线粒体-内质网动态关联与胰岛β细胞功能稳态

摘要: 胰岛β细胞内的能量及物质代谢稳态是维持其正常功能及存活的关键,细胞器线粒体与内质网之间的动态关联在此过程中起到了重要的调控作用。线粒体自身的动态变化响应胞内能量和物质代谢,为胰岛β细胞稳态提供物质基础;内质网则可根据线粒体的变化,实时调节相关代谢酶及蛋白的合成,从而调控胰岛β细胞功能。本文将综述近年围绕线粒体与内质网间动态联系的相关研究,阐述二者通过结构或功能变化,相互影响、协同作用,进而维持胰岛β细胞正常状态以及生理功能的过程。线粒体与内质网之间的动态关联,以全新视角对胰岛β细胞损伤及保护的可能途径进行了阐释,其中也不乏一些潜在的药物治疗靶点,对于2型糖尿病的相关机制探讨及新药研发具有十分重要的意义。

Dynamic associations of mitochondria-endoplasmic reticulum in maintenance of pancreatic beta cell homeostasis

Abstract: The appropriate regulation of intracellular bioenergy and nutrient metabolism is a basic requirement for proper function and survival of pancreatic beta cells, where mitochondria-endoplasmic reticulum (ER)-associations play crucial roles. Mitochondria are changed dynamically according to intracellular energy and nutrients, which provides material foundation for energy homeostasis; while ER regulates metabolic enzymes and protein synthesis in different pathways. This review sheds light upon the development of mitochondria-ER associations and its role in the regulation of insulin secretion in pancreatic beta cell. The impact on beta cell viability is discussed. Interruption of calcium and redox oxidative species results in reduction of glucose-stimulated insulin secretion, while intracellular calcium levels could be partial altered by depleting calcium from the ER. Given the tight link between ER and mitochondria, the association are crucial to the homeostasis and are an indicator of overall beta cell status, with a potential as a novel drug target for treatment of type 2 diabetes mellitus.

线粒体功能异常及内质网应激与胰岛β细胞功能紊乱密切相关, 参与2型糖尿病 (type 2 diabetes mellitus, T2DM) 的发生与发展[1]。线粒体作为细胞代谢网络和信号传导网络的调控中心, 具有多种重要的生理功能。并且现有研究表明, 内质网与线粒体之间存在结构与功能方面的双重联系, 而这种关联对于维持细胞生理功能及存活具有重要意义。因此, 以线粒体相关功能调控为目的的药物研究已成为新药研发的热点之一, 包括通过直接调节线粒体功能的抗氧化剂和化学解偶联剂, 以及作用于线粒体和内质网二者间动态关联的新靶点, 均可为糖尿病以及其他代谢性疾病的防治提供新的机遇。

1 线粒体动态变化与胰岛β细胞 1.1 线粒体动态变化

线粒体融合、线粒体分裂以及线粒体自噬在线粒体形态、数量和功能调控过程中起到极其重要的作用[2, 3]。研究表明, 线粒体的动态变化与能量及营养物质供应之间具有直接联系, 线粒体结构的变化可能是适应生化或代谢需求的重要机制[4]。通过对自身结构的调整, 线粒体可精确调控胞内能量利用及消耗过程[5]

1.2 胰岛β细胞内线粒体参与的能量调控

胰岛β细胞可直接利用葡萄糖、游离脂肪酸及氨基酸等营养物质供能, 以调控胰岛素分泌[6, 7], 这种精确的调控作用主要通过上述营养物质的氧化而得以实现。其中, 葡萄糖氧化上调胞内ATP/ADP比率是刺激胰岛素分泌的最主要因素。另外, 多个研究显示, 线粒体内营养物质代谢的副产物[如琥珀酰辅酶A、活性氧簇 (reactive oxygen species, ROS) 以及三磷酸鸟苷 (guanosine triphosphoate, GTP)]也是胰岛素分泌过程中的调控因子[8-10]; 此外, 某些氨基酸也可通过提供代谢产物乙酰辅酶A促进线粒体ATP合成, 从而刺激胰岛素分泌[11, 12]

值得注意的是, 不同营养物质还可通过其他途径影响胰岛素分泌环节, 如脂肪酸可通过线粒体内β氧化过程或与细胞膜受体的直接作用, 协同调控葡萄糖刺激的胰岛素分泌[13]。由于胰岛β细胞具有基于最大处置能力 (而非实际需求) 利用营养物质的特性, 所以在营养物质过量的糖尿病状态下, 胰岛β细胞常处于超载状态。因此, 其对于过量营养物质的代偿处置方式更为重要。

1.3 线粒体应对胰岛β细胞内过量营养物质的代谢调节

研究表明, 过量营养物质对β细胞的毒性主要来自葡萄糖与游离脂肪酸[14]。若使胰岛β细胞长期暴露于过量的葡萄糖或脂质环境, 则会导致其功能紊乱。

多数研究认为, 该现象主要是因胰岛β细胞对ROS十分敏感, 但其抗氧化能力却较弱[15]。2型糖尿病状态下, 过量营养物质在胰岛β细胞线粒体中进行氧化和利用, 同时产生一定量的ROS, 但该过程与线粒体内ATP的合成和利用关联不甚密切[16], 即使胰岛β细胞对ATP需求很低时, 也会刺激胰岛素分泌。因此, 长期营养物质过量的病理环境 (如2型糖尿病) 使ROS持续产生和累积, 从而导致胰岛β细胞抗氧化能力降低, 将会引起线粒体损伤, 进而损伤胰岛β细胞功能[17-19]

鉴于胰岛素分泌过程中ATP生成、ROS以及线粒体产生的偶联因子扮演着如此重要的角色, 可推测对于胰岛β细胞内ATP合成而言, ATP生成的主要途径--线粒体参与的呼吸作用至关重要。然而, 事实却恰恰相反。胰岛β细胞线粒体内源性质子外流水平高于其他组织 (如肌肉等)[20]。尽管不甚相符, 但解偶联的呼吸作用对于营养物质过量引起的ROS毒性具有一定的限制作用, 而这也与胰岛β细胞内ROS生成的复杂机制相关。据此可得出结论, 与肌肉或棕色脂肪等其他组织内的线粒体相比, 胰岛β细胞内的线粒体具有某种特有属性, 令其对于能量稳态变化具有精确的感知能力[21]

1.4 线粒体结构与能量代谢变化对胰岛β细胞功能的影响

β细胞内的物质氧化及能量利用, 与胰岛素分泌过程密切相关。因此, 胰岛β细胞可作为研究线粒体动态变化以及细胞内能量代谢效率的理想模型。

线粒体是一种动态细胞器, 其形态结构的维持在调节能量代谢中发挥重要作用, 并可通过不断的融合、分裂过程调控其功能[22]。在胰岛β细胞株MIN-6及NIT-1细胞中, 给予高脂条件刺激24 h, 或同时给予葡萄糖刺激10 h后, 均可引起线粒体外膜破裂[23, 24]; 类似研究显示, 在原代小鼠胰岛细胞中, 给予高糖及高脂处理48 h后, 亦可观察到线粒体融合水平显著下调[25]。上述研究提示, β细胞中的线粒体受损是与胞内营养物质氧化水平直接相关的早期事件。同时Corkey等[6]的研究结果提示, 过量营养物质引起胞内营养物质氧化水平以及代谢循环水平升高, 致使胰岛β细胞内线粒体负担加重, 可能是胰岛β细胞损伤的重要机制。

研究显示, 过量脂肪酸对胰岛β细胞形态结构的损伤较高糖更为严重。目前, 针对脂肪酸与葡萄糖调控胰岛β细胞内线粒体外膜破裂的不同机制有以下两种假说: ① 以脂肪酸为底物的呼吸作用常伴随膜电位降低, 这与线粒体内质子外流作用增强相关; 而以葡萄糖为底物的氧化过程则伴随相对较高的膜电位以及较弱的质子外流作用[26]。由此推测, 脂肪酸的氧化过程及质子外流增强引发的附加效应更易引起线粒体外膜破裂。研究发现, 脂肪酸氧化生成的超氧化物与ROS水平升高紧密相关, ROS亦是线粒体外膜破裂以及解偶联中的主要触发因子[27]。因此, 线粒体外膜破裂以及解偶联可能是抑制其受到进一步氧化应激损伤的防御机制。② 脂肪酸可能通过直接干扰线粒体融合, 诱导线粒体外膜破裂。研究显示, 棕色脂肪组织内的脂肪酸可通过解偶联蛋白1 (uncoupling protein 1, UCP1) 激活解偶联呼吸作用[28], 该激活机制与其化学结构 (而不是脂肪酸代谢或内源性质子流动调控过程) 关系更为密切[29]。由此推测, 特定种类的脂肪酸除了作为线粒体物质氧化的能源外, 还可通过UCP1介导, 激活线粒体分裂信号并促进解偶联呼吸作用发生。此外, 过量脂肪酸可能通过干扰磷脂酶相关的生理过程, 对线粒体融合过程起到调控作用。研究发现, 线粒体融合蛋白 (mitofusin, Mfn) 调控线粒体融合的过程依赖线粒体内磷脂酶活性, 该研究将线粒体内磷脂酶介导产生的酸性脂质与线粒体融合过程直接联系起来[30]

回归胰岛β细胞本身, 其线粒体解偶联呼吸作用增强, 可满足其减少过量营养物质并维持生物能量代谢效率的需求, 从而保持胞内营养物质供需稳态[31]。解偶联呼吸作用增强, 促使细胞通过产热消耗多余营养物质, 可能是降低胰岛β细胞内生物能量效率的机制之一。因此, 针对解偶联呼吸环节研发更为合适的解偶联剂, 以促进β细胞线粒体呼吸, 同时减少ATP的生成, 有望改善胰岛β细胞功能紊乱。

2 线粒体与内质网动态关联共调控胰岛β细胞稳态 2.1 线粒体与内质网的功能联系

内质网作为细胞内生物合成以及蛋白加工的重要细胞器, 几乎参与物质代谢的所有环节, 并将物质代谢与细胞内信号传导联系起来。多种病理及生理因素均会影响内质网功能, 甚至引起内质网应激。内质网应激过程可启动下游信号级联反应即未折叠蛋白反应 (unfolded protein response, UPR) 以释放压力。若UPR过程不足以消除内质网应激, 则会引起诸如胰岛β细胞功能异常及凋亡的病理状态发生, 最终导致2型糖尿病的发生[32]

高糖 (糖毒性)、高脂 (脂毒性) 或二者同时存在条件下, 内质网可抑制ROS生成, 尤其是来源于线粒体的ROS, 同时, 可降低某些内质网应激相关因子的表达, 并可一定程度上抵御棕榈酸诱导的胰岛β细胞凋亡[33]; 但内质网应激却可通过干扰线粒体内钙离子稳态, 引起胰岛β细胞凋亡。除此以外, 近期研究表明, 线粒体功能异常亦会引起内质网应激; 上述研究均提示, 内质网与线粒体在维持胰岛β细胞存活及功能方面具有一定的协同作用。

线粒体与内质网功能之间存在多种间接或直接的联系, 共同调节细胞内物质及能量代谢稳态, 其中即包括脂质转运过程及钙离子交换过程 (对钙离子方面影响见2.3线粒体-内质网动态关联与胰岛素相关信号通路)。以脂质转运过程为例, Lahiri等[34]研究表明, 内质网膜蛋白复合物 (EMS) 可通过与线粒体外膜移位酶蛋白复合物5 (translocase of the outer membrane 5, Tom5) 发生相互作用, 促使脂质由内质网向线粒体转运。

2.2 线粒体与内质网的结构偶联关系

近年, 针对内质网与线粒体之间膜接触位点 (即结构偶联) 形成机制的研究进展迅速, 也进一步揭示了两个重要细胞器之间的相互调控对细胞生理生化功能的影响。研究发现, HeLa细胞中5%~20%的线粒体表面膜结构 (线粒体外膜) 与内质网存在联系[35]。与此类似, 针对酵母的研究发现, 内质网与线粒体之间存在约100个紧密联系 ( < 30 nm) 的位点[36]。目前认为, 这些接触位点对于内质网与线粒体之间的钙离子以及脂质交换具有重要作用。de Brito等[37]证实, 同时存在于内质网及线粒体上的线粒体融合蛋白2 (mitofusin 2, Mfn2) 对于线粒体的融合过程具有重要调控作用。绝大多数Mfn2位于线粒体外膜 (mitochondrial outer membrane, OMM), 但内质网中同样表达一部分Mfn2, 存在于内质网上的这部分Mfn2可与线粒体外膜上的Mfn2及Mfn1形成二聚体, 从而连接内质网与线粒体。更为重要的是, 敲除Mfn2可减少细胞内质网及线粒体之间的钙离子运输, 这与Mfn2在二者连接之间的作用相一致。亦有研究表明, 这种连接作用同样受到角蛋白结合蛋白多毛蛋白 (trichloplein) 或肌肉生长抑制素 (mitostatin) 的调控[38]

最近, Kornmann及其团队[39, 40]在酵母中发现了一种可连接内质网与线粒体的复合物, 并将其命名为ERMES (ER-mitochondria encounter structure), 该研究采用一种巧妙的方法分离出内质网与线粒体连接缺陷的突变体, 并发现该复合物由内质网蛋白Mmm1、线粒体外膜蛋白Mdm10以及Mdm34和胞内蛋白Mdm12组成, 敲除其中任何一种蛋白均会导致该复合物的解离, ERMES突变也可抑制内质网与线粒体之间的脂质交换过程。研究中也发现, ERMES复合物的4种组成蛋白中, 有3种均具有形成管状结构的结构域, 这种结构可能与膜间脂质交换过程密切相关[41]

线粒体与内质网具有结构上及生理功能上的相互联系。这种联系一般发生在与线粒体相连接的内质网膜 (MAM)、内质网的特定区域或OMM。有研究表明, 通过调控内质网与线粒体之间的接触, 可影响γ-分泌酶 (γ-secretase) 活性及β-淀粉样蛋白 (Aβ) 的生成。若内质网与线粒体之间联系增加, 则会引起γ-secretase活性降低[42, 43]。Lewis等[43]研究发现, 内质网与线粒体的接触还决定着线粒体的复制、分裂和分布。整个细胞中, 有上百个这样的接触点决定着线粒体DNA分裂发生的位置及线粒体在细胞中分裂的方式, 这表明内质网与线粒体间存在更高层级的调控作用。本实验室在探索线粒体蛋白SIRT5的生理功能时发现, 过表达SIRT5可抑制糖脂诱导的胰岛β细胞凋亡, 同时也会显著抑制内质网应激相关的多个关键分子[24], 亦提示线粒体中某些蛋白可能通过某种调控机制影响内质网的分子表达。

随着对内质网与线粒体的结构偶联现象及其调节机制的深入了解, 可推测这种结构上的联系直接或间接地参与某些疾病的病理机制, 对其功能异常与相关疾病的研究有助于发现潜在药物靶点和形成新的治疗策略。

2.3 线粒体-内质网动态关联与胰岛β细胞调控

线粒体功能受损及内质网应激与2型糖尿病的发生发展密切相关。一方面, 线粒体与内质网之间的联系对于维持胰岛β细胞存活、抵御病理状态具有重要作用; 另一方面, 二者间联系也对胰岛β细胞的正常分泌功能具有关键调控作用。

2.3.1 线粒体-内质网动态关联与胰岛β细胞存活

在2型糖尿病状态下, β细胞线粒体内氧消耗水平降低, 胞内ROS种类增加, 引起多种分子氧化及细胞损伤。同样, 内质网稳态遭到破坏时, 如内质网内发生UPR, 也会启动细胞死亡信号通路[44]。高糖或高脂对线粒体活性及内质网稳态产生的损伤作用, 均可引发能量代谢失衡及UPR加剧, 最终导致胰岛β细胞死亡[45]。亲联蛋白 (Junctophilin, JPH) 在细胞膜及内质网之间起到通道作用, 为细胞提供内部交流的通道。有研究表明, 特异性敲低小鼠胰岛内Jph3, 可下调葡萄糖刺激的胰岛素分泌 (glucose-stimulated insulin secretion, GSIS) 水平, 且伴随线粒体功能损伤[46]

细胞内能量代谢及营养物质代谢稳态的正常调控是维持胰岛β细胞功能的关键。在此过程中, 线粒体与内质网之间的联系起到重要作用。当线粒体内钙离子稳态遭到破坏时, 葡萄糖刺激的胰岛素分泌水平显著降低, 但释放内质网中的钙离子可一定程度抵消该作用; 然而, 当内质网中钙离子浓度下降时, 内质网内蛋白错误折叠及内质网应激效应风险将会增加, 并进一步影响胰岛素分泌过程[47, 48]

2.3.2 线粒体-内质网动态关联与胰岛素相关信号通路

有研究表明, 特异性敲除肝脏中Mfn2会导致线粒体与内质网间联系减少, 从而导致线粒体功能异常、胰岛素抵抗及葡萄糖耐量异常[49], 提示线粒体与内质网的动态关联对于胰岛素信号传导具有重要影响, 该观点得到了一系列研究的支持。Tubbs等[50]研究证实, 胰岛素的信号传导与MAM的形成和维持直接相关, 且T2DM模型中MAM联系被破坏, 其中蛋白激酶B (PKB, 也被称为Akt) 是参与MAM水平调节的关键激酶, 可能通过与IP3R1相互作用, 调控其磷酸化水平, 进而影响内质网内Ca2+的释放。胞内Ca2+稳态在胰岛素信号通路中作用尤为关键, 尤其对于胰岛β细胞而言, 葡萄糖引起的胞内Ca2+水平变化在胰岛素分泌过程中的作用已十分明确[7], 且其异常可能导致T2DM的发生[51]

虽然已证实线粒体的完整性与胰岛β细胞对于葡萄糖反应性密切相关[22], 但线粒体-内质网间动态联系与胰岛β细胞相关研究仍十分有限。考虑到胰岛β细胞可能也是胰岛素作用的关键靶点之一[52], 其中线粒体-内质网间联系的相关研究可能具有更为重要的意义。然而, 对于线粒体-内质网动态关联, Ca2+、ROS及脂质 (亦或三者共同作用) 造成的影响孰轻孰重目前尚无定论, 仍需后续研究进一步探讨。

3 展望

线粒体与内质网作为重要的亚细胞结构, 与细胞的功能状态密切相关, 线粒体与内质网结构偶联亦加强了二者联系。线粒体自身动态变化响应胞内能量代谢过程, 并参与部分物质代谢过程, 为胰岛β细胞稳态提供物质基础; 内质网则可实时响应线粒体变化, 并通过调控相关蛋白合成, 影响胰岛β细胞功能。除二者各自功能外, 线粒体与内质网结构或能量代谢状态中的相互影响、协同作用, 对于维持胰岛β细胞的状态及生理功能具有十分重要的意义。对线粒体-内质网动态关联共调控胰岛β细胞功能稳态的深入探索, 将进一步揭示亚细胞水平细胞器间对β细胞功能的精密调节, 有助于糖尿病发病机制研究及药物新靶点的发现, 也将为糖尿病的防治提供全新思路。
