饮料饮用会导致青少年体重上升,增加肥胖的可能性。James等[21]通过实验证实能量饮料在儿童和青少年的肥胖流行中起了重要作用。管爽等[22]调查发现长期饮用含糖饮料会导致青少年肥胖。
2.2 头痛能量饮料的主要成分是咖啡因,咖啡因过量摄入会增加青少年患头痛的危险。Hering-Hanit等[23]对青少年进行持续5年的研究发现,从能量饮料中摄入高咖啡因量可能会使其患上慢性头痛,逐渐减少饮用量能完全治愈头痛。Shapiro[24]也提出咖啡因能够导致头痛的发生,建议临床医生劝病人限制咖啡因使用,降低头痛发生的风险。
2.3 对心脏影响能量饮料饮用可能会导致心律不齐。孔维敏等[25]指出过量饮用功能饮料会使青少年心律失常、心肌缺血。Nagajothi等[26]报告,一位23岁的健康女性在饮用了能量饮料之后因为心悸和胸部压迫感去医院就医,经过检查,发现其血压为120/55 mm Hg(1 mm Hg=0.133 3kPa),心跳为219次/min,心律失常;这个案例提醒我们应该让公众认识到能量饮料危害的重要性。
2.4 对神经系统影响能量饮料饮用可能会使中枢神经系统受到影响,导致癫痫、精神病、短暂性脑缺血等疾病发生。Iyadurai等[18]认为大量饮用能量饮料会引起癫痫;Bramstedt[27]指出癫痫病人一旦停止饮用能量饮料,其癫痫就不会发作。Chelben等[28]发现饮用能量饮料会使精神病人的病情恶化,使其出现精神状态不稳定、心理不安等情况,提醒易患精神疾病的个体应该谨慎消费能量饮料。Dikici等[29]报告了一起饮用能量饮料后导致的短暂性脑缺血发作病例,分析过量或空腹饮用能量饮料,会摄入过多咖啡因和牛磺酸,使神经系统和心脏中毒,诱发心血管疾病和短暂性脑缺血。
2.5 死亡能量饮料的饮用可能会诱发个体疾病,进而导致死亡。2009年,英国媒体报道了一则新闻,一名学生在夜店喝了四罐能量饮料和一些含咖啡因的伏特加饮品后死亡,死亡原因是过量能量饮料和酒精摄入导致的心脏病[5]。2012年,加拿大和美国媒体共报道了能量饮料导致的16起死亡[30]。
3 能量饮料对青少年的心理危害 3.1 问题行为问题行为会影响青少年的身心健康。研究表明,能量饮料的消费会增加青少年物质滥用、危险行为、沉迷游戏等问题行为的发生率。(1)增加物质滥用。研究表明,能量饮料饮用和青少年烟、酒、大麻和非法药物的使用有关,能量饮料饮用多的青少年会出现更多物质滥用行为[6, 13, 15, 31-32]。(2)表现出更多危险行为。Miller[33]指出高中生能量饮料的消费频率越高,越容易出现危险性行为、打架、不系安全带等危险行为。Malinauskas等[34]调查发现大学生在长时间驾驶和饮酒时更喜欢喝能量饮料。Larson等[20]指出能量饮料饮用会增加青少年沉迷游戏的可能性。
3.2 心理特征 3.2.1 自我认同能量饮料饮用与大学生对运动能力、男子气的认同有关。Miller[35]在调查中发现,消费能量饮料的频率与大学生体育运动认同、男子气、与健康有关的冒险行为(增加的酒精使用、暴力和不安全的性行为)有显著关系,消费能量饮料的大学生对自己的运动认同更好,认为自己更具有男子气概,并表现出更多的冒险行为。
3.2.2 负性情绪与幸福感能量饮料饮用会导致焦虑、抑郁负性情绪的增加。Azagba等[30]研究表明能量饮料消费的增加会导致抑郁症状增加;能量饮料(每天100 mL)的饮用与焦虑密切相关,但这种相关只表现在男性身上[36]。孔维敏等[25]指出过量饮用能量饮料会引起青少年产生焦虑、抑郁负性情绪。能量饮料的使用会影响青少年的幸福感。Rockett等[37]调查发现高中生能量饮料导致的咖啡因成瘾和健康、幸福感相关,咖啡因成瘾的高中生处于严重的压力状态,表现出慢性抑郁,整体身体健康状况糟糕,报告出更多的酒精和药物问题,这些会降低高中生的幸福感。
3.2.3 感觉寻求能量饮料饮用可能会增加青少年的感觉寻求。研究发现,饮用能量饮料频率高的中学生会表现出更多的感觉寻求,更喜欢寻求新异、多变的体验和感觉,更喜欢表现出冒险行为[30]。
3.2.4 社会问题解决能力能量饮料饮用可能会降低社会问题解决能力。研究表明,大学生能量饮料的饮用与学业成绩和社会问题解决能力的降低有关,低学业成绩和社会问题解决能力的学生更容易饮用能量饮料[38]。
4 能量饮料与酒精混合饮用危害随着能量饮料在青少年中的日益流行,一种新的消费趋势在青少年中不断增加,这就是能量饮料与酒精的混合饮料(alcohol mixed with energy drink,AmED)。AmED所产生的问题日益引起研究者们的关注,认为这是一种危险的饮用行为,相对于单独饮用能量饮料或酒精,混合饮料带来的危害更大[1, 31, 39-40]。关于AmED的研究,概括起来主要围绕其使用动机和导致的行为问题2个方面开展。
4.1 AmED的使用动机青少年之所以喜欢饮用AmED,主要是因为与单独饮用能量饮料或酒精相比,AmED饮用具有以下特点:(1)能降低醉酒带来的负面影响,特别是使个体感受不到醉酒,延长饮酒时间[41]。这容易导致青少年出现酒精依赖,并增加与酒精饮用相关事故发生的可能性。(2)能使个体更加兴奋。对大学生AmED的消费方式和动机调查发现,AmED在大学生中很普遍,使用动机是能够使他们保持清醒,维持兴奋状态,能够喝下更多酒[31, 42-44]。(3)能给个体带来娱乐和放松。Patrick等[45]发现,学生选择AmED的动机更多是为了放松、娱乐,这种需求高于通过饮用AmED满足身体需求。
4.2 AmED导致的问题行为相对于单独饮酒或能量饮料来说,AmED更容易导致青少年出现以下问题行为:(1)导致酒精依赖。研究表明,AmED的使用能预测青少年酒精的使用,喜欢AmED的青少年表现出更多的酒精使用[46];AmED会增加中学生饮酒量和酒精依赖的可能性、提高滥饮的比率[13]。(2)增加物质滥用的可能性。Martz等[40]对6 498名美国中学生调查发现,AmED饮用与酗酒、大麻使用、非法药物使用显著相关。(3)表现出更多危险行为。AmED饮用与青少年危险驾驶、逃课、晚上在外娱乐等危险行为有很高的相关。众多研究发现,使用AmED的大学生比只饮酒的大学生表现出更多的危险驾驶行为(如醉酒后驾驶等)和危险饮酒行为(如醉酒次数更多等), 增加与酒精相关的事故和伤害,出现后悔性行为,增加自杀观念的出现[47-49]。
5 小结能量饮料对青少年健康的危害不容忽视,社会和学校应该采取措施降低其负面影响。首先,社会监管部门应加强对能量饮料生产公司的管理,规定其在产品上标注成分并警示可能带来的副作用[50],禁止媒体为能量饮料做广告[11];同时,社会媒体应该向公众普及能量饮料对健康的危害[25, 30, 50],向相关管理部门建议禁止向未成年人销售能量饮料[51-52]。其次,学校应该加强校内医护人员、营养师等的培训,使其知晓能量饮料对青少年健康的负面影响;并在校内限制能量饮料的售卖[5, 22]。目前,国外研究者相关研究较多,中国还较少有研究者重视这一问题。为此,我们应立足国情全面调查能量饮料对青少年健康的影响。(1)转换研究方式。现有研究多是采用问卷调查的方式,在后续研究中,要突破这一局限,采用实验法、访谈法等来验证能量饮料对青少年健康的危害[44]。(2)拓宽研究视角。目前,关于能量饮料的研究多是从营养学、医学、物质滥用成瘾的角度进行,更多探讨了能量饮料对青少年生理的影响;在后续研究中,可以从心理学视角出发,深入探讨能量饮料对青少年心理的影响,如对负性情绪、压力应对、自杀意向、身体自我等的影响。(3)研究本土化。现有研究大多是针对不同国家青少年能量饮料对身心健康的影响开展,其结果是否和我国青少年一致,还有待进一步验证。我们应该开展立足我国青少年实际情况的本土研究,对能量饮料的流行现状、原因、对身心健康的影响等方面进行调查,为能量饮料管理及青少年健康发展提供参考。
