基于改善抗肿瘤药物疗效的聚合物混合胶束研究进展

摘要: 聚合物胶束已成为一种很有潜力的药物载体,然而部分单聚合物胶束往往存在一些性能缺陷,如载药量偏低、稳定性不佳和微环境响应能力弱等。采用两种或两种以上高分子聚合物自组装形成的混合聚合物胶束是解决这些问题的有效方法。本文尝试从聚合物混合胶束的分子间作用力、制备表征、性能优势和生物活性等方面进行综述,展示聚合物混合胶束的优势,分析存在的问题和不足,希望为抗肿瘤纳米胶束的发展提供借鉴。

Research progress of polymer mixed micelles based on improving the efficacy of antitumor drugs

Abstract: Polymeric micelles have become a potential drug carrier. However, some single-copolymer micelles often have some performance defects, such as low drug loading, poor stability and weak response to microenvironment. Mixed micelles self-assembled from two or more dissimilar macromolecular polymers provides a direct and convenient approach to improve drug delivery. This paper attempts to review the internal force, preparation and characterization, performance advantages and biological activity of mixed polymer micelles, aims to show the advantages of polymer mixed micelles, analyzes the problems and shortcomings, and hopes to provide reference for the development of anti-tumor nano-micelles.

聚合物胶束(polymeric micelles, PMs) 已成为一种很有潜力的药物载体, 广泛用于抗肿瘤药物的载体, 主要包括小分子药物、核酸和蛋白等物质。当水中的聚合物浓度超过临界胶束浓度(critical micelle concentration, CMC), 两亲性聚合物则自组装形成核-壳结构的纳米粒, 疏水性链段在内部聚集成核作为难溶性药物贮库, 亲水性链段在表面分布成壳赋予微粒稳定性。在PMs的研究开发过程中, 载药能力和稳定性是主要的表征参数。此外, 粒径分布、荷电情况和响应功能等也是胶束评价的核心指标, 这些与胶束的体内过程[1, 2]、组织分布[3, 4]和生物利用度[5, 6]息息相关。然而, 由于聚合物分子的结构类型和长度限制, 部分单聚合物胶束(single-copolymer micelles, SMs) 往往存在一些性能缺陷, 或体内稳定性不足, 载药量偏低; 或对肿瘤微环境无特异响应, 未能有序地调节药物摄取和释放。

采用两种或两种以上两亲性高分子聚合物的组合构建混合聚合物胶束(mixed polymer micelles, MMs), 是解决这些问题的有效方法。如Hou等[7]考察不同HS15/F127比率对icariside Ⅱ胶束理化性质的影响, 发现Solutol HS15/Pluronic F127 (4∶1) 具有较高的增溶能力和包封效果; Jin等[8]考察不同1, 2-二硬脂酰-sn-甘油基-3-磷酸乙醇胺-聚乙二醇-2000 (DSPE-PEG)/D-α-生育酚聚乙二醇1000琥珀酸酯(TPGS) 比率对药物滞留和肿瘤渗透的影响, 发现DSPE-PEG/TPGS (1∶1) 具有较低的CMC和较慢的释药行为, 且对MCF-7细胞有较好的渗透能力和细胞毒性; Bernabeu等[9]考察MCF-7、MDA-MB-231和SKOV-3三种不同细胞的摄取及毒性情况, 发现紫杉醇(paclitaxel, PTX)-Soluplus/TPGS (4∶1) 组的细胞摄取明显高于PTX-Soluplus组, 且TPGS的加入显著降低了各细胞的半数抑制浓度(half maximal inhibitory concentration, IC50), 表现出最强的细胞毒性; Liu等[10]考察发现, 聚[2, 8-(N-乙酰神经氨酸钠盐)]-胱胺-氯甲酸胆甾醇酯(PSA-CS-CH)/Pluronic F127混合胶束相比于Pluronic F127单聚合物胶束, 能显著增加多柔比星(doxorubicin, DOX) 摄取和细胞毒性, 且对高表达谷胱甘肽(glutathione, GSH) 的酸性肿瘤微环境响应拥有良好靶向作用和调节释放能力, 具备较强的抗肿瘤活性。

因此, 可以通过对混合聚合物的筛选及优化, 能获得多功能响应的纳米胶束, 然而两种或两种以上聚合物能否共胶束化, 则受到聚合物类型和比例的影响。MMs相比于SMs, 可通过疏水相互作用、离子相互作用、立体络合和氢键等不同作用力, 显著提高热力学、动力学稳定性, 同时, 可在载药负荷、粒径控制和功能响应等方面获得更多优势。本文尝试从聚合物混合胶束的内部作用力、制备表征、性能优势和生物活性等4个方面进行综述, 展示聚合物混合胶束的优势, 分析存在的问题和不足, 希望为抗肿瘤药物纳米胶束的发展提供借鉴。

1 混合聚合物胶束内的相互作用

不同两亲性高分子自组装形成的混合胶束, 主要依靠疏水相互作用、分子识别相互作用(如立体络合、氢键、离子相互作用) 和疏水链化学交联等作用力[11, 12], 维持纳米胶束的热力学稳定性, 见图 1

1.1 疏水相互作用

疏水相互作用是混合胶束体系中最常见的非共价相互作用。在这种情况下, 两种不同的两亲性聚合物在水溶液中通过疏水链段间的疏水作用形成MMs, 疏水链的类型和长短会影响疏水作用。一般而言, CMC较大的聚合物疏水作用较弱, 如Pluronic F68为聚环氧乙烷-聚环氧丙烷-聚环氧乙烷(PEO-PPO-PEO) 的嵌段聚合物, 疏水链PPO之间相互作用力影响核-壳结构中核的稳定性, 由于其亲水亲油平衡值(HLB值) 大于24, PPO链段间相互作用较弱, 自组装形成胶束的CMC较高, 辅以Gelucire 44/14时, 烷基嵌段增加了与PPO的疏水相互作用, 增加了其载药能力和稳定性能[13]。此外, 难溶性药物与疏水链段之间也会发生疏水相互作用, 以增加载药性能, 如Wang等[14]发现胶束中Tween与姜黄素的疏水相互作用随着饱和烷基链长度增加而增加, 随着链不饱和度的增加而降低; Guo等[15]研究发现四环素(tetracycline, TC) 与聚乳酸(polylactic acid, PLA) 链段发生疏水相互作用, 更容易被包载在聚乳酸-聚乙二醇-聚乙烯亚胺(PLA-PEG-PEI) 胶束内核中, 包封率达90%。

1.2 立体络合作用

对于许多聚合物而言, 手性相反高分子链间的相互作用比手性相同高分子链间的相互作用更强, 这种由不同立体化学性质对映体之间的相互作用, 称为立体络合。由于环丙交酯单体有L-和D-对映体, 可以形成手性相反高分子链, 使得聚乳酸链之间具有更强的相互作用。Guo等[16]利用聚L-乳酸-b-聚赖氨酸(PLLA-b-PLys)/聚D-乳酸-b-甲氧基聚乙二醇(PDLA-b-mPEG) 制备混合胶束, 由于PLLA和PDLA链段的立体络合作用, 该胶束在不同pH值体系中都具有较低CMC值和较慢释药行为; Liu等[17]利用叶酸-聚乙二醇-聚D-丙交酯(FA-PEG-b-PDLA)/甲氧基聚乙二醇-聚L-丙交酯-2, 2, 6, 6-四甲基哌啶-1-氧基(mPEG-b-PLLATEMPO) 制备获得DOX混合胶束, 具有优良的载药能力、较好的细胞摄取, 差示扫描量热法分析发现1个190 ℃熔融峰, 明显高于PLA单晶的吸热峰, 证实PLA对映体络合在胶束核内已有效形成。

1.3 氢键相互作用

与立体络合类似, 氢键是一种短程相互作用, 当两个作用基团非常接近时才会形成。假设不同两亲性聚合物由于疏水作用而自组装形成胶束, 聚合物疏水基团与药物分子的部分基团相互作用形成氢键, 对疏水核起到稳定作用。Xu等[18]利用聚N-异丙基丙烯酰胺-丙烯酸四苯乙烯酯-聚甲基丙烯酸聚乙二醇酯[P(NIPAAm-co-TPE)-b-POEGMA] 聚合物包载水溶性胸腺肽, 制备得到的胶束具有较高包封率和载药量, 通过FTIR测试证实NIPAAm链段与胸腺肽之间形成强烈的氢键作用; Li等[19]利用聚乙二醇-聚ε己内酯-聚乙烯亚胺(PEG-PCL-PEI) 制备多西他赛(docetaxel, DTX) 胶束, 发现PEG和PEI链容易形成氢键, 以保证胶束内聚合物的稳定排列。

1.4 静电吸引作用

静电吸引有利于复合胶束的形成, 带相反电荷的链段通过静电相互作用聚集在一起, 形成聚离子复合胶束, 以利于包载叶酸、辅酶A等离子型药物/复合物。高分子链的电离度受到体系pH值的制约, 从而影响荷电情况和静电相互作用。Ge等[20]利用Pluronic F127-壳聚糖(F127-CS)/胆酸钠(NaC) 制备PTX胶束, 带负电的F127-CS与带正电NaC发生静电吸引, 使得F127-CS/NaC胶束内核紧凑, 获得较高的包封率, 当F127-CS/NaC比例发生变化使得聚合物之间的静电吸引减弱, 疏水相互作用则重新占主导地位。

2 聚合物混合胶束的制备与表征 2.1 制备方法

混合胶束的制备方法与单聚合物胶束相似, 根据分散介质的类型, 主要分为逆向蒸发法和薄膜分散法两类。将药物用有机溶剂分散再滴入聚合物溶液, 并旋转蒸发除去有机溶剂获得胶束, 即为逆向蒸发法, 适用于中等疏水性聚合物。该法较为简单, 易规模化生产, 但仅少数药物能获得较高载药能力。采用四氢呋喃和甲醇等亲水有机溶剂溶解聚合物和药物, 用旋蒸减压挥去有机溶剂后水化形成胶束, 即为薄膜分散法。该法将聚合物和药物在同相中溶解, 相互作用提高载药性能, 但是操作步骤相对较多, 且会发生有机溶剂残留, 不易放大规模化生产。另外, 采用逆向蒸发或薄膜分散处理后, 或使用二甲基甲酰胺等较高沸点溶剂制备胶束时, 利用蒸馏水进行多次透析, 除去有机溶剂或游离药物的方法, 被称为透析法。

选择不同的制备方法, 可获得不同的载药能力和溶蚀速度, 如Lin等[21]分别采用透析法、薄膜分散法和逆向蒸发法制备新藤黄酸胶束, 获得胶束粒径分别为222、269和179 nm, 电位分别为-22、-18和-37 mV, 包封率分别为62%、72%和85%。Tonglairoum等[22]分别采用O/W乳化法、逆向蒸发和透析法制备克霉唑胶束, 发现O/W乳化法制备的胶束粒径最大, 包封率和载药量也最高; Yang等[23]在8周考察期内发现, 透析法制备的PLA-PEG胶束较为紧凑, 粒径维持不变, 而直接溶解法制备的胶束粒径先增加后下降。因此, 聚合物胶束的制备时, 需要根据药物和载体材料的性能, 选择合适的制备方法是非常必要的。

2.2 性能表征

混合胶束性能表征也与单聚合物胶束相似, 主要涉及粒径及粒度分布、药物溶解度、释药行为和稳定性等理化性质。粒径大小与增强渗透滞留效应(enhanced permeability and retention effect, EPR)、溶解度与载药性能、释药行为与功能响应、稳定性与体内过程之间存在紧密联系。因此, 选择聚合物作为胶束的药物载体时, 这些理化性质必须考量和优化。

2.2.1 稳定性

聚合物胶束稳定性评价主要集中在物理稳定性方面, 包含稀释稳定性和储存稳定性两个方面。当胶束进入血液循环后被无限稀释, CMC较大的聚合物胶束有可能过早解体, 导致药物的快速释放或析出。如TPGS的CMC为0.13 mmol·L-1, 稀释后胶束容易解体, 药物沉淀[24]; 当胶束遇到冷冻干燥或长期储存等情况时, 可能对药物溶解度和胶束荷电情况产生影响, 导致粒径增大或药物析出。一般而言, 随着疏水基团比例的增加, 聚合物的CMC降低。当聚合物浓度低于CMC时, 胶束会发生缓慢解体, 其速率却受到核芯疏水链段的物理状态、疏水基团相互作用等因素影响。因此, 通过考察CMC、粒径分布和载药能力等参数, 对聚合物胶束的稳定性进行评价[25, 26]

2.2.2 载药性能

疏水相互作用是聚合物胶束形成及荷载药物的主要驱动力, 其大小取决于聚合物和药物的相容性, 直接影响药物溶解度和包封率。Fares等[27]采用Pluronic P123/F127 (4∶1) 包载拉西地平后, 药物溶解度由0.001 6 μg·mL-1提升至0.72 mg·mL-1; Younes等[28]采用HS15/Brij 58 (1∶1) 制备硝酸舍他康唑胶束, 发现药物的溶解度由0.023 mg·mL-1提升至7.79 mg·mL-1; Elsaid等[29]采用硫辛酸-壳聚糖-聚乙二醇(LACPEG)和DSPE-PEG以1∶1比例作为载体材料替代LACPEG时, 药物的包封率从45.6%增加至80.3%; 此外, 药物与聚合物的相容性可利用Flory-Huggins方程预测, 以此评估聚合物胶束的载药能力, 当χ药物-聚合物值小于5时, 则表示药物和聚合物具有良好的相容性[30], 因此, 可以通过选择与药物性质相适应的高分子聚合物以提高胶束的载药性能。

2.2.3 粒度分布

聚合物胶束的粒径一般为10~400 nm, 粒径大小主要取决于高分子链的性质和载药含量。理想的聚合物胶束, 不仅能避免由于粒子过小而被肾小球滤过提前消除, 还能避免由于粒子过大而被网状内皮系统吞噬破坏, 能从血管选择性外渗并基于EPR效应被动靶向进入肿瘤组织。因此, 调控粒子对于组织分布、细胞摄取有着非常重要的影响。然而多大粒子是聚合物胶束克服生理屏障所需要的, 且能体现较强抗癌活性和较低毒副作用, 却始终没有研究清楚[31]。Fang等[32]考察血浆吸附与聚乙二醇-聚十六烷基氰基丙烯酸酯(PEG-PHDCA) 微粒大小的关系, 发现243 nm粒子的吸附率为34%, 显著高于80 nm粒子的吸附率8%; Yu等[33]考察不同粒径胶束对肿瘤组织的渗透能力和抑制能力, 相比于118 nm粒子, 30、45和60 nm粒子的组织渗透能力较高, 抑制肿瘤生长能力较强, 但药时曲线下面积(AUC) 和平均滞留时间(MRT) 却较小; Cabral等[34]发现30 nm胶束比100 nm胶束能有效的肿瘤渗透, 获得较强的体内抗肿瘤效果; Mikhail等[35]发现15 nm粒子比60 nm粒子能更快在肿瘤聚集。粒径较大、粒度分布不均匀影响胶束的体内药动学性质, 通过筛选混合聚合物类型和比例, 可优化胶束粒径大小及分布指数, 提高药物抗肿瘤效果。

2.2.4 释药行为

聚合物胶束药物的释放曲线考察常采用透析袋方法, 以不同pH值磷酸盐缓冲液作为分散介质, 取适量胶束于37 ℃条件下进行透析, 计算各时间点的药物累积释放量, 绘制曲线分析释药行为。Li等[36]以泊洛沙姆403/407包载白藜芦醇制备胶束, 释药行为符合Higuchi方程, 具有较好的缓释行为; Huang等[37]以胆固醇聚氨酯-聚乙二醇-胆固醇聚氨酯[(PAE-g-Chol)-b-PEG-b-(PAE-g-Chol)] 为载体材料制备DOX纳米胶束, 发现pH 5.0和7.4体系中药物的释放分别为80%和37%, 主要依赖pH的药物扩散和聚合物溶蚀; Dong等[38]以聚乙二醇单甲醚-ss-硬脂酸(MPEG-ss-2SA)/TPGS为载体材料制备PTX胶束, 在10 mmol·L-1二硫苏糖醇体系中24 h内药物的释放由40%增加至70%, 主要依靠二硫键断裂后的药物扩散; 本课题组[39]利用透明质酸(HA) 和鱼精蛋白(PRM) 对藤黄酸进行层层包载, 发现胶束能响应透明质酸酶和酸性体系, 通过酶解和质子化效应促进药物释放。混合聚合物胶束的药物释放是以扩散和溶蚀为主, 常具有一定的缓释效果, 释药行为受到混合聚合物构成的影响; 另外, 当赋予混合聚合物胶束一定功能后, 响应肿瘤微环境(酸、酶、氧化还原等) 时释放行为获得改善。

3 混合聚合物胶束的性能优势

在制备胶束过程中, 聚合物和制备方法的选择, 直接影响聚合物胶束性能。现阶段, 有许多采用单聚合物进行胶束研究, 但部分存在稳定性差、载药量低、靶向效率不高和释药行为不佳等问题。根据药物的理化性质选择混合载体材料, 可以获得较好的性能优势, 见图 2

3.1 理化性质

合理选择两种或多种聚合物作为载体材料, 可以获得相关物理化学性能明显提升。通过优化后加入的第2种聚合物, 可以改变疏水/亲水链段比例, 调整聚合物的CMC改善药物溶解性能, 提高药物的包封率和载药量, 同时影响胶束的亲水性能和荷电情况, 改变粒径大小及分布。以Soluplus/TPGS (3∶2) 代替Soluplus作为载体材料时, 薯蓣皂苷的包封率从66.7%增加至92.6%, 载药率由3.3%增加至4.6%[24]; 利用Soluplus/TPGS (4∶1) 制备PTX聚合物胶束, 粒径由12.3 nm增加至120 nm, 药物溶解度由0.3 mg·mL-1增加至7.16 mg·mL-1[9]; 当选用LACPEG/DSPE-PEG (1∶1) 为载体材料时, 粒径由115 nm降低至49.8 nm, 荷电由5.6 mV降至2.7 mV[29], 见表 1[9, 24, 29, 40-49]

聚合物胶束由于内核的疏水性能, 通过“相似相溶”原理, 不仅能增加难溶性药物的溶解度, 还能延缓药物的释放, 从而获得长效作用。胶束释药行为的快慢与聚合物的亲水性能有关, 亲水性强则水分更容易进入核芯并形成水性孔道, 促使聚合物溶蚀及药物扩散。文献[50]研究发现, 部分聚合物胶束制剂存在突释行为, 这是由于核壳界面的药物快速释放导致, 表明药物并没能完全包载在核内, 也还有部分胶束内部相互作用较强导致药物释放过缓现象, 这些处方有待进一步优化, 或许可以根据释药位置、释放速率等需求, 选择合适的混合聚合物, 以获得期望的释药行为。在制备拉莫三嗪胶束时, 分别以mPEG-PLA和mPEG-PLA/TPGS (2∶5) 为载体材料, 胶束在4 h累积释放68%和47%, 说明TPGS的加入降低了药物突释行为, 具备更好的缓释效果; 以叶酸-白芨多糖-硬脂酸(FA-BSP-SA)/TPGS (3∶1) 替代FA-BSP-SA制备PTX胶束, 发现pH 5.0体系中12 h累积释放由74.4%增加至93.6%, 在pH 7.4体系中24 h累积释放仍然维持在64%, 说明FA-BSP-SA/TPGS混合胶束在偏酸性微环境有更好的释药速率[51]; 利用TPGS/F127/F68 (1∶2∶1) 制备得到丁香酸胶束, 相比于F127/F68 (2∶1) 混合胶束, 其在pH 1.2体系中0.5 h累积释放维持在50%, 但在pH 7.4体系中0.5 h累积释放从75%降低至45%, 说明TPGS加入增加了混合胶束在体循环的稳定性, 具有更好的肿瘤定位释放能力[52], 见表 2[38, 44, 45, 47, 48, 50-54]

3.2 药动学性质

聚合物胶束的粒径较小, 能够减少网状内皮细胞的吞噬作用, 同时, 表面亲水性能阻止胶束与血白蛋白的调理作用, 减少单核吞噬细胞的亲和作用, 可以延长在体循环时间, 然而, 部分单聚合物仍很难获得理想的体内药动学行为, 采用两种或多种聚合物作为载体材料, 可以对胶束粒径、荷电和释药性能进行调控, 从而改变其在体内的吸收、分布、代谢和排泄, 表现出不同的药物动力学性质, 药物半衰期(t1/2) 增加, 平均滞留时间(MRT) 延长, 血药浓度(Cmax) 和生物利用度(AUC0-t) 升高。如利用FA-F87-PLA/TPGS制备获得PTX胶束, 相比于FA-F87-PLA胶束, AUC0-t由306.0 μg·h·mL-1增加至432.5 μg·h·mL-1, 这是由于TPGS的加入延长体内循环时间和抑制P-gp药物外排作用[45]; 在以HA-ss-PLA制备PTX胶束时加入适量TPGS, t1/2由3.59 h增加至5.87 h, Cmax由3.49 μg·L-1提升至4.64 μg·L-1, MRT由4.57 h延长至6.97 h, AUC0-t从3.96 μg·h·mL-1增加至8.12 μg·h·mL-1, 说明混合胶束能增加PTX体内滞留时间, 改善药物生物利用度[55]; 利用TPGS/F127/F68 (1∶2∶1) 制备丁香酸胶束, 相比于F127/F68 (2∶1) 混合胶束, t1/2增加至62.6 min, MRT延长至112.6 min, AUC0-t提高至1 501.8 μg·min·mL-1, 说明TPGS的加入可以通过抑制P-gp外排增加肠道吸收, 减小粒子粒径, 避免网状内皮系统的快速清除, 增加粒子表面亲水性, 降低与血浆蛋白结合, 延长体内循环时间, 从而改善药物生物利用度[52], 见表 3[45, 47, 50, 52, 55-59]

4 抗肿瘤效果评价

混合聚合物作为载体材料, 不仅可改善胶束的理化性质和生物利用度, 还可通过对核-壳表面的聚合物亲水链段调控, 赋予多功能响应性质, 在保证生物相容性的前提下, 提高聚合物胶束的肿瘤靶向能力、细胞摄取能力, 乃至抑制肿瘤组织生长, 为临床研究提供服务。

4.1 体外细胞毒性

在抗肿瘤活性评价中, 常采用MTT或CCK-8实验测定IC50或半数生长抑制浓度(growth inhibition of 50%, GI50) 值, 以表征药物及制剂的细胞毒性, 数值越低则表示毒性越强。一般而言, 聚合物胶束通过对难溶性药物的增溶和纳米化, 可显著提高抗肿瘤活性。在此基础上, 采用混合聚合物处方将纳米胶束功能化, 不仅可以肿瘤微环境(受体、pH、酶、氧化还原等) 响应, 增加细胞摄取、抑制P-gp外排, 还能产生活性氧促进线粒体凋亡, 从而获得更好的生物活性。利用FA-F87-PLA制备PTX胶束时加入TPGS, 可显著增加OVCAR-3的细胞毒性, IC50值(24 h) 由4.69 mg·L-1降低至2.63 mg·L-1, 这是由于TPGS能增加细胞摄取、抑制P-gp药物外排[45]的结果; 利用FA-PF127/F68制备获得的白杨素混合胶束, 通过叶酸受体介导的细胞内吞作用增加药物胞内浓度, MCF-7细胞24 h的GI50值由31.9 μmol·L-1降低至16.5 μmol·L-1[60], 见表 4[9, 29, 45, 46, 54, 60-65]

4.2 细胞摄取能力

细胞毒性高低与药物摄取密切相关。一般而言, 纳米粒和核壳结构利于胶束的细胞内化。对聚合物胶束的载体材料进行优化, 可利用穿膜肽介导摄取或利用特异性高表达CD44、转铁蛋白和叶酸等受体介导细胞内吞, 也可利用P-gp外排抑制剂TPGS、P105等, 抑制药物细胞外排。常采用流式细胞仪、激光共聚焦测定荧光强弱以表征细胞摄取量。利用FA/P127制备白藜芦醇混合胶束, 孵育MCF-7细胞2 h, 荧光强度约是P127组的3倍, 这是由于FA介导细胞内吞增加了药物摄取[61]; 分别利用PPG-g-HA、PPG-g-HA/L61包载DOX, 发现含3% L61的PPG-g-HA/L61混合胶束的细胞摄取率为PPG-g-HA胶束的198.8%, 但随着L61比例增加, 细胞摄取率降低, 这是由于胶束表面含有较多的PEO基团, 空间位阻减少了HA-CD44相互作用[46], 见表 5[10, 55, 65-76]

4.3 动物体内药效

混合聚合物通过改变胶束的理化性质, 一是可以减少巨噬细胞吞噬, 受体介导细胞摄取, 提高细胞毒性; 二是可以延长体内循环时间, 特定组织靶向, 提高生物利用度, 从而改善体内抗肿瘤活性。现阶段, 动物模型是评价药物抗肿瘤能力大小的可靠方法, 常以肿瘤抑制率或肿瘤体积为评价指标, 肿瘤体积越小、抑制率越大, 则药效越好。利用FA-TPGS2K/HDP为载体材料制备PTX混合胶束, MCF-7/PTX荷瘤裸鼠给药16天, PTX-HDP、PTX-FA-TPGS2K/HDP组的肿瘤抑制率分别为29.2%、64.1%, 说明FA-TPGS2K/HDP可显著提高抗肿瘤活性, 这是由于FA受体介导细胞内吞、TPGS2K中PEG2K基团延长体循环、维生素E琥珀酸酯(vitamin E succinate, VES) 基团抑制P-gp外排作用的协同结果[49]; 利用HA-ss-PLA/TPGS制备PTX胶束, H22荷瘤小鼠给药11天后, PTX-HA-ss-PLA和PTX-HA-ss-PLA/TPGS组的肿瘤抑制率分别为49.76% 和69.08%, 取得较好的逆转肿瘤耐药性, 这是由于HA受体介导细胞内吞和TPGS的P-gp外排作用[55], 见表 6[51, 66, 77-81]

4.4 生物相容性及临床研究

与单体聚合物胶束一样, 混合胶束在体内最终会解体成单个高分子链, 易蓄积在组织或器官产生毒性, 这就要求聚合物有较好的生物相容性和可降解性。Katekar等[82]利用溶血实验考察白藜芦醇-F127/TPGS混合胶束的毒性, 发现红细胞耐受性良好; Liang等[83]利用Tween 80/大豆磷脂制备PTX混合胶束并进行溶血实验, 发现当PTX质量浓度为1.2 mg·mL-1时, 溶血比率从78%降低至19%; Hou等[7]观察连续口服10周淫羊藿次苷Ⅱ (20 mg·kg-1) 对胃肠道的影响, 苏木素伊红染色(hematoxylin-eosin, HE) 分析发现HS15/F127混合胶束具有较好的胃肠道安全性; Ke等[84]发现Soluplus/P105混合胶束没有增加冬凌草甲素的胃肠道刺激性, 这些研究说明混合胶束具有较好的生物相容性, 然而生物降解研究主要集中在响应微环境的胶束解体[85, 86], 而在聚合物层面的生物降解研究却较少。对于不可降解聚合物用于制备非肠道用胶束, 则需要保证聚合物分子质量在20 kDa以下, 以保证在胶束解体后聚合物分子能通过肾脏排泄[87]。此外, 免疫原性的研究也受到更多关注, Podaralla等[88]制备了mPEG/zein胶束, 利用小鼠测试发现胶束是非免疫原性; Garg等[89]利用PEO-b-PCL制备胶束并测试, 发现没有免疫原性, 与骨髓来源的树突状细胞(DCS) 具有很好生物相容性。

混合胶束的性能表征、体内外生物活性实验已经获得广泛开展, 其测试结果明确了MMs在抗肿瘤临床应用的良好前景, 目前已有不少胶束应用于抗肿瘤临床研究, 白藜芦醇、紫杉醇和姜黄素等已经完成Ⅰ、Ⅱ期研究, Genexol-PM、NK105和NC-6004等已经完成Ⅲ期临床研究, 然而, 混合胶束的临床研究较少, Surpatek Pharma公司的DOX胶束(SP1049C), 是由L61和PF127作为载体材料, L61可选择性消耗细胞内的ATP, 从而抑制细胞膜上的P-gp功能, 临床前研究发现SP1049C显示出优于DOX的抗肿瘤活性, Ⅰ期临床研究通过观察毒性特征、剂量限制毒性、最大耐受剂量和药代动力学特征, 发现SP1049C的体内清除速率较DOX慢, 剂量限制性毒性是骨髓抑制, 最大耐受剂量为70 mg·m-2[90]。Ⅱ期临床研究观察SP1049C对晚期食管和胃食管连接部腺癌的抑制作用, 给药剂量为75 mg·m-2, 每3周静脉注射1次, 主要评价指标为客观有效率, 次要指标为无进展生存率(PFS)、总生存率和安全性等。21名患者接受了SP1049C治疗, 其中19名患者可进行疗效评价, 发现9名患者部分缓解, 8名患者轻微缓解或病情稳定, 可评价患者群体、意向治疗群体的客观有效率分别为47%和43%, 中位生存期和无进展生存期分别为10、6.6个月; 中性粒细胞减少是SP1049C的主要毒性。另外, 4名患者的左心室射血分数至少下降15%, 但没有下降到45%以下[91], Ⅲ期临床研究还在开展中。

5 总结与展望

恶性肿瘤是一种严重威胁人类健康和生命的常见病、多发病, 防治任务异常艰巨。相比于系统性治疗, 靶向治疗则往往更加有效。胶束是利用两亲性载体材料依靠疏水相互作用、立体络合等非共价结合力, 形成稳定核壳结构的纳米粒, 可以增加载药性能、延长体内循环时间、促进摄取及提高生物活性。然而单聚合物胶束存在一些不足, 正如上述的研究实例, 可通过调控两种或两种以上聚合物组合获得理想的CMC值, 从而改善其溶解度、载药量和释药行为等性质; 胶束还可以集多个功能于一体以响应肿瘤微环境, 如利用合适配体促进受体介导内吞, 利用质子效应促进溶酶体逃逸, 利用P-gp抑制剂减少胞内药物外排, 从而提高靶向能力及抗肿瘤疗效。

在混合胶束的成型过程中, 聚合物之间、聚合物与药物之间的相互作用力, 直接影响制剂的理化性质、稳定性和溶出行为, 从而影响胶束的解体速率、释放性能和降解速率, 最终体现出不同的药动学性质和治疗效应; 另外, 聚合物生物相容性和生物可降解性直接影响MMs临床应用前景。因此, 聚合物的筛选及用量优化是保证MMs有效性、安全性的核心内容[92], 是调控粒子大小、载药性能、释药行为和细胞摄取的关键措施, 也是影响药理活性、药动学性质和体内毒性等临床研究的重要因素, 是抗肿瘤药物混合胶束安全、有效的临床应用基础。本文尝试从分子间作用力、制备表征、性能优势、生物活性等多个方面进行综述分析, 以期为抗肿瘤药物混合胶束的开发与应用提供一些借鉴。

然而, 结合对混合胶束研究的整理分析, 发现存在以下不足: ①性能优良的混合胶束的开发需要有相互作用力的更多数据作为支撑, 相比于疏水作用, 立体络合、氢键、离子相互作用等方式的研究较少, 在MMs形成中的优势有待进一步探索。同时, 缺乏可靠的相互作用力表征方法; ②载体材料除TPGS、PLA和泊洛沙姆外, 多采用自行设计合成的聚合物, 与药用辅料要求还有较大差距, 这就需要开发更多聚合物药用辅料, 作为处方优化的重要支撑; ③ MMs对肿瘤生长抑制有一定优势, 但荷瘤生存优势研究相对缺乏, 可通过MMs和SMs总体存活率的对比分析, 展示混合聚合物处方优势; ④临床研究及聚合物生物降解的相关数据较少, 体内毒性和药动学性质有待进一步评价。

综上, MMs相比于SMs, 在抗肿瘤方面具备较多优势, 但仍存在许多问题待解决, 随着药学及各种相关学科的发展, 各种新技术、新材料不断出现, 将推动MMs不断向前发展, 或最终可以成为一种可靠的药物递送体系, 服务于临床。
