线粒体靶向药物输送抗肿瘤的研究进展

摘要: 线粒体是人体组织细胞中的重要细胞器,参与多种代谢过程,其功能障碍与癌症、神经退行性疾病、肥胖和缺血再灌注损伤等难治愈疾病密切相关。因此,线粒体靶向药物研究日益受到重视,本文将简要介绍线粒体与人类疾病,尤其是与肿瘤的关系,概述当前线粒体靶向药物传递系统的设计策略和发展趋势。

Abstract: Mitochondrion is one of the most vital organelles in cells of human body, and it is involved in many metabolic processes. Mitochondrion dysfunction is closely related to many diseases such as cancers, neurodegenerative diseases, obesity and ischemia reperfusion injury. As a result, mitochondrial drug delivery has gained more and more attention in the drug discovery against these diseases. This review gives a brief introduction to the relationship between mitochondria and human diseases (e.g., cancer), and summarizes the latest trend of mitochondrial targeting drug delivery system (MTDDS).

靶向给药系统 (targeting drug delivery system,TDDS) 经血管、胃肠道或局部给药后,可借助配体的修饰或外加磁、热、光、超声等影响,选择性地浓集定位药物于特定器官、组织、靶细胞或细胞器内[1]。目前,已有较多的针对器官、组织或细胞靶向的研究,然而药物发挥治疗作用的靶点大多是蛋白质、核酸、酶和受体等功能性生物分子,这些分子通常位于细胞中。因此,改变药物的细胞内分布,输送药物至特定细胞器 (如细胞核、线粒体和内质网等) 的输送技术研究成为当前药学研究的新兴热点之一[2]

大部分临床化疗药物的作用靶点为细胞核内的DNA,如蒽环类抗生素、顺铂和喜树碱等,它们通过损伤癌细胞核内的DNA或抑制拓扑异构酶来诱导细胞凋亡。而有些耐药细胞可能会将已进入细胞核的药物 (阿霉素) 重新排出至细胞质中 (sequestration in cytoplasm),从而将药物与其作用靶点 (细胞核) 相隔离,产生耐药现象。作者前期研究中制备了透明质酸 (hyaluronic acid) 修饰的树状聚合物 [poly(amido amine)] 的纳米载体,共载阿霉素和小干扰RNA,可明显提高阿霉素在细胞核内的分布及其细胞毒性[3]。此外,线粒体是细胞呼吸链和氧化磷酸化的中心,与电子转移、调控细胞凋亡、钙储存、自由基生成和脂质代谢等重要生化过程密切相关。因此,线粒体功能失调和一系列疾病如心脑血管病、糖尿病、肠胃病及神经退行性疾病,尤其是肿瘤凋亡,关系密切[4],其靶向药物输送成为近年来研究热点之一,本文将对这一领域的最新进展进行概述。

1 细胞内药物靶向与肿瘤 1.1 药物的细胞内靶向

细胞内许多亚结构可以作为肿瘤治疗的靶点,干扰DNA复制和转录,可有效抑制肿瘤组织的增殖,因此一些药物通过作用于细胞核发挥抗肿瘤作用。但较小的细胞核孔影响部分药物如多肽的递送,Her2-NLP多肽可以发射荧光,特异性地与Her2/neu阳性乳腺癌循环细胞相结合,PEG修饰后的Her2-NLP多肽与MDA-MB-231细胞共同孵育,结果发现多肽仅出现在细胞质内,几乎无法进入细胞核[5]。核糖体是细胞内蛋白质合成的重要场所,通过抑制RNA聚合酶I mRNA的转录,阻断肿瘤细胞的蛋白质合成,可以抑制肿瘤的生长[6],然而目前作用于核糖体的药物依然相对较少。另外,肿瘤细胞过度增殖,中心体的复制频繁,通过抑制中心体的聚集,纺锤体出现多极化,最终可导致细胞死亡。抗癌药物通过改变纺锤体张力发挥疗效,此类药物可抑制细胞增殖,但无法直接杀灭细胞[7]。细胞内氨基酸代谢平衡与细胞的正常生长密切相关,当细胞内营养物质不足,将通过自噬过程,将多余或受损的蛋白质降解为游离氨基酸,并释放到胞浆。整个过程受到氨基酸浓度、ATP及氧气浓度精确调控[8]。因此改变线粒体氧化呼吸链的功能,同样可以调节蛋白质合成与分解,达到靶向核糖体的类似效果。而阻断药物呼吸作用,也可直接杀死细胞。相对于细胞核及其他细胞器,线粒体以其较高的膜电位、脂溶性和特异性的蛋白转入途径等特点而引起了更多研究者关注。

1.2 线粒体与肿瘤

肿瘤细胞具有一系列生物学特征,如无限复制潜能、抗生长抑制信号、抗凋亡、持续产生增殖信号、诱导血管再生和浸润转移等,由于线粒体可以调控凋亡,因此肿瘤细胞的线粒体大多功能异常,如外膜通透性差、不能及时释放凋亡信号等,线粒体也由此成为肿瘤治疗的靶点。

线粒体由两层磷脂膜包被,分为基质、膜间隙两个腔,膜上和腔内含有大量参与细胞代谢生长的酶 (如NADH脱氢酶和ATP合成酶等) 和少量线粒体DNA[9]。线粒体在细胞中发挥着重要作用: 一方面,线粒体是细胞的能量工厂,可以通过电子传递链和氧化磷酸化将碳水化合物分解产生ATP,为生命体提供代谢和运动所需的大部分能量[10]; 另一方面,线粒体还参与细胞凋亡过程,凋亡有外在、内在两条信号途径,前者与其细胞表面受体如TNF和Fas有关,后者为线粒体途径,主要是在细胞受到刺激时,线粒体释放存在于线粒体膜间隙内的细胞色素C和凋亡诱导因子,激活caspases,启动细胞凋亡。因此,增加线粒体外膜通透性的因素均可能引发细胞凋亡。如内膜膜电势降低、电子传递链和ATP生产出现障碍、膜上的渗透性转移通道 (permeability transition pore) 开放引起线粒体吸水膨胀而破裂等[1, 11]

线粒体靶向药物包括己糖激酶抑制剂、Bcl-2家族类似物和硫醇氧化还原抑制剂等,其作用途径包括线粒体内膜的亲脂性阳离子、线粒体DNA、电压依赖型阴离子通道、氧化还原电子传递链以及三羧酸循环等[12]。位于线粒体外膜的VDAC1可以调节线粒体与细胞其他部分的信号传递,作为细胞代谢产物的转运体,VDAC1还可通过调节自噬蛋白的释放影响线粒体介导的自噬过程[13]。线粒体产生的活性氧 (ROS) 可促进肿瘤细胞的增殖,将抗氧化剂送入线粒体内,可改变线粒体三羧酸循环过程,诱导肿瘤细胞自噬[14]。线粒体上的辅酶Q-细胞色素C还原酶结合蛋白与血管生成相关,靶向肿瘤线粒体可改善肿瘤外周血管稳态[15]。另外,研究发现,肿瘤干细胞的线粒体膜电位更高,并伴有更高水平的三磷酸腺苷、钙离子以及ROS[16]

2 线粒体靶向药物传递方法

目前线粒体研究的作用靶点主要包括线粒体DNA、呼吸链、通透转变孔复合物 (mitochondrial permeability transition pore complex,mPTPC)、钾通 道以及线粒体相关的凋亡因子。糖酵解抑制剂氯尼 达明、凋亡诱导剂氨氯地平和神经酰胺是研究较多 的作用于线粒体的抗肿瘤小分子,而大多数传统化疗药物,其主要作用靶点仍以细胞核或微管为主。但有研究表明紫杉醇的作用机制除了细胞质内微管外,也与其作用于线粒体并触发凋亡有关[17]; 而阿霉素除了作用于细胞核内DNA,也可通过其提高线粒体ROS水平,促进细胞凋亡[18]

线粒体靶向属于细胞器水平的靶向,其较大的跨膜电位和线粒体蛋白的进入机制是其被用来靶向输送药物的两个基本特性,因此,线粒体的药物靶向主要包括亲脂阳离子 (lipophillic cations) 介导和线粒体蛋白导入两种途径,此外,还可以通过对药物进行硝基氧化物或短杆菌肽等的连接修饰来实现线粒体靶向[15]。由于到达靶点前给药系统要克服复杂的多重屏障,因此对给药系统的设计提出了更高的要求。目前研究大多采用双靶向策略,如在纳米载体表面同时修饰两种配体或抗体,使其先后靶向至肿瘤细胞和线粒体,而进入细胞浆中的药物分子在到达线粒体前仍面临着胞浆中蛋白骨架和其他细胞器及生物大分子的阻碍等问题[19]

线粒体靶向的配体主要有三苯基膦 (TPP) 及 其衍生物如十八烷基三苯基膦 (STPP) 等,以及金 离子与氮杂环构成的复合物 (NHCs)、KLA多肽 (KLAKLAK)、αvβ3家族多肽 (如RGD4C) 等[8, 20]。金离子与氮杂环构成的复合物 (NHCs) 兼有线粒体靶向、硫氧还原蛋白酶抑制剂的作用[21]。KLA多肽属于穿膜肽的一种,能够选择性诱导肿瘤细胞凋亡[22]

2.1 亲脂阳离子介导的靶向

体外线粒体膜电位180~200 mV时,内侧带负电; 在活细胞和机体内略低,为130~150 mV。利用亲脂阳离子进行药物递送,其中研究最为成功的是TPP[23],TPP含有三个苯环可增大分子表面积,并形成离域正电荷,可穿过线粒体双层疏水膜。许多生物活性分子经TPP修饰后,显示出线粒体的靶向性,如抗氧化剂维生素E与TPP相 连,能更有效地保护线粒体不被氧化[24, 25]。作者前期研究中,将阿霉素与TPP化学连接,显示了良好的线粒体靶向作用,并且通过线粒体途径诱导了肿瘤细胞的凋亡,克服肿瘤细胞耐药性[26] (图 1)。将TPP与纳米制剂化学结合,也具有线粒体靶向的作用,如TPP修饰硬脂酰 (stearyl) 制得的脂质体[27]、TPP修饰PEG-PE制得的紫杉醇脂质体[20],表面修饰TPP的树状大分子PAMAM[28]、TPP修饰PLGA-PEG后的纳米粒[29],以及TPP修饰TPGS后制得的紫杉醇脂质体[30]等,均显示不同程度的线粒体靶向作用,并具有良好的体内外抗肿瘤作用。

2.2 细胞穿膜肽与线粒体蛋白导入机制

线粒体中的蛋白质大多由核基因编码、在胞浆核糖体上以前体蛋白的形式合成,这些蛋白在经靶向信号介导转运至线粒体内,经过适当的水解、折叠,与其他辅助因子结合成为成熟的蛋白[31]。该前体蛋白大都含一段特定的线粒体靶向的氨基酸序列信号 (MTS),通常含有20~30个氨基酸,结构上会形成富含碱基、羟基化和疏水残基的α螺旋[32]。将这些信号肽/蛋白与药物及载体结合,或将编码线粒体靶向信号肽MTSs的DNA序列与治疗性DNA整合,也可实现分子或微粒的线粒体靶向[33]

细胞穿膜肽 (cell penetrating peptides,CPP) 是从人类免疫缺陷病毒-1 (HIV-1) 中获得的一段蛋白转导区域(protein transduction domain,PTD),这种肽可以促进多肽、蛋白、质粒DNA和siRNA等生物大分子,以及脂质体等纳米载体的入胞[34]。Shokolenko等[35]将PTD和MTS结合,可促进exonuclease III递送系统的入胞和线粒体靶向。线粒体膜与细胞膜均具有膜电势,但前者的疏水性更强,因此增加CPP的正电性和疏水性有利于其进一步靶向至线粒体,使其同时具有线粒体靶向的功能[36]

3 线粒体靶向传递系统研究现状

目前,线粒体靶向药物的传递主要是通过线粒体靶向基团如罗丹明 (rhodamine)、TPP和地喹氯铵 (dequalinium chloride) 等,对活性药物或纳米载体(以血管内给药途径为主) 进行化学连接或表面修饰来实现的。另外通过提高药物或载体的正电荷或亲脂性,也有助于其趋向线粒体 (mitochondriotropics)。研究较多的线粒体靶向载体包括囊泡、脂质体、胶束、纳米粒、树状大分子、碳纳米管及金纳米粒等无机纳米载体。除了治疗药物输送,亦有一些影像试剂 (如FITC) 用于线粒体靶向的研究[11,18,25,26,28-30,37-55],如表 1所示。

3.1 脂质体

脂质体能够包载亲脂性和亲水性药物,对脂质体表面进行不同的修饰有望实现相应的靶向作用。有研究设计了一种新型的多功能脂质给药系统 (dual-function MITO-Porter,DF-MITO-Porter),通过进一步的R8或线粒体靶向信号肽MTS的修饰,可实现载体的入胞、内涵体逃逸以及线粒体靶向等多级转运功能。该类给药系统主要包括两重的双分子层脂质膜: 内膜包括药物; 外膜借助R8的作用促进入胞后,进一步通过载体的pH敏感或膜融合脂质的作用,从内涵体中逃逸,并再次通过内层膜上的R8或MTS,实现载体的线粒体靶向[44, 45, 48]

传统脂质体可通过对表面进行TPP、地喹氯铵的修饰,或表面进行阳离子的改造以及线粒体靶向肽的修饰,来实现载药脂质体的线粒体靶向[18, 30, 38, 41, 51]

3.2 纳米粒

Cuchelkar等[56]将肿瘤靶向和线粒体靶向结合,采用赖氨酸连接TPP和药物,再利用可降 解的二硫键与聚合物HPM结合。该聚合物-药物的结合物靶向进入肿瘤细胞后,其二硫健断裂释放TPP-赖氨酸-药物。另外,亦有研究采用TPP修饰PLGA-b- PEG ,得到PLGA-b-PEG-TPP,并与PLGA或PLGA- PEG混合制备得到了混合纳米粒,包载药物后,在抗肿瘤和阿尔兹海默症中,显示了良好的治疗效果[29]

金属纳米粒近年来也开始用于药物靶向传递和影像诊断研究,Wang等[43]用溴化十六烷基三甲铵 (CTAB) 修饰金纳米粒,研究其在癌细胞、普通细胞和干细胞中的摄取、转运和对溶酶体的作用,发现金纳米粒能在杀死癌细胞的同时,保持对正常细胞的低毒性,显示了良好的研究应用前景。目前研究涉及的常见金属纳米粒有金、铂和铁等,但其毒性还有待于进一步验证。

3.3 胶束

聚合物胶束内核通常是由两亲性多聚体的疏水部分组成,可以用来包裹难溶性药物并控制其释放,外壳通常是亲水端,用于提高系统水溶性并避免被体内网状内皮系统识别。以往大多数胶束采 用直链聚合物,而支链或星形聚合物因其聚集后独特的形态学结构和在溶液中的自组装特性引起人们的关注,有研究采用ABC星形多聚物实现CoQ10的线粒体靶向纳米传递系统,结合使用点击化学 (click chemistry) 和开环聚合法合成多功能杂臂聚合物[37]。实验证明,CoQ10胶束能够有效传递至线粒体,并在神经细胞中呈现良好的抗氧化特性。

3.4 其他载体

除了脂质体、纳米粒和胶束外,另一种常用的纳米载体就是树状大分子。树状大分子既能将药物包载进内核,也能与药物通过化学结合使其附着在表面。利用不同的分子对树状大分子进行表面 修饰可使其具有不同性质,如PEG修饰可提高溶解度,而连接细胞识别配体可实现靶向作用,另外连接发光物质还可用于成像等。Biswas等[28]在PAMAM (G(5)-D) 树状大分子上接TPP作为靶向线粒体的配基; 通过对树状大分子表面的氨基部分乙酰化可中和一部分正电荷,这样可以避免树状大分子与其他细胞的非特异性结合,从而降低其毒性,链接FITC显像后,可明显靶向于细胞内线粒体。

4 总结

目前,关于线粒体靶向药物输送系统的研究取得了一定进展,但仍面临一些选择和挑战。① 药物的选择: 包括传统化疗药物、新型小分子活性药物、蛋白多肽和基因等生物大分子药物; ② 作用靶点的选择: 如电子传递链中的酶、线粒体DNA以及线粒体膜通透性等; ③ 线粒体靶向与肿瘤靶向的兼顾: 线粒体广泛存在于各种细胞中,如何避免线粒体靶向载体的非特异性输送,需要与肿瘤细胞靶向的策略联合应用; ④ 载体材料的靶向效率: 如何在确保药物输送系统进入细胞并经内涵体/溶酶体逃逸后,依然保持其靶向功能,并克服胞内如生物大分子等各种阻碍,进一步靶向至线粒体,仍有一定困难。⑤ 载体的生物安全性: 如何确保载体材料的体内降解和生物相容性,降低其毒性,是目前纳米材料应用的一大挑战。因此,今后的研究需进一步探讨和明确线粒体的作用途径及机制。另外,在线粒体靶向输送系统的研究中,目前多采用双靶向或多靶向的设计策略,也可进一步探索兼顾线粒体靶向和肿瘤细胞靶向的配体,同时对纳米载体自身的性质,如粒径、电位、形貌和化学组成等进行优化。

从细胞生物学角度来看,线粒体在不同生理条件下,数量和形状都会发生改变,线粒体DNA拷贝数量与肿瘤的发生是否存在关联仍有争议,这为制剂作用机制的研究带来挑战。建立动物模型时,不同个体的线粒体差异较大,难以找到统一的治疗方案。不同肿瘤细胞系之间线粒体的区别,在治疗方案筛选上的异同还有待进一步研究。

随着生物和纳米技术的发展,细胞器靶向药物输送的研究成为靶向研究的一个趋势和热点,也有望在今后多种疾病的治疗中发挥更大的作用。
