奇异变形杆菌是变形杆菌属临床最常见的病原体,是导致泌尿系统感染排名第2位的肠杆菌科细菌,仅次于大肠埃希菌[1]。在院内感染中,由奇异变形杆菌引起的比例占到3%[2]。奇异变形杆菌还可以引起肠道感染,目前我国公布的细菌性食物中毒病原谱中,奇异变形杆菌仅次于副溶血弧菌、沙门菌和金黄色葡萄球菌,是引起食物中毒的第四大主要病原菌[3]。奇异变形杆菌还可引起血流感染,病死率较高,值得关注[4]。
随着抗生素的大量不规范使用,奇异变形杆菌的耐药性不断增强,1991年,法国首次报道了产β-内酰胺酶的奇异变形杆菌[5]。随后,美国、法国、日本、中国等国家陆续报道携带不同类型β-内酰胺酶的奇异变形杆菌[6-20]。耐多药奇异变形杆菌和耐碳青霉烯奇异变形杆菌的出现,给临床治疗提出了极大的挑战。本研究将从生物学特征,对β-内酰胺类、喹诺酮类等抗菌药物耐药机制,耐药基因在各国的流行情况,临床感染及治疗情况、未来发展趋势5个方面来介绍奇异变形杆菌耐药性的研究进展。
1 奇异变形杆菌的生物学特征奇异变形杆菌是具有细胞迁徙现象的兼性厌氧菌,营养需要不高,广泛存在于泥土、水和机体消化道中,有周鞭毛,运动活泼。奇异变形杆菌与感染相关的毒力因子主要为高黏附性的菌毛、荚膜多糖、各种蛋白水解酶以及具有细胞毒活性的溶血素如HlyA、HpmA、HpmB等。菌毛增加了细菌的侵袭力,蛋白水解酶促进感染扩散并帮助菌株逃避免疫系统的攻击,荚膜多糖不仅增加了细菌黏附性,其形成的生物被膜更阻断了抗菌药物的杀菌抑菌作用,因此奇异变形杆菌导致的感染往往比较严重而且难以治疗。
奇异变形杆菌氧化酶、赖氨酸脱羧酶、吲哚、枸橼酸、麦芽糖、甘露醇、蔗糖、赖氨酸、鸟氨酸均呈阴性;苯丙氨酸脱氨酶、脲酶、鸟氨酸脱羧酶呈阳性;能发酵葡萄糖,产酸产少量气;不能发酵乳糖和卫矛醇;不能产生靛基质;有动力;能产生H2S;不能分解枸橼酸盐[21]。
2 奇异变形杆菌的耐药情况奇异变形杆菌耐药谱广,除对呋喃妥因、四环素、替加环素和多粘菌素天然耐药外,近年来对β-内酰胺类、喹诺酮类药物耐药性持续上升,多重耐药菌株也日益增多。
2.1 对β-内酰胺类抗菌药物的耐药流行情况奇异变形杆菌因青霉素结合蛋白2(PBP2)的功能缺陷[22],对亚胺培南天然不敏感,最低抑菌浓度MIC(minimum inhibitory concentration)值在16~64 μg/ml 之间。1991年法国首次报道了产β-内酰胺酶奇异变形杆菌[5]。1995年美国报道了产TEM-10β-内酰胺酶的临床来源奇异变形杆菌[6]。从1998年至今,南非、意大利、西班牙、希腊、巴西、日本、保加利亚、中国、欧洲、瑞士、阿根廷、韩国、中国台湾、俄国、波兰、以色列、中国香港等多个国家和地区报道了产各种类型β-内酰胺酶的奇异变形杆菌[8-20]。2008年美国首次报道了产KPC碳青霉烯酶的奇异变形杆菌,对亚胺培南、美罗培南、厄他培南均耐药[23]。近几年波兰和法国又陆续报道了产VIM-1型和产NDM-1型碳青霉烯酶的奇异变形杆菌[24-25]。
近25年有关奇异变形杆菌耐药报道中,最多的是产β-内酰胺酶的奇异变形杆菌,分布于世界各地,有很多种类型,包括广谱β-内酰胺酶(ESBLs)、头孢菌素酶(AmpC)和碳青霉烯酶等,其中ESBLs以TEM型和CTX-M型为主,头孢菌素酶以CMY型为主。近年来,奇异变形杆菌对第三代头孢菌素等的耐药率不断上升,产ESBL和(或)AmpC酶是奇异变形杆菌对头孢菌素耐药最重要的机制[4]。
2.1.1 ESBLs的耐药流行情况ESBLs主要由肠杆菌科细菌产生,尤其是在大肠埃希菌和克雷伯菌属中,已成为肠杆菌科对β-内酰胺类抗生素产生耐药性的主要原因。其类型主要有TEM、SHV、CTX-M、PER和VEB等。汇总最近二十几年有关奇异变形杆菌耐药相关文献发现产ESBLs的奇异变形杆菌主要以TEM型和CTX-M型为主。
2.1.1.1 TEM型的耐药流行情况、主要耐药基因类型及相关的耐药机制TEM型是ESBLs中研究最多的酶,目前报道的已有200多种类型,主要是由于特定位点氨基酸的取代使酶具有ESBLs活性。产TEM型ESBLs的奇异变形杆菌一般对头孢类抗 生素耐药,在世界各地的分布情况见表 1。

TEM-1型分布的地域最广,出现的频率最高,法国报道的类型最多。有研究证明TEM-8、TEM-10、TEM-24、TEM-26和TEM-87是导致头孢他啶耐药的原因,TEM-3、TEM-21、TEM-66和TEM-72是导致头孢噻肟耐药的原因[26];TEM-110是导致克拉维酸耐药的原因[27],也有研究报道特定位点氨基酸的取代使酶具有ESBLs活性,比如TEM-1进化为TEM-187,产生ESBLs活性就是因为21、164、184和265位点的氨基酸发生了替换导致的[28]。
2.1.1.2 CTX型的耐药流行情况、主要耐药基因类型及相关的耐药机制CTX型酶耐头孢噻肟的能力最强,报道的已有100多种,分布于世界各地。酶抑制剂中他唑巴坦比舒巴坦或棒酸的抑制作用强,主要由质粒介导[29]。产CTX型的奇异变形杆菌也是主要介导第三、四代头孢类抗生素和青霉素类抗生素耐药。在世界各地的分布情况见表 1。
产CTX-M型酶的奇异变形杆菌主要分布于亚洲和欧洲的国家,blaCTX-M-15、blaCTX-M-2和blaCTX-M-14是最流行的耐药基因类型。有研究报道在奇异变形杆菌中,blaCTX-M-15、blaCTX-M-2和位于染色体上,它们的移动与插入元件ISEcp1有关,ISEcp1将blaCTX-M-15、blaCTX-M-2整合到奇异变形杆菌染色体上,从而引起耐药性的传播[30-33]。
2.1.2 产AmpC酶的耐药流行情况、主要耐药基因类型及对应的耐药机制AmpC酶是最重要的β-内酰胺酶之一,对三代头孢菌素耐药但对四代头孢菌素如头孢吡肟敏感,且不被酶抑制剂克拉维酸所抑制,有染色体介导和质粒介导2种。目前在奇异变形杆菌中发现的AmpC酶以CMY型为主[33]。首先在法国零星的发现有blaCMY-3、blaCMY-4和blaCMY-12耐药基因类型,来源于希腊和阿尔及利亚地区的患者,blaCMY属于染色体介导的耐药基因[32],主要是对青霉素类抗生素耐药,尤其是阿莫西林。在世界各国的分布见表 1。
产CMY型酶的奇异变形杆菌在欧洲多个国家和中国大陆、中国香港及中国台湾均有报道,CMY-2是最主要的类型。最近,日本、中国香港分别报道了从犬体表、鸡肉制品中分离到了产CMY-2型酶的奇异变形杆菌[20, 34]。有研究报道编码CMY-16的耐药基因定位在一个新的和SXT/R391相关的共轭元件(ICE)上,这个元件具有可移动性,提示CMY-16具有更高的水平传播能力[35-36]。
2.1.3 产碳青霉烯酶奇异变形杆菌流行情况、主要耐药基因类型及相关的耐药机制在已报道的奇异变形杆菌中,主要的碳青霉烯酶的类型有KPC-2、NDM、VIM。其中报道产KPC-2型碳青霉烯酶的国家有美国、中国和巴西[23, 38, 46]。KPC-2型耐药基因定位在质粒上,介导菌株对包括亚胺培南、厄他培南在内的β-内酰胺类药物耐药[23, 46]。
2006年希腊首次报道了携带VIM-1型碳青霉烯酶的奇异变形杆菌[14]。2014年保加利亚也报道了携带VIM-1型的奇异变形杆菌[42],其他国家目前还未发现产VIM型碳青霉烯酶的奇异变形杆菌。
最近,法国首次报道了产NDM-1型碳青霉烯酶的奇异变形杆菌。研究表明NDM-1基因位于一个38.5 kb的多耐药(Multi-Drug Resistance,MDR)基因岛上,该基因岛定位在染色体上,具有潜在的可移动性[25]。
另外,最近巴勒斯坦首次报道了携带OXA-48型碳青霉烯酶的奇异变形杆菌[37]。
2.1.4 其他耐药基因型的耐药流行情况及主要耐药基因类型法国、西班牙等国家和地区均报道了产VEB类型的β-内酰胺酶的奇异变形杆菌[16, 28, 35],主要有VEB-1、VEB-2、VEB-4、VEB-6 4种类型,在世界各地的分布情况如表 1所示,其中,blaVEB-6定位于染色体上[45]。
另外,法国报道了产DHA-1型的AmpC酶的奇异变形杆菌[47],西班牙、中国报道了产PER-1型超广谱β-内酰胺酶的奇异变形杆菌[33, 48]。
2.2 对喹诺酮类抗菌药物的流行情况、主要耐药基因类型及相关的耐药机制2000年美国报道奇异变形杆菌对喹诺酮类抗菌药物的敏感性开始下降,环丙沙星(CIP)的MIC90>512 μg/ml[49];随后日本、中国、希腊、韩国、意大利、中国香港陆续报道了耐喹诺酮类抗菌药物的奇异变形杆菌[18, 39, 46, 50-53]。
奇异变形杆菌对喹诺酮类抗菌药物耐药主要是和染色体上的喹诺酮耐药决定区(QRDRs)突变相关[46, 52-54],是gyrA、gyrB和parC基因发生点突变引起的。gyrA上发生的点突变为83位点上的丝氨酸突变为异亮氨酸或者精氨酸,或87位点上的谷氨酸突变为赖氨酸;gyrB上发生的点突变为466位点上的谷氨酸突变为天冬氨酸,或464位点上的丝氨酸突变为酪氨酸或者苯丙氨酸;parC上发生的点突变为80位点上的丝氨酸突变为异亮氨酸或精氨酸。不同国家来源的不同耐药菌株具体的突变位点不同,但均发生在上述3个基因上,可能只有1个,或2个基因上的位点发生了点突变,也或是3个基因的氨基酸位点均发生突变。QRDRs基因突变是菌株对喹诺酮类药物高水平耐药的主要原因。同时,质粒携带的耐药基因也介导了菌株对喹诺酮类抗菌药物敏感性降低,如qnrC、qnrA、qnrD和aac(6’)-Ib-cr等[18]。还有研究报道AcrAB外排泵的过度表达和前述的QRDRs上的点突变协同导致对喹诺酮的高水平耐药[54]。
2.3 耐多药的奇异变形杆菌的流行情况2001年意大利首次报道了耐多药的产广谱β-内酰胺酶的奇异变形杆菌[55],接着美国、欧洲、波兰、中国香港、巴西、中国、韩国等多国家和地区均报道了耐多药的奇异变形杆菌[20, 32, 34, 41, 50, 53, 56],呈现出向全球蔓延的趋势。
最近法国报道了从犬体表分离的耐多药奇异变形杆菌,提示多重耐药菌株可以在人和动物之间传播[57]。香港也在鸡肉制品中发现了耐多药的奇异变形杆菌[20]。
3 临床感染治疗情况奇异变形杆菌是引起泌尿系感染的主要病原菌之一(仅次于大肠埃希菌),引起的泌尿系感染持久且难治。其中,约90%的泌尿系感染由留置导尿管引起[58],临床提示应密切关注留置导尿患者中奇异变形杆菌引发的泌尿系感染,争取及早诊断,有效治疗;并且对于留置的导尿管应注意及时更换或拔除,防止奇异变形杆菌的吸附及生物被膜的生成,在一定程度上避免奇异变形杆菌引起的泌尿系感染持续并且反复发作[59]。针对产ESBL耐药的奇异变形杆菌,在β-内酰胺抑制复合剂中最有效的药物是哌拉西林/他唑巴坦,在氨基糖苷类抗生素中最有效的是阿米卡星[60-61]。针对耐喹诺酮类抗菌药物的奇异变形杆菌,左氧氟沙星和环丙沙星的敏感率呈逐年下降趋势,临床提示应慎重应用[39, 46, 51-54] 。针对一般的非碳青霉烯类耐药的奇异变形杆菌耐药菌,对亚胺培南以外的碳青霉烯类药物基本维持高度敏感性,可选择作为临床用药[61]。研究发现,产ESBLs的奇异变形杆菌常对氟喹诺酮类等非β-内酰胺类耐药,极大限制了抗菌药物的临床选择,对抗感染治疗构成了严重威胁[33]。有研究报道用氨曲南可治愈耐多药奇异变形杆菌的感染[62]。奇异变形杆菌的多重耐药性已成为临床治疗中一个非常棘手的问题,临床医生建议,最好在抗菌药物使用前做菌株的药敏试验,根据药敏试验指导用药,联合用药,以达到控制感染的目的[63]。
4 未来发展趋势肠杆菌科菌中常见的很多耐药现象在奇异变形杆菌中都有报道,目前主要是对超广谱β-内酰胺类抗生素和喹诺酮类抗菌药物表现出耐药,近几年来,有对碳青霉烯类抗生素耐药的陆续报道,多重耐药菌的报道,动物源的耐药菌有向人类传播的可能的报道,奇异变形杆菌未来将面临着严峻形势。因此,奇异变形杆菌的耐药现状应得到临床上和相关部门的重视,重点研究引起奇异变形杆菌耐药的耐药机制、变异特征以及传播扩散的规律,从而采取有效措施来控制耐药奇异变形杆菌的传播。
