Nrf2抑制剂在肿瘤治疗中的研究进展

摘要: 核因子E2相关因子2(Nrf2)是一种多功能的转录因子,在细胞保护机制中起着至关重要的作用。随着Nrf2调控新模式和新功能研究的不断深入,Nrf2在恶性肿瘤中的促癌作用日益凸显。越来越多的证据表明Nrf2参与了肿瘤的发生、发展、转移和预后的整个过程,抑制Nrf2是一种较有前景的肿瘤治疗策略。目前,Nrf2抑制剂的研究尚处在早期阶段。本文简要介绍Nrf2的生物学功能,以及其在肿瘤发生中的双重作用,并根据结构类型对代表性的Nrf2抑制剂进行综述,为围绕Nrf2调控开发抗肿瘤药物提供借鉴及思路。

Abstract: Nrf2 is a multi-effect transcription factor, which plays a crucial role in cytoprotective system. With the deepening of research on new regulatory modes and biologic functions of Nrf2, the oncogenic role of Nrf2 in malignant transformed tumors is increasingly obvious. More and more evidences show that Nrf2 is involved in the whole process of tumor occurrence, development, metastasis and prognosis, and inhibiting Nrf2 may be a promising strategy in tumor therapy. However, the development of Nrf2 inhibitors is still in early stage. In this paper, the biological function of Nrf2 and its dual role in tumor are briefly introduced, and representative Nrf2 inhibitors are reviewed according to their structure types, so as to provide reference and ideas for the development of anti-tumor drugs centering on the regulation of Nrf2.

核因子E2相关因子2 (nuclear factor erythroid 2-related factor, Nrf2) 是一类碱性亮氨酸拉链(basic region-leucine zipper, bZIP) 转录因子, 由NFE2L2基因编码[1]。Nrf2可与小肌肉筋膜性纤维肉瘤(small musculoaponeurotic fibrosarcoma, sMAF) 蛋白形成异源二聚体, 该二聚体可识别并结合位于调控区域的抗氧化反应元件(antioxidant response element, ARE) 来增强靶基因的转录。一般而言, Nrf2调控的基因主要参与氧化应激调节, 包括谷胱甘肽系统、硫氧还蛋白抗氧化系统以及Ⅰ相、Ⅱ相和Ⅲ相药物代谢相关的酶。通过诱导这些基因的转录, Nrf2可以激活细胞防御系统, 保护细胞免受外源性和内源性的侵袭。因此Nrf2被认为是调节细胞保护系统的核心转录因子[2]

Nrf2除了在细胞保护中发挥重要作用, 其还调节多种功能基因的转录, 特别是细胞代谢相关的基因[3]。Nrf2通过调控关键酶的表达, 以调节脂类、碳水化合物、核苷酸和氨基酸的代谢。此外, Nrf2还参与调控几种蛋白酶体亚基和自噬基因的表达, 进一步增强了Nrf2在代谢调节网络中的影响。一般而言, Nrf2的激活可增强合成代谢途径, 有利于癌细胞的增殖。在肿瘤发生发展过程中, 激活Nrf2可提高肿瘤细胞的抗氧化能力和药物代谢能力, 从而提高肿瘤细胞的存活率[4]。此外, Nrf2还参与肿瘤代谢网络改变, 使肿瘤更倾向于有氧酵解[5]。Nrf2在多种不同类型的肿瘤细胞中过度激活[6], 这些研究结果进一步证实了Nrf2的致癌作用, 开发Nrf2抑制剂将可能是一种有效肿瘤抑制策略。

然而靶向转录因子的抑制剂开发具有更大的挑战性, 特别是Nrf2复杂的调控网络进一步增加其研究的难度。到目前为止, Nrf2抑制剂的开发仍处于早期阶段。本文主要介绍Nrf2的生物学功能和调控网络以及Nrf2的致癌作用, 并对已报道的Nrf2抑制剂进行综述, 希望能够对Nrf2抑制剂的进一步研究有所帮助。

1 Nrf2的生物学功能和活性调节系统

机体时刻面对各种来源的氧化性和亲电性的化学物质, 这些化学物质可能对组织和细胞产生危害。为降低和缓解这种伤害, 机体形成了一套防御系统, 即严格控制参与应激反应的各种途径。在应激条件下, 防御系统可以感知异常, 上调细胞保护酶基因表达水平, 以抵抗各种有害损伤。在这些应激反应通路中, Nrf2属于bZIP转录因子CNC (cap-n-collar) 亚家族, 是细胞对环境应激反应的主要调节因子[7]。Nrf2和sMAF蛋白的异源二聚体可以识别并结合位于这些基因上游的顺式调控元件序列(5′-GTGACnnnGC-3′), 从而诱导下游细胞保护酶基因的转录。

Nrf2可同时调控两个最重要的抗氧化系统, 分别为谷胱甘肽(GSH) 和硫氧还蛋白依赖性的抗氧化系统[4]。在谷胱甘肽依赖性的抗氧化系统中, Nrf2可调节GSH的生物合成、利用和相应的再生酶。在GSH的生物合成中, 谷氨酸-半胱氨酸连接酶催化(GCLC) 和调节(GCLM) 亚基可结合形成异源二聚体, 在催化GSH生物合成中发挥限速作用。Nrf2通过控制GCLC和GCLM的转录, 影响谷胱甘肽的生物合成[8, 9]。谷胱甘肽过氧化物酶(GPx) 可利用谷胱甘肽作为还原剂来减少过氧化物。Nrf2可通过调节GPx-2和GPx-4的表达来调节谷胱甘肽的消耗过程。此外, 谷胱甘肽还原酶(glutathione reductase, GSR) 也是Nrf2的靶基因之一, 主要负责从氧化态的GSSG转化为还原态的GSH, 维持还原态和氧化态GSH的平衡。硫氧还蛋白也是一类广泛存在的蛋白质, 可通过半胱氨酸-硫醇-二硫化物交换促进其他蛋白质的还原, 发挥抗氧化剂的作用。硫氧还蛋白抗氧化系统包括硫氧还蛋白(thioredoxin, TXN)、硫氧还蛋白还原酶(thioredoxin reductase, TXNRD) 和sulfiredoxin (SRXN), 而这些均可被Nrf2调控[10-13]

除了这些抗氧化蛋白和酶外, Nrf2还介导一系列药物代谢酶的转录, 包括Ⅰ相和Ⅱ相药物代谢酶以及Ⅱ相代谢相关的药物转运体。这些药物代谢酶可催化多种类型的药物代谢反应, 包括由细胞色素P450蛋白(cytochrome P450 proteins, CYPs) 氧化, 醛脱氢酶(aldehyde dehydrogenases, ALDHs) 和醇脱氢酶(alcohol dehydrogenases, ADHs) 等介导的氧化反应; 由NAD(P)H还原醌氧化还原酶1 (NQO1) 和醛酮还原酶(aldo-keto reductases, AKRs) 介导的还原反应; 由葡萄糖醛酸转移酶(UDP-glucuronosyltransferases, UGTs) 和磺酸转移酶(sulfotransferases, SULTs) 介导的结合反应, 这些反应几乎涵盖了所有药物代谢过程, 有利于促进肿瘤细胞对药物代谢[14, 15]。Nrf2还可调节NADPH的循环过程[16]。NADPH是体内重要的还原性物质, 在多种抗氧化和代谢途径中起辅因子作用。值得注意的是, 几乎所有可产生NADPH的酶, 包括葡萄糖-6-磷酸脱氢酶(glucose-6-phosphate dehydrogenase, G6PD)、6-葡萄糖磷酸脱氢酶(6-phosphogluconate dehydrogenase, PGD)、异柠檬酸脱氢酶1 (isocitrate dehydrogenase 1, IDH1) 等都受Nrf2调控[15]。磷酸戊糖途径(pentose phosphate pathway, PPP) 是一种生成NADPH、五碳糖戊糖以及5-磷酸核糖的糖酵解途径。G6PD和PGD是PPP过程中的关键酶, 可通过PPP途径影响NADPH的生成。Nrf2还可调控PPP中的其他酶, 如酮糖转移酶(Tkt) 和醛缩转移酶1 (Taldo1) 等[17], 以上研究表明Nrf2在增强PPP中起着关键作用。

由于Nrf2在细胞应激、代谢和氧化还原调节中起中枢性的作用, 细胞内有复杂和精细的Nrf2的活性调节机制。Nrf2是一种组成型表达的蛋白, 在细胞内的半衰期较短, 细胞内有多重降解机制负责Nrf2蛋白的降解, 构成了以Nrf2含量调节为核心的Nrf2活性调节的主导机制[18]。其中, 泛素E3连接酶Keap1介导的泛素化降解途径是控制Nrf2蛋白水平最主要的途径。Nrf2的Neh2结构域中的两个保守序列ETGE (AA 79-82) 和DLG (AA 29-31) 是与Keap1的Kelch结构域相互作用的结合位点[19, 20]。“铰链和门闩”模型是Keap1和Nrf2相互作用模式的主要假说[20]。根据此模型, Nrf2的ETGE序列能够以高亲和力与Keap1结合, 被称为铰链, 而Nrf2的DLG序列与Keap1的亲和力较弱, 被称为门闩。在正常生理状况下, Keap1同源二聚体同时与Nrf2的ETGE和DLG序列结合, 形成2∶1的复合物, 锁定在闭合构象。随后, Keap1的BTB结构域与CUL3 (cullin 3) 的N端区域发生相互作用, 组成完整的E3泛素连接酶Keap1-CUL3-RING (CRLKEAP1) 复合物[21], 执行对Nrf2多泛素化标记并促使其被26S蛋白酶体快速降解[22]。Keap1含有多个高反应活性的半胱氨酸残基[23]。这些半胱氨酸残基对Keap1二聚体与Nrf2结合形成三元复合物起重要调节作用。其中典型的是Keap1的BTB结构域的Cys151, 以及位于IVR结构域的Cys273和Cys288[24]。当细胞遭到氧化应激或亲电物质攻击时, Keap1的部分半胱氨酸残基被修饰, 使蛋白构象改变, 不能形成稳定的闭合构象, 而主要处于开放构象[25]。此构象下, Nrf2不能被Keap1构成的E3泛素连接酶复合体泛素化(图 1)。因此, 当细胞处于氧化应激或是亲电物质含量升高时, Nrf2蛋白的水平在短时间内迅速升高, Nrf2随后转移入核, 诱导下游靶基因的表达, 激活细胞内防御系统[26, 27]

除此之外, β-TRCP也能在糖原合成激酶3 (glycogen synthase kinase 3, GSK-3) 的帮助下介导Nrf2的泛素化降解。Nrf2的Neh6结构域含有两段保守序列DSGIS (AA 334-338) 和DSAPGS (AA 373-378) 可作为β-Trcp蛋白的底物识别序列[28]。其中位于DSGIS序列中的Ser335和Ser338能够被GSK-3磷酸化[29], 磷酸化的DSGIS序列能够被β-Trcp识别, 随后Nrf2可被SKP1-CUL1-β-Trcp (SCFβ-TrCP) 复合物泛素化进而通过蛋白酶体降解。同时, 由于体内的GSK-3激酶活性受到PI3K-Akt通路的调节, 通过此种模式, 就将Nrf2的活性与细胞内和细胞外的多种信号通路建立其调控的关联性。此外, HRD1也能介导Nrf2的泛素化降解并负调控Nrf2活性[30]。例如, 在肝硬化等导致内质网应激时, HRD1水平上调并导致Nrf2泛素化增加以及Nrf2介导的抗氧化通路受损[30]。另外, CRIF1也能促进Nrf2的泛素化降解, 且该过程与细胞是否受到氧化应激无关, 在正常生理条件下同样能够进行, 其具体调节机制尚不明确[31]。除了泛素化修饰以外, 以蛋白质磷酸化修饰为代表的其他翻译后修饰途径也参与了Nrf2的活性调节[32], 从而实现了细胞内Nrf2活性的精细调节网络(图 2)。

2 Nrf2对肿瘤的两面性

Nrf2在癌症中的双重作用已经被广泛认可[33, 34]。正常情况下, Nrf2可阻止正常组织和癌前组织癌变, 阻止正常细胞向癌变细胞的转化; 在肿瘤组织中, Nrf2的过度激活可能有利于已经转化的癌细胞的存活和增殖。

早期研究表明, 增强Nrf2活性可抑制肿瘤发生, 而Nrf2表达缺失可提高多种致癌物对肿瘤发生的易感性[35]。Nrf2通过清除ROS和活性氮氧化物(RNS)、促进化学致癌物的代谢或者通过诱导其靶基因的表达修复氧化损伤等途径, 抑制肿瘤发生[36]。多项研究证实, Nrf2激活剂可使Nrf2-/-小鼠预防化学和辐射(电离、紫外线) 诱导的癌变。

另一方面, Nrf2作为一种细胞保护剂, 可增强细胞的存活率, 因此提高Nrf2活性也可使肿瘤细胞受益。Nrf2能显著抑制氧化应激, 降低肿瘤对化疗药物的敏感性[33, 37]; 此外Nrf2的激活还可提高谷胱甘肽和硫氧还蛋白抗氧化系统的活性, 增强肿瘤对细胞毒药物的解毒作用。Nrf2还可提高ATP依赖的药物外排泵如多药耐药系统(multidrug resistance system, MDR) 的活性, 导致肿瘤细胞内药物有效浓度降低, 增强肿瘤耐药。进一步的生物学研究表明, Nrf2可以通过复杂的机制为肿瘤细胞提供多种营养物质, 为肿瘤细胞的生长创造有利条件[38]。因此, Nrf2激活可能会对肿瘤细胞产生保护作用, 并且能够促进耐药肿瘤的发生和发展。

目前已经在多种癌症中发现转录因子Nrf2的过度激活, 其中NFE2L2功能获得突变和KEAP1功能缺失突变在肺癌的发生和发展中得到了深入的研究[39]KEAP1功能缺失性突变首次在人肺腺癌细胞系中发现, 突变的KEAP1蛋白对Nrf2蛋白的亲和力降低, 从而导致其对Nrf2的抑制作用下降。在61%的NSCLC细胞系和41%的肿瘤样本中, 等位基因缺失导致KEAP1活性下降[40, 41]。对不同组患者的分析得出相似的结果, 即KEAP1功能缺失突变激活Nrf2, 促进了肺癌细胞生长。最近, 众多的研究表明NFE2L2功能获得突变和KEAP1功能缺失突变对于肺癌细胞抵抗免疫治疗、放射治疗和化学治疗均起到了重要的促进作用[42-44]。另有研究显示应激蛋白TRIB3能够抑制肺癌细胞中Keap1-Nrf2相互作用, 从而上调Nrf2转录激活活性, 促进肿瘤增殖并降低化疗药物敏感性[45]

而在一些特定的肝癌中, 肿瘤细胞可利用过表达p62蛋白等方式, 通过异常的蛋白-蛋白相互作用, 在NFE2L2KEAP1基因未突变的情况下, 实现Nrf2的过度活化。p62蛋白可以识别Keap1中的Nrf2结合位点, 通过竞争性的方式结合到Keap1蛋白上, 从而使得Nrf2免于被泛素化降解, 实现Nrf2的异常活化[46]。而肿瘤细胞内信号通路异常导致的p62磷酸化可进一步增加p62和Keap1蛋白的亲和力, 增强Nrf2激活效应[47]。这种Nrf2的激活模式既可以有效帮助肿瘤起始细胞的存活从而促进肿瘤发生[48], 也有利于肿瘤细胞实现凋亡抵抗、药物耐药等效应促进肿瘤的进一步发展和恶化[49]

以上研究表明, 不同肿瘤细胞可通过多种模式实现Nrf2的过度激活。上调Nrf2活性不仅保护肿瘤细胞免受各种压力应激和氧化损伤, 还可调节下游靶基因的转录, 增加抗氧化蛋白、药物代谢酶、药物转运蛋白、还原酶等的表达, 同时还参与肿瘤的发病、进展、转移和预后的全过程。抑制肿瘤细胞内的Nrf2活性可通过多种机制实现肿瘤治疗, 包括破坏肿瘤细胞内氧化还原平衡、促进肿瘤细胞凋亡、逆转肿瘤细胞耐药并增强抗肿瘤免疫等[50, 51]。因此, 开发安全有效的Nrf2抑制剂可能是一种很好的肿瘤治疗策略。

3 Nrf2抑制剂研究进展

随着Nrf2激活的促癌功能被越来越关注, 越来越多的研究者致力于发现Nrf2抑制剂。迄今为止, 具有不同化学类型和结合靶点的分子已被报道显示Nrf2抑制作用[52]。尽管这些药物作用机制各异, 但它们在不同的肿瘤细胞中均显示了明确的Nrf2抑制作用。

3.1 黄酮类Nrf2抑制剂

一些天然黄酮类化合物被报道具有Nrf2抑制活性。但值得注意的是其中一些黄酮衍生物在不同的研究中对Nrf2活性表现出相反的作用。黄酮类衍生物汉黄芩素(wogonin, 图 3) 是传统中草药黄芩的主要有效成分之一, 具有抗炎、抗肿瘤等多种生物活性, 特别是耐药肿瘤的治疗中具有较好的疗效。研究表明, 在慢性粒细胞白血病K562/A02和K562R细胞中, 汉黄芩素可通过下调Nrf2来克服多柔比星(DOX) 和伊马替尼的耐药。此外, 在K562/A02异种移植模型中, 汉黄芩素(40 mg·kg-1) 显著增强DOX对肿瘤的抑制作用[53]

木犀草素(luteolin) 是一类存在于苔藓、蕨类、木兰花等植物中的黄酮类化合物。木犀草素可增强Nrf2的mRNA不稳定性, 降低Nrf2的mRNA和蛋白水平, 进而抑制Nrf2-ARE系统的活性, 提高A549细胞对抗肿瘤药物的敏感度[54]。另一项研究也显示luteolin可以通过抑制Nrf2+/+小鼠A549细胞移植瘤模型上的Nrf2通路来增强顺铂的抗癌作用, 而对Nrf2-/-小鼠则没有作用[55]。在奥沙利铂耐药和Nrf2活化的结直肠肿瘤HCT116-OX和SW620-OX细胞中, 木犀草素可浓度依赖地抑制细胞增殖[56]。木犀草素(5 μmol·L-1) 能显著提高奥沙利铂、顺铂和DOX在耐药细胞株HCT116-OX和SW620-OX中的抗癌功效。另一方面, 在急性氯化汞暴露引起的肝中毒模型中, luteolin又可增强Nrf2-ARE, 提高抗氧化防御系统活性, 减轻肝损伤[57]。以上研究表明, luteolin在不同的病理条件下, 对Nrf2可表现出不同的作用。

白杨黄素(chrysin), 又名5, 7-二羟黄酮, 已经被证明可以通过减少Nrf2进入细胞核的转运和抑制HO-1和NQO1的表达来抑制Nrf2信号通路[58]。白杨素(10 μmol·L-1) 可显著增加DOX在耐DOX细胞BEL-7402/ADM中疗效, 这一类细胞高表达Nrf2。通过在mRNA和蛋白表达水平抑制Nrf2, 同时下调其下游基因HO-1和AKR1B10, 白杨素(20 μmol·L-1) 可显著增加细胞内DOX浓度[59]

3.2 甾体类Nrf2抑制剂

不同于许多在微摩尔浓度下才能显示Nrf2抑制活性的化合物, 一些糖皮质激素在纳摩尔浓度下表现出显著的Nrf2抑制活性[60]。以地塞米松(dexamethasone, 图 4) 为代表的糖皮质激素可表现出糖皮质激素受体(GCR) 依赖的Nrf2抑制活性[61]。这些化合物的Nrf2抑制活性与GCR的激活活性相关, 表明了GCR参与Nrf2的活性调节机制。其中, 丙酸氯倍他索(clobetasol propionate) 能够通过激活糖皮质激素受体, 促进Nrf2的泛素化降解, 进而抑制Nrf2活性[62]

3.3 维甲酸类Nrf2抑制剂

全反式维甲酸(ATRA, 图 5) 又称维生素A酸或维甲酸, 与其他一些维甲酸受体(RAR-α) 激动剂是一类作用机制较明确的Nrf2抑制剂。ATRA不影响Nrf2的蛋白水平和核累积, 但其通过促进RARα与Nrf2形成复合物, 抑制了Nrf2与ARE结合[63]。研究发现RARα可结合Nrf2的Neh7区域, 复合物的形成显著抑制了Nrf2对ARE序列的识别。贝沙罗汀(bexarotene) 是一种RXRα特异性配体, 也可以通过依赖RXRα的方式抑制Nrf2的转录活性[64]

3.4 异烟肼类Nrf2抑制剂

异烟肼是一类经典的抗结核病药物, 对结核杆菌有抑制和杀灭作用。研究报道, 异烟肼可在多种小鼠和人来源的细胞系中抑制Nrf2-ARE的活性。其对Nrf2的抑制可有效干扰脂肪组织的形成[65]。进一步的研究发现, 异烟肼可以提高Hep3B细胞内Nrf2的胞质蛋白水平, 同时降低核内Nrf2蛋白水平[66]。一些异烟肼类似物, 比如异烟酸酰胺(isonicotinic acid, 图 6) 和乙硫异烟胺(ethionamide), 也表现出Nrf2抑制活性。其中乙硫异烟胺能以Nrf2依赖的方式增加急性单核细胞白血病THP-1细胞在肿瘤化疗中对As2O3的敏感性[67]。到目前为止, 这些抗结核小分子已被证实不会影响Nrf2的mRNA和蛋白质水平, 但是其抑制Nrf2机制仍不清楚, 需要进一步的研究来阐明这一独特的机制。

3.5 其他Nrf2抑制剂

二甲双胍(meformin, 图 7) 作为治疗2型糖尿病的一线药物, 也被证实能够抑制Nrf2-ARE系统。研究表明二甲双胍可降低HepG2细胞中Nrf2的mRNA和蛋白水平, 其下调机制不依赖Keap1和AMPK[68]。进一步研究表明, 二甲双胍诱导miR-34a抑制Nrf2通路, 增加野生型p53肿瘤细胞对氧化应激和凋亡的敏感性[69]。值得注意的是, 一些研究也报道了二甲双胍对Nrf2的激活作用, 说明二甲双胍在不同条件下对Nrf2会产生不同效应[70]

Brusatol是较早被报道的一种强效Nrf2抑制剂[71]。Brusatol能有效降低细胞内的Nrf2水平, 增强A549细胞等多种肿瘤细胞对化疗药物的敏感性[72]。研究表明, Brusatol的作用机制主要是通过抑制蛋白质翻译途径发挥作用。这种非选择性的作用机制限制该化合物的进一步研究。喜树碱(camptothecin) 是一种拓扑异构酶抑制剂, 同时也能抑制Nrf2。喜树碱(0.1或0.5 μmol·L-1) 能显著抑制HepG2、SMMC-7721和A549细胞中Nrf2表达和转录活动, 可在体外和异种移植瘤模型中(3 mg·kg-1) 均可增加这些细胞对表柔比星和5-FU的敏感性[73]

葫芦巴碱(trigonelline) 是一种豆科植物葫芦巴的提取物, 能够通过减少Nrf2蛋白的细胞核累积而抑制Nrf2活性。在抑制Nrf2的同时, 葫芦巴碱可阻断Nrf2依赖性的蛋白酶体基因表达, 导致蛋白酶体活性降低[74]。葫芦巴碱对Nrf2抑制作用可增加肿瘤细胞在体内外水平对化疗药物敏感性。常山酮(halofuginone) 是一种广谱抗球虫药, 其也被发现具有抑制Nrf2的活性[75]。作用机制研究表明, 常山酮主要通过引起氨基酸缺乏应激反应, 降低细胞内蛋白质合成进而减少细胞内Nrf2的含量。初步的药理学研究显示常山酮同样可以有效增加多种耐药肿瘤细胞对药物的敏感性。

最近, 被广泛研究的天然产物雷公藤甲素(triptolide) 也被报道具有Nrf2抑制活性。ChIP实验研究表明, 雷公藤甲素能有效抑制转录因子Nrf2结合到靶基因的启动子区域, 进而实现抑制Nrf2的转录活性。在IDH1突变的神经胶质瘤细胞中, 雷公藤甲素能显著地抑制谷胱甘肽的合成, 从而表现出对脑胶质瘤的选择性毒性作用[76]。但是雷公藤甲素抑制Nrf2的具体靶标和分子机制尚不清楚。

3.6 靶向Nrf2抑制剂

化合物ML385 (图 8) 是通过高通量筛选发现具有Nrf2-ARE抑制活性的化合物, 其能有效抑制Nrf2与靶基因的结合, 从而抑制其下游靶基因表达[77]。体外作用机制研究表明, ML385能够直接抑制Nrf2-MafG蛋白和含有ARE的DNA序列结合。ML385 (1或5 μmol·L-1) 与铂类药物、DOX或紫杉醇联合使用时, 能够显著增加这些药物在NSCLC细胞中的细胞毒性。在Nrf2高度活化的NSCLC细胞中, ML385 (30 mg·kg-1) 联合卡铂用药, 可产生显著抗肿瘤作用。

最近, Merck公司的研究小组报道了一种新的Nrf2抑制剂策略[78]。他们采用Nrf2/MafG/DNA三元复合物作为模型, 以和DNA结合的多肽片段作为结构模板, 设计获得了一系列能够有效抑制Nrf2/MafG复合物结合到ARE序列上的多肽先导物。这种直接抑制Nrf2/MafG和DNA中的ARE序列相互作用的作用模式具有较好的选择性, 避免了已有的Nrf2抑制剂特异性不足的问题, 为进一步探索发现具有临床开发价值的选择性Nrf2探索了新的路径。

针对部分肿瘤细胞系利用过表达的p62实现Nrf2的过度活化的特征, 已有研究团队利用此特点, 针对Keap1-p62的相互作用, 开发Keap1-p62选择性的抑制剂, 恢复Keap1对Nrf2的负调节作用, 进而实现Nrf2抑制的目标。K67是第一个报道的具有一定选择性的Keap1-p62抑制剂。K67能在p62过表达的Huh-1肝细胞癌细胞系中促进Nrf2的泛素化降解, 抑制Nrf2系统的活性, 从而增强顺铂等治疗效果[79]。随后, Tadahiko Mashino小组针对K67的化学结构开展了构效关系和结构衍生化研究。研究虽然未获得显著提升的化合物结构, 不过这些衍生物同样表现出在p62过表达的Huh-1肝细胞癌细胞系的Nrf2抑制和化疗增敏作用[80, 81]

4 Nrf2抑制剂在肿瘤治疗的问题与展望

Nrf2在肿瘤治疗中的双重功能已经进行了较深入的研究[82]。随着对Nrf2调控新模式和新功能研究的不断增多, Nrf2根据其在癌症特征中的作用而发挥的抑癌促癌作用也越来越明显[83]。Nrf2是否属于癌基因尚不确定, 但Nrf2的促肿瘤作用已在不同类型肿瘤中得到了较充分的验证。Nrf2活化与铁死亡[84, 85]、细胞自噬[86, 87]等肿瘤新特征的关系也得到了揭示。这些新进展进一步支持了抑制Nrf2可能是一种新颖的肿瘤治疗方法。相对于Nrf2的促癌功能研究的快速进展, 开发Nrf2抑制剂用于癌症治疗的研究仍然有限。目前现有的Nrf2抑制剂研究大多缺乏具体作用靶标, 很少有表现出直接和选择性的作用机制。考虑到Keap1依赖的Nrf2抑制系统在癌症中经常失活, 应该在癌症背景下探索不依赖于Keap1的Nrf2调控网络的全景图, 发掘能抑制Nrf2活性的潜在药物靶点。此外, 通过靶向细胞核中Nrf2相关的PPIs直接抑制Nrf2的转录活性, 特别是干扰Nrf2-MafG-ARE的相互作用, 是一种有前景的策略。然而, 对于Nrf2-MafG-ARE作用界面知之甚少, 限制了靶向抑制剂的发现。对Nrf2-MafG-ARE复合物的结构生物学研究将有助于相应抑制剂的开发。目前, Nrf2抑制剂的研究尚处在较早期的阶段, 突破性的进展尤其需要学科的交叉和合作。
