干扰素(interferons,IFNs)是由脊椎动物细胞产生的一类分泌型糖蛋白[1-2],是由Issacs和Lindenmann 1957年在鸡胚绒毛尿囊膜培养物中检测流感病毒多重复制的干扰现象时发现[3]。干扰素应答是抵御病毒入侵的第一道防线[1, 4-5],机体通过天然免疫识别病毒感染激活IFNs应答,IFNs与以自分泌和旁分泌方式表达而呈广泛分布的干扰素受体(interferon receptor,IFNAR)结合,激活数百个干扰素刺激基因(interferon-stimulated genes,ISGs)参与的抗病毒防御程序,这些基因能够干扰病毒复制的每一步[6-8],使机体快速进入一种抗病毒状态,同时激活对病毒的适应性免疫应答[9-10]。干扰素的研究非常广泛,在人及猴、小鼠、牛、羊、犬、猪、马、大熊猫、兔、鼬等哺乳动物,以及野生动物禽类、龟类、鱼类等都发现干扰素物质的存在[11-13]。根据干扰素的染色体定位、受体特异性和基因特征将其分为3种类型,Ⅰ型、Ⅱ型和Ⅲ型。Ⅰ型干扰素具有抗病毒、抗肿瘤作用[7, 14];Ⅱ型由单基编码的IFN-γ构成,具有免疫调节作用[15-17]。Ⅲ型为IFN-λ,与Ⅰ型干扰素(IFN-α和IFN-β)的功能相似[18-20],此外,IFN-λ在黏膜屏障的先天抗病毒、抗真菌和抗原虫感染中具有重要防御作用[20-22]。
Ⅰ型干扰素家族具有相同的受体类型和基因位点,迄今为止已确认有9类,分别是IFN-α、IFN-β、IFN-ω、IFN-κ、IFN-ε、IFN-δ、IFN-τ、IFN-ν和IFN-ζ[4, 23]。由于IFN-ε发现较晚,相关研究和报道较少,其研究还停留在初步阶段,其很多性质和生物学作用尚不明确。IFN-ε作为Ⅰ型干扰素具有与IFN-α、IFN-β等其他Ⅰ型干扰素不同的、独特的内源性表达模式,不需要病毒刺激或者干扰素诱导因子诱导,并在各个器官和组织黏膜上皮细胞中组成型表达,表达水平受雌激素调节[24-26]。其具有重要的抗病毒和黏膜免疫调节功能[27],对神经系统的有利调节也优于IFN-α/β[26]。尽管IFN-ε与IFN-α、IFN-β等其他Ⅰ型干扰素存在不同,但与其他类型一样,可与IFNAR结合,激活JAK/STAT信号,产生抗病毒、抗增殖和免疫调节反应[28-29]。研究发现,IFN-ε在人和其他动物体内器官和组织的分布规律不同,不同动物基因序列差异显著[30]。目前,研究仅证实人、小鼠、牛、羊和犬IFN-ε具有明显的抗病毒作用[31-32],骆驼IFN-ε具有抗肿瘤作用[26],其他生物学功能需要开展进一步研究。
1 IFN-ε的起源早在1981年,美国的Allan和Jarvis[33]在专利中描述其发现了一种新的具有抗病毒活性的物质——干扰素-ε,是将原代二倍体人上皮细胞接种病毒并分泌到培养液中产生,其分子量约20 ku,这种新物质与IFN-α、β和γ具有抗原特异性和种属特异性,在人上皮细胞中具有明显的抗病毒特性,但在其他类型的人细胞以及牛和小鼠细胞均未显示抗病毒活性[32-33]。由此,发展了利用培养上皮细胞产生IFN-ε治疗人上皮细胞病毒感染的方法。随后,1984年由加拿大学者Roy和Pang[34]公开了一种定量检测IFN-ε的方法,该方法将IFN-ε与人角质细胞和人成纤维细胞一起孵育,然后进行病毒攻击,同时设立标准品,以此来确定抗病毒活性单位。直到1999年,由美国基因泰克公司首次公开了人IFN-ε基因序列[35]。研究发现,人IFN-ε在不同器官组织表达水平不同,人IFN-ε基因在胎盘组织、支气管上皮细胞、微血管内皮细胞、动脉血管平滑肌细胞、卵巢、子宫、阴道、脑、小肠等组织中高表达[26, 36],并受雌激素调控。
2 IFN-ε在动物体内的分布规律IFN-ε具有种属特异性,在不同物种中,IFN-ε基因在各个器官、组织中表达水平也不尽相同。IFN-ε可在大部分动物生殖器官组织中高表达。包括猕猴、小鼠和雌性绵羊卵巢、子宫以及雄性睾丸组织中高水平表达,且随着雌激素水平的升高,IFN-ε转录水平升高[28]。其次,在猕猴、猪和牛的胸腺、小肠、肠系膜淋巴结、骨髓和皮肤中也检测到IFN-ε表达[31, 37-38]。在猕猴和牛的肾,猪、牛和绵羊的肝也能检测到表达[33]。由于IFN-ε处于研究的初步阶段,研究人员选取检测器官组织类别不一致会导致IFN-ε在不同组织中的分布规律差异,特别是脾、淋巴结、骨髓、肌肉、膀胱和气管检测数据较少,想要明确IFN-ε的分布规律还需要开展完善的试验检测。
3 IFN-ε表达与黏膜免疫的关系IFN-ε是一种独特的Ⅰ型IFN,不受模式识别反应元件的诱导。在女性体内,IFN-ε基因在子宫、宫颈、阴道和卵巢组织高表达。在正常生理状态下,IFN-ε可增强女性生殖道(female reproductive tract,FRT)黏膜免疫,具有保护女性生殖系统不受病毒和细菌感染的重要作用[24, 28-39]。推测IFN-ε从上皮细胞分泌到FRT管腔和间质中,可作用于应答细胞,但其分布的确切机制和模式尚待阐明。IFN-ε的表达受雌激素和孕激素调节,分别促进或抑制IFN-ε的表达[24]。IFN-ε通过免疫细胞和上皮细胞表面的Ⅰ型干扰素受体(IFNAR)信号上调干扰素刺激基因(ISGs)(图 1),激活细胞对IFN-ε的特异性应答,增加免疫细胞的数量和活性,上调上皮细胞的抗病毒和抗菌免疫应答。
有研究显示,IFN-ε也可能是女性新冠病毒患者死亡率低的原因之一。通过对COVID-19感染死亡人数数据分析发现,男性死亡率高于女性[40-43]。土耳其巴尔基西尔大学医学研究人员在COVID-19感染治疗研究中发现,IFN-ε抗病毒效果降低了COVID-19女性感染者的致死率[44]。分析认为,IFN-ε与其他类型的Ⅰ型IFNs一样,在抵抗病毒感染时具备基本自然免疫防御反应,同时女性受雌激素水平影响又促进了IFN-ε在体内表达,这可能是降低女性新冠病毒患者死亡率的原因。另一份报告认为,男性死亡率较高与冠状病毒功能性受体血管紧张素转化酶2(angiotensin-converting enzyme 2,ACE2)水平较高有关,ACE2受体在睾丸细胞中大量表达,且其能够帮助SARS-CoV-2进入宿主细胞,故较高的ACE2浓度可能导致患者对SARS-CoV-2的易感性增加[45-46]。其他IFNs,如IFN-λ可减轻病毒载量,改善患者临床症状,是治疗COVID-19的理想药物,但对死亡率无影响[47]。此外,IFN-ε还保护女性生殖道免受细菌感染,在月经周期、生殖和更年期受激素调节,并受外源性激素调节[24, 28]。
对穿山甲免疫系分析发现,IFN-ε具有相似的黏膜免疫保护作用。穿山甲被认为是SARS-CoV-2样冠状病毒的天然蓄水池[45],其免疫系统的进化与其他哺乳动物不同,非洲、亚洲穿山甲的皮肤和黏膜上皮细胞(肺、肠和生殖组织)高水平表达IFN-ε,对皮肤和黏膜免疫具有重要防御作用[28, 46]。这可能是穿山甲在冠状病毒感中呈现轻微症状的原因之一。
4 IFN-ε对神经系统的调节作用IFN-α是一种多效性细胞因子,应用于临床治疗已有30多年,许多干扰素治疗的副作用也相继被报道[48-49],特别是对神经系统的不良影响。IFN受体存在于中枢神经系统中,临床上使用的IFN与这些受体结合,可引起痴呆、抑郁、厌食、运动神经系统抑制等症状[49-53]。此外,IFN-α干扰神经营养信号,阻碍神经突起生长、突触可塑性、内源性神经发生和神经元存活[54]。与其他干扰素不同,IFN-ε的一个特点是在脑内呈组成型高表达,提示IFN-ε可能对大脑和中枢神经系统有特殊影响。对与大脑和神经系统相关的IFN-ε调节基因进行分析发现,这些基因参与许多生理功能,如维持大脑结构、神经细胞的分化、兴奋调节等。例如,rhIFN-ε可促进中枢神经系统钙调蛋白结合蛋白striatin与细胞内Ca2 +结合,调节神经兴奋性突触的功能,而IFN-α没有此功能。rhIFN-ε也可调控与神经细胞结构相关的轴周蛋白(Periaxin,PRX)基因的的表达,影响神经鞘磷脂的形成和神经细胞正常结构的维持[26]。IFN-ε调控的透明质酸介导的运动受体被脑微血管内皮细胞高表达,可能参与脑微血管的形成[55]。最后,IFN-ε激活了胰岛淀粉样多肽的表达,在非胰岛素依赖型糖尿病和阿尔茨海默病神经元损伤的病理过程中发挥作用[26]。IFN-ε由于其持续表达于血管细胞表面,抑制细胞增殖,有望用于动脉粥样硬化症的治疗。此外,经IFN-ε作用后,细胞产生有利于神经细胞的调节因子,可用于治疗中枢神经系统的异常,包括焦虑、抑郁、神经分裂症、帕金森氏症等疾病。由于目前临床应用治疗的干扰素(IFN-α、IFN-β)普遍会产生神经抑郁的副反应,因此,IFN-ε有望成为对干扰素治疗产生不良反应严重患者的替代药品。
5 IFN-ε的抗病毒作用近期的研究发现,利用基因工程技术体外表达的动物IFN-ε具有明显的抗病毒活性。在同源和异源动物体外细胞上,IFN-ε显示交叉保护抑制病毒复制的跨物种特性[56]。例如,应用大肠杆菌表达重组牛IFN-ε能够抑制水泡性口炎病毒(vesicular stomatitis virus,VSV)在牛肾细胞(Madin-Darby bovine kidney,MDBK)、牛鼻甲细胞(bovine turbinate cells,BT)、胎牛肾细胞(embryonic bovine kidney cells,EBK)和猪肾细胞(porcine kidney 15 cell,PK-15)上的复制[56]。重组绵羊IFN-ε在体外可诱导ISG15、黏病毒抗性蛋白1(myxovirus resistanceproteinA,Mx1)和2’-5′寡聚腺苷酸合成酶(2’-5′ oligoadenylate synthetase,OAS)的转录,产生抗病毒蛋白抑制外周血淋巴细胞和MDBK、RK-13、F81、PK-15、BT细胞上的病毒复制,而在MDCK、BHK-21细胞无抗病毒活性[57]。
IFN-ε也可作用于HIV感染的早期阶段,包括病毒进入、反转录和核导入。在人巨噬细胞中诱导一种阻断HIV-1复制的抗病毒状态。IFN-ε刺激的免疫介质和途径具有Ⅰ型IFN的特征,但与巨噬细胞中的IFN-α不同。IFN-ε诱导了大量的吞噬和活性氧(reactive oxygen species,ROS)杀灭作用,从而阻碍了HIV的复制[58]。除此之外,IFN-ε具有抗增殖的作用,重组犬IFN-ε可激活JAK-STAT信号通路,具有显著的抗增殖作用[59]。有学者通过噻唑蓝(3-(4,5)-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide,MTT)细胞增殖检测、形态学观察和细胞凋亡试验发现,包涵体表达的重组骆驼IFN-ε以剂量依赖性抑制乳腺癌细胞MDA-MB-231和MCF-7生长,与对照组相比,IFN-ε处理过的MDA-MB-231细胞Caspase-3的表达量明显升高[60]。
Ⅰ型IFN都具有结构上的同源性,并与由IFNAR1和IFNAR2亚基构成的异二聚体受体结合,该类受体在大多数细胞表面均有表达[61]。IFN受体两个亚基IFNAR1和IFNAR2存在两个独立的配体结合位点,这种双重结构保证了不同类型IFN被识别并结合启动干扰素信号转导[62]。IFN-ε与IFNAR结合,激活ISGs的表达,进而激活多种抗病毒蛋白表达抑制病毒复制。但是,在体内和体外试验中,病毒诱导不能产生IFN-ε[63],而IFN-ε在生殖系统黏膜和支气管黏膜上皮细胞组成型高表达,这些黏膜部位存在着不同的解剖结构、生理作用和微生物的差异,因此,要明确IFN-ε对不同病原体在不同黏膜组织内的保护作用需要进一步深入研究其作用机理。在女性生殖系统中IFN-ε表达水平受雌激素和孕激素的调节,是否存在其它的特异性作用机制调节IFN-ε的表达也有待进一步证实。
6 展望干扰素是国内外第一个被批准投放市场用于临床抗病毒、抗肿瘤治疗的基因工程药物,其治疗的适应症有数十种[64-65],目前,也用于治疗COVID-19感染的临床试验[44, 66-67]。而IFN-ε作为Ⅰ型IFNs中新发现的一个独特家族成员,在多种细胞上具有明显的抗病毒作用,其在黏膜免疫和对神经调节的作用明显优于其它Ⅰ型IFNs。IFN-ε正在被研究用于由HPV感染引起的尖锐湿疣、寻常疣、喉乳头状瘤、宫颈癌等多种疾病,女性泌尿生殖系统感染以及部分肿瘤的辅助治疗,有望成为其他治疗用干扰素不良反应的替代品。目前的研究表明,病毒感染不能诱导机体和体外细胞产生IFN-ε,尽管IFN-ε对生殖道、肠道和肺部气管黏膜有预防微生物感染的保护作用,这种保护作用可能通过某种组织特异性的机制进行调节,例如在雌性生殖系统通过分泌雌激素和孕激素调节IFN-ε的表达。其他黏膜部位的IFN-ε表达是通过什么机制调节的?是否存在依赖于细胞或特定组织中表达的其他辅助因子?IFN-ε对不同病原体在不同黏膜组织内保护作用的机制、广度和潜力都需要进一步探索。
