cGAS-STING在肿瘤转移中的研究进展

摘要: 环磷酸鸟苷-腺苷酸合成酶(cyclic GMP-AMP synthase,cGAS)-干扰素基因刺激因子(stimulator of interferon gene,STING)信号通路是细胞质内一条重要的免疫应答通路,通过识别胞浆DNA调节起始免疫和适应性免疫应答。研究表明,来自肿瘤自身的DNA和基因组不稳定性都能够激活cGAS-STING通路,促进或抑制肿瘤的发展及转移。本文主要从cGAS和STING的结构功能特点、通路转导调节、抑制剂/激动剂研究现状及其在肿瘤发生发展及转移中的作用等方面进行系统阐述,为临床开发靶向cGAS-STING的抗肿瘤新药提供理论依据,为肿瘤临床及肿瘤研究工作者提供参考。

Abstract: Cyclic GMP-AMP synthase (cGAS)-stimulator of interferon gene (STING) signaling pathway as an essential immune response pathway in cytoplasm, can find cytoplasmic DNA to regulate the innate immune and adaptive immune response. Studies have shown that the signaling pathway can be activated by both tumor self-DNA and genomic instability, thus to promote or inhibit the development and metastasis of tumors. Therefore, the role of cGAS-STING in tumor genesis, development and metastasis will be systematically expounded from the structures, physiological functions, inhibitors and agonists of cGAS and STING as well as its pathway transduction regulation in this paper. The paper aims to offer theoretical basis and reference for targeting cGAS-STING anti-tumor drugs in clinical practice and cancer clinical and cancer research workers.

环磷酸鸟苷-腺苷酸合成酶-干扰素基因刺激因子(cGAS-STING)通路在识别微生物DNA后诱导Ⅰ型干扰素(interferon, IFN)和炎症细胞因子的产生, 介导抗微生物先天免疫。最近的研究表明, cGAS-STING通路也参与调控癌症形成的过程。肿瘤来源的DNA, 如死亡肿瘤细胞的DNA、微核、胞质染色质片段、游离端粒DNA等, 可以激活cGAS-STING通路, 诱导细胞衰老、炎症和抗肿瘤免疫, 对肿瘤的发生具有不同的作用[1-7]

环鸟嘌呤腺嘌呤(cyclic GMP-AMP, cGAMP)由cGAS催化生成, 激活下游STING, 是cGAS-STING通路的中间介导物。它属于环二核苷酸(cyclic dinucleotide, CDNs)。研究表明, 小鼠结肠、乳腺、皮肤肿瘤内注射cGAMP后能够起到明显的抗肿瘤效应[8-10]。然而, 亦有部分研究发现肿瘤细胞可以通过高表达STING促进其生长和转移, 如在小鼠LLC肺腺癌模型中, 过表达STING能够促进肿瘤生长[11]; 舌鳞状细胞癌通过高表达STING招募Treg细胞介导免疫逃逸, 促进病程发展, 同时高表达STING患者预后往往较差[12]。因此, STING可能是一个双向调节因子介导肿瘤的发生发展。本综述重点介绍了最近关于cGAS-STING通路在抑制和促进肿瘤发生发展及转移的发现。

1 cGAS及STING的结构与生理功能 1.1 cGAS的结构特点

cGAS又称C6ORF150或MB21D1, 是一种分子质量在60 kDa的蛋白质, 由大约130~150个残基组成的非结构、不保守的氨基末端延伸, 随后是一个高度保守的属于核苷酸转移酶(NTase)超家族的Mab21结构域。N-末端1~212位残基是结合dsDNA必需的, 这个区域可能包含两个不同的DNA结合域。C-末端213~522位残基含有部分核苷酸转移酶(NTase)基序和Mab21结构域, 并在从斑马鱼到人类的不同生物中高度保守。这些区域对于形成结合cGAS底物GTP和ATP的催化口袋并进行必要的环化反应是必需的[13, 14]

1.2 cGAS生理学功能

cGAS是一种DNA感受器, 存在于胞浆中, 能够通过带正电荷的氨基酸残基与带负电荷的DNA磷酸主链相互作用。cGAS与异常出现在胞质的DNA直接结合后, cGAS二聚体化, 并经历构象变化, 打开催化核苷酸结合口袋, 允许GTP和ATP进入, 催化ATP和GTP, 催化GMP的2'OH和AMP的5'磷酸, AMP的3'OH和GMP的5'磷酸盐之间分别形成2'-5'和3'-5'磷酸二酯键, 最终产生2'3'-cGAMP。

2'3'-cGAMP是由陈志坚和他的同事[15]通过收集不同类型DNA转染细胞的胞浆提取物时发现的。通过基因沉默技术沉默小鼠纤维肉瘤细胞系L929的STING基因, 然后将不同种类的双链DNA转染该细胞系, 收集细胞提取物, 采用亲和层析法分离, 并通过液相色谱分析法鉴别出这种STING激活剂, 即环GMP-AMP (cGAMP)。cGAMP作为第二信使, 与STING结合, 触发干扰素调节因子3 (interferon regulatory factor 3, IRF-3)的激活, IRF-3导致Ⅰ型干扰素-β的转录。另外, RNA不能激活cGAS产生2'3'-cGAMP。

1.3 STING的结构特点

STING又称ERIS/MYPS/MITA, 是一个由TMEM173基因编码的多功能接头蛋白[16, 17], 由378个氨基酸构成, 其N端区域(1~154位残基)包含4个跨膜结构域, 将STING锚定在内质网上; C端包括二聚化结构域、环二核苷酸相互作用域, 以及与TANK结合激酶1 (TANK-binding kinase 1, TBK1)相互作用和激活的结构域。结构生物学研究提出了STING的激活模型, 在静息状态下, STING呈现蝴蝶状二聚体的形式, 其处于自抑制失活状态。一个环二核苷酸分子可以通过广泛的疏水相互作用以及氢键结合至STING二聚体的中心缝隙处[18], 在与2'3'-cGAMP结合后, STING经历了一个重要的构象变化(向内旋转约20 ), 并包围cGAMP[19]

1.4 STING的生理学功能

STING与2'3'-cGAMP或其他CDNs (来自细菌的c-di-GMP、c-di-AMP和3'3'-cGAMP)结合后, STING发生二聚反应, 构象改变, 通过高尔基体从内质网转运至核周微粒体。STING可激活TBK1并磷酸化下游转录因子IRF-3 (诱导Ⅰ型IFN反应)、信号传导和转录激活因子6 (signal transduction and activator of transcription 6, STAT-6), 诱导CCL2和CCL20等趋化因子。STING还可通过IκB激酶(IKK)的活性激活核因子κB (nuclear factor kappa-B, NF-κB)转录因子。新的研究表明, 除了通过结合cGAMP而被激活, STING还可以被其他细胞浆DNA感受器蛋白所激活, 这些蛋白包括DDX41、DAI和IFI16等[20-22]。STING也可刺激自噬体形成相关标志LC3斑点的形成[23, 24]。此外, cGAS和STING受到翻译后修饰, 并与其他蛋白相互作用的广泛调控[21], 如磷酸化、泛素化、棕榈酰化等, 这可能说明该通路紧密调控的重要性。

2 cGAS-STING信号通路转导

cGAS-STING信号通路是一种免疫应答通路, 参与胞质DNA介导的固有免疫反应。cGAS作为DNA感受器, 可识别细菌、病毒和肿瘤细胞产生的双链DNA, 进而产生2'3'-cGAMP。cGAMP结合并激活STING, 改变其构象, 招募TBK1, 然后磷酸化IRF3。磷酸化的IRF3进入细胞核, 诱导Ⅰ型IFN和其他与免疫调节相关的细胞因子产生。Ⅰ型IFN是连接先天免疫和适应性免疫发挥抗肿瘤免疫的重要桥梁, 因为肿瘤微环境中抗原提呈树突状细胞产生Ⅰ型IFN, 不仅可以激活固有免疫, 同时促进T细胞的交叉浸润渗透, 而且cGAMP并不受限于固有的细胞信号转导方式, 可以通过缝隙连接介导的信号转导产生更广泛的区域免疫应答。

3 cGAS-STING通路在肿瘤中的作用 3.1 促进肿瘤的发展

cGAS-STING通路的激活促进了肿瘤的发展。慢性刺激cGAS-STING通路可能导致炎症驱动的癌变。例如, 7, 12二甲基苯(a)蒽(7, 12-dimethylbenz[a]anthracene, DMBA)是一种致癌物质, 可导致核小体释放到细胞质中, 从而触发STING的激活, 促进小鼠皮肤肿瘤的发生, 而STING缺陷的小鼠对DMBA诱导的皮肤癌具有抵抗性[25]

最近的一项研究报道, cGAS过表达增强了肺癌模型的双链DNA损伤, 导致其基因组不稳定, 诱导恶性转化, 刺激体外增殖, 加速肺癌细胞在体内的生长[26]。Ye[27]通过对STING在肺腺癌发生发展中的作用及初步机制的研究, 同样发现STING异常高表达能够显著促进肺腺癌细胞的生长增殖能力。宿主cGAS-STING通路在Lewis肺癌(LLC)诱导肿瘤生长过程中也有类似的作用[28]

3.2 抑制肿瘤作用

肿瘤细胞中激活的cGAS-STING通路促进了抗肿瘤作用。Woo等[1]发现抗原提呈细胞中的cGAS-STING通路负责肿瘤应答, 产生Ⅰ型干扰素和引起T细胞激活, 从而引起肿瘤消退。Li等[29]同样在结肠腺癌模型中, 发现使用STING激活剂激活树突细胞(dendritic cell, DC), 促进提呈抗原对CD8+ T细胞发挥抗肿瘤作用, 而STING缺陷的小鼠显著抑制cGAMP的抗肿瘤效果。在抗原性肿瘤中, 辐照导致的DNA损伤会激活STING信号调控的起始免疫应答[30], 而STING敲除会极大影响辐照的肿瘤治疗效果; 在辐照治疗时加入cGAMP表现出更好的辐照效果[31]。5, 6-二甲基呫吨酮-4-乙酸[2-(5, 6-dimethyl-9-oxoxanthen-4-yl) acetic acid, DMXAA]是小鼠STING的直接配基。体内实验结果表明, DMXAA能显著减少肿瘤体积, 且能抵抗同种肿瘤细胞的再次入侵, 还可抑制腹侧未给药部位的肿瘤生长, 这些作用都依赖于STING的存在[32]

此外, 由偶氮甲烷/葡聚糖硫酸钠(azomethane/dextran sodium sulfate, AOM/DSS)[33]诱导的大肠杆菌相关肿瘤小鼠模型中, 也观察到STING的抗肿瘤作用。AOM触发DNA损伤, 并通过STING信号通路诱导炎症细胞因子基因的表达。STING敲除的小鼠经AOM/DSS处理后, 结肠表现出明显的炎症细胞的浸润及腺癌的发展。这些发现表明, 缺乏cGAS或STING的宿主骨髓源性树突状细胞中肿瘤DNA先天免疫传感缺陷会破坏肿瘤浸润性CD8+ T细胞的生成。因此, DC中宿主cGAS-STING通路的激活和Ⅰ型IFN诱导可促进肿瘤抗原交叉表达, 激活T细胞以控制肿瘤。

胞质DNA激活cGAS-STING通路在细胞衰老中发挥一定作用, 这是一种显著的肿瘤抑制机制。各种应激状态导致细胞进入衰老状态, 细胞周期处于不可逆的停滞状态[34, 35]。在大气O2水平培养小鼠胚胎成纤维细胞(mouse embryonic fibroblast, MEFs)可诱导细胞衰老。有趣的是, 在cGAS和STING缺乏的MEFs中[5], 细胞增殖加速, 衰老表型减弱[4, 5]。此外, cGAS-STING通路还可促进MEFs和原代人成纤维细胞对氧化应激、基因毒性应激、辐照和癌基因表达的衰老反应[4, 5]。衰老细胞中cGAS-STING通路的激活与核层蛋白lamin B1的丢失及cGAS识别异常的胞质染色质片段有关[5, 6]。因此, cGAS-STING通路介导Ⅰ型IFNs和衰老相关分泌表型(senescence-associated secretory phenotype, SASP)因子的产生, 促进衰老。值得注意的是, cGAS-STING通路还促进SASP因子的体内生产, 以及辐射和致癌Ras蛋白诱导的衰老。这些研究表明, cGAS-STING通路的激活可能通过诱导细胞衰老抑制癌症的发展。

STING激动剂的作用可能不局限于促进免疫检测和诱导细胞衰老抑制癌症的发展, 也可以促进癌细胞的直接凋亡。Tang等[36]在慢性淋巴细胞血管病的Eμ-TCL1小鼠中注射STING激动剂3′3′-cGAMP, 发现可诱导细胞凋亡。Liang等[12]发现STING的激活促进宫颈癌细胞株的凋亡。STING的激动剂通过激活STING则可有效释放T细胞并摧毁癌细胞, 显示出在肿瘤免疫治疗中的巨大潜力。

3.3 在肿瘤转移中的作用

虽然STING参与肿瘤的发生发展, 但是STING在肿瘤迁移和转移中的作用少有研究报道。早期研究发现, 单核细胞被李斯特菌单细胞基因感染后, 可通过STING诱导MCP-1和MCP-3的产生, 使单核细胞向肝脏迁移, 而MCP-1和MCP-3的产生与Ⅰ型IFNs无关[37]。这说明STING可能在诱导免疫细胞迁移中起重要作用。然而, STING对肿瘤细胞迁移和转移的影响似乎不同。

3.3.1 抑制肿瘤转移

肿瘤细胞中STING的激活, 至少在乳腺癌环境中可通过NF-κB信号诱导细胞死亡, 有效地限制了它们的迁移和转移。STING在乳腺癌细胞株MCF-7中表达下调, 而在表达上调细胞系导致细胞迁移减少[38], 说明STING可能抑制乳腺癌细胞[39]的迁移和转移。但是, 基本机制仍有待确定, 可能与NF-κB有关[39]。同样, STING沉默导致胃癌细胞[40]的迁移和侵袭增多, 并且STING敲除抑制了胃癌中胞质DNA的感受和cGAMP的活性。此外, 与野生型小鼠相比, 接种的小鼠黑色素瘤细胞在STING敲除的小鼠体内[41]更容易发生肺转移。这可能与交叉传递树突状细胞中STING激活而产生Ⅰ型IFN, 导致肿瘤特异性CD8+T细胞的自发诱导和肿瘤排斥有关。

3.3.2 促进肿瘤转移

STING可通过免疫性调节吲哚胺2, 3-双加氧酶(indoleamine 2, 3-dioxygenase, IDO)促进肿瘤生长, IDO可被STING激活[28]。IDO能催化L-色氨酸转化为N-甲酰犬尿氨酸, 促进肿瘤细胞免疫逃逸并限制T细胞增殖[42]。值得关注的是, IDO的表达在肿瘤引流淋巴结中升高, 并且STING和IDO同时缺陷的小鼠更能抵抗LLC的远处转移。cGAS-STING信号的激活也会刺激PD-L1在癌细胞中表达, 从而介导癌细胞的免疫逃逸[38]。Chen等[43]发现, 人类和小鼠的乳腺癌和肺癌细胞可以表达原钙黏蛋白7 (procadherin 7, PCDH7), 促进由连接蛋白43组成的癌-星形胶质细胞间隙连接的形成。一旦与星形胶质细胞间隙连接, 癌细胞传输第二信使cGAMP到星形胶质细胞中, 激活STING通路并产生炎性细胞因子干扰素-α (interferon-α, IFNα)和肿瘤坏死因子-α (tumor necrosis factor-α, TNF-α)。作为旁分泌信号, 这些因素激活信号传导与转录激活因子1 (signal transduction and activator of transcription 1, STAT1)和NF-κB通路, 从而促进脑转移。同时, 敲低星形胶质细胞中STING的表达, 可抑制IFNα和TNF-α的产生。Bakhoum等[44]发现染色体不稳定性通过维持肿瘤细胞对胞质DNA的自主反应来促进转移。染色体不稳定的癌细胞会产生大量DNA碎片通过微核分泌到细胞质中, 一旦细胞质内出现DNA碎片, 就会激活cGAS-STING通路, 但随后不会激活经典的NF-κB通路。相反, cGAS-STING通路会激活非经典的NF-κB通路, 促使p52基因的表达, 使癌细胞的基因表达特征与骨髓先天性免疫细胞具有某些相似性, 并且可逃避T细胞的杀伤, 抑制cGAS-STING通路可显著抑制癌细胞转移。

4 cGAS和STING激动剂及抑制剂

虽然cGAS-STING信号通路在固有免疫和适应性免疫至关重要, 但异常的自我DNA对该通路的不恰当激活已经在自身免疫性或自身炎症性疾病中被发现, 如Aicardi-Goutieres综合征、系统性红斑狼疮[45]。因此, 研究者发现一些cGAS的高亲和力抑制剂见表 1, STING的抑制剂见表 2

4.1 cGAS抑制剂

Wang等[46]基于HPLC的介质吞吐率筛选确定苏拉明(suramin)是一种有效的抑制剂, 发现苏拉明取代与cGAS结合的DNA, 从而抑制了cGAS酶的活性, 并且苏拉明处理的THP1细胞降低了IFN-β信使RNA和蛋白质的水平。喹那克林(quinacrine)和氯喹(chloroquine)属于抗疟药物, 可减弱cGAS活性, 并已被用于改善Ⅰ型干扰素病的治疗[47, 48]。An等[47]通过对药物库进行硅筛选, 计算分析证实了喹那克林和氯喹是IFN-β合成的有效抑制剂, 并且通过抑制cGAS的dsDNA刺激而发挥作用。

Hall等[49]发现一种新的高通量cGAS荧光偏振(fluorescence polarization, FP)检测方法, 可快速识别和优化cGAS抑制剂。FP检测使用cy5标记的cGAMP结合一种新的高亲和力单克隆抗体PF-06928215, 该抗体特异性识别cGAMP, 不与cAMP、cGMP、ATP或GTP交叉反应。鉴于其在先天免疫反应中的作用, cGAS是一种有希望的治疗自身炎症疾病的靶点。

4.2 STING抑制剂

两个硝基呋喃衍生物C-178和C-176化合物共价作用于预测的跨膜半胱氨酸残基Cys91, 从而阻断活化诱导的STING的棕榈酰化。C-178和C-176的物种特异性表明, 该化合物直接针对小鼠STING (mmSTING), 而不针对人STING (hsSTING)。H-151对hsSTING有明显的抑制作用, 可以通过抑制Ⅰ型IFN反应、降低TBK1磷酸化和抑制hsSTING棕榈酰化来证明[50]。小分子环肽astin C可特异性抑制cGAS-STING信号通路及胞质DNA所诱发的炎症应答。在HSV-1感染Trex1缺失的小鼠中, astin C可显著降低其自身免疫炎症反应, 阻断IRF3被招募到STING信号小体[51]

4.3 STING激动剂

cGAS-STING通路的激活能够增强抗肿瘤免疫, STING的激动剂见表 3。CDNs是一种非常有效的STING激活剂, 是原核生物[52]和真核生物[53]免疫系统中普遍存在的第二信使。CDNs的来源有两种:一种是cGAS途径在检测到胞质DNA[15]后产生非典型的二核苷酸2', 3'-cGAMP; 另一种是由于病原体[18]的存在而直接在胞质中发现。研究发现, CDNs对体内黑色素瘤[54-56]、结肠癌[54]和口腔癌[57]的治疗中, 显示出了强大的抗肿瘤作用和应用前景。

然而, CDNs作为候选药物面临着明显的实际问题和局限性。CDNs的分子量大, 净电荷和极性分布强烈限制了其膜通道和细胞吸收, 且磷酸二酯键易于酶解。磷酸二酯基团的修饰一直是候选药物ADU-S100的核心策略, 该药物目前正处于Ⅰ期临床试验[58]阶段, 用于晚期/转移性实体肿瘤或淋巴瘤患者。

DMXAA是一种能有效激活小鼠STING蛋白的非核苷激动剂。在小鼠体内, DMXAA不仅诱导Ⅰ型IFN的产生, 而且具有极强的抗癌作用。DMXAA激活STING可以刺激白血病模型中CD8+ T细胞的反应, 并通过适应性免疫提高体内生存。接种B16黑色素瘤的野生型小鼠, 经瘤内剂量为500 μg的DMXAA治疗, 大多数小鼠出现肿瘤消退和肿瘤排斥, 而STING敲除的小鼠没有治疗效果。DMXAA可引起小鼠非小细胞肺癌肿瘤部位特异性血管破坏, 与内源性非典型环二核苷酸STING激动剂2'3'-cGAMP类似, 可诱导M2巨噬细胞复极化[59]。但是DMXAA对STING的激活存在种属差异, 不能诱导人STING的激活。目前研究表明, 利用稳定表达cGAS-STING通路的人源性THP-1-Dual细胞和稳定敲除STING的THP-1 KO-STING细胞, 建立荧光素酶检测的STING激动剂筛选模型, 可排除种属差异[60]

5 总结与展望

cGAS-STING介导的胞质DNA识别模式、激动剂和抑制剂如图 1所示。综上所述, cGAS-STING在肿瘤的发生发展及转移中起着双重作用。cGAS-STING通路在抑制肿瘤的作用可能是通过增强抗肿瘤免疫、诱导细胞衰老和促进细胞凋亡这三种途径。在免疫细胞和肿瘤细胞中, cGAS-STING的激活, 促进Ⅰ型干扰素的表达, 激活CD8+ T细胞, 促进抗肿瘤免疫。同时, STING激活可促进宫颈癌的凋亡。基于此, STING激动剂的开发与使用具有广大的前景。在小鼠结肠、脑、皮肤、胰腺、乳腺和B细胞恶性肿瘤小鼠模型中, 瘤内注射STING激动剂cGAMP和其他环二核苷酸, 可见肿瘤体积减小和生长减缓。机体能够通过调节cGAS-STING信号通路的激活, 增强天然抗肿瘤免疫, 因此, 了解其机制可能为抗肿瘤疫苗和治疗的研发提供新的理论依据。

然而, 还有很多问题需要进一步明确, cGAS-STING的过度激活导致STING高表达, 可招募免疫抑制细胞如Treg细胞, 帮助肿瘤细胞逃避免疫。STING也可以激活IDO, 从而促进肿瘤的增长转移, 这种对肿瘤发生、转移的双向调节机制尚不明确; 基于该信号研发的抑制剂和激动剂的作用机制和使用条件还需要进一步深入研究。另外, 由于该信号对肿瘤的双向调控可能与其表达于不同细胞有关, 在治疗过程中相关抑制剂/激动剂的靶向递送也是值得关注的问题。因此, 加强对通路的深入研究, 有助于对STING激活剂临床治疗的把控, 并且研发抑制或促进STING相关信号通路的药物将在未来抗肿瘤、抗病原体以及自身免疫疾病等领域具有广阔的前景, 使其在基础免疫学、肿瘤生物学及肿瘤临床治疗中发挥更大的作用。
