双孔钾通道TREK-1中枢神经系统药理学研究进展

摘要: 双孔钾离子通道(two-pore domain potassium channels,K2P)是20世纪90年代发现的一类钾离子通道亚家族,其中TREK-1是研究最为广泛的一种K2P亚型。TREK-1广泛存在于机体内,特别是在中枢神经系统中高表达,它的主要作用是控制细胞兴奋性并维持膜电位低于去极化阈值,因此参与调节多种生理和病理过程。TREK-1也是多种疾病潜在的药物作用靶标,很多已上市药物可影响TREK-1的功能,但目前缺少特异性的TREK-1调节剂和以TREK-1为靶点的药物。本文在介绍TREK-1的结构、分布以及调控的基础上,重点介绍近年来TREK-1在神经保护、癫痫、抑郁以及麻醉等方面的药理学研究进展和相关的TREK-1调节剂的研究现状。

Progress in understanding the central nervous system pharmacology of the two-pore domain potassium channel TREK-1

Abstract: Two-pore domain potassium channels (K2P) make up a subfamily of potassium channels discovered in the 1990s, and TREK-1 is the most widely studied subtype of K2P. TREK-1 is widely expressed in the body and especially in the central nervous system, where its main role is to control cell excitability and maintain the membrane potential below the depolarization threshold. It thereby participates in regulating various physiological and pathological processes. TREK-1 is also a potential drug target in many diseases. It is known that many marketed drugs can affect the function of TREK-1, but currently there are no specific TREK-1 modulators or drugs. We review the structure, distribution and regulation of TREK-1 and focus on recent progress in understanding the pharmacology of TREK-1 and its role in neuroprotection, depression, anesthesia and epilepsy. The research status of TREK-1 modulators is discussed.

双孔钾通道是20世纪90年代发现的钾离子通道, 包括15个成员, 并根据它们的序列相似性将其分为6个亚家族[1-4], TWIK、THIK、TASK、TALK、TREK和TRESK 6大类[1]。TREK-1属于TWIK相关的双孔钾通道亚家族[1]。TREK-1是一种背景钾通道, 在可兴奋细胞和不可兴奋细胞中均有表达, 在维持细胞膜静息电位、动作电位时程、神经递质的释放中发挥作用[5]

1 TREK-1的结构、调控和分布

TREK-1具有4个跨膜片段(M1~M4)和2个孔道结构域(P1、P2), 胞内侧有1个较短的氨基端和1个较长的羧基端, 胞外侧M1和P1之间形成1个环形结构[6, 7], 2个通道亚基通过该环形结构的共价二硫键形成二聚体, 发挥其功能, 结构如图 1所示[5]。K2P通道具有独特的孔特征序列:在第1孔(P1)上的Gly-Tyr(Phe)-Gly和在第2孔(P2)上的Gly-Leu(Phe)-Gly[8]。编码TREK-1的基因KCNK2位于染色体1q41上[9]

TREK-1的功能受各种物理、化学因素和生物效应器的调节, 如温度、张力、酸度、麻醉剂和第二信使等[10-12], 这里不再详细论述。TREK-1的一个特征是对经典的钾通道阻滞剂如四乙基铵(TEA)、4-氨基吡啶(4-AP)、Ba2+等均不敏感[8, 13]。TREK-1分布广泛, 在大鼠和小鼠中枢神经系统中均有表达[14, 15], 据报道TREK-1首先是从小鼠大脑中克隆出来[16]。在人类中枢神经系统中, TREK-1在嗅球、海马体、大脑皮层以及尾状核和壳核中的γ-氨基丁酸(GABA)中间神经元中高表达[16], 在前额叶皮质、海马、下丘脑和中脑的5-羟色胺能神经元和背根神经节中的感觉神经元中亦有丰富的表达[17]。TREK-1在众多外周组织中如胃肠平滑肌、肺组织以及血管内皮细胞、心房和心室肌细胞都有表达[15, 18]。此外, TREK-1也表达于神经元树突以及海马区成熟的星形胶质细胞[19]

2 TREK-1与神经保护作用

TREK-1广泛分布于中枢神经系统, 其在神经保护特别是抗缺血缺氧损伤/保护的作用一直倍受关注。发生脑缺血时, 细胞会产生花生四烯酸(arachidonic acid, AA)和溶血磷脂A, 并形成酸中毒, 上述因素均可激活TREK-1[8]。据报道TREK-1激活剂异氟烷预处理能够提高缺糖缺氧(oxygen glucose deprivation, OGD)模型中SH-SY5Y细胞生存率, 还明显降低大脑中动脉栓塞(middle cerebral artery occlusion, MCAO)模型中大鼠脑梗死体积。这些发现进一步证实, 异氟烷预处理对缺血的神经保护作用与TREK-1密切相关[20]。不饱和脂肪酸亚麻酸(linolenic acid, LNA)和AA对TREK-1都具有激活开放作用, 能够起到一定的神经保护作用[21-23]。此外, 神经保护剂一氧化氮和利鲁唑均是TREK-1的潜在开放剂[24, 25]。对TREK-1基因敲除小鼠模型研究发现, 在双侧颈动脉夹闭模型中, TREK-1基因敲除鼠与野生型小鼠相比, 在缺血再灌注时神经元丢失量和动物死亡率明显增加[26]; 在缺血前30 min给予溶血磷脂酰胆碱(lysophosphatidyl choline, LPC)后, TREK-1基因敲除鼠与野生型小鼠相比神经元死亡数明显减少、动物存活率增高[26]。激活TREK-1参与脑缺血保护作用的机制可能为: TREK-1被一些病理因素激活, 降低神经元的兴奋性, 减少其代谢需求。突触前神经元超极化, 限制电压依赖性钙通道激活, 减少钙内流, 突触后神经元超极化, 增加Mg2+N-甲基天冬氨酸(NMDA)受体的阻断作用, 降低谷氨酸对神经元的兴奋性毒性[8, 27]。但是也有大量文献表明, 抑制TREK-1也能产生神经保护作用。本课题组早期的研究[27]结果显示, MCAO 2和24 h以后, 大鼠海马和皮层中的TREK-1的mRNA表达量和TREK-1蛋白表达量均明显增加, 说明TREK-1参与了急性脑缺血过程。细胞水平实验也证明了TREK-1有神经保护作用。原代培养的大鼠皮层神经元和海马神经元联合OGD 24 h后, TREK-1蛋白表达上调[27]。抗脑缺血药L-NBP (L-3-n-butylphthalide)及其衍生物lig4-4可以显著抑制TREK-1电流[28, 29]。此外, 神经保护剂西帕曲近、Spadin以及氟西汀均可以显著抑制TREK-1电流[30-32]。TREK-1抑制剂五氟利多和甲硫氨酸均可促进星形胶质细胞脑源性神经营养因子的合成[33]。TREK-1还可以对抗H2O2和硝普钠(SNP)引起的氧化损伤而产生保护作用[13]。抑制TREK-1参与脑缺血保护作用的机制可能为:脑缺血期间, 大量钾离子外流, 抑制TREK-1可减少钾离子的过度外流, 从而减少钾离子过度外流诱发的神经元凋亡产生保护作用[29]。在脑缺血发生的不同阶段, TREK-1产生保护作用的调节模式可能有所不同[29]。鉴于TREK-1参与抗缺血缺氧损伤/保护作用的具体机制还存在争议, 其在脑缺血的功能还需进一步研究。

除脑缺血外, 还有研究表明TREK-1与缺血后损伤、神经炎症及细胞增殖有关。与野生型小鼠相比, TREK-1基因敲除小鼠脑出血后血脑屏障损伤程度增加, 促炎因子白细胞介素-1β、肿瘤坏死因子α和细胞黏附分子分泌增加[34], 神经损伤加重; Nissl染色结果表明, 脑出血后第3天TREK-1基因敲除小鼠的神经元丢失量明显多于野生型鼠[34]; TUNEL染色结果表明, TREK-1基因敲除小鼠神经元凋亡数明显多于野生型小鼠。这些结果提示, TREK-1可能是治疗脑缺血后继发性损伤的一个潜在靶点[34]。在脊髓损伤模型组中, TREK-1基因敲除小鼠同野生型小鼠相比, 反应性星形胶质细胞增生加重, 神经元和少突胶质细胞凋亡增加, 运动功能恢复变慢[35]。TREK-1可能也是治疗脊髓损伤的一个有前途的药物靶点。在小鼠实验性自身免疫性脑脊髓炎模型中, 药物阻断或基因敲除TREK-1通道均可导致炎症浸润增加和神经损伤[36, 37]。其他研究表明, 激活TREK-1能够增强缺氧后的星形胶质细胞对谷氨酸清除能力, 降低炎症因子的分泌[33, 38], 推断TREK-1可能通过调节钾离子外流和膜电位参与神经炎症。本课题组前期的研究[39]结果显示, TREK-1在中国仓鼠卵巢(Chinese hamster ovary, CHO)细胞中的过表达能够抑制蛋白激酶A (protein kinase A, PKA)的活性, 增加p38的活性, 从而抑制细胞周期蛋白D1的表达, 最终导致细胞周期阻滞在DNA合成前期/DNA合成期(G1/S)。尽管TREK-1和PKA的调控机制需要进一步研究, 但该实验证明了TREK-1过表达能够使得DNA合成前期/DNA合成期(G1/S)的细胞增多, 提示TREK-1可能参与细胞的增殖。

3 TREK-1与癫痫

TREK-1和癫痫也具有一定的相关性。TREK-1在皮层和海马突触前和突触后大量表达, 对癫痫发作特别敏感[40, 41]。TREK-1激活剂LNA和LPC亦能够降低由谷氨酸受体激动剂海人藻酸诱发的野生型小鼠癫痫发作率, 而在TREK-1基因敲除小鼠中没有观察到LNA和LPC对癫痫模型小鼠的治疗作用, 表明TREK-1基因敲除小鼠对海人藻酸引起的癫痫状态更为敏感[26]。此外, 体内实验结果表明LNA还可保护大鼠免于海人藻酸诱发的癫痫发作和海马损伤[20]。TREK-1激活剂多不饱和脂肪酸如LNA还能够降低红藻氨酸诱发的大鼠癫痫发作率[21]。在另一项研究中, TREK-1通道突变可使培养的海马神经元遭受癫痫持续状态的风险降低50%[42]。这些研究证明, TREK-1通道在癫痫发作过程中具有关键的神经保护作用, 并表明它们有望成为治疗这种常见神经系统疾病的药物靶标。实际上, 咖啡因和茶碱的致癫痫作用已被认为是由于TREK-1通道的阻滞所致[43]

4 TREK-1与抑郁症

TREK-1在抑郁症中的研究也比较广泛。研究表明, TREK-1基因敲除小鼠在强迫游泳和悬尾等5种不同的抑郁模型中都表现为抗抑郁表型[44-46]。此外, 临床常用抗抑郁药氟西汀(IC50 = 19 μmol·L-1)和氟西汀代谢产物去甲氟西汀(IC50 = 9 μmol·L-1)也可拮抗TREK-1通道[30]。选择性五羟色胺再摄取抑制剂(selective serotonin reuptake inhibitors, SSRI)如西酞普兰和依西酞普兰对TREK-1通道也具有不同程度的阻断作用。推测TREK-1抑制剂可能成为临床抗抑郁药物的靶点。除了上述非选择性的TREK-1阻滞剂以外, Spadin作为TREK-1通道的特异性阻滞剂, 能刺激海马神经发生, 增强背缝核神经元5-HT的释放, 已被作为治疗抑郁症的潜在药物开展临床试验[31, 47, 48]。体内实验结果显示, 用Spadin处理的小鼠表现出快速诱导的抗抑郁(但不能抗焦虑)表型, 结果与在TREK-1敲除小鼠或用氟西汀治疗的野生小鼠中获得的结果相似, 可能是由于背缝核的5-羟色胺能神经元活性增加导致[30]。另外, TREK-1与脑卒中后抑郁(post-stroke depression, PSD)也有关, 已经有实验室研究了TREK-1通道在PSD病理过程中的可能作用, PSD是脑卒中后发生的一种高发病率的神经精神疾病。在大鼠PSD模型中, TREK-1在海马和前额叶皮质中的表达上调, 经TREK-1抑制剂艾司西酞普兰的慢性治疗后, TREK-1的表达上调被抑制[49]。综合上述结果, TREK-1阻滞剂是治疗抑郁的潜在靶点, 其具体作用机制还有待深入研究。

5 TREK-1与麻醉

在全身麻醉中, 临床剂量的异氟烷、乙醚、氟烷和氯仿可以激活TREK-1通道[47, 50-52], 并且TREK-1结构域的C末端结构是挥发性麻醉剂发挥激活作用的关键部分[53]。体内实验显示, TREK-1基因敲除的小鼠对TREK-1激活剂异氟烷、氟烷和氯仿的敏感性降低, 引起麻醉作用的阈值和剂量需要更高[26]。而全身麻醉剂苯巴比妥无法调节TREK-1通道, 在TREK-1-/-小鼠中无法重现这些效果[26]。本课题组的前期研究[39]结果表明, 静脉麻醉剂氯胺酮和依托咪酯都能够激活TREK-1, 氯仿也能够激活TREK-1。在临床上, 氯胺酮不仅作为麻醉剂使用, 而且还有抗抑郁作用, Berman等[54]发现, 抑郁症患者注射0.5 mg·kg-1氯胺酮4 h后, 患者的汉密尔顿抑郁量表(Hamilton depression rating scale, HAMD)评分明显下降, 而且氯胺酮抗抑郁作用可以持续3天。氯胺酮于2019年被FDA批准为抗抑郁药物。依托咪酯是临床上常用的麻醉剂, 依托咪酯发挥麻醉作用的靶点认为与GABA有关。据报道, 依托咪酯在大鼠急性分离的骶髓后连合核(sacral dorsal commissural nucleus, SDCN)神经元中可通过作用于GABAA受体而诱发氯通道电流[55], 且激活GABAA的EC50为33.35 ± 3.07 μmol·L-1, 但无法排除依托咪酯的麻醉作用与TREK-1的相关性。

6 TREK-1在其他疾病方面的作用

TREK-1还可能是非甾体抗炎药(non-steroidal anti-inflammatory drugs, NSAIDs)镇痛作用的靶点之一。常用的NSAIDs氟芬那酸和甲芬那酸都能够显著激活开放TREK-1通道, 而不受环氧合酶-环氧化酶抑制[56, 57]。而抗精神病药物(治疗精神分裂症和双相情感障碍)多有阻滞TREK-1通道的作用[58], 典型的和非典型的抗精神病药都对TREK-1有阻滞作用, 如氯丙嗪(IC50 = 2.7 μmol·L-1)和洛沙平(IC50 = 19.7 μmol·L-1)均能够剂量依赖性阻断TREK-1, 推测精神分裂症和双相情感障碍可能存在一定的联系, 且都与TREK-1有一定的关联。目前, 关于这方面的研究还较少, 具体相关性还需要大量的实验去验证。TREK-1在癌症中的表达也有变化。据报道, TREK-1在宫颈癌和前列腺癌中的表达上调[59]

7 TREK-1调节剂

由于上调TREK-1和下调TREK-1均能产生病理生理效应, TREK-1的激活剂和抑制剂也逐渐成为药物作用靶点用来寻找新的神经保护剂, 部分现有的TREK-1调节剂主要有:

3-正丁基苯酞(3-n-butylphthalide, NBP)是2002年底国家食品药品监督管理局批准的一种有效的神经保护剂, 是治疗缺血性卒中的药物[28, 60], NBP的光学异构体L-NBP对TREK-1电流有浓度依赖性抑制作用(IC50 = 0.6 ± 0.03 μmol·L-1), 10 μmol·L-1 L-NBP使TREK-1/CHO细胞的静息膜电位从-55.3 mV升高到-42.9 mV, 但对空载CHO的细胞膜电位无影响。L-NBP的作用与TREK-1激活剂(如不饱和脂肪酸、利鲁唑和挥发性麻醉剂等)的神经保护作用有所不同。L-NBP通过抑制TREK-1产生显著的脑保护作用, 类似于神经保护剂西帕曲近对TREK-1通道的作用[28]。虽然, 目前已证明NBP对TREK-1具有明显的抑制作用, 但它的神经保护作用还可能与其他靶点有关。最近, 对NBP类似物lig4-4的研究表明, lig4-4也可抑制TREK-1电流(IC50约为2 μmol·L-1), 还可抑制缺氧引起的神经元凋亡, 减少MCAO大鼠的脑梗死体积从而显示出明显的抗脑缺血保护作用[29]。此类小分子的缺点是虽然对TREK-1的特异性相对较好, 但其神经保护作用靶点较多。

Spadin及其衍生物是目前研究最为深入的TREK-1调节剂。不同于传统的小分子化合物, Spadin是一种多肽, 包含17个氨基酸, 是迄今为止最强的TREK-1阻断剂, IC50为40~70 nmol·L-1 [28, 47, 49]。为了提高生物利用度和体内稳定性, 根据Spadin结构开发了长度更短的Spadin类似物(spadin retro-inverso peptides), 在强迫游泳实验中, 连续静脉注射该类似物(10 μg·kg-1) 4天, 可显著减少模型小鼠静止时间, 表现出很好的抗抑郁活性和体内稳定性[45, 61]。Spadin和/或Spadin类似物目前正在进行临床试验中, 有可能为抑郁患者提供新的治疗方法[62]。此类多肽的优点是对TREK-1的抑制活性高并且特异性较好。

化合物SID1900也能够阻断HEK293/TREK-1以及原代培养的大鼠海马神经元细胞中的双孔钾离子通道, IC50为30~50 μmol·L-1, 并在慢性不可预测轻度应激(chronic unpredictable mild stress, CUMS)大鼠模型中显示出抗抑郁样特性[63], 显著提高CUMS大鼠中缝背核和前额皮质内5-HT能神经元的放电率。SID1900的生物利用度和血脑屏障渗透性良好, 是一个治疗抑郁症的潜在化合物, 而且很有可能成为研究TREK-1在其他神经病理学研究中有用的工具药。此化合物的缺点是抑制活性和选择性均较差。

BL-1249是一种四氮唑类化合物, 是人膀胱肌细胞TREK-1样电流的激活剂(EC50 = 1.5 μmol·L-1), 也被证明能够直接激活TREK-1通道[64]。BL-1249通过TREK-1的C末端发挥作用, 跨膜结构域M2和M3对选择性至关重要[65]。镇定剂如氯化锂和抗癫痫药物如加巴喷丁、丙戊酸钠和卡马西平等, 也可激活TREK-1通道。据报道, 有实验室筛选出的化合物ML67及其优化的类似物ML67-33对TREK-1有明显的激活作用[66]。紧接着同一实验室又开发出2种具有更高效价的分子ML335和ML402, 都能够明显激活TREK-1, EC50分别为5.2 ± 0.8和5.9 ± 1.6 μmol·L-1 [67]。此类化合物的缺点是激活活性和选择性均较差。

利鲁唑是一种神经保护分子, 临床上用于抗惊厥, 也在临床上用于延长肌萎缩性侧索硬化症患者的生存率, 其作用机制涉及阻断谷氨酸受体。已显示利鲁唑可作为瞬时TREK-1非选择性激活剂在30 s内可产生激活作用, 在90 s后可产生强抑制作用[24]。其对TREK-1的这种双重活性被认为是通过PKA激活环磷腺苷(cAMP)介导的。利鲁唑的缺点是选择性较差, 对多种离子通道均有活性。

此外, 二氢吡啶类似物氨氯地平和尼古地平都是L-Ca2+阻滞剂, 文献[65]报道它们也具有较强的阻断TREK-1通道的作用, IC50分别为0.43和0.75 μmol·L-1, 其在中枢和外周的钙拮抗剂以外的作用有待进一步研究。

8 展望

近年来越来越多的研究证明, TREK-1在神经系统疾病、麻醉以及抑郁症中发挥重要的作用。过去的十多年, 有大量的数据显示了神经保护剂对TREK-1通道的调节作用, 但是仍然有很多信息未知, 需要未来进一步的深入研究和充分挖掘TREK-1通道作为多种疾病药物作用靶点的潜能。特别是TREK-1不同的调节模式引起的不同病理生理功能。TREK-1基因敲除小鼠的广泛应用以及TREK-1转染表达株的构建都为研究TREK-1通道在病理生理条件下的作用提供了强有力的工具。建立TREK-1特异性调节剂筛选方法, 寻找作用于TREK-1的安全有效的药物是今后研究的主要目标。无论是TREK-1通道活性被激活还是被抑制都与很多疾病相关, 因而TREK-1会为治疗中枢神经系统疾病开辟新的视野。
