药物通透性在新药发现和开发阶段的评估策略

摘要: 通透性是影响口服药物生物利用度的一个关键性因素,因此,新药研发的早期阶段,准确及高效地评估药物通透性至关重要。工业界通常将平行人工膜渗透技术(parallel artificial membrane permeability assay,PAMPA)和Caco-2细胞模型作为早期评估方法,目前,尤斯灌流大鼠模型也被广泛的应用。本篇综述首先总结了人体内单次灌注技术(in vivo single-pass perfusion technique,Loc-I-Gut)的数据—金标准,然后着重介绍了三种体外方法的基本原理、实验操作、效率、与人体内有效通透性(effective permeability coefficient,Peff)数据和人体吸收分数(fraction absorbed,Fa)的相关性,以期为在新药发现和开发的不同阶段使用正确的通透性方法提供建议。

Assessment strategies for drug permeability during drug discovery and development

Abstract: Permeability is a key factor in the bioavailability of oral drugs. Therefore, in the early stage of drug discovery, accurate and efficient evaluation of drug permeability is essential. The parallel artificial membrane permeability assay (PAMPA) with Caco-2 cells model was used by the industry as early evaluation methods. At present, the Ussing chamber rat model is also widely used. This review summarizes the human data for the in vivo single-pass perfusion technique (Loc-I-Gut)-the gold standard, and then focuses on the basic principles, experimental operation, and efficiency of the three in vitro methods, with correlation to the effective permeability coefficient (Peff) and fractional absorbed (Fa) in man. We provide recommendations for the use of proper permeability methods at different stages in drug discovery and development.

口服给药在现代药物治疗方案中占据主导地位, 与其他给药方式相比, 它安全、有效且患者依从性好[1]。口服制剂的开发需要深入进行吸收机制的研究, 不但可以预测口服给药的生物利用度的大小, 还可以预测食物、pH等因素对生物利用度的影响, 以便为制剂的处方优化、未来上市后仿制药的生物豁免等提供依据。因此如果能在研发早期了解化合物的肠道吸收机制, 可以提高研发效率和节约成本[2]。口服药物的吸收机制研究, 通透性是决定性参数之一[3]。本文将药物的“permeability”译为通透性, 而不是渗透性。因为肠道吸收是一个复杂的动态过程, 包括被动跨细胞扩散、载体介导的主动转运和外排以及细胞旁路途径, 而渗透性从字面意思看仅意味着一种被动的过程[4]

工业界通常将平行人工膜渗透技术(parallel artificial membrane permeability assay, PAMPA) 和Caco-2细胞模型两种方法合用作为药物研发早期的通透性预测方法, 但是目前尤斯灌流(Ussing chamber) 大鼠模型以其吸收窗评估优势也被应用[1, 4]。这些方法的准确性主要依赖人体内单次灌注技术(in vivo single-pass perfusion technique, Loc-I-Gut) 30个药物的通透性结果进行判别[5]

本文首先介绍了金标准实验Loc-I-Gut的基本内容, 总结已有的人类通透性数据, 然后介绍了三种方法的基本原理、实验操作、效率、与人体内通透性数据(effective permeability coefficient, Peff) 和人体吸收分数(fraction absorbed, Fa) 的相关性。期望为在药物研发早期的不同阶段正确地使用各种体外通透性模型提供建议。

1 人体Loc-I-Gut实验 1.1 方法介绍

空肠是多数哺乳动物对大部分药物的主要吸收区, 它具有最大的表面积, 是肠道中最活跃的载体介导转运位点[6]。最常用和最有效的临床上测定空肠近端Peff的技术是Loc-I-Gut[7]。任何用于预测人体肠道吸收的体外模型都必须从对照相应的人体体内数据进行验证开始, 这些预测的基础是与Loc-I-Gut的历史Peff测量值具有相关性[6]。目前, 已有30种药物的人体肠道通透性通过Loc-I-Gut被测定, 见表 1[7-9]

1.2 实验操作

Loc-I-Gut研究是在健康的受试者进行的。如图 1所示, 受试者的上喉被麻醉后, 将外径为5.3 mm的聚氯乙烯管经口插入小肠。该管有6个通道, 末端连有两个相距10 cm, 各40 mm长的乳胶囊, 它们分别与其中一个较小的通道相连, 在管的中心的两个较粗的通道分别为灌流的流入管和流出管, 其余的周围小通道用于给予标记物质和/或引流[5]。根据充分搅拌模型来计算Peff:

式(1) 中CinCout是灌注液流入和流出时药物的浓度; Qin为单次灌注的速率; r为灌注肠段半径(约1.75 cm); L为灌注肠段长度。

1.3 效率

Loc-I-Gut实验操作繁琐, 价格昂贵, 且涉及伦理学的问题, 因此无法大规模地用于药物的筛选。

1.4 准确性

将这些空肠Peff值转化为肠道吸收半衰期(half-life of absorption, t1/2, abs) 值, 再与传统药代动力学(pharmacokinetic, PK) 数据分析, 即血药浓度曲线计算出的相应t1/2, abs值进行比较, 进一步分析这些空肠Peff值的有效性。从选定的药物子集的空肠Peff值(基于公式2计算) 获得的t1/2, abs值与临床PK研究(口服溶液或速释制剂) 获得的“真实”体内吸收数据非常吻合(图 2)[7]。并且这些药物的人体空肠Peff值具有位置和时间依赖性, 但仍然能够预测整体Fa[7]。这都说明了Peff能反映药物在体内的“真实”吸收[6, 7]。因此, 目前人体Loc-I-Gut实验是通透性实验的“金标准”。

式(2) 中V是经灌注肠段的液体体积, 约为60 mL[7]

2 PAMPA 2.1 基本原理

Kansy等[10]于1998年首次提出PAMPA技术。PAMPA是以人造的非细胞脂质膜(可在滤膜上涂上不同的有机溶剂, 如十六烷[11], 或用不同的磷脂和胆固醇溶解于十二烷或1, 7-辛二烯中[12-16]) 为基础, 它没有孔隙、主动转运载体和代谢酶, 被用于预测候选药物的被动跨细胞渗透性[17]。PAMPA可耐受大的pH范围及高的二甲基亚砜含量(最高5%), 因此可以评估不同pH条件(酸性和碱性) 的化合物的通透性[18, 19]

2.2 实验操作

PAMPA方法是用滤膜将96孔板和96孔滤板分成两个隔间(供体室和受体室)。供体室中加入待测化合物的缓冲溶液(pH 5.0~pH 7.4), 受体室中加入空白缓冲液(pH 7.4左右), 如图 3所示[20]。在37 ℃环境下, 通过一段时间内测定受体室透过的药物量进而计算得到通透性, 这种通透性常被称为有效通透性(effective permeability, Pe)[3], 根据公式(3) 计算:

式(3) 中, A为人工磷脂膜的面积(cm2); VD为供体腔的体积(mL); VA为受体腔的体积(mL); t为通透时间(s); CA(t) 为在t时间受体液的浓度; CD(t) 为在t时间供体液的浓度。

2.3 效率

PAMPA可以采用平行的方法进行, 不需要培养, 人工屏障是在进行实验时制备[20], 并且PAMPA中化合物的浓度可以使用96孔微板, 通过紫外分光光度法测定, 这大大提高了效率。PAMPA筛选一批药物大概需要4 h, 每周可测定约650个化合物, 是一种高通量的通透性筛选模型[2]

3 Caco-2细胞模型 3.1 基本原理

Caco-2细胞源自人结肠腺癌细胞系, 以标准条件在半透膜上培养, 分化完全的Caco-2细胞与正常人肠上皮细胞的形态结构非常相似, 具有紧密连接, 形成顶膜侧(apical, AP) 和基底外侧(basolateral, BL) 结构, 能在顶膜表面分化出刷状缘绒毛[21, 22]。并且与PAMPA不同的是, Caco-2细胞能表达肠道转运体和酶, 有可能预测主动转运和有多种转运体的药物相互作用[23]

3.2 实验操作

Caco-2细胞单层实验如图 4所示, 将细胞置于细胞培养附着的装置中(如Transwell小室), 细胞接种在多孔滤膜上。大约21天后, 细胞在滤膜上形成分化的单层。培养好的Caco-2细胞模型可以进行待测物的双向转运实验, 加入转运蛋白抑制剂可以确定化合物是否为转运蛋白的底物。根据公式(4) 计算表观通透系数(apparent permeability coefficient, Papp)。

式(4) 中, dQ/dt为单位时间内药物通过Caco-2细胞单层的转运量, 以药物的累积转运量(Q) 对取样时间(t) 作图后, 该直线的斜率为dQ/dt; A为小室的膜面积; C0为给药侧药物的初始浓度。

3.3 效率

Caco-2细胞模型需要21天的培养周期, 并且必须使用LC/MS或HPLC/UV用于药物定量[1]。与PAMPA相比, Caco-2细胞模型的通量较低, 成本较高。但是, Caco-2细胞模型可同时对大批量的药物进行快速筛选, 并且所需药量较少, 与繁琐的动物模型相比, 省时又经济。因此, Caco-2细胞模型通常被认为是中等通量的通透性筛选模型, 大概每周可以测定50个化合物[2]

4 尤斯灌流大鼠模型 4.1 基本原理

尤斯灌流技术是目前常用的研究胃肠道通透性的体外方法之一[24]。尤斯灌流大鼠模型使用切除大鼠的肠组织, 能够保持肠道结构的完整性, 模拟胃肠道的生理环境, 可结合肠道代谢来研究药物的转运[25], 同时研究不同肠段(十二指肠、空肠、回肠和结肠) 的吸收, 并且该方法也可以研究各种药物-药物相互作用对吸收的影响。

4.2 实验操作

该方法(图 5) 是将大鼠肠道截取后, 将离体肠段的浆膜层组织分离, 随后将肠片固定在互不相通的黏膜和浆膜腔室之间的孔上, 将测试化合物添加到组织的黏膜或浆膜侧, 可分别研究吸收和分泌方向的转运[26]Papp的计算方法与Caco-2细胞模型相同。

4.3 效率

尤斯灌流大鼠模型实验需先剥离大鼠肠段, 再将黏膜层的组织分离, 且黏膜易破损, 操作技术复杂, 并且由于前置时间较长加上肠道活力的限制, 该方法与PAMPA和Caco-2细胞模型相比通量低, 一套装置1天大概只能检测1个药物浓度[2]

5 PAMPA、Caco-2细胞模型和尤斯灌流大鼠模型的准确性评估

尽管PAMPA、Caco-2细胞模型和尤斯灌流大鼠模型均已被广泛使用, 但是受实验方法、质量控制值及环境等因素的影响, 对药物通透性的测定在实验室与实验室之间存在差异, 有的差异甚至高达100倍[27]。因此, 汇总不同实验室之间的体外通透性数据与人体内通透性数据进行绝对值比对是不明智的。

作者汇总了不同实验室之间的独立研究的药物在这三种体外模型的通透系数, 与人体有效通透性(Peff) 进行相关性研究: 纵坐标为以Lg (人Peff), Lg (体外PappPe) 为横坐标做线性回归所得的相关系数(r2), 见图 6。PAMPA数据[28-31]中仅包含以被动扩散为主要转运方式的药物, 相关性良好; Caco-2细胞模型数据[6, 32-40]中阿莫西林、头孢氨苄、左旋多巴、赖诺普利和依那普利拉是氨基酸或肽转运蛋白的底物, 这些蛋白在人和细胞中表达差异≥10倍, 剔除这些药物后, 相关性良好; 尤斯灌流是大鼠空肠的数据[41-45], 相关性很好。

作者从三种模型中各选择一个相关系数较高的实验室[29, 40, 44], 将其获得的药物通透性数据结合人体空肠通透性数据与Fa进行相关性研究[7], 如图 7所示, 四种通透性评估方法和Fa之间呈现类似的趋势且相关性良好, 同样的Fa, 不同方法对应的通透性由小到大的顺序依次是: Caco-2细胞模型、PAMPA、尤斯灌流大鼠模型、人体Loc-I-Gut方法。因为在单位面积下, 由于环状皱褶、绒毛和微绒毛的存在, 使小肠黏膜的表面积增加600倍, 因此人和大鼠肠道所吸收药量远高于Caco-2单细胞层[7]。尤斯灌流大鼠模型方法为离体的大鼠组织, 没有血液循环, 导致漏槽条件不满足, 所以同样是小肠组织, 其通透性结果低于人体结果。PMAPA的结果受不同实验室人工膜制备方法的影响。

6 结语

PAMPA、Caco-2细胞模型和尤斯灌流大鼠模型均已证明与人体内Peff和Fa相关性良好, 是目前用于筛选化合物通透性和吸收的有价值的研究工具。在药物发现和开发的初始阶段, 它们是可以在短时间内以最少的资源预测新化学实体(new chemical entity, NCE) 的通透特性和肠道吸收。从这些体外方法中获得的结果不应孤立地解释, 而应一起评估, 甚至需要结合化合物特征(溶解度和稳定性等) 的数据。

因为三种模型不同的特性(表 2), 能提供不同的通透性信息, 三种体外模型可以灵活应用在药物发现的不同阶段, 见图 8。首先, 应使用在两个pH条件(pH 5.5~6.5和pH 7.4) 下进行的PAMPA筛选所有测试化合物的被动跨细胞扩散, 这分别提供了化合物在酸和碱中的信息, 并提供了肠道吸收和组织分布的初步评估。如果新药在PAMPA上具有良好的通透性, 则可利用Caco-2细胞模型进行AP-BL方向上通透性的二次评估, 并将其数据进行比较, 以确定主要的通透性机制: PAMPA和Caco-2细胞模型线性相关表明被动扩散占优势; 如果PAMPA < < Caco-2细胞模型, 表明可能存在主动转运机制, 而如果PAMPA > > Caco-2细胞模型, 则表明可能存在外排机制。在Caco-2细胞模型分析过程中, 建议使用特定的转运蛋白抑制剂对BL-AP方向也进行通透性研究。尤斯灌流大鼠模型的建立有助于进一步证实前期的研究结果, 了解药物吸收过程中的代谢信息, 深入了解药物的通透性机制。

另外, 仿制药人体生物等效性的生物豁免需要申请人提供药物通透性信息, 这类注册申报用数据需要符合GLP要求。FDA生物豁免审评结果表明, 通透性实验不规范是审评发补或不批准的重要原因[46]。如何建设规范的通透性实验室我国尚处在摸索之中。
