基于线粒体靶向纳米药物递送系统的设计及研究进展

摘要: 线粒体参与能量提供、信号传导、细胞分化等诸多生理活动, 在肿瘤的发生发展中起重要作用, 以线粒体为靶标是一种癌症治疗新策略。利用纳米技术构建线粒体靶向纳米药物递送系统可改善传统药物溶解性, 延长药物半衰期, 提高其生物利用度及降低不良反应, 有望解决肿瘤治疗中出现的耐药性问题。本综述着重于癌症治疗领域, 介绍了线粒体靶向纳米药物递送系统用于癌症治疗的机制, 并概述了近5年线粒体靶向纳米药物递送系统的设计思路、分类及应用研究, 最后拓展分析了以线粒体为靶点的其他研究如仿生载体等, 并讨论其存在的优势及不足, 为未来纳米药物靶向线粒体的深入研究提供了依据。

Design and research progress of nano-drug delivery system based on mitochondria targeting

Abstract: Mitochondria is involved in many important physiological activities such as energy supply, signal transduction, cell differentiation, etc., and plays an significant role in the occurrence and development of diseases. Using mitochondria as a target is a new strategy for cancer treatment. The use of nanotechnology to construct a mitochondrial targeted nano-drug delivery system can improve the solubility of traditional drugs, prolong the half-life of drugs in the body, increase the bioavailability and concentration of drugs at the tumor site, and reduce the toxic and side effects of drugs. It is expected to solve the resistance in the process of tumor treatment. This review focuses on the field of cancer treatment. Firstly, it introduces the mechanism of mitochondrial targeted nano-drug delivery system for cancer treatment. Secondly, it outlines the design ideas, classification and application research of mitochondrial targeted nano-drug delivery systems in the past five years. Finally, it expands the analysis of other studies that target mitochondria, such as bionic vectors, and presents its advantages and disadvantages, which provide a basis for in-depth research on drug delivery systems in the future.

癌症是人类死亡主要原因[1], 世界卫生组织国际癌症研究机构(International Agency for Research on Cancer, IARC) 发布的《全球癌症数据统计(2020)》显示全球新发癌症1 929万例, 死亡996万例, 预计2040年出现2 840万例, 比2020年增加47%[2, 3]。化疗是治疗癌症的主要方法, 但传统药物分子存在水溶性差、溶出率低、渗透性低等缺点, 导致药效差、不良反应大[4]。因此, 探寻高效的癌症化疗新策略是临床亟待解决的课题, 具有重要意义。

相比于传统癌症药物分子治疗方法, 纳米医学具有如下优势: 通过纳米材料携载传统药物可改善药物溶解性, 延长药物体内半衰期, 提高生物利用度[5]; 通过高通透性和滞留(enhanced permeability and retention, EPR) 效应在肿瘤组织富集, 提高药物在肿瘤部位的浓度及治疗指数, 降低不良反应; 可搭载多种药物同时作用于肿瘤, 提高治疗效果[6]。目前纳米制剂NC-6004 Nanoplatin[7]、Nab-paclitaxel (Abraxane)[8]和PEGPH20[9]已进入癌症临床研究阶段, 免疫磁珠、MM-398 (Onivyde)[10]和紫杉醇白蛋白纳米制剂也已应用于临床诊断和治疗。

线粒体作为半自主性细胞器, 是细胞的能量工厂, 负责许多重要功能如电子传递、ATP合成、活性氧(ROS) 生成和凋亡途径的启动[11], 同时参与多种代谢途径如脂肪酸氧化、柠檬酸循环、糖异生及类固醇激素合成[12]。线粒体损伤导致的细胞能量代谢紊乱与多种疾病密切相关如神经退行性疾病、癌症和心血管疾病等[13], 许多临床批准药物可直接作用于线粒体触发癌细胞死亡, 如奈必洛尔(nebivolol) 可抑制线粒体呼吸和线粒体ATP合酶活性, 从而限制癌细胞能量供应[14]。大量研究发现一些药物作用于线粒体内靶点可能取得良好疗效, 如原阿片碱通过线粒体凋亡途径抑制肝细胞癌生长[15], 表明线粒体是肿瘤化疗的关键靶点。线粒体靶向纳米载体可跨越生理障碍直接将化疗药物选择性递送至线粒体内的靶标, 通过干扰能量代谢、调控细胞内氧化还原稳态[16]、参与细胞增殖和分化及线粒体自噬等来引发细胞凋亡[17]。因此, 与癌症治疗中传统纳米药物相比, 线粒体靶向可能是一种更有效的策略[18]。此外, 线粒体靶向纳米药物递送系统作为一种功能性材料, 经改性修饰可与其他辅助疗法如光动力疗法(photodynamic therapy, PDT)[19]、声动力疗法(sonodynamic therapy, SDT)[20]等联合, 起到“1+1 > 2”的效果[21]

本文将概述线粒体与肿瘤的关系, 综述近5年靶向线粒体纳米药物递送系统的设计和应用进展, 为新型药物输送系统的进一步发展提供建议。

1 线粒体靶向纳米系统用于癌症治疗的机制

线粒体通过信号转导、囊泡转运和膜接触位点与内质网、细胞核等相互作用来调节能量代谢、生物合成和免疫反应, 是细胞内生理功能的重要枢纽[22]。现阶段以线粒体为靶点的抗肿瘤药物治疗研究主要集中在对线粒体内的动力学、DNA链及凋亡通路相关蛋白质(caspase-3、caspase-9、Bax和Bcl-2等) 表达进行干扰或破坏, 调节能量代谢和ROS水平、诱导线粒体DNA和线粒体自噬稳态失调等。此外, 如光热疗法(PTT)、PDT、SDT、光声成像(PAI) 及免疫疗法等联合疗法的介入进一步完善了癌症的治疗方式和机制。本节对线粒体靶向癌症治疗的机制进行了分类和讨论。

1.1 线粒体动力学与肿瘤

线粒体动力学是指线粒体处在融合(fusion) 与裂变(fission) 的动态平衡中, 表现为形态上的异质性, 包括裂变状态和融合状态[23]。线粒体融合是两步过程(外膜融合和内膜融合), 分别由线粒体外膜上的线粒体融合蛋白1 (MFN1)、MFN2和内膜上的视神经萎缩蛋白1 (OPA1) 介导[24]。外膜和内膜的融合机制相似, 其中膜上的融合蛋白形成互锁卷曲状线圈, 通过三磷酸鸟苷水解为融合提供动力[25]。线粒体裂变主要由高度保守的动力相关蛋白1 (DRP1) 介导, 其被膜蛋白Mff和Fis1招募到线粒体外膜后, 通过在线粒体外膜周围自聚合, 驱动线粒体裂变[26] (图 1)。线粒体融合导致线粒体网络扩展, 为高能量需求的细胞提供了优势, 且已证明线粒体融合的破坏会导致线粒体功能障碍[25]。线粒体融合可修复受损线粒体, 恢复细胞器功能和同质性, 而线粒体裂变产生较小的细胞器, 有利于其在细胞中的运动和遗传信息保护。

肿瘤细胞通过改变线粒体动力学稳态来实现增殖和生存优势[27, 28]。通过研究线粒体动力学与肿瘤关系, 可以找到潜在治疗靶点。Gao等[29]研究了线粒体动力学在诱导先天免疫极化和抗肿瘤应答中的重要调控作用, 并阐明了相关的分子机制。结果表明Miga2作为线粒体外膜蛋白, 对线粒体形态的改变具有重要作用, 对其敲除会导致线粒体分裂, 并特异性上调白介素-12的表达, 促进T细胞产生干扰素-γ, 显著增强巨噬细胞抗肿瘤免疫能力。因此线粒体的动力学形态相关调控蛋白Miga2可作为抗肿瘤免疫疗法的潜在新靶点。

1.2 线粒体DNA与肿瘤

线粒体DNA (mtDNA) 是约16.5 kb的环状分子, 编码13种蛋白[30]。由于mtDNA裸露, 故易受损伤且突变比例较高[31], 编码呼吸链关键酶的区域突变会导致线粒体功能障碍, 尤其是控制mtDNA复制和转录过程的D-loop环区, 其突变率比核DNA高100~200倍。研究表明D-loop区域与急性髓系白血病(AML) 的发病机制存在关联, 其中T152C突变与AML风险相关性较高, 尤其是M3亚型[32]。2020年, 研究人员开发了首个特异性线粒体转录抑制剂IMT1B (inhibitors of mitochondrial transcription 1B)[33], 其可特异地靶向线粒体RNA聚合酶(POLRMT, 在多种癌细胞中过表达) 并改变POLRMT构象, 从而抑制mtDNA转录。此外, mtDNA是癌细胞代谢的关键, 故携带干扰mtDNA的基因或药物在治疗线粒体疾病方面有很大潜力。线粒体DNA拓扑异构酶I (Top1mt) 在维持线粒体DNA稳态、完整性和修复方面发挥着关键作用。Top1mt已被证明与癌症和耐药性有关。Mallick等[34]设计了三苯基膦(triphenylphosphine, TPP) 修饰的Cerberus纳米粒(mt-CN), 其中包含琥珀酸α-生育酚酯(α-TOS; 电子传递链中的复合物II抑制剂)、顺铂(DNA损伤药物) 和拓扑替康或SN38 (Top1mt抑制剂)。该纳米粒定位于MCF-7细胞线粒体中, 实现了对Top1mt的抑制和mtDNA损伤, 从而抑制线粒体蛋白质合成。Top1mt和mtDNA的共靶向导致线粒体膜透化, 释放细胞色素C (cytochrome C, Cyto C) 并产生ROS导致线粒体损伤及裂解caspase-9从而启动细胞凋亡。负载干扰mtDNA的基因或药物的纳米递药系统可能会成为治疗线粒体相关疾病基因调控的通用平台。

1.3 线粒体凋亡和肿瘤

细胞凋亡包括外源性途径(通过胞外信号激活细胞内caspase)[35]和内源性途径(又称线粒体凋亡途径, 通过线粒体释放凋亡酶激活因子激活caspase)[36]。线粒体凋亡途径中, Bcl-2蛋白家族通过影响电压依赖性阴离子通道(VDAC) 而控制线粒体通透性来调节细胞凋亡。抗凋亡蛋白Bcl-2和Bcl-xL驻留在线粒体外膜中, 抑制Cyto C释放[37]。促凋亡的Bcl-2家族Bad、Bid、Bax和Bim可驻留在细胞质中, 在接受死亡信号转导后转位至线粒体, 促进Cyto C释放[38]。Cyto C释放后与APAF-1结合, 并与caspase-9形成激活复合体进一步激活caspase-3、caspase-7等切割细胞底物实现凋亡[39]。研究者不断开发靶向线粒体纳米药物递送系统, 通过递送抗肿瘤药或敏化剂等, 调控凋亡相关蛋白表达水平, 破坏线粒体并发挥抗肿瘤作用。

1.4 能量代谢干扰

根据Warburg效应, 即使在氧气充足的条件下, 肿瘤细胞也优先利用糖酵解而不是产能效率更高的氧化磷酸化来满足其能量需求。癌细胞的能量供应很大程度依赖于糖酵解, 而正常细胞仅在缺氧时才启动糖酵解。针对这种独特的代谢途径可选择性破坏癌细胞而不影响正常细胞。开发线粒体靶向纳米系统以干扰肿瘤细胞中的能量代谢途径可能实现癌症治疗[40]。Jin等[41]开发了一种具有代谢阻断作用的肿瘤特异性多重刺激激活的树枝状纳米系统以克服化疗耐药性, 并逐级突破化疗耐药性的多重生理障碍, 因为其具有多重响应特性, 包括隐形树突状电晕以延长血液循环, 稳定的纳米结构可有效被动靶向和肿瘤积累, 酶激活介导肿瘤微环境靶向以加深肿瘤渗透并促进药物内化、细胞质氧化还原敏感性分解以充分释放氯尼达明以快速达到治疗窗口, 以及溶酶体中酸触发抗肿瘤药物的核递送。此外, 代谢阻断策略终止了MCF-7R细胞中过度表达的己糖激酶(HK) 活性, 并进一步激发了多种途径来克服细胞耐药性因素, 包括ATP限制、凋亡激活和抗凋亡抑制。在体外和体内对MCF-7R肿瘤的疗效表明该策略有力地克服了生理障碍和MDR。此研究对能量代谢干扰以克服化疗耐药性而治疗癌症非常有价值。

1.5 ROS调节

ROS包括超氧阴离子、过氧化氢、单线态氧等, 主要来源是线粒体[42]。一方面, 高水平ROS可导致细胞结构的氧化损伤。如为了最大限度利用和响应肿瘤部位的高氧化应激环境, Li等[43]通过化学酶法合成了线粒体靶向的荧光自报告纳米胶束载体1K-TPP和2K-TPP, 以负载多柔比星(DOX) 并增强内源性ROS反应。在体外, 该平台显示出对ROS的高灵敏度(检测限 < 200 nmol·L-1 H2O2) 和比率荧光自报告特性, 同时实现DOX快速释放。与不含TPP修饰的1K-OH和2K-OH相比, 1K-TPP和2K-TPP在共定位实验中表现优异的线粒体靶向性。负载DOX的1K-TPP和2K-TPP大大增强了对肿瘤细胞的毒性和选择性。该研究证实了1K-TPP和2K-TPP可联合增强内源性ROS治疗肿瘤。另一方面, 清除过量的ROS也可作为治疗癌症的有效策略。如环孢素A (CsA) 通过与位于线粒体内膜的亲环素D (CypD) 结合, 抑制线粒体通透性转换孔(mPTP) 的开放并能降低ROS水平, 是一种极有前景的心肌缺血再灌注损伤治疗药物。而由于CsA对其他正常器官和组织的免疫抑制作用, 其应用受到限制。Zhang等[44]制备了靶向线粒体的纳米粒CsA@PLGA-PEG-SS31, 由于SS31的膜穿透特性和正电荷, CsA将被精准递送到缺血心肌细胞的线粒体内膜, 用于治疗心肌缺血/心肌梗死。在体外呈现双相释药模式, 初期快速释放是由于吸附在纳米粒表面的药物的释放, 这对于心肌细胞急性损伤的快速恢复大有裨益。后期缓释是由CsA@PLGA-PEG-SS31与释放介质之间较低的药物浓度梯度。实验表明CsA@PLGA-PEG-SS31对H/R损伤的H9c2细胞mPTP开放和线粒体膜电位崩溃有较强的抑制作用, 同时降低了细胞的ROS水平和caspase-3活性。血液动力学结果表明CsA@PLGA-PEG-SS31增加了左室射血分数和心率, 减少心肌缺血/再灌注损伤心肌中乳酸脱氢酶、AST和cTnI的释放, 这表明CsA@PLGA-PEG-SS31减少了心肌梗死大鼠心肌细胞的凋亡和梗死面积, 改善了心肌梗死/缺血再灌注大鼠的心功能。因此, 基于线粒体ROS调节的纳米载体设计是值得关注的治疗策略。

1.6 蛋白质靶向

许多线粒体蛋白参与了细胞代谢途径, 而癌细胞主要依赖有氧糖酵解产生能量。如HK是一种糖酵解限速酶, 其磷酸化状态催化糖酵解的第一步[45], 有4种亚型, 其中HKII在癌细胞中高度过表达[46]。HKII与位于线粒体外膜(OMM) 的VDAC1结合。后者与抗凋亡蛋白结合在线粒体介导的凋亡中起关键作用, 而VDAC1和HKII的结合抑制细胞凋亡。因此, 阻碍HKII与OMM的结合或VDAC1-HKII复合物解离, 可激活癌细胞凋亡, 有助于克服抗癌治疗中的耐药性问题[47]。Liu等[48]报道了一种靶向癌细胞线粒体的两亲性肽超分子自组装体, 旨在通过减弱线粒体VDAC1-HKII的相互作用来放大凋亡刺激。将带正电荷的片段(pKV) 锚定在特定氨基酸序列产生细胞穿透肽(pHK-pKV) 并将脂质链(Pal) 与pHK-pKV的N末端结合, 以增强pHK的细胞内递送。细胞毒性和凋亡研究结果表明减弱VDAC1-HKII的相互作用而抑制糖酵解并放大凋亡信号可作为一种有前途的癌症治疗策略。

1.7 线粒体自噬

线粒体自噬是一种自我保护过程, 可降解受损或功能失调的线粒体, 该过程介导了癌细胞耐药性从而降低线粒体靶向治疗的有效性, 然而过度自噬可能会导致细胞代谢紊乱和死亡, 对癌症治疗产生有益的影响[49]。因此, 不同条件下线粒体自噬在癌症耐药性中起双重作用。受线粒体靶向癌症治疗和线粒体自噬之间关系的启发, Zhu等[50]开发了一种基于能量消耗的抗癌策略, 通过设计线粒体靶向的TPGS/dc-IR825纳米胶束, 利用dc-IR825的线粒体靶向能力和TPGS (D-alpha-tocopheryl polyethylene glycol 1000 succinate) 在癌细胞中引起高水平的线粒体自噬, 引起能量高度消耗而治疗癌症。该策略的抑癌机制有两种: ①激活线粒体自噬; ②健康线粒体数量的减少导致细胞ATP水平降低, 激活能量感应途径, 触发癌细胞中的自噬以补偿ATP短缺。双自噬途径产生了大量自噬体与溶酶体, 最终降解细胞内成分。由于自噬下游被阻断, 癌细胞的自噬活动无法回收细胞成分, 反而加速了ATP的消耗, 最终导致ATP耗竭和细胞死亡[50]。鉴于以线粒体为靶点的PDT/PTT也会引起线粒体功能障碍, 在808 nm激光照射下可进一步增强过度的线粒体自噬/自噬引起的能量消耗。该研究通过人为调节癌细胞的自噬过程为抗癌治疗提供了一种新的能量消耗策略, 并可能利于对自噬与癌细胞死亡之间关系的进一步理解。

1.8 联合疗法

基于纳米技术的联合疗法包括PTT、PDT、SDT、PAI及免疫疗法等。

PDT是通过光敏剂在光照射下产生ROS杀死癌细胞的疗法, 具有非侵入性、低毒性和精确可控等优点。但受肿瘤缺氧、ROS极短的扩散距离(< 0.02 μm) 和寿命(< 0.04 μs) 影响, PDT疗效不佳[51]。破坏线粒体可能逆转肿瘤缺氧微环境, 而目前很少利用线粒体抑制策略来辅助PDT, 因为线粒体的膜渗透性低阻碍了治疗剂的进入。为克服以上难题, Li等[52]开发了一种由光敏剂二氢卟酚e6 (Ce6) 的明胶核和抗癌药多柔比星-甘草次酸共轭物(DOX-GA) 化学键合的聚合物壳组成的纳米制剂。Ce6产生过量的ROS, 以协助DOX-GA从溶酶体逃逸。由于DOX-GA引起显著的肿瘤线粒体功能障碍和O2消耗减少, 增强了PDT疗效[52]。结果表明, 线粒体靶向化疗和PDT在诱导肿瘤细胞凋亡, 抑制肿瘤生长和转移中显示出协同作用。体内实验结果证明肿瘤转移几乎被完全抑制。通过整合线粒体靶向化疗和PDT用于转移性乳腺癌治疗是一种有前途的策略。

SDT是通过声敏剂结合超声(US) 辐射来杀伤肿瘤细胞。与PDT相比, SDT具有以下优势: ① US是机械波, 具有微创性和深层组织穿透性; ②廉价且无放射性; ③高精度瞄准病变区域, 因此保持正常组织未受损[53]。声敏剂在肿瘤细胞特异性积累的能力可以极大影响治疗效率。Costley等[54]设计了一种纳米液滴IR780-NDs, 由于US激发的声学液滴汽化(acoustic droplet vaporization, ADV) 效应显著促进了IR780-NDs从循环系统向肿瘤区域的传输, 增强肿瘤组织内的穿透深度。此外, IR780-NDs具有线粒体靶向能力, 可提高SDT精度和准确性。在体外, 线粒体靶向后观察到ROS的过量产生, 使得癌细胞更易受到ROS诱导的细胞凋亡的影响[55]。IR780-ND可实现光声和荧光成像, 且由于ADV产生的气泡, 还可增强US成像, 这为SDT引导和监测提供了潜力。因此, IR780-NDs可成为用于癌症治疗的有前途的治疗诊断纳米平台。

PAI是一种常用于联合治疗的非侵入性成像技术, 能利用非电离辐射提供具有较高空间分辨率和成像深度的三维图像, 实现生物体内线粒体靶向纳米系统的实时跟踪, 以进行癌症治疗期间的监测和指导[56]。Yang等[57]设计了用于PAI指导诊疗的纳米粒Pt(IV)-NPs, 由生物素标记的Pt (IV) 前药衍生物和负载IR780的环糊精构成。它将靶向配体、显像剂和药物整合, 避免了不稳定的Pt载药量、过早泄漏, 实现了诊疗一体化。Pt(IV)-NPs克服顺铂耐药性主要基于IR780对癌症线粒体的特异性靶向, 其在线粒体内局部释放顺铂以诱导显著的ROS升高和线粒体功能障碍如ATP消耗、线粒体膜电位损失和mtDNA损伤, 从而大大降低细胞内谷胱甘肽(GSH) 水平, 避免了GSH对顺铂耐药癌细胞(A549R) 解毒[57]。同时, 与近红外激发的IR780光热疗联合, 进一步消耗GSH以克服顺铂耐药性。

2 线粒体靶向纳米系统的设计与构建

为实现线粒体特异性靶向和高效药物释放, 通过化学合成构建线粒体靶向纳米系统, 需要满足以下条件: ①血液循环中稳定性好; ②能选择性地从血液输送至肿瘤组织而非正常组织区域; ③能特异性地从组织液中被肿瘤细胞摄取; ④进入细胞基质后逃逸其他细胞器摄取和防止酶裂解影响并顺利靶向线粒体; ⑤进入线粒体能充分释放药物作用于靶点。因此, 线粒体靶向纳米材料的设计要求和应用标准比传统药物递送材料更加严格。表 1[44, 58-73]总结了部分成功靶向线粒体的纳米系统最新实例, 并对小分子及小分子前药组装形成的纳米系统、脂质体、纳米胶束、介孔二氧化硅、树突状聚合物、电荷可翻转的纳米共聚物、脂质壳-聚合物杂交纳米粒、金属有机骨架和多肽修饰的纳米系统进行介绍。

2.1 基于小分子及小分子前药形成的纳米系统

与正常细胞线粒体膜电位(-160 mV) 相比, 癌细胞中线粒体膜电位有所增加(-220 mV)[74, 75], 同时, 为了药物高效跨过线粒体膜, 纳米系统需一定的亲脂性, 目前已报道了一些具亲脂性且带正电的小分子, 通常称为离域亲脂性阳离子(delocalized lipophilic cations, DLCs)[76]

TPP是用于线粒体靶向的小分子之一[77], 已被广泛用于线粒体靶向递药纳米系统中。最具代表性的是, Battogtokh等[58]将多西他赛(DTX) 与TPP偶联实现线粒体靶向性, 并将TPP-DTX负载到叶酸-胆固醇白蛋白纳米粒(FA-chol-BSA NPs) 用于抗肿瘤且疗效显著。

除了TPP, 还有其他DLCs如胍和罗丹明及最新报道的小檗碱(berberine, BBR)[59]、5BMF[60]、生物素等都可靶向线粒体。如Cheng等[59]组装了GSH响应释放的PTX-ss-BBR纳米粒, 用于协同治疗癌症。BBR协助纳米粒靶向线粒体, 在肿瘤环境高浓度GSH的触发下, 在线粒体内聚集和药物释放, 提高了抗癌效率, 降低了药物毒性。此外, 小分子5BMF是一种新型的线粒体靶向DLC, 具有良好的抗肿瘤活性且有生物成像功能, 目前逐渐应用于纳米药物抗肿瘤研究[60, 78]。Qian等[60]开发了5BMF与人血清白蛋白HSA偶联的5BMF@HSA复合纳米体系以进一步提高5BMF溶解度(1.61~5.41 mg·mL-1) 和荧光强度(> 2倍)。细胞内分布评估结果表明5BMF@HSA能从溶酶体逃逸并成功靶向进入线粒体。与中/碱性环境相比, 酸性条件下5BMF@HSA复合物释放出近10倍5BMF。5BMF@HSA和5BMF在μmol·L-1和nmol·L-1范围内对肿瘤细胞均表现出选择性毒性, 并有效抑制小鼠模型的肿瘤生长。

此外, 钌(Ru) 和铱(Ir) 配合物也可靶向定位于线粒体。如EGFR (epidermal growth factor receptor) 广泛存在于肿瘤细胞表面, 其抑制剂可通过改变ErbB轴的功能从而影响肿瘤细胞周期、增殖和生长等过程实现抗肿瘤[79], 其主要耐药机制之一是肿瘤细胞表面EGFR易位至线粒体而导致的化疗耐药性[80]。Ilmi等[61]设计了一种靶向线粒体中EGFR的新型诊疗剂, 将金属络合物[Ru(bipyridine)3]2+的荧光特性与小分子((4-bromophenyl)amino)quinazoline对易位至线粒体内的EGFR的抑制作用相结合, 改变线粒体膜电位释放Cyto C, 实现肿瘤细胞线粒体靶向诊疗的平台构建。

2.2 基于脂质体的纳米系统

脂质体是一种具有双分子层的人工膜, 一般采用高压均质法、乙醇注入法、旋转蒸发超声法和微流控法制备[81], 其可携带亲水性和亲脂性药物, 前者分布在核心隔室内, 后者分布在双层膜内[82]。如Peng等[62]制备了葡萄糖和TPP共修饰的多靶点氧化还原敏感脂质体Lip-SPG (图 2), 以有效递送DOX和化学增敏剂氯尼达明(LND) 用于胶质瘤治疗。体外实验表明, Lip-SPG由葡萄糖和TPP介导, 对大脑、肿瘤和线粒体具有多靶点定位的能力, 可提高癌细胞线粒体对DOX和LND的摄取, 有效抑制肿瘤细胞增殖和诱导细胞凋亡。此外, Lip-SPG对线粒体功能有显著干扰, 如减少细胞内ATP的产生、诱导ROS产生、促进线粒体膜电位去极化。通过引入GSH敏感的二硫键和聚乙二醇使DOX和LND具有良好的药代动力学特性、脑靶向能力、对正常组织的低毒性和良好的抗胶质瘤疗效。

2.3 基于纳米胶束的纳米系统

纳米胶束是由两亲性共聚物在水性介质中自组装形成的具有核-壳结构的纳米载体, 具有制备简易、工艺稳定、粒径小的优势。胶束不仅能显著改善药物溶解性, 同时能通过EPR效应增加药物肿瘤部位的积累, 提高肿瘤化疗效果[83], 一定程度上逆转肿瘤的耐药性。

多种天然药物活性成分通过刺激内源性线粒体信号转导途径诱导肿瘤细胞凋亡, 但其溶解性和膜渗透性差严重抑制了其抗肿瘤效果。Han等[63]制备了一种装载去甲斑蝥素的纳米胶束系统TPP-PEG-PCL, 其明显增加肿瘤细胞对天然药物活性成分去甲斑蝥素的摄取, 并将药物聚集在线粒体, 通过降低线粒体膜电位、提高细胞内ROS水平、增加Bcl-2、减少Bax的表达等增强抗肿瘤疗效。Kang等[64]采用薄膜水化法制备载紫杉醇的纳米胶束TPP-PEG-PE, 肺癌细胞毒性评价结果显示载紫杉醇纳米胶束TPP-PEG-PE促细胞凋亡效果良好, Hoechst染色提示凋亡肺癌细胞出现了大量形态学改变, 纳米胶束可明显提高促凋亡caspase-3活性及减少抗凋亡蛋白Bcl-2和c-IAP1表达量, 优于PEG-PE纳米胶束和紫杉醇。

2.4 基于介孔二氧化硅的纳米系统

介孔二氧化硅(mesoporous silica nanoparticles, MSNs) 是通过溶胶-凝胶法、微波辅助技术、化学蚀刻技术、模板法等制备的一种有序介孔材料[84, 85]。该材料具有生物相容性好、比表面积高、尺寸可控、可降解等特点[86, 87]。通常, 快速膨胀的肿瘤细胞压迫附近淋巴管, 延长了纳米载体到病灶的累积时间, 因此, 单纯的被动靶向往往因短时间无法实现的药物积累而影响疗效。为了进一步提高MSNs载药靶向性, López等[65]采用肿瘤细胞和线粒体双靶向策略, 开发了由2个靶向部分修饰的MSNs, 一半采用叶酸修饰, 能结合肿瘤细胞膜表面的叶酸受体, 另一半采用TPP修饰, 能响应线粒体膜电位驱动进入线粒体中, 多数肿瘤细胞的膜表面叶酸受体分布较多, 故采用叶酸修饰纳米载体可较大程度增加药物在肿瘤部位的累积(图 3)。

2.5 基于树突状聚合物的纳米系统

树突状聚合物(dendrimers) 具有超支化分支结构, 该结构可填充疏水性药物小分子至聚合物空隙, 也可将药物接枝到聚合物链上(图 4)[88]。为有效调控dendrimers靶向线粒体的能力, 有学者研究TPP的共轭密度对聚(酰胺基胺) 树状大分子线粒体靶向的影响[89], 以TPP与胺封端的第四代聚(酰胺基胺) (PAMAM) 树枝状聚合物(G4NH2) 纳米载体的缀合物为研究对象, 通过调节TPP与G4NH2间的PEG接头长度, 控制表面的TPP密度进行优化, 并评估其生物活性和线粒体靶向能力。结果表明, 未加PEG时, 在低TPP密度(~5 eq) 下, 细胞摄取和线粒体靶向均增加。在高TPP密度(~10 eq) 下, 细胞摄取和线粒体靶向性进一步提高, 而纳米载体也显示出明显细胞毒性。添加PEG后, 毒性降低, 随着PEG长度的增加, 纳米载体的线粒体靶向能力不受影响[90]

姜黄素抗癌活性强, 但芳香结构的电子致密性显著减少非极性姜黄素摄取到线粒体中, 阻碍了疗效[90]。Kianamiri等[66]采用第四代树突状聚合物PAMAM接枝TPP并缀合姜黄素进行体内外活性评价。研究表明, 靶向树状聚姜黄素(TDC) 可成功将药物递送到癌细胞线粒体, 并阻滞细胞周期G2/M。此外, 在小于25 μmol·L-1姜黄素剂量下, TDC显著降低了ATP和GSH水平, 并增加了离体大鼠肝细胞线粒体的ROS水平。体内研究表明, 与游离姜黄素相比, TDC治疗后具有显著的肿瘤抑制作用和存活率改善。除其靶向性和安全性外, 溶解性和稳定性也得到改善。

胶质母细胞瘤由于BBB阻碍了治疗剂向大脑的输送, 限制了传统疗法和新型疗法在目标部位的疗效[91]。Sharma等[67]报道了一种与易位蛋白(TSPO) 配体DPA点击修饰的dendrimers, 并在体外证明了dendrimers-DPA共轭物(D-DPA) 显著增加了树状聚合物与线粒体的共定位。体内研究表明D-DPA特异性靶向肿瘤相关巨噬细胞内的线粒体, 且可进一步修饰以靶向特定的细胞内区室以进行细胞器特异性药物递送。

BBB和多药耐药(MDR) 被认为是PTX化疗失败的主要障碍。肿瘤细胞因自身增殖消耗大量葡萄糖, 导致GLUT1过度表达, 约为转铁蛋白受体的100倍[92]。肿瘤细胞内(2~10 mmol·L-1) 的GSH远高于细胞外浓度(2~20 μmol·L-1)[93]。Ma等[68]设计了一种针对PTX耐药性的靶向递药载体glucose-PEG-peptide-TPP-PAMAM-PTX, 以PAMAM为核心, 分别通过氨基和TPP偶联, 二硫键和药物PTX偶联, MMP2敏感肽(GPLGIAGQ) 与长循环PEG层偶联。该材料首先靶向富含GLUT1的肿瘤细胞, 接着MMP2水解肽段导致PEG层剥离, 暴露的TPP引导靶向线粒体, PTX将在细胞质和线粒体中快速释放。由于细胞内高浓度PTX阻碍P-gp的外排效应, 从而克服多药耐药性[68]

2.6 基于电荷可翻转的纳米共聚物的纳米系统

电荷可翻转的纳米共聚物是一种通过阴离子改性用于屏蔽纳米系统的正电荷的共聚物, 避免与其他蛋白非特异性结合而被消除[94]。正常生理环境中(pH 7.4) 该共聚物呈中性或负电性, 可降低网状内皮系统巨噬细胞对纳米药物的摄取, 同时保证其血液循环中的稳定性; 但当到达肿瘤部位时, 其电位发生电荷反转, 与肿瘤细胞表面的亲和力显著增强, 有利于纳米粒子在肿瘤细胞中的蓄积[95]

Wang等[69]开发了基于N-(2-羟丙基) 甲基丙烯酰胺(HPMA) 共聚物(MPC) 的新型线粒体靶向电荷可逆的多功能纳米载体MPC, 由2个带相反电荷的HPMA共聚物形成(图 5), 即带正电荷的胍氨基分子改性的共聚物和电荷可逆的2, 3-二甲基丙酸酐(DMA) 修饰的共聚物(P-DMA), 经验证MPC可在血液循环中保持稳定, 可响应肿瘤微酸环境立即暴露胍基的正电荷, 同时促进内吞作用、溶酶体逸出和线粒体靶向。体外实验表明, 与不具有胍改性的共聚物相比, MPC在模拟肿瘤环境中的细胞吸收和线粒体靶向能力分别增加了4.3%和23.8倍。在携带B16F10肿瘤C57小鼠上进行体内实验, MPC显示肿瘤部位的最高积累, 肿瘤抑制率为82.9%[69]

2.7 基于脂质壳-聚合物杂交纳米粒的纳米系统

脂质壳-聚合物杂交纳米粒(lipid-polymer nanoparticles, LPNPs) 由聚合物核心和生物相容的脂质壳构成。与一般脂质体相比, LPNPs具有较长半衰期。Zhang等[70]提出了一种通过改变脂质体组分/比例来调节脂壳-聚丙交酯-共乙酰化(PLGA) 核壳纳米粒(图 6) 在细胞器的分布。通过微流控技术逐层组装和高通量筛选杂化纳米粒子, 制造具有不同表面性质的LPNPs。通过提高脂质壳中的DOTAP与DOPE比例, 分析了杂化纳米粒在线粒体内的分布趋势。同时系统研究了DOTAP和DOPE的功能, 表明阳离子和中性辅助脂质对溶酶体逸出以及线粒体靶向的增量作用。

2.8 基于金属有机骨架的纳米系统

金属有机骨架(metal-organic framework, MOF) 是一类由无机金属离子与有机配体配位形成的多孔材料[96]。与其他纳米药物载体相比, MOF具有高孔隙率、结构可调、尺寸可控、易于修饰等优点, 此外MOF表现出独特优势: ①易制备且稳定性好, 由无毒金属(Fe、Zn、Ca、Mg等) 和低毒性羧酸或膦酸组装; ②生物可降解, 尤其暴露于水时; ③其内部微环境适用于递送具有不同活性的药物分子[96]。这些特性使MOF成为理想的生物医药应用材料, 如药物或成像剂的递送等, 材料表面修饰改性进一步丰富了MOF作为递药平台治疗疾病的策略, 如光热疗和化疗联合, 超声疗法和化疗联合等组合治疗策略[97]。目前研究较多的是ZIF[98, 99]、UiO[100]、PCN[101]等系列。Haddad等[102]设计并开发了TPP@(DCA5-UiO-66), 又称“特洛伊木马”, 该材料提高了二氯乙酸钠(DCA) 的抗癌疗效, 与游离药物相比, 所需剂量减少至不足1%, 与非线粒体靶向MOF相比, 减少至约10%。

2.9 基于多肽修饰的纳米系统

多肽由于良好的生物相容性和低毒性, 与小分子相比, 其作为线粒体靶向配体更具有优势。目前广泛用于线粒体靶向纳米系统的多肽是线粒体穿透肽(MPPs)、线粒体靶向信号肽(MTSs) 和Szeto-Schiller (SS) 肽。基于多肽的纳米系统还可运输功能性小分子、肽、聚合物、蛋白质和核酸进入细胞。

MPPs是人工合成的线粒体定位肽, 由4~16个氨基酸组成, 含阳离子和疏水残基[103]。与DLC相似, MPPs的线粒体靶向性取决于表面电位和亲脂性。如Mallick等[71]利用苯丙氨酸-精氨酸-苯丙氨酸-赖氨酸的MPP序列开发了疏水性药物抗霉素A (AMA) 的Chol-FRFK/D纳米脂质体制剂。脂质体的小尺寸和适度的阳离子电荷促进了有效的细胞摄取而降低毒性。线粒体膜电位评估结果表明Chol-FRFK/D脂质体和AMA协同作用导致肿瘤细胞线粒体内膜电位的完全破坏。

与MPPs不同的是, MTSs是由20~40个氨基酸组成的多肽, 主要通过结合线粒体外膜进入线粒体, 但其溶解性差限制了该药物靶向递送方面的应用[71]。Battogtokh等[72]设计了表面修饰线粒体靶向信号肽的纳米脂质体MITO-Porter, 并验证了纳米粒表面的MTSs可与线粒体外膜上的TOM/TIM复合物结合, 增强了对线粒体的选择性结合能力。

SS肽是一种亲脂性阳离子的替代品, 通常由4个带正电的氨基酸组成[104, 105]。SS肽主要有SS01 (Tyr-D-Arg-Phe-Lys-NH2)、SS02 (Dmt-D-Arg-Phe-Lys-NH2) 和SS31 (D-Arg-Dmt-Lys-Phe-NH2)[105]。此外, 它还具有一定抗氧化作用, 是治疗线粒体相关疾病的一种载体成分。这些SS肽可减少线粒体ROS产生, 抑制线粒体通透性转换孔开放, 在防止氧化应激或抑制线粒体电子传递链诱导的细胞凋亡和坏死方面效果显著[106]。TPP的靶向性依赖于线粒体膜电位, 在治疗线粒体膜电位减弱或丧失心肌缺血患者时存在局限性。SS-31通过与心磷脂相互作用特异性地集中在线粒体内膜, 可应用于线粒体膜电位异常的患者的治疗中[107]

两亲性尾部锚定肽(amphipathic tail-anchoring peptide, ATAP) 作为一种新型线粒体靶向肽已被证明具有诱导线粒体依赖性凋亡的活性。Yamada等[73]报道了磁性核壳纳米粒子(magnetic core-shell nanoparticle, MCNP) 搭载ATAP至恶性脑瘤和转移性乳腺癌细胞。与其他线粒体靶向肽不同, ATAP的细胞毒性效应不需促凋亡蛋白的存在, 也不受肿瘤中常发生突变的抗凋亡蛋白浓度的影响, 而是选择性靶向线粒体并通过干扰线粒体膜通透性释放Cyto C, 从而引发一系列变化, 导致细胞凋亡, 而MCNPs介导的局部热疗进一步改善疗效。

3 其他

关于线粒体作为靶点的研究, 除了纳米载体靶向递送药物到线粒体之外, 最近还有许多相关报道如线粒体移植、线粒体作为药物载体及线粒体膜包裹纳米载体作为生物仿生载体的研究等。

3.1 线粒体移植

癌细胞能量代谢的改变与线粒体功能障碍有关。癌细胞的线粒体功能障碍包括有氧糖酵解增加、ROS水平升高、细胞凋亡减少和对化疗药物的抵抗。而线粒体移植疗法是一种治疗线粒体功能障碍的新型策略, 即将正常细胞的线粒体引入癌细胞可能恢复线粒体功能并抑制癌细胞生长, 逆转化疗耐药性[108]。有研究指出, 分离出的功能性线粒体选择性进入人乳腺癌MCF-7和MDA-MB-231细胞, 但不进入正常乳腺上皮MCF-12a细胞。线粒体移植后, 抑制了有氧糖酵解并增加了人乳腺癌细胞对多种抗肿瘤药物(DOX、abraxane和卡铂) 的敏感性[109]。除化学疗法外, 放射疗法是癌症最重要的治疗方式之一。线粒体缺陷增加了人肝癌HepG2细胞的辐射抗性, 减轻了ROS介导的氧化损伤, 阻断线粒体凋亡途径。这表明线粒体移植可能是一种有效的放射增敏方式。但这项技术仍有许多关键问题需解决, 所以除了正在进行的临床研究外, 目前还未能广泛使用线粒体移植[110]。最重要的问题之一就是免疫反应, 目前研究显示自体线粒体移植是安全的, 但使用同种异体线粒体作为药物更实用。同种异体线粒体比自体更适合异体线粒体, 包括细胞和组织中存在线粒体DNA突变和线粒体功能障碍的患者。还有其他如怎样提取能保证线粒体的质量和功能的完整性和稳定性及储存等问题待解决[111]

3.2 线粒体作为药物载体

近年来, “用自身物质治疗自身疾病”的精准医学理念逐渐引起关注, 一些来源于细胞本身的物质在药物递送方面体现出良好的应用前景, 如外泌体和细胞外囊泡等。线粒体移植在治疗线粒体缺陷疾病的成功表明外源的线粒体能进入细胞中。有学者[112]设计了一种以正常结肠上皮细胞来源的线粒体为载体, 搭载化疗药物, 并具有肿瘤微环境响应的药物递送体系, 将其应用于结直肠癌的治疗。结果表明线粒体搭载药物后增强了药物杀伤肿瘤细胞的能力, 结合已有的线粒体在疾病治疗中的研究, 说明线粒体作为药物载体在结直肠癌治疗方面具有一定的应用前景。

3.3 线粒体膜包裹纳米载体作为仿生载体

在过去几年中, 越来越多地使用天然细胞膜来涂覆纳米材料以进行功能化制备仿生载体。其具备以下优点: ①跳过分子识别, 为纳米材料提供了类似细胞的功能; ②膜涂层提供了一系列细胞特异性蛋白质, 用于动态和多重结合相互作用, 从而产生功能驱动的广谱生物活性; ③不同膜涂层的特性也不同, 选择合适的涂层可满足延长半衰期, 降低免疫排斥反应和药物不良反应等。细胞膜包被的纳米材料在药物递送、解毒、免疫调节和生物传感方面具有广泛的应用。Gong等[113]通过膜-基质融合过程将从小鼠肝脏中提取的OMM包覆在纳米粒和场效应晶体管(FETs)。结果表明, OMM-NPs可以与靶向OMM的Bcl-2抑制剂ABT-263结合。OMM-NPs在体外可有效保护细胞免受ABT-263诱导的细胞凋亡, 同时也能在体内缓解由ABT-263造成的血小板减少。同时, 包覆有OMM的FET传感器(OMM-FET) 可对Bcl-2抗体和小分子激动剂进行检测和区分。上述研究结果表明OMM可被用于涂覆纳米粒子和功能器件, 证明OMM也可作为构建新型仿生载体的包覆材料。目前报道的线粒体膜包裹纳米载体作为仿生载体的研究较少, 但在药物递送系统开发领域有很大潜力。

4 总结与展望

本综述总结了线粒体靶向纳米药物递送系统用于癌症治疗的最新成果和突破, 同时对其进行了分类和讨论。纳米药物递送系统搭载药物至肿瘤部位, 通过干扰线粒体内能量代谢、调节ROS水平、调控细胞凋亡相关蛋白、损伤mtDNA、调控线粒体自噬等策略改善了癌症治疗效果, 线粒体靶向PDT、PTT和CDT等已成为有前景的治疗策略, 避免了手术等传统治疗方式的高风险性。

线粒体靶向纳米药物递送系统的优点包括: ①精准给药, 直接将药物递送到靶点, 改善化疗疗效和患者适用性, 降低不良反应; ②便于基团修饰和化合物高效负载, 以配合不同联合疗法的需求; ③可控释放药物, 良好的药物释放特性适应临床治疗需求。线粒体靶向纳米材料的设计中也存在一些问题: ①生物安全性有待改善, 线粒体靶向分子多带正电荷易引起毒性, 更安全的靶向分子亟待进一步开发; ②线粒体靶向纳米材料缺乏统一的评价标准; ③监测疾病生物标志物的线粒体靶向探针仍需进一步开发。

目前线粒体靶向纳米材料递药系统的靶头设计偏向采用小分子如TPP、guanidine等, 但许多研究者也意识到生物安全性问题, 近几年逐渐采用各种序列的多肽作为靶头, 甚至向仿生领域进发, 希望能挖掘低毒甚至无毒的线粒体靶向仿生药物载体, 靶头作为最关键部分, 其安全性和靶向效果将是未来开发该类纳米载体的重点考察因素。另外, 联合治疗往往比单药治疗效果要好, 但联合治疗的方法研究和安全性评价还不够细化, 如光热源、声源等使用条件的考察、纳米载体的生物可降解性及药物的代谢产物、免疫排斥反应等。重要的是, 潜在的分子机制需要进一步研究, 需要药学、化学、生物学、材料学等各领域人员更频繁地合作, 开展深入研究, 实现精准治疗, 攻克癌症。还有很多工作亟待学者们共同探索研究, 希望充分发挥线粒体靶向纳米材料递药系统设计优势, 为解决人类健康问题提供更有效的方案。
